Percutaneous ablation is a low-invasive, repeatable, and curative local treatment that is now recommended for early-stage hepatocellular carcinoma (HCC) that is not suitable for surgical resection. Poorly differentiated HCC has high-grade malignancy potential. Microvascular invasion is frequently seen, even in tumors smaller than 3 cm in diameter, and prognosis is poor after percutaneous ablation. Biopsy has a high risk of complications such as bleeding and dissemination; therefore, it has limitations in determining HCC tumor malignancy prior to treatment. Advances in diagnostic imaging have enabled non-invasive diagnosis of tumor malignancy. We describe the usefulness of ultrasonography, computed tomography, magnetic resonance imaging, and 18F-fluorodeoxyglucose positron emission tomography for predicting outcome after percutaneous ablation for HCC. Background: The classification of Parkinson disease by age of onset has proven to be a valuable method for subtyping, given its practical application in clinical settings. However, the interactions between the metabolic brain changes, dopaminergic dysfunction, and clinical manifestations in patients with early-onset (early-iPD) and late-onset (late-iPD) idiopathic Parkinson disease have not been adequately evaluated. Therefore, this study aimed to investigate the difference in cerebral metabolism and presynaptic dopaminergic function between patients with early-iPD and those with late-onset disease using 18 F-fluorodeoxyglucose ( 18 F-FDG) and [ 18 F] 9-fluoropropyl-(+)-dihydrotetrabenazine ( 18 F-FP-DTBZ) positron emission tomography (PET). Furthermore, the goal was to further explore the correlation between imaging measurements and clinical manifestations in the early and late idiopathic patients with Parkinson disease., Methods: This cross-sectional study included 80 patients with idiopathic Parkinson disease and 29 healthy control participants who underwent 18 F-FDG and 18 F-FP-DTBZ PET imaging at Xuanwu Hospital, Capital Medical University from August 2022 to August 2023. The patients were categorized into early-iPD (n=27) and late-iPD (n=53) groups based on an age threshold of 50 years. The mean standardized uptake value of 18 F-FDG and the standardized uptake value ratio (SUVR) of 18 F-FP-DTBZ were compared between the early-iPD and late-iPD groups using unpaired Student t -tests. Furthermore, pairwise correlations among cerebral metabolism, dopaminergic function, and corresponding clinical ratings in all patients were conducted using Pearson correlation analysis., Results: Patients with late-iPD exhibited a significant metabolic decrease in the frontal, parietal, and temporal cortex, along with the globus pallidus, putamen, thalamus, and cerebellum, compared to those with early-iPD in 18 F-FDG PET imaging (all P values <0.05). Furthermore, the 18 F-FP-DTBZ binding potential was significantly lower in the contralateral caudate and anterior putamen of patients with late-iPD compared to those with early-iPD (contralateral caudate: 3.16±1.2 vs. 2.63±0.7, P=0.020; contralateral anterior putamen: 2.49±1.2 vs. 2.05±0.7, P=0.040). Further analysis of the correlations between imaging clinical features revealed that glucose metabolism increases and dopaminergic function decreases with higher motor ratings., Conclusions: 18 F-FDG and 18 F-FP-DTBZ PET offer an objective molecular imaging basis for distinguishing between early-onset and late-onset idiopathic with Parkinson disease. Spontaneous hemoperitoneum is a rare and potentially life-threatening condition with a wide differential diagnosis. Gastrointestinal stromal tumors (GIST) can present with spontaneous hemoperitoneum, although diagnosing GIST as the cause of hemoperitoneum is challenging due to its rarity. A 76-year-old Japanese man presented with sudden epigastric pain and was found to have a 10 cm space-occupying lesion and ascites on ultrasonography. Despite stable vital signs, computed tomography (CT) findings showed a 10×15 cm mass with heterogeneously enhanced solid and cystic lesions, and the patient opted for conservative treatment. Two months later, a contrast-enhanced CT scan revealed a high-density area within the hematoma, prompting further investigation with fluorodeoxyglucose-positron emission tomography/CT (FDG-PET/CT), which showed FDG accumulation suggestive of malignancy. Exploratory laparotomy revealed