3. Exploring Global Climate Model Downscaling Based on Tile-Level Output

Abstract: Statistical and dynamical modeling techniques are used to downscale global climate model (GCM) outputs to practical resolutions for local- or regional-scale applications. Current techniques do not incorporate the effects of land-use and land-cover changes, although research has shown that such changes can substantially affect climate locally. Here, we explore a new downscaling technique that uses tile-level GCM outputs provided under phase 6 of the Coupled Model Intercomparison Project (CMIP6). The method, land-cover tile downscaling (LTD), spatially locates the tile-level GCM outputs by mapping them to corresponding classes in high-resolution land-cover maps. Furthermore, it applies an elevation-based correction to account for the effect of topography on the local climate. LTD is applied to near-surface temperature outputs from the Community Earth System Model, version 2 (CESM2) and U.K. Earth System Model, version 1 (UKESM1), and surface temperature output from CESM2 and evaluated against observations. In comparison with grid-averaged control data, LTD outputs show an overall bias reduction that is not spatially consistent. Moreover, LTD performs better on air temperature data than on surface temperature and better on areas dominated by primary/secondary land and crops than on urban land. This could arise from simplifications in methods, like land-cover reclassification and simplified lapse rate estimates. However, the difference in response between the two variables and land-cover types implies that biases also stem from model structural features involved in estimating their tile-level outputs. This is supported by the differences between grid average data provided by the models and the same data reconstructed from tile-level outputs. Therefore, a thorough evaluation and quality control of tile-level outputs is recommended.

FRR was supported by The Commission of the European Communities through contract No. SC1000488 with The Niels Bohr Institute., A variation on the abelian Higgs model, with SU(2)global x U(1)local symmetry broken to U(1)global, was recently shown by Vachaspati and Achucarro to admit stable, finite-energy cosmic string solutions, even though the manifold of minima of the potential energy does not have non-contractible loops. This new and unexpected feature motivates a full investigation of the properties of the model. Here we exploit the existence of first-order Bogomol'nyi equations to classify all static finite-energy vortex solutions in the Bogomol'nyi limit. We find a 4n-dimensional moduli space for the nth topological (n-vortex) sector. Single-vortex configurations depend on a position coordinate and on an additional complex parameter and may be regarded as hybrids of Nielsen-Olesen vortices and CP1 lumps. The model is also shown to obey Bogomol'nyi equations in curved space, and these allow a simple calculation of the gravitational field of the above configurations. Finally, monopole-like solutions interpolating between a Dirac monopole and a global monopole are found. Abstract: We present the first spatially resolved measurements of galaxy properties in the JWST ERO SMACS 0723 field. We perform a comprehensive analysis of five 5 < z < 9 galaxies with spectroscopic redshifts from NIRSpec observations. We perform spatially resolved spectral energy distribution fitting with Bagpipes, using six NIRCam imaging bands spanning the wavelength range 0.8-5 mu m. This approach allows us to study the internal structure and assembly of the first generations of galaxies. We find clear gradients both in the empirical color maps and in most of the estimated physical parameters. We find regions of considerably different specific star formation rates across each galaxy, which points to very bursty star formation happening on small scales, not galaxy-wide. The integrated light is dominated by these bursty regions, which exhibit strong line emission, with the equivalent width of [O iii]+H beta reaching up to similar to 3000-4000 angstrom rest frame. Abstract: Surveys with the James Webb Space Telescope (JWST) have discovered candidate galaxies in the first 400 Myr of cosmic time. Preliminary indications have suggested these candidate galaxies may be more massive and abundant than previously thought. However, without confirmed distances, their inferred properties remain uncertain. Here we identify four galaxies located in the JWST Advanced Deep Extragalactic Survey Near-Infrared Camera imaging with photometric redshifts z of roughly 10-13. These galaxies include the first redshift z > 12 systems discovered with distances spectroscopically confirmed by JWST in a companion paper. Using stellar population modelling, we find the galaxies typically contain 100 million solar masses in stars, in stellar populations that are less than 100 million years old. The moderate star-formation rates and compact sizes suggest elevated star-formation rate surface densities, a key indicator of their formation pathways. Taken together, these measurements show that the first galaxies contributing to cosmic reionization formed rapidly and with intense internal radiation fields.The JWST Advanced Deep Extragalactic Survey using NIRCam to find the earliest galaxies reveals the size and star formation rates of four extreme redshift (z > 10) galaxies of the distant Universe.

9. Temporary mechanical circulatory support for COVID-19 patients

Abstract: OBJECTIVE: Myocardial damage occurs in up to 25% of coronavirus disease 2019 (COVID-19) cases. While veno-venous extracorporeal life support (V-V ECLS) is used as respiratory support, mechanical circulatory support (MCS) may be required for severe cardiac dysfunction. This systematic review summarizes the available literature regarding MCS use rates, disease drivers for MCS initiation, and MCS outcomes in COVID-19 patients.METHODS: PubMed/EMBASE were searched until October 14, 2021. Articles including adults receiving ECLS for COVID-19 were included. The primary outcome was the rate of MCS use. Secondary outcomes included mortality at follow-up, ECLS conversion rate, intubation-to-cannulation time, time on ECLS, cardiac diseases, use of inotropes, and vasopressors.RESULTS: Twenty-eight observational studies (comprising both ECLS-only populations and ECLS patients as part of larger populations) included 4218 COVID-19 patients (females: 28.8%; median age: 54.3 years, 95%CI: 50.7-57.8) of whom 2774 (65.8%) required ECLS with the majority (92.7%) on V-V ECLS, 4.7% on veno-arterial ECLS and/or Impella, and 2.6% on other ECLS. Acute heart failure, cardiogenic shock, and cardiac arrest were reported in 7.8%, 9.7%, and 6.6% of patients, respectively. Vasopressors were used in 37.2%. Overall, 3.1% of patients required an ECLS change from V-V ECLS to MCS for heart failure, myocarditis, or myocardial infarction. The median ECLS duration was 15.9 days (95%CI: 13.9-16.3), with an overall survival of 54.6% and 28.1% in V-V ECLS and MCS patients. One study reported 61.1% survival with oxy-right ventricular assist device.CONCLUSION: MCS use for cardiocirculatory compromise has been reported in 7.3% of COVID-19 patients requiring ECLS, which is a lower percentage compared to the incidence of any severe cardiocirculatory complication. Based on the poor survival rates, further investigations are warranted to establish the most appropriated indications and timing for MCS in COVID-19.

