1. Effectiveness of Virtual Reality-based Rehabilitation and High-intensity Exercise Program for Total Knee Arthroplasty Patients: A Randomised Controlled Trial

Abstract

Introduction: Progression of articular cartilage loss and wear and strain are the usual causes of Osteoarthritis (OA), sometimes referred to as degenerative joint disease. India had roughly 200,000 knee arthroplasty procedures in 2020 in which nearly 72% were because of OA. Rebuilding the knee joint through knee arthroplasty is a great alternative for treating symptomatic OA in patients who have not responded to conservative treatment. Aim: To determine the effect of Virtual Reality (VR)- based rehabilitation and high-intensity exercise program for Total Knee Arthroplasty (TKA). Materials and Methods: In this double-blinded randomised controlled trial, 36 participants matched the inclusion criteria who underwent Total Knee Replacement (TKR) at the Department of Physiotherapy, Saveetha College of Physiotherapy, SIMATS, Chennai, Tamil Nadu, India. The study was started in the month of October 2023 and ended in January 2024. Then the participants were randomly allotted to an experimental group-VR (n=18) and a conventional group-high-intensity exercises (n=18). Outcome measures used are the Numeric Pain Rating Scale (NPRS) pain scale, knee range of motion, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and Timed Up and Go (TUG) test. A paired t-test was utilised to evaluate significant variations between the pre- and post-test measurements. In order to find any meaningful differences between the two groups, an unpaired t-test was employed. Result: The average mean±Standard Deviation (SD) of age and Body Mass Index (BMI) was found to be 51.2±5.2 yearsand 28.3±2.0 kg/m². The experimental group and the conventional group both exhibited notable changes in terms of within-group differences. Numeric Pain Rating Scale (NPRS) significantly showed the same between the groups, but the range of motion showed better output in VR-based rehabilitation than indifferently supporting the pain outcome of the experimental group (p≥0.0001). Balance, gait and functional activities were improved in the experimental group compared to the conventional group and in the VR-based rehabilitation, the functional independence of the patient was achieved in nine weeks compared to the High-intensity (HI) exercises. Conclusion: The VR-based rehabilitation showed better outcomes in pain, range of motion, balance, gait and functional independence than a high-intensity exercise programme.

Abstract

Impacted tooth is often a common problem in dentistry. Maxillary canines are seen impacted most commonly in patients undergoing orthodontic treatment without reaching the line of occlusion. These impactions are commonly due to physical obstructions and the difference in the direction of movement of the tooth. Canines are the cornerstones for esthetic smile and functional occlusion. Hence, surgical exposure of these impacted canines, guiding them toward the line of occlusion, plays a very important role in the maintenance of the health of periodontium. A series of cases are discussed on various techniques that can be used in guiding the direction or altering the direction of impacted canines to bring them into a proper occlusion without disturbing the health of periodontium.

3. The Causal Effect of Parenting Style on Early Child Development. EdWorkingPaper No. 24-996

Abstract

This paper presents causal evidence on the impact of parenting practices on early child development. We exploit exogenous changes in nurturing care induced by a parent training intervention to estimate the impact of nurturing parenting practices on child outcomes. We find a large and significant impact measured at age two; in contrast, at age four nurturing care has only a modest, and imprecisely estimated, impact on child outcomes. This is despite the fact that the intervention induced substantial changes in parenting practices at both ages. The differential relationship between child development and nurturing care at ages two and four explains the fade-out in treatment effects for the intervention as a whole: although parents continued to respond, their response no longer had the intended effect on child outcomes. 4. Rare Disease Differential Diagnosis with Large Language Models at Scale: From Abdominal Actinomycosis to Wilson's Disease

Abstract

Large language models (LLMs) have demonstrated impressive capabilities in disease diagnosis. However, their effectiveness in identifying rarer diseases, which are inherently more challenging to diagnose, remains an open question. Rare disease performance is critical with the increasing use of LLMs in healthcare settings. This is especially true if a primary care physician needs to make a rarer prognosis from only a patient conversation so that they can take the appropriate next step. To that end, several clinical decision support systems are designed to support providers in rare disease identification. Yet their utility is limited due to their lack of knowledge of common disorders and difficulty of use. In this paper, we propose RareScale to combine the knowledge LLMs with expert systems. We use jointly use an expert system and LLM to simulate rare disease chats. This data is used to train a rare disease candidate predictor model. Candidates from this smaller model are then used as additional inputs to black-box LLM to make the final differential diagnosis. Thus, RareScale allows for a balance between rare and common diagnoses. We present results on over 575 rare diseases, beginning with Abdominal Actinomycosis and ending with Wilson's Disease. Our approach significantly improves the baseline performance of black-box LLMs by over 17% in Top-5 accuracy. We also find that our candidate generation performance is high (e.g. 88.8% on gpt-4o generated chats).