a large encapsulated mass from the greater omentum, and histopathology confirmed a diagnosis of high-risk extraluminal gastric GIST. The patient was successfully treated with surgical resection. This case highlights two important clinical issues. First, follow-up CT and FDG-PET/CT are useful in detecting GIST when an unexplained intraperitoneal hematoma is identified. Second, surgical intervention is recommended in such cases to determine the cause. Contrast-enhanced follow-up CT and FDG-PET/CT are valuable in clarifying the presence of GIST, and surgical intervention is recommended to identify the causes of intraperitoneal hematoma. Objective: 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) not only gives information about anatomical features of lung lesions but also about their metabolic activity. This study aimed to determine the mediastinal and parenchymal pathologies of the lung with false-positive FDG involvement and characteristics that could benefit differential diagnosis. Materials and Methods: This study retrospectively analyzed 1,924 subjects that underwent 18F-FDG PET/CT from December 2010 to January 2015. Subjects with a maximum standardized uptake value (SUVmax) value of =2.5 in the lung tissue, mediastinal lymph nodes with no primary lung malignancy, and a benign pathology result were included. Results: The mean age of the 143 subjects was 61.7±11.0 years. The pathologies anthracosis, granulomatous inflammatory events, and organized pneumonia (OP) were 64 (45%), 36 (25%), and 18 (13%) cases, respectively, in order of frequency. The median SUVmax value of all lesions was 5.2 (min-max, 2.5-37.6). Moreover, mediastinal lymph node involvement was frequent (59%). The median SUVmax value of anthracotic lesions was significantly lower than the median SUVmax value of granulomatous and OP lesions (p<0.001 and p=0.007, respectively). No significant difference in the median SUVmax value was noted between granulomatous and OP lesions. Conclusion: Anthracosis and tuberculosis should be considered as benign causes of mediastinal lymph node positivity in developing countries. 18F-FDG PET/BT is an unreliable method for the differential diagnosis of benign diseases characterized by inflammation. Giant-cell arteritis (GCA) is a type of vasculitis characterised by the presence of granulomas. It is the predominant form of systemic vasculitis in adults and primarily affects the larger arteries in individuals aged ≥ 50 years. GCA affects the major arteries, such as the aorta and its branches, particularly the outer branches of the external carotid artery. Signs and symptoms can be categorised into cranial, extracranial, and systemic manifestations. Patients with headaches, jaw claudication, and vision disturbances usually have extracranial branches of the external carotid artery. Aside from being the prevailing manifestation of GCA, our primary concern regarding this variant is the potential for irreversible vision loss if not properly identified and addressed. Conversely, the GCA can also affect other major blood vessels such as the aorta. Here, we present the case of a 70-year-old Caucasian female patient with cranial GCA who had experienced a temporal headache three years prior. The patient was successfully treated with prednisolone, which was gradually reduced to a very low level with the assistance of methotrexate. Recently, the patient presented with a dry cough that lasted for two months and elevated inflammatory markers. After thorough research, it was determined that there was no evidence of infection, including atypical infections, and that no abnormalities were found in the lungs. Ultimately, via an 18F-fluorodeoxyglucose-positron emission tomography (FDG-PET) scan, the patient was diagnosed with large vessel giant cell arteritis (LV-GCA). This impacted the aorta, carotid arteries, and subclavian arteries. The patient experienced notable improvement in her cough and a reduction in inflammatory markers after receiving a high dosage of oral prednisolone. This case exemplifies the unusual manifestation of LV-GCA and verifies that recurring symptoms may differ from the original presentation. Cancer diagnosis and therapy is quickly moving from the traditional histology-based approaches to genomic stratification, providing a huge opportunity for radiogenomics, associating imaging features with genomic data. Genome