12. Validated tests for language research with university students whose native language is English : tests of vocabulary, general knowledge, author recognition, and reading comprehension

Abstract: We present five studies aimed at developing an L1 vocabulary test for English-speaking university students. Such a test is useful as an indicator of crystallized intelligence and because vocabulary size correlates well with reading comprehension. In the first study, we tested 100 written words with four answer alternatives, based on Nation's Vocabulary Size Test. Analysis suggested two factors, which we interpreted as the possible existence of two types of difficult words: unknown words for general knowledge and unknown words for specialized knowledge. In Study 2, we attempted to develop a vocabulary test for each type of word, and these tests were then validated in Study 3. Since the test for general words proved too easy for the target population, we improved it in a fourth study by creating and testing more difficult items. Finally, a fifth study was conducted to validate the new test. Unexpectedly, Study 5 found a high correlation (r = .82) between the general knowledge vocabulary test and the specialized knowledge vocabulary test, suggesting that they measure the same latent factor, contrary to our initial assumption. Both tests have high reliability (r > .85) and correlate well (r > .4) with general knowledge, author recognition, and reading comprehension. In addition, a collection of other language tests was used and improved to verify the validity of the vocabulary tests. An exploratory factor analysis of all tests identified three factors (text comprehension, crystallized intelligence, and reading speed), with the vocabulary tests loading on the factor crystallized intelligence, which in turn correlates with reading comprehension. Structural equation modeling confirmed the interpretation.

Abstract: Introduction Hormonal contraception is a well-known risk factor for venous thromboembolism (VTE). APC resistance and impaired functions of protein S and TFPI are thought to play an important role in the pathogenesis of hormone-related VTE. It is unknown, whether women, who develop VTE during hormonal contraception possess a vulnerability in these pathways, making them susceptible to thrombosis. Materials and methods Plasma samples were obtained from 57 premenopausal women in average 15.3 years after hormone-associated VTE and from 31 healthy controls. Thrombin generation at high tissue factor (TF) in the absence and in the presence of activated protein C (APC) and at low TF without and with inhibiting anti-protein S- and anti-TFPI-antibodies was measured via calibrated automated thrombography. Results Women with previous hormone-related thrombosis had higher thrombin generation at low TF, higher APC resistance, protein S- and TFPI ratios, differences: 219.9 nM IIa.min (95%CI:90.4 to 349.3); 1.88 (95%CI:0.71 to 3.05); 0.13 (95%CI:0.01 to 0.26) and 0.19 (95%CI:0.08 to 0.30), respectively. Thrombin generation at high TF without APC did not differ between the groups. Smoking decreased thrombin generation at low TF by −222.6 nM IIa.min (95%CI: −381.1 to −64.1), the APC sensitivity ratio by −2.20 (95%CI: −3.63 to −0.77) and the TFPI ratio by −0.16 (95%CI: −0.29 to −0.03), but did not influence thrombin generation at high TF. Discussion We demonstrated impairment of the protein S/TFPI system and increased APC resistance in women with previous hormone-induced VTE. Smoking decreased thrombin generation at assay conditions, dependent on the function of the TFPI system.

Abstract: The modelling of unequal mass binary black hole systems is of high importanceto detect and estimate parameters from these systems. Numerical relativity (NR)is well suited to study systems with comparable component masses, $m_1\simm_2$, whereas small mass ratio (SMR) perturbation theory applies to binarieswhere $q=m_2/m_1<< 1$. This work investigates the applicability for NR and SMRas a function of mass ratio for eccentric non-spinning binary black holes. Abstract: ObjectivesThe aim of this study was to compare percutaneous catheter ablation vs. minimally invasive surgical ablation, evaluating the impact of repeated ablation on atrial function, and evaluating predictors of atrial fibrillation (AF) recurrence.BackgroundWhen AF ablation fails, re-ablations are required in up to 40% of patients to treat recurrent arrhythmia; surgical ablation is more effective than catheter ablation.MethodsThirty-two patients with failed prior catheter ablation and referred for a second ablation (18 catheter and 14 surgical) were included in a descriptive observational study. Left atrial volumes, strain, and strain rate were measured with 2D speckle tracking echocardiography at baseline and 6 months after the procedures to assess left atrial functions. Patients received up to 1 year of clinical and Holter follow-up.ResultsAt the 12-month follow-up, catheter ablation was effective in 56% and surgical ablation in 72% of patients (OR 2 (CI 0.45–8.84), p 0.36). Left atrial booster function was similar in all patients, but left atrial reservoir function was more impaired in those patients who underwent surgical ablation. Left atrial booster function was predictive of arrhythmia recurrence after both catheter and surgical ablation: late diastolic strain rate (LASRa) cut-off ≤ -0.89 s–1 (sensitivity 88%, specificity 70%, AUC 0.82) and ≤ -0.85 s–1 (sensitivity 60%, specificity 100%, AUC 0.82), respectively.ConclusionSurgical ablation has a more negative impact on LA reservoir function despite being slightly more effective in arrhythmia suppression. LA booster function is not significantly impaired by either procedure. LA booster function predicts arrhythmia elimination after a re-ablation (catheter or surgical).

