摘要:目的分析患者入院时血清白蛋白与纤维蛋白原比值(AFR)、全身免疫炎症指数(SII)及降钙素原(PCT) 水平对老年重症肺炎(SP)预后不良的预测效能。方法 老年SP患者120例,根据病情最终转归分为治愈组84例与 预后不良组36例。收集两组患者的年龄、性别、体质量指数、合并症、病史及入院第1天时的血生化指标等资料,并 计算 AFR､SII。将老年SP是否预后不良作为因变量,以患者血清AFR､SII､PCT水平为自变量,采用多因素 Logistic 回归分析法分析老年SP预后不良的影响因素。采用受试者工作特征曲线(ROC)分析血清AFR、SII、PCT及三者联 合对老年SP预后不良的预测效能。结果预后不良组患者血清PCT、SII、WBC、Neu水平均高于治愈组(P均< 0.05),血清AFR、ALB、FIB水平均低于治愈组(P均<0.05)。血清AFR､SII､PCT水平是老年SP预后不良的影响因 素(P均<0.05)。血清AFR水平预测老年SP预后不良的AUC为0.711,取截断值为4.96时,血清AFR水平预测老年 SP预后不良的灵敏度为47.22%,特异度为88. 10%。血清SII预测老年SP预后不良的AUC为0.904,取截断值为 370.71时,血清SII预测老年SP预后不良的灵敏度为91.67%,特异度为73.81%。血清PCT水平预测老年SP预后 不良的AUC为0.879,取截断值为3.47 ng/mL时,血清PCT水平预测老年SP预后不良的灵敏度为83.33%,特异度 为84.52%。血清 AFR､SII､PCT联合预测老年SP预后不良的灵敏度为86.11%,特异度为95.24%。结论 患者入 院时血清AFR､SII、PCT水平是老年SP预后不良的影响因素,检测血清AFR、SII、PCT可用于老年SP预后不良的预 测,且三者联合预测价值更高。 [ABSTRACT FROM AUTHOR]