sequencing is time consuming, expensive and invasive whereas 18F-FDG PET/CT is readily available, fast and noninvasive. The aim of this study was to determine the relationship between the maximum standardized uptake value (SUVmax) and the frequency of 11 common oncogenic anomalies determined by specific common genomic alterations in tissue biopsies from patients with cancer. We retrospectively studied 102 consecutive untreated patients with gastrointestinal, lung, and breast cancer who underwent 18F-FDG PET/CT imaging, shortly prior to molecular testing by a biopsy for genomic profiling that consisted of 11 common DNA alterations: (1) TP53, (2) DNA repair, (3) EGFR, (4) PI3K/AKT/MTOR (PAM) pathway including PTEN, PIK3CA, AKT, TSC, CCNB1, MTOR, FBXW2, and NF2, (5) MEK, (6) CYCLIN including CCND,CDK, CDKN, and RB, (7) WNT, (8) ALK, (9) MYC, (10) MET, and (11) FGF/FGFR. Higher SUVmax was associated with the presence of TP53 and PAM genomic anomalies (p <.05), but not the other 9 gene groups (p >.05). More importantly, SUVmax was positively correlated with total number of oncogenic anomalies (r = 0.27, p =.005). We propose higher SUVmax as an indicator for total number of common oncogenic anomalies. Background The pro-tumoral action of the cluster of differentiation 147 (CD147), which is associated with the chemotherapy resistance of lung adenocarcinoma, is partly due to accelerated tumor cell glycolysis. 18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET) metabolic parameters included maximal standardized uptake value (SUVmax), mean standardized uptake value (SUVmean), metabolic tumor volume (MTV), and total lesion glycolysis (TLG), which are non-invasive markers of the glucose metabolism of tumor cells in vivo. This study aimed to clarify the correlation between PET metabolic parameters and CD147 expression, and to evaluate the prognostic value of CD147 expression in resectable lung adenocarcinoma patients. Methods A total of 89 lung adenocarcinoma chemotherapy-naive patients who underwent 18F-fluorodeoxyglucose positron emission tomography and computerized tomography scan before pulmonary surgery were retrospectively analyzed. The PET metabolic parameters were calculated by 18F-FDG PET imaging, and CD147 expression was analyzed by immunohistochemistry. SUVmax, SUVmean, MTV, and TLG compared for their performance in predicting the expression of CD147 were illustrated with statistical analysis. All patients were then followed-up for survival analysis. Results The SUVmax was significantly correlated with the CD147 expression and was the primary predictor for the CD147 expression of lung adenocarcinoma. A cut-off value of the SUVmax, 9.77 allowed 85.1% sensitivity and 64.3% specificity for predicting the CD147 positive lung adenocarcinoma. CD147 expression was correlated with tumor differentiation and metastasis. Univariate survival analysis showed that CD147 expression was significantly associated with a shorter overall survival (OS) time. Multivariate analysis revealed that CD147 was an independent prognostic factor of lung adenocarcinoma patients. Conclusion The SUVmax of a primary tumor measured with 18F-FDG PET may be a simple and non-invasive marker for predicting CD147 expression in lung adenocarcinoma. Abstract: Skeletal muscle involvement is uncommon in lymphoma, occurring in less than 1.5% of patients. We report the original case of a 61-year-old man who presented with pseudotumoral muscle lesions of the lower limbs, revealing nonHodgkin T-cell lymphoma. In our patient, magnetic resonance imaging (MRI) was useful in clearly revealing the detailed anatomic extent of muscle change; indeed, MRI showed muscle enhancement after intravenous administration of gadolinium on T 1-weighted images as well as high-signal intensity on T 2-weighted images. Moreover, MRI was helpful in guiding the optimal site for muscle biopsy. [Copyright &y& Elsevier]