Abstract: Background: Season of birth is a risk factor of schizophrenia, and it is possible that cumulative exposure to cli-matic factors during childhood affects the risk of schizophrenia. We conducted a cohort study among 365,482 persons born in Finland in 1990-1995 to examine associations of 10-year cumulative exposure to global solar radiation and ambient temperature in childhood with schizophrenia. Methods: Data on schizophrenia diagnoses and sociodemographic factors from the Finnish population register and health care register were linked to daily meteorological data using residential information. The study population was followed from age 10 until the first schizophrenia diagnosis, death, emigration or December 31, 2017, whichever came first. Hazard ratios (HR) for the risk of schizophrenia were estimated using Cox pro-portional hazards model.Results: Compared to the lowest quintile of global solar radiation or ambient temperature, growing up in the second highest quintile (Q4) was associated with greater risk of schizophrenia. These hazard ratios were attenuated after adjustment for parental mental disorder, parental education, parental income, area-level so-cioeconomic characteristics and urbanicity (HR = 1.29, 95 % CI 1.06-1.58 for radiation; HR = 1.24, 95 % CI, 1.02-1.52 for temperature). Continuous linear terms evaluated in secondary models suggested a greater risk of schizophrenia at greater childhood exposure to global radiation and ambient temperature, but these associations did not remain in fully adjusted models.Conclusions: We found no consistent evidence that cumulative exposure to sunlight and ambient temperature in childhood is associated with the risk of developing schizophrenia. Studies in other populations residing in different latitudes are needed.

Abstract: Caesarean section is associated with an increased risk of abnormal placental implantation and adverse pregnancy outcomes in subsequent pregnancies. Besides the placenta accrete spectrum, only a few of the previous studies focused on other placental development alterations in the scarred uterus. We assessed placental development deviations in the uterus with a Caesarean section scar by evaluating placental volume (PV) and vascular flow indexes. From 1 January 2021 until 31 March 2022, placental volumes and vascularization indexes (VI, FI, VFI) were prospectively measured by 3D power Doppler and VOCAL techniques in 221 patients attending the first trimester screening program. We also calculated the placental quotient to standardize PV to the gestational age. No statistically significant differences in the values of placental volume, placental quotient and placental vascularization indexes were detected between women with previous Caesarean section delivery or women with vaginal delivery. FI was significantly lower in nulliparous in the first trimester. The results of our study suggest that 3D placental evaluation was not able to detect placental development alteration in the uterus with a Caesarean section scar. The use of contrast agents including O-18, Zn-68 or Cu-63, isotopes with a high cross section for low-energy protons in nuclear reactions producing PET emitters, has been proposed to enhance the PET signal in the last millimeters of the proton path. However, it remains a challenge to achieve sufficient concentrations of these isotopes in the target volume. Here we investigate the possibilities of O-18-enriched water (18-W), a potential contrast agent that could be incorporated in large proportions in live tissues by replacing regular water. We hypothesize that 18-W could also mitigate the problem of biological washout, as PET (F-18) isotopes created inside live cells would remain trapped in the form of fluoride anions (F-), allowing its signal to be detected even hours after irradiation. To test our hypothesis, we designed an experiment with two main goals: first, prove that 18-W can incorporate enough O-18 into a living organism to produce a detectable signal from F-18 after proton irradiation, and second, determine the amount of activity that remains trapped inside the cells. The experiment was performed on a chicken embryo chorioallantoic membrane tumor model of head and neck cancer. Seven eggs with visible tumors were infused with 18-W and irradiated with 8-MeV protons (range in water: 0.74 mm), equivalent to clinical protons at the end of particle range. The activity produced after irradiation was detected and quantified in a small-animal PET-CT scanner, and further studied by placing ex-vivo tumours in a gamma radiation detector. Abstract: The modeling of unequal mass binary black hole systems is of high importance to detect and estimate parameters from these systems. Numerical relativity (NR) is well suited to study systems with comparable component masses, m1 similar to m2, whereas small mass ratio (SMR) perturbation theory applies to binaries where q = m2/m1 MUCH LESS-THAN 1. This work investigates the applicability for NR and SMR as a function of mass ratio for eccentric nonspinning binary black holes. We produce 52 NR simulations with mass ratios between 1:10 and 1:1 and initial eccentricities up to 0.8. From these we extract quantities like gravitational wave energy and angular momentum fluxes and periastron advance, and assess their accuracy. To facilitate comparison, we develop tools to map between NR and SMR inspiral evolutions of eccentric binary black holes. We derive post-Newtonian accurate relations between different definitions of eccentricity. Abstract: This study examines the distinction between knowing the meaning of a word and experiencing the feelings associated with it. We collected affective ratings for a set of emotional and neutral English words from a group of English native speakers and a group of European Portuguese-English bilinguals. Half of the emotional words named emotions (emotion words) and the other half did not name emotions but could provoke them (emotion-laden words). Some participants were asked to focus on the meaning of words while others were asked to focus on the feeling produced by the words. Native speakers of English produced more intense affective ratings that Portuguese-English bilinguals. Such difference was larger when participants focused on their feelings than when they focused on the words' meaning. Accordingly, such distinction should be considered in the study of bilingual affective language processing. Finally, the type of emotional word (emotion vs. emotion-laden) had only modest effects.