However, malevolent actors can exploit these capabilities to orchestrate sophisticated attacks, posing significant challenges to defenders and traditional security measures. Adversarial attacks, particularly those targeting vulnerabilities in deep learning models, present a nuanced and substantial threat to cybersecurity. Our study delves into adversarial learning threats such as Data Poisoning, Test Time Evasion, and Reverse Engineering, specifically impacting Network Intrusion Detection Systems. Our research explores the intricacies and countermeasures of attacks to deepen understanding of network security challenges amidst adversarial threats. In our study, we present insights into the dynamic realm of adversarial learning and its implications for network intrusion. The intersection of adversarial attacks and defenses within network traffic data, coupled with advances in machine learning and deep learning techniques, represents a relatively underexplored domain. Our research lays the groundwork for strengthening defense mechanisms to address the potential breaches in network security and privacy posed by adversarial attacks. Through our in-depth analysis, we identify domain-specific research gaps, such as the scarcity of real-life attack data and the evaluation of AI-based solutions for network traffic. 6. LLM4DS: Evaluating Large Language Models for Data Science Code Generation

Abstract

The adoption of Large Language Models (LLMs) for code generation in data science offers substantial potential for enhancing tasks such as data manipulation, statistical analysis, and visualization. However, the effectiveness of these models in the data science domain remains underexplored. This paper presents a controlled experiment that empirically assesses the performance of four leading LLM-based AI assistants-Microsoft Copilot (GPT-4 Turbo), ChatGPT (o1-preview), Claude (3.5 Sonnet), and Perplexity Labs (Llama-3.1-70b-instruct)-on a diverse set of data science coding challenges sourced from the Stratacratch platform. Using the Goal-Question-Metric (GQM) approach, we evaluated each model's effectiveness across task types (Analytical, Algorithm, Visualization) and varying difficulty levels. Our findings reveal that all models exceeded a 50% baseline success rate, confirming their capability beyond random chance. Notably, only ChatGPT and Claude achieved success rates significantly above a 60% baseline, though none of the models reached a 70% threshold, indicating limitations in higher standards. ChatGPT demonstrated consistent performance across varying difficulty levels, while Claude's success rate fluctuated with task complexity. Hypothesis testing indicates that task type does not significantly impact success rate overall. For analytical tasks, efficiency analysis shows no significant differences in execution times, though ChatGPT tended to be slower and less predictable despite high success rates. This study provides a structured, empirical evaluation of LLMs in data science, delivering insights that support informed model selection tailored to specific task demands. Abstract

The ocean ecology is badly impacted by large-scale oil spills, plastic waste, and chemical pollution, which destroy ecosystems and endanger marine life. Acknowledging the detrimental effects of oil spills on ecosystems, our research aims to establish the foundation for creative methods to lessen their impact. With an emphasis on the containment and prediction of oil spills, this research investigates the potential of acoustic levitation as a cutting-edge technique for environmental cleanup. Effectively separating and eliminating pollutants without causing additional ecological harm is a major issue for traditional oil spill cleanup techniques. Acoustic levitation provides a non-invasive, accurate, and effective alternative by using sound waves to precisely and subtly separate oil droplets from water in controlled environments. This proposed approach can reduce the negative effects on the environment and increase the efficacy of cleanup efforts. The findings have been examined and assessed by proof of concept experiments with oil droplets, identifying the relationship between the intensity of ultrasonic pressure and the proportion of oil droplets collected.

Abstract

Assurance cases offer a structured way to present arguments and evidence for certification of systems where safety and security are critical. However, creating and evaluating these assurance cases can be complex and challenging, even for systems of moderate complexity. Therefore, there is a growing need to develop new automation methods for these tasks. While most existing assurance case tools focus on automating structural aspects, they lack the ability to fully assess the semantic coherence and correctness of the assurance arguments. In prior work, we introduced the Assurance 2.0 framework that prioritizes the reasoning process, evidence utilization, and explicit delineation of counter-claims (defeaters) and counter-evidence. In this paper, we present our approach to enhancing Assurance 2.0 with semantic rule-based analysis capabilities using common-sense reasoning and answer set programming solvers, specifically s(CASP). By employing these analysis techniques, we examine the unique semantic aspects of assurance cases, such as logical consistency, adequacy, indefeasibility, etc. The application of these analyses provides both system developers and evaluators with increased confidence about the assurance case.

Thomas [4,5] Key Message: Perturbations in neurodevelopment [...], Autism spectrum disorder (ASD) is a childhood-onset complex neurodevelopmental disorder. We carried out a comprehensive genetic study of a quadruplet discordant for ASD to identify the candidate genes of ASD. Whole exome sequencing (WES) was done for the quadruplet and their parents. We identified 218 proband-specific de novo single nucleotide variants (SNVs) and 100 indels, none of which were deleterious. Among these, nine SNVs and six indels are reported in autism databases. A homozygous recessive non-synonymous SNV in TRAM2, and a pair of compound heterozygous non-synonymous SNVs in DGKD, all of which were proband-specific, were predicted to be deleterious. These are novel candidate genes for ASD. Genes harboring proband-specific de novo and inherited variants were enriched in the biological processes related to synaptic transmission and neurodevelopment. Abstract