Background: Parkinson's disease (PD) is an irreversible neurodegenerative disease. The diagnosis of PD based on neuroimaging is usually with low-level or deep learning features, which results in difficulties in achieving precision classification or interpreting the clinical significance. Herein, we aimed to extract high-order features by using radiomics approach and achieve acceptable diagnosis accuracy in PD., Methods: In this retrospective multicohort study, we collected 18 F-fluorodeoxyglucose positron emission tomography ( 18 F-FDG PET) images and clinical scale [the Unified Parkinson's Disease Rating Scale (UPDRS) and Hoehn & Yahr scale (H&Y)] from two cohorts. One cohort from Huashan Hospital had 91 normal controls (NC) and 91 PD patients (UPDRS: 22.7±11.7, H&Y: 1.8±0.8), and the other cohort from Wuxi 904 Hospital had 26 NC and 22 PD patients (UPDRS: 20.9±11.6, H&Y: 1.7±0.9). The Huashan cohort was used as the training and test sets by 5-fold cross-validation and the Wuxi cohort was used as another separate test set. After identifying regions of interests (ROIs) based on the atlas-based method, radiomic features were extracted and selected by using autocorrelation and fisher score algorithm. A support vector machine (SVM) was trained to classify PD and NC based on selected radiomic features. In the comparative experiment, we compared our method with the traditional voxel values method. To guarantee the robustness, above processes were repeated in 500 times., Results: Twenty-six brain ROIs were identified. Six thousand one hundred and ten radiomic features were extracted in total. Among them 30 features were remained after feature selection. Background: Many benign pulmonary lesions, especially sarcoidosis, are metabolically active and are indistinguishable from lung cancer using 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) imaging. This study sought to analyze the 18F-FDG PET/CT imaging features of benign pulmonary lesions and to improve the differential diagnosis of benign pulmonary lesions by 18F-FDG PET/CT imaging., Methods: One hundred and thirteen patients with benign pulmonary lesions were studied retrospectively. Each patient underwent an 18F-FDG PET/CT scan. All cases were identified by pathology, diagnostic therapy or follow-up. The maximum standardized uptake value (SUVmax) was calculated for each pulmonary lesion., Results: According to the final results, the benign pulmonary lesions were classified as inflammatory lesions (n=77) and granulomas (n=36) by histopathological diagnoses. The SUVmax of inflammatory lesions and granulomas were both high (4.55±2.77 and 6.81±3.96, respectively; P<0.05). When the benign pulmonary lesions were classified by clinical diagnoses, the SUVmax of sarcoidosis was significantly different from other diseases (15.12±5.67; P<0.01)., Conclusions: Inflammatory lesions and granulomas show moderate or high FDG uptake on 18F-FDG PET/CT, but granulomas have higher values. 18F-FDG PET/CT appeared to have a higher SUVmax for the differential diagnosis of sarcoidosis and benign pulmonary lesions., Competing Interests: Conflicts of Interest: The authors have no conflicts of interest to declare.

Quantitative analysis of glucose consumption measured by maximum standardized uptake value (SUVmax) in lung adenocarcinoma (LA) remains in discussion and metabolic information provided by FDG-PET is not included in cancer staging. The first aim of this work was to evaluate the correlation between SUVmax and different histologic subtypes of LA. The second aim was to establish the correlation between SUVmax and TNM, genetic mutations and prognostic. Glucose consumption of primary tumor was quantified using SUVmax in 112 patients with histologically-confirmed LA. Specimens were classified according to the IASLC/ATS/ERS into in situ -AIS-, minimally invasive -MIA-, invasive (lepidic, papillary, acinar, solid and micropapillary) and invasive mucinous adenocarcinoma. Tumors were grouped according to three histological grades; low-grade: AIS, MIA, intermediate-grade: lepidic, acinar, papillary and high-grade: micropapillary, solid and mucinous. Comparisons between SUVmax and histologic subtypes were performed with Kruskal-Wallis followed by a Dunn's test. Overall (OS) and disease-free survival (DFS) were calculated. SUVmax was histologically-dependent (P<0.001): AIS 0.5±0.1, MIA 1.1±0.9 lepidic 3.3±3.1, acinar 8.6±6.7, papillary 3.9±5.1, micropapillary 4.9±3.4, solid 10.4±5.4 and invasive mucinous 2.7±1.2. SUVmax was associated with TNM stage in stage IA and IB. SUVmax was significantly lower in patients with K-RAS and EGFR mutation. Low SUVmax was associated with low-grade histology and with a higher OS and DSF compared to high SUVmax (intermediate and high-grade histology). SUVmax on FDG-PET is a powerful information in the presurgical evaluation of LA patients.