Abstract: Background To explore long-term cardiovascular prognosis after myocardial infarction (MI) among patients with type 1 diabetes. Methods Patients with type 1 diabetes surviving 90 days after MI (n = 1508; 60% male, mean age = 62.1 years) or without any type of diabetes (n = 62,785) in Finland during 2005–2018 were retrospectively studied using multiple national registries. The primary outcome of interest was a combined major adverse cardiovascular event (MACE; cardiovascular death, recurrent MI, ischemic stroke, or heart failure hospitalization) studied with a competing risk Fine-Gray analyses. Median follow-up was 3.9 years (maximum 12 years). Differences between groups were balanced by multivariable adjustments and propensity score matching (n = 1401 patient pairs). Results Cumulative incidence of MACE after MI was higher in patients with type 1 diabetes (67.6%) compared to propensity score-matched patients without diabetes (46.0%) (sub-distribution hazard ratio [sHR]: 1.94; 95% confidence interval [CI]: 1.74–2.17; p Conclusions Patients with type 1 diabetes have notably poorer long-term cardiovascular prognosis after an MI compared to patients without diabetes. These results underline the importance of effective secondary prevention after MI in patients with type 1 diabetes.

Abstract: ObjectivesTo evaluate the risk of subsequent fractures in patients who attended the Fracture Liaison Service (FLS), with and without incident falls after the index fracture.DesignA 3-year prospective observational cohort study.SettingAn outpatient FLS in the Netherlands.ParticipantsPatients aged 50+ years with a recent clinical fracture.Outcome measuresIncident falls and subsequent fractures.ResultsThe study included 488 patients (71.9% women, mean age: 64.6±8.6 years). During the 3-year follow-up, 959 falls had been ascertained in 296 patients (60.7%) (ie, fallers), and 60 subsequent fractures were ascertained in 53 patients (10.9%). Of the fractures, 47 (78.3%) were fall related, of which 25 (53.2%) were sustained at the first fall incident at a median of 34 weeks. An incident fall was associated with an approximately 9-fold (HR: 8.6, 95% CI 3.1 to 23.8) increase in the risk of subsequent fractures.ConclusionThese data suggest that subsequent fractures among patients on treatment prescribed in an FLS setting are common, and that an incident fall is a strong predictor of subsequent fracture risk. Immediate attention for fall risk could be beneficial in an FLS model of care.Trial registration numberNL45707.072.13.

Abstract: Purpose The second-generation multi-electrode catheter, pulmonary vein ablation catheter (PVAC) GOLD, was designed to improve the delivery of phased radiofrequency energy and reduce procedure times using a ‘single-shot’ approach for pulmonary vein isolation (PVI), while retaining efficacy and safety. This large registry presents acute success rates and safety outcomes in a daily practice setting. Methods A total of 1017 patients undergoing first-time ablation for atrial fibrillation (AF) using PVAC GOLD were included, 644 patients with paroxysmal AF and 373 patients with non-paroxysmal AF, divided into 175 patients receiving PVI only and 198 patients receiving PVI with additional substrate modification. Results High and comparable percentages of successful PVI could be achieved in all groups (98%, 95% and 99%; p = 0.108). The median total procedure time for all groups was 90 min [70–100]. As expected, the total procedure, ablation and fluoroscopy time were significantly longer in the PVI + substrate modification group compared with the PVI-only cases (all p p = 0.306, p = 0.088, p = 0.233, respectively). A total of 44 complications were observed in 43 patients (4.2%). Major complications were seen in 19 patients (1.87%) and non-major procedure–related complications were seen in 25 patients (2.46%). Complications leaving permanent sequelae were rare and occurred in only four patients (0.39%). Complications did not differ between groups (p = 0.199, p = 0.438, p = 0.240 and p = 0.465 respectively). Conclusion PVAC GOLD performs successful PVI, while reducing procedure times and retaining safety for paroxysmal, persistent and long-standing persistent AF. Safety was unaffected by additional substrate modification.

Abstract: Rationale: Resection of soft tissue sarcomas (STS) is accompanied by a high rate of tumor-positive surgical margins (14-34%), which potentially leads to decreased disease-free survival. Vascular Endothelial Growth Factor-A (VEGF-A) is overexpressed in malignant tumors, including STS, and can be targeted with bevacizumab-800CW during fluorescence-guided surgery for real-time tumor detection. In this phase 1 clinical trial, we determined the feasibility, safety and optimal dose of bevacizumab-800CW for fluorescence-guided surgery (FGS) in STS for in- and ex vivo tumor detection. Methods: Patients with a histopathologically diagnosis of STS were included. In the dose-escalation phase, patients received bevacizumab-800CW intravenously 3 days prior to surgery (10, 25 and 50 mg, n = 8). In the subsequent dose-expansion phase, 7 additional patients received bevacizumab-800CW at the optimal dose. Fluorescence images in- and ex vivo were obtained during all stages of standard of care. The optimal dose was determined by calculating in- and ex vivo Tumor-to-Background ratios (TBR) and correlating these results with histopathology. Results: Fifteen patients with STS completed this study. All tumors could be visualized during in- and ex vivo imaging. The optimal bevacizumab-800CW dose proved to be 10 mg with a median in vivo TBR of 2.0 (±0.58) and an ex vivo TBR of 2.67 (±1.6). All seven tumor-positive margins could be observed real-time after surgical resection. Conclusion: FGS using 10 mg bevacizumab-800CW is feasible and safe for intra-operative imaging of STS, potentially allowing tumor detection and margin assessment during surgery. An additional follow-up phase II study is needed to confirm the diagnostic accuracy.