We consider analogues of Grigorchuk-Gupta-Sidki (GGS-)groups acting on trees of growing degree; the so-called growing GGS-groups. These groups are not just infinite and do not possess the congruence subgroup property, but many of them are branch and have the $p$-congruence subgroup property, for a prime $p$. Among them, we find groups with maximal subgroups only of finite index, and with infinitely many such maximal subgroups. These give the first examples of finitely generated branch groups with infinitely many finite-index maximal subgroups. Additionally, we prove that congruence quotients of growing GGS-groups associated to a defining vector of zero sum give rise to Beauville groups., Comment: 28 pages

41. 'Let's Keep Calm and Breathe'--A Mindfulness Meditation Program in School and Its Effects on Children's Behavior and Emotional Awareness: An Australian Pilot Study

Abstract

This study aimed to understand how mindfulness meditation affects young people by examining its impact on self-regulation, happiness, emotional awareness, and school performance among two groups of school children. A 10-week mindfulness program was conducted by a meditation expert for 552 children aged 4-8 (Group 1) and 287 children aged 9-11 (Group 2). Results for the 4-8 years group (Group 1) showed meditation predicted an increase in happiness (R[superscript 2] = 0.003, p < 0.001) and self-reported school performance (R[superscript 2] = 0.005, p < 0.001) and a decrease in emotional (R[superscript 2] = 0.017, p < 0.001) and behavioral difficulties (R[superscript 2] = 0.009, p < 0.001); however, the effect sizes were small. In the 9-11 years group (Group 2), meditation predicted an increase in emotional awareness (R[superscript 2] = 0.02, p < 0.001), and a decrease in emotional (R[superscript 2] = 0.014, p < 0.001) and behavioral difficulties (R[superscript 2] = 0.009, p < 0.001) as measured by scores from baseline to postintervention. For Group 2, there was no significant change in happiness over the 10 weeks. The findings support incorporating mindfulness meditation in schools, noting significant enhancements in self-regulation with just 5 min of daily practice.

42. Scale of Outcome-Based Education: Beyond the Knowledge-Skill Dichotomy

Abstract

This article fills a notable gap in existing research on outcome-based education (OBE) in India. It reports findings from a multi-sited field-based investigation of OBE across five relatively highly ranked institutions in India. Building on actor-network theory the article argues that attempting to study OBE opens up a range of concerns such as disciplinary dispositions, teacher training, methodological limitations of OBE, concerns of labour, management and the problem of designating the scalar boundaries of OBE. The article argues that OBE allows problematizing the distinction between skill and knowledge and the hierarchy that exists between them. It argues that OBE need not be construed as a degradation of higher education into trade schools, rather this moment provides us an opportunity to rethink the relationship between vocational, technical, and general education. This assumes significance in the current context as the new clientele of higher education do not have the luxury nor often the aspiration for further education but want to use undergraduate education as a take-off to build meaningful careers outside the academia.

Abstract

Introduction: The monsoon and peri-monsoon season often entail, a high patient influx with fever and thrombocytopenia. In severe cases, blood transfusion is necessary, but as there are no guidelines or a precise laboratory cut-off for platelets at which transfusion is requisite, unwarranted blood transfusions adversely affect the patients and overwhelm blood banks. Kshirsagar P et al., developed a risk assessment score system for febrile thrombocytopenia to aid in determining therapeutic intervention. Validation of this scoring system on a larger sample size is imperative, taking into consideration both aetiology and comorbidities. Aim: To validate clinical scoring system developed by Kshirshagar P et al., and to correlate etiology, comorbidities and platelet count with clinical score and outcome in patients with febrile thrombocytopenia. Methods: NF1 patients with confirmed molecular diagnosis from 2014 to 2021 were included in the study. The molecular tests used for the diagnosis were exome sequencing, targeted gene sequencing, and Multiple Ligation Probe Assay. Results: Forty-two clinically diagnosed NF1 patients who had confirmed molecular diagnosis, which is now part of the revised diagnostic criteria, were included in the analysis. Nonsense variants were the most frequently observed (35.71%), followed by frameshift (23.8%), splice site (14.29%), deletion (11.9%), missense (9.5%), and in-frame deletions (4.76%) in our case series. Three variants (c. 5269-1G > C, c. 1541_1542del AG, and c. 6853_6854insA) were identified in more than one patient, suggesting that the variants are widely distributed in the gene and lack any mutational hotspot. This study supports the previous findings that patients with the variant c. 2970_2972delAAT do not develop neurofibromas; however, it was not necessary for those with whole gene deletion to have dysmorphic features as reported by other studies. The study could not establish any correlation between the type of variants and specific clinical features. Around 28% of mutations could be identified by screening exons 14, 28, 37, 46 and intron 37 in this population. Conclusion: This study will contribute to a better understanding of the phenotypic variability of neurofibromatosis patients. The variable expressivity seen in NF1 suggests that modifying genes may be involved in the development of particular clinical features in addition to NF1 mutations.