Abstract: This paper presents the gravitational-wave measurement of the Hubble constant (H 0) using the detections from the first and second observing runs of the Advanced LIGO and Virgo detector network. The presence of the transient electromagnetic counterpart of the binary neutron star GW170817 led to the first standard-siren measurement of H 0. Here we additionally use binary black hole detections in conjunction with galaxy catalogs and report a joint measurement. Our updated measurement is H 0 = km s-1 Mpc-1 (68.3% of the highest density posterior interval with a flat-in-log prior) which is an improvement by a factor of 1.04 (about 4%) over the GW170817-only value of km s-1 Mpc-1. A significant additional contribution currently comes from GW170814, a loud and well-localized detection from a part of the sky thoroughly covered by the Dark Energy Survey. With numerous detections anticipated over the upcoming years, an exhaustive understanding of other systematic effects are also going to become increasingly important. Abstract: Cattle traction was a technological innovation that made a significant impact on production, individual and household wealth, and social organisation. Despite ongoing debates regarding the origins and extent of the harnessing of cattle power among early agropastoral societies, only a few studies have attempted at addressing this matter systematically. In Neolithic catalho center dot yuk, several studies have explored the symbolism and domestication of aurochs and cattle, while the systematic investigation of Bos skeletal remains regarding the presence of cattle traction has been missing. This study focuses on Neolithic catalho center dot yuk in Central Anatolia, renowned for its cattle symbolism, to explore the possibility of cattle traction in the 7th and 6th millennium BCE. We studied the palaeopathological traces on the lower limbs of Bos from catalho center dot yuk East (Early, Middle, and Late) and West Mounds. Our results suggest that arthropathies are present on the Bos lower limbs (particularly anterior elements) in all phases of Neolithic catalho center dot yuk. Pathological and sub-pathological changes are on average more severe among the small cattle of catalho center dot yuk West than in the preceding periods at catalho center dot yuk East, a result affected by a few rather deformed specimens in catalho center dot yuk West. We did not observe any clear correlation between cattle survivorship, size, and pathology severity. Although an unequivocal association between pathologies and traction in prehistoric cattle remains challenging, we discuss plausible explanations for the changing nature and intensity of cattle pathologies at catalho center dot yuk throughout time. Abstract: Background: Current evidence suggests that the composition of infant formula (IF) affects the gut microbiome, intestinal function, and immune responses during infancy. However, the impact of IF on circulating lipid profiles in infants is still poorly understood. The objectives of this study were to (1) investigate how extensively hydrolyzed IF impacts serum lipidome compared to conventional formula and (2) to associate changes in circulatory lipids with gastrointestinal biomarkers including intestinal permeability. Methods: In a randomized, double-blind controlled nutritional intervention study (n = 73), we applied mass spectrometry-based lipidomics to analyze serum lipids in infants who were fed extensively hydrolyzed formula (HF) or conventional, regular formula (RF). Serum samples were collected at 3, 9, and 12 months of age. Child’s growth (weight and length) and intestinal functional markers, including lactulose mannitol (LM) ratio, fecal calprotectin, and fecal beta-defensin, were also measured at given time points. At 3 months of age, stool samples were analyzed by shotgun metagenomics. Results: Concentrations of sphingomyelins were higher in the HF group as compared to the RF group. Triacylglycerols (TGs) containing saturated and monounsaturated fatty acyl chains were found in higher levels in the HF group at 3 months, but downregulated at 9 and 12 months of age. LM ratio was lower in the HF group at 9 months of age. In the RF group, the LM ratio was positively associated with ether-linked lipids. Such an association was, however, not observed in the HF group. Conclusion: Our study suggests that HF intervention changes the circulating lipidome, including those lipids previously found to be associated with progression to islet autoimmunity or overt T1D. Abstract: Objectives: We aimed to compare COVID-19 outcomes in the Omicron-driven fourth wave with prior waves in the Western Cape, the contribution of undiagnosed prior infection to differences in outcomes in a context of high seroprevalence due to prior infection, and whether protection against severe disease conferred by prior infection and/or vaccination was maintained. Methods: In this cohort study, we included public sector patients aged ≥20 years with a laboratory confirmed COVID-19 diagnosis between 14 November-11 December 2021 (wave four) and equivalent prior wave periods. We compared the risk between waves of the following outcomes using Cox regression: death, severe hospitalization or death and any hospitalization or death (all ≤14 days after diagnosis) adjusted for age, sex, comorbidities, geography, vaccination and prior infection. Results: We included 5,144 patients from wave four and 11,609 from prior waves. Risk of all outcomes was lower in wave four compared to the Delta-driven wave three (adjusted Hazard Ratio (aHR) [95% confidence interval (CI)] for death 0.27 [0.19; 0.38]. Risk reduction was lower when adjusting for vaccination and prior diagnosed infection (aHR:0.41, 95% CI: 0.29; 0.59) and reduced further when accounting for unascertained prior infections (aHR: 0.72). Vaccine protection was maintained in wave four (aHR for outcome of death: 0.24; 95% CI: 0.10; 0.58). Conclusions: In the Omicron-driven wave, severe COVID-19 outcomes were reduced mostly due to protection conferred by prior infection and/or vaccination, but intrinsically reduced virulence may account for an approximately 25% reduced risk of severe hospitalization or death compared to Delta.

Abstract: We present 850 $��$m polarimetric observations toward the Serpens Main molecular cloud obtained using the POL-2 polarimeter on the James Clerk Maxwell Telescope (JCMT) as part of the B-fields In STar-forming Region Observations (BISTRO) survey. These observations probe the magnetic field morphology of the Serpens Main molecular cloud on about 6000 au scales, which consists of cores and six filaments with different physical properties such as density and star formation activity. Using the histogram of relative orientation (HRO) technique, we find that magnetic fields are parallel to filaments in less dense filamentary structures where $N_{H_2} < 0.93\times 10^{22}$ cm$^{-2}$ (magnetic fields perpendicular to density gradients), while being perpendicular to filaments (magnetic fields parallel to density gradients) in dense filamentary structures with star formation activity. Moreover, applying the HRO technique to denser core regions, we find that magnetic field orientations change to become perpendicular to density gradients again at $N_{H_2} \approx 4.6 \times 10^{22}$ cm$^{-2}$. This can be interpreted as a signature of core formation. At $N_{H_2} \approx 16 \times 10^{22}$ cm$^{-2}$ magnetic fields change back to being parallel to density gradients once again, which can be understood to be due to magnetic fields being dragged in by infalling material. In addition, we estimate the magnetic field strengths of the filaments ($B_{POS} = 60-300~��$G)) using the Davis-Chandrasekhar-Fermi method and discuss whether the filaments are gravitationally unstable based on magnetic field and turbulence energy densities., 18 pages, accepted for publication in ApJ

Abstract: Tao-Chung Ching et al., We present 850 μm dust polarization observations of the massive DR21 filament from the B-fields In STar-forming Region Observations (BISTRO) survey, using the POL-2 polarimeter and the SCUBA-2 camera on the James Clerk Maxwell Telescope. We detect ordered magnetic fields perpendicular to the parsec-scale ridge of the DR21 main filament. In the subfilaments, the magnetic fields are mainly parallel to the filamentary structures and smoothly connect to the magnetic fields of the main filament. We compare the POL-2 and Planck dust polarization observations to study the magnetic field structures of the DR21 filament on 0.1–10 pc scales. The magnetic fields revealed in the Planck data are well-aligned with those of the POL-2 data, indicating a smooth variation of magnetic fields from large to small scales. The plane-of-sky magnetic field strengths derived from angular dispersion functions of dust polarization are 0.6–1.0 mG in the DR21 filament and ∼0.1 mG in the surrounding ambient gas. The mass-to-flux ratios are found to be magnetically supercritical in the filament and slightly subcritical to nearly critical in the ambient gas. The alignment between column density structures and magnetic fields changes from random alignment in the low-density ambient gas probed by Planck to mostly perpendicular in the high-density main filament probed by James Clerk Maxwell Telescope. Abstract: Background: Serological screening of the relatives of coeliac disease patients is widely endorsed. However, the need for and the optimal timing of possible re-testing of once seronegative at-risk individuals for coeliac disease remain unclear. Objective: We investigated this issue by inviting a large cohort of previously screening-negative relatives of patients with coeliac disease to participate in a follow-up study. Methods: Altogether 599 relatives of coeliac disease index patients not diagnosed with coeliac disease in a screening study carried out in 2006–2010 were asked about possible later diagnosis or re-tested with coeliac disease autoantibodies in 2017–2021. Besides incidence, the possible impact of various patient-related clinical factors and HLA haplotype on the later diagnosis or screening positivity was examined. Results: Fifteen (2.5%) relatives were either diagnosed with a coeliac disease (n = 8) during the follow-up period or were found to be screening-positive in the re-testing (n = 7), giving a combined annual incidence of 221/100,000 person-years in all relatives and 336/100,000 among those carrying coeliac disease-associated HLA DQ2/DQ8. Abstract: Hwang et al., We present and analyze observations of polarized dust emission at 850 μm toward the central 1 × 1 pc hub-filament structure of Monoceros R2 (Mon R2). The data are obtained with SCUBA-2/POL-2 on the James Clerk Maxwell Telescope (JCMT) as part of the B-fields in Star-forming Region Observations survey. The orientations of the magnetic field follow the spiral structure of Mon R2, which are well described by an axisymmetric magnetic field model. We estimate the turbulent component of the magnetic field using the angle difference between our observations and the best-fit model of the underlying large-scale mean magnetic field. This estimate is used to calculate the magnetic field strength using the Davis–Chandrasekhar–Fermi method, for which we also obtain the distribution of volume density and velocity dispersion using a column density map derived from Herschel data and the C18O (J = 3 − 2) data taken with HARP on the JCMT, respectively. We make maps of magnetic field strengths and mass-to-flux ratios, finding that magnetic field strengths vary from 0.02 to 3.64 mG with a mean value of 1.0 ± 0.06 mG, and the mean critical mass-to-flux ratio is 0.47 ± 0.02. Additionally, the mean Alfvén Mach number is 0.35 ± 0.01. This suggests that, in Mon R2, the magnetic fields provide resistance against large-scale gravitational collapse, and the magnetic pressure exceeds the turbulent pressure. We also investigate the properties of each filament in Mon R2. Abstract: Background HIV patients face considerable acute and chronic healthcare needs and battling the HIV epidemic remains of the utmost importance. By focusing on health outcomes in relation to the cost of care, value-based healthcare (VBHC) proposes a strategy to optimize quality of care and cost-efficiency. Its implementation may provide an answer to the increasing pressure to optimize spending in healthcare while improving patient outcomes. This paper describes a pragmatic value-based healthcare framework for HIV care. Methods A value-based HIV healthcare framework was developed during a series of roundtable discussions bringing together 16 clinical stakeholder representatives from the Belgian HIV reference centers and 2 VBHC specialists. Each round of discussions was focused on a central question translating a concept or idea to the next level of practical implementation: 1) how can VBHC principles be translated into value-based HIV care drivers; 2) how can these value-based HIV care divers be translated into value-based care objectives and activities; and 3) how can value-based HIV care objectives and activities be translated into value-based care indicators. Value drivers were linked to concrete objectives and activities using a logical framework approach. Finally, specific, measurable, and acceptable structure, process and outcomes indicators were defined to complement the framework. Results Our framework identifies 4 core value areas where HIV care would benefit most from improvements: Prevention, improvement of the cascade of care, providing patient-centered HIV care and sustaining a state-of-the-art HIV disease management context. These 4 core value areas were translated into 12 actionable core value objectives. For each objective, example activities were proposed. Indicators are suggested for each level of the framework (outcome indicators for value areas and objectives, process indicators for suggested activities). Conclusions This framework approach outlines how to define a patient- and public health centered value-based HIV care paradigm. It proposes how to translate core value drivers to practical objectives and activities and suggests defining indicators that can be used to track and improve the framework’s implementation in practice.

Abstract: The purpose of this study is to compare whether or not there is a difference between venous and capillary blood samples in blood glucose measurements and investigate the effects of different aseptic methods used in skin cleaning before collecting blood samples on measurement results. This quasi-experimental study was conducted with 109 patients. The capillary first and second blood drop values taken from the patients after fasting and at 2 hours following 75 g oral glucose tolerance test (OGTT) and capillary and venous blood glucose values were compared. There was no significant difference between the median venous blood glucose value and the capillary second blood drop value taken after wiping the finger with alcohol. There was no significant difference between the first and second blood drop values of capillary blood glucose 2 hours after OGTT.

Abstract: Aims and Objectives : To determine sputum conversion rate at monthly intervals of 1st, 2nd and 3rd month in new smear positive cases (cat-1) under treatment under RNTCP. Material and Methods : The study was conducted at DOTS Center, Gauhati Medical College and Hospital; Guwahati between July 2005 to June 2006.The study is a prospec-tive study and consists of 100 cases of new smear positive pulmonary tuberculosis cases (category 1) irrespective of age and sex. Results and Observations : The age & sex distribution of 100 patients showed that majority of the patients (74%) belonged to 2nd, 3rd and 4th decades & 75% were males and 25% were female with male to female ratio 3:1 respectively. The chest x-ray of 100 Smear Positive patients shows that only 60 (60%) patients had x-ray evidence of pulmonary Koch. In the present study, sputum conversion i.e. from smear positive to smear negative at the end of the 1st month is 71%, at the end of 2nd month is 84% and at the end of 3rd month is 92%. Summary & Conclusion : In conclusion, the overall sputum conversion rate under Directly Observed Treatment, Short Course (DOTS) chemotherapy in 100 sputum smear positive Pulmonary Tuberculosis in DOTS centre, Gauhati Medical College & Hospital was 92%.The chest x-ray evidence of pulmonary Koch in 100 patients is 60%.The sputum conversion at the end of 1st month is 71%, at the end of 2nd month it is 84% and at the end of 3rd month the same is 92%. In the present study, the infectivity decreases from the baseline with significant P value for sputum conversion of 3+, 2+ and 1+ sputum positivity. Directly Observed Treatment is an effective intervention for improving adherence to tuberculosis treatment programme in a resource-poor country. A significant decrease in conversion rate was observed with the initial high grade smear positive cases.

Abstract: 18 Pág. Departamento de Reproducción Animal (INIA), Tick-borne pathogens cause the majority of diseases in the cattle population in Morocco. In this study, ticks were collected from cattle in the Rabat-Sale-Kenitra region of Morocco and identified morphologically, while tick-borne pathogens were detected in cattle blood samples via polymerase chain reaction assay and sequencing. A total of 3394 adult ixodid ticks were collected from cattle and identified as eight different tick species representing two genera, Hyalomma and Rhipicephalus. The collected ticks consisted of Hyalomma marginatum, Hyalomma anatolicum excavatum, Rhipicephalus sanguineus sensu lato, Rhipicephalus bursa, Hyalomma detritum, Hyalomma lusitanicum, Hyalomma dromedarii, and Hyalomma impeltatum. The overall prevalence of tick-borne pathogens in blood samples was 63.8%, with 29.3% positive for Babesia/Theileria spp., 51.2% for Anaplasma/Ehrlichia spp., and none of the samples positive for Rickettsia spp. Sequencing results revealed the presence of Theileria annulata, Babesia bovis, Anaplasma marginale, Theileria buffeli, Theileria orientalis, Babesia occultans, Anaplasma phagocytophilum, Anaplasma capra, Anaplasma platys, Anaplasma bovis, Ehrlichia minasensis, and one isolate of an unknown bovine Anaplasma sp. Abstract: With academic internationalisation at full speed, English is increasingly used as a medium of instruction in higher education. The question arises of whether unbalanced bilinguals remember study materials in a non-native language (L2) as well as in a first language (L1). In previous studies, we found a disadvantage for students recalling short, expository texts in L2 compared with L1, but no such disadvantage for a true/false recognition test, not even on delayed tests after a month. As no additional forgetting occurs, the quality of the memory trace seems to be equally strong in both languages and the recall cost might be caused by a lack of production skill in L2. To test this hypothesis, we ran experiments in L1–L1, L2–L1, and L2–L2 conditions with free and cued recall (short open questions). We replicate the L2 free recall cost reported earlier and show that it is due to the encoding in L2 rather than to an L2 production cost. In contrast, we found no significant difference in a new pair of texts with short, cued recall questions, though there was a trend in the expected direction. A summary of the effect sizes obtained so far shows a considerable variety in magnitudes (with rather big confidence intervals), suggesting that the cost of studying in L2 depends on several factors such as study time, test requirement, and language proficiency level.

This study retraces how the metal supply shifts in response to the transforming power relations and how this change is related to Rome's rise to the virtually uncontested ruler of the region.

Abstract: Background: Nonbullous pemphigoid is an under-recognized phenotype of the autoimmune bullous disease pemphigoid, characterized by the absence of blisters. Several disease aspects have not been studied previously.Objective: To describe the characteristics of nonbullous pemphigoid.Methods: A retrospective review study of medical records. The diagnosis of pemphigoid was based on meeting 2 of the following 3 criteria: (1) pruritus, (2) positive direct immunofluorescence microscopy, or (3) positive indirect immunofluorescence microscopy on salt-split skin.Results: The review included 69 patients. The mean delay in diagnosis was 29 months. Skin examination most often showed pruritic papules/nodules (37%) or pruritus without primary skin lesions (22%). Histopathologic findings were mainly nonspecific. Results of direct and indirect immunofluorescence microscopy were positive in 60% and 69%, respectively. During follow-up, blisters formed in 17%, which was associated with a positive indirect immunofluorescence microscopy (P = .014) and a positive BP180 immunoblot result (P = .032). The Kaplan-Meier estimates of mortality at 1, 2, and 3 years were 14%, 34%, and 46%, respectively, with an 8.6-fold increased all-cause mortality risk.Limitations: The retrospective study design.Conclusions: Nonbullous pemphigoid presented with heterogeneous pruritic skin lesions, resulting in delayed diagnosis. Direct and indirect immunofluorescence microscopy are essential to diagnose nonbullous pemphigoid, in contrast to histopathology, mainly showing nonspecific findings. An increased all-cause mortality risk was observed during follow-up.

46. Resistance to activated protein C and impaired TFPI activity in women with previous hormone-induced venous thromboembolism

Abstract: Introduction: Hormonal contraception is a well-known risk factor for venous thromboembolism (VTE). APC resistance and impaired functions of protein S and TFPI are thought to play an important role in the pathogenesis of hormone-related VTE. It is unknown, whether women, who develop VTE during hormonal contraception possess a vulnerability in these pathways, making them susceptible to thrombosis. Materials and methods: Plasma samples were obtained from 57 premenopausal women in average 15.3 years after hormone-associated VTE and from 31 healthy controls. Thrombin generation at high tissue factor (TF) in the absence and in the presence of activated protein C (APC) and at low TF without and with inhibiting anti-protein S- and anti-TFPI-antibodies was measured via calibrated automated thrombography. Results: Women with previous hormone-related thrombosis had higher thrombin generation at low TF, higher APC resistance, protein S- and TFPI ratios, differences: 219.9 nM IIa.min (95%CI:90.4 to 349.3); 1.88 (95% CI:0.71 to 3.05); 0.13 (95%CI:0.01 to 0.26) and 0.19 (95%CI:0.08 to 0.30), respectively. Thrombin generation at high TF without APC did not differ between the groups. Smoking decreased thrombin generation at low TF by -222.6 nM IIa.min (95%CI: -381.1 to -64.1), the APC sensitivity ratio by -2.20 (95%CI: -3.63 to -0.77) and the TFPI ratio by -0.16 (95%CI: -0.29 to -0.03), but did not influence thrombin generation at high TF. Discussion: We demonstrated impairment of the protein S/TFPI system and increased APC resistance in women with previous hormone-induced VTE. Smoking decreased thrombin generation at assay conditions, dependent on the function of the TFPI system.

48. Real-time Search for Compact Binary Mergers in Advanced LIGO and Virgo's Third Observing Run Using PyCBC Live

Abstract: The third observing run of Advanced LIGO and Advanced Virgo took place between 2019 April and 2020 March and resulted in dozens of gravitational-wave candidates, many of which are now published as confident detections. A crucial requirement of the third observing run was the rapid identification and public reporting of compact binary mergers, which enabled massive follow-up observation campaigns with electromagnetic and neutrino observatories. PyCBC Live is a low-latency search for compact binary mergers based on frequency-domain matched filtering, which was used during the second and third observing runs, together with other low-latency analyses, to generate these rapid alerts from the data acquired by LIGO and Virgo. This paper describes and evaluates the improvements made to PyCBC Live after the second observing run, which defined its operation and performance during the third observing run.

49. Adding eccentricity to quasicircular binary-black-hole waveform models

Abstract: The detection of gravitational-wave signals from coalescing eccentric binary black holes would yield unprecedented information about the formation and evolution of compact binaries in specific scenarios, such as dynamical formation in dense stellar clusters and three-body interactions. The gravitational-wave searches by the ground-based interferometers, LIGO and Virgo, rely on analytical waveform models for binaries on quasi-circular orbits. Eccentric merger waveform models are less developed, and only a few numerical simulations of eccentric mergers are publicly available, but several eccentric inspiral models have been developed from the post-Newtonian expansion. Here we present a novel method to convert the dominant quadrupolar mode of any circular analytical binary-black-hole model into an eccentric model. First, using numerical simulations, we examine the additional amplitude and frequency modulations of eccentric signals that are not present in their circular counterparts. Subsequently, we identify suitable analytical descriptions of those modulations and inter-polate key parameters from twelve numerical simulations designated as our training dataset. This allows us to reconstruct the modulated amplitude and phase of any waveform up to mass ratio 3 and eccentricity 0.2. We find that the minimum overlap of the new model with numerical simulations is around 0.98 over all of our test dataset that are scaled to a 50 M⊙ black-hole binary starting at 35 Hz with aLIGO A+ design sensitivity. A python package pyrex easily carries out the computation of this method.

50. Tests of general relativity with binary black holes from the second LIGO-Virgo gravitational-wave transient catalog

Abstract: Gravitational waves enable tests of general relativity in the highly dynamical and strong-field regime. Using events detected by LIGO-Virgo up to 1 October 2019, we evaluate the consistency of the data with predictions from the theory. We first establish that residuals from the best-fit waveform are consistent with detector noise, and that the low- and high-frequency parts of the signals are in agreement. We then consider parametrized modifications to the waveform by varying post-Newtonian and phenomenological coefficients, improving past constraints by factors of similar to 2; we also find consistency with Kerr black holes when we specifically target signatures of the spin-induced quadrupole moment. Looking for gravitational-wave dispersion, we tighten constraints on Lorentz-violating coefficients by a factor of similar to 2.6 and bound the mass of the graviton to m(g) <= 1.76 x 10(-23) eV/c(2) with 90% credibility. We also analyze the properties of the merger remnants by measuring ringdown frequencies and damping times, constraining fractional deviations away from the Kerr frequency to delta(f) over cap (220) = 0.03(-0.35)(+0.38) for the fundamental quadrupolar mode, and delta(f) over cap (221) = 0.04(-0.32)(+0.27) for the first overtone; additionally, we find no evidence for postmerger echoes. Finally, we determine that our data are consistent with tensorial polarizations through a template-independent method. When possible, we assess the validity of general relativity based on collections of events analyzed jointly. We find no evidence for new physics beyond general relativity, for black hole mimickers, or for any unaccounted systematics.
