Background: Congenital heart defects (CHDs) are one of the most common associated anomalies in patients with an orofacial cleft (OFC). However, few studies have shown the association between cleft type and CHDs in our population. This study aimed to assess the prevalence of CHDs in a cohort of OFC patients at a tertiary health facility in Nigeria, as well as assess the risk of CHD by OFC type. Materials and Methods: This was a prospective study design. Patients with an OFC were consecutively enrolled at a single OFC treatment facility. All subjects were assessed by a paediatric cardiologist and had echocardiography done. They were categorised based on the presence of CHDs, as well as the OFC phenotypic type (cleft lip and/or alveolus, cleft lip and palate and cleft palate only). Statistical analysis was done using STATA version 14 (College Station, Texas), and significance was set at P < 0.05. Results: A total of 150 subjects enrolled in the study over a period of 2 years (2018–2020). The median age of subjects was 6 months (interquartile range: 2–24), and 54.7% were female. The prevalence of CHDs in the subjects reviewed was 30.7%. Based on the severity of CHDs, the majority presented with simple defects (95.6%). Overall, the most common presentation was patent foramen ovale (12.7%), followed by septal defects (8.0%). There was no significant association between cleft type and the odds of a CHD. Conclusion: The study reports a relatively high prevalence of CHDs in patients with OFC; however, there was no association between the risk of CHD by cleft type. Although a majority of CHDs may pose a low operative risk, cardiac evaluation is recommended for all cases of OFC to aid the identification of potentially high-risk cases.

Introduction: Orofacial clefts (OFCs) are among the most common craniofacial developmental abnormalities worldwide and a significant cause of childhood morbidity and mortality. This study aimed to identify patterns of patient presentation, treatment approaches, and changes in our overall cleft care service between 2007 and 2019. Methods and Methodology: A retrospective review of patients managed at a tertiary health facility in Nigeria of all OFC cases operated between 2007 and 2019 was done using the postintervention data retrieved from the Smile Train database. Data of all OFC cases operated within the period were analyzed using the Statistical Package for the Social Sciences. Descriptive statistics were performed using the Statistical Package for the Social Sciences version 20.0. Results: A total number of 740 OFC surgeries were performed in 565 patients, consisting of 269 females (48.2%) and 289 males (51.8%). The majority (63%) of the patients presented before the age of 2 years. Thirty-seven percent presented with cleft lip and alveolus, 27.1% with cleft palate only, and 36.7% with cleft lip, alveolus, and palate. Primary cleft lip repair was the most performed surgery (n = 320, 43.2%), the mean age at repair was 2.1 years. Since 2017, additional services such as speech therapy, mixed dentition orthodontics, and nutritional support were added to services provided to our cleft patients. Fifteen patients have undergone speech assessment and three have completed speech treatment. Eight patients have undergone mixed dentition stage orthodontic treatment. Discussion: Our services have evolved from simply providing surgical care to comprehensive care with a multidisciplinary team approach and provision of a wide range of services including nutritional counseling, pediatric care, orthodontic services, and speech therapy. We believe these will improve the overall well-being of our patients while we continue to improve on services based on clinical research outcomes.

Aim The aim of the study was to investigate the role of variants in GJB2 gene in the etiology of hearing defects in nonsyndromic cleft lip/palate. Method Saliva samples were obtained from cases (subjects with orofacial clefts) and control (subjects without orofacial clefts) who consented to the study. Deoxyribonucleic acid (DNA) was extracted using standardized protocol at Butali Lab (Iowa, IA). Primers for the coding region of GJB2 was designed using Primer 3 (http://bioinfo.ut.ee/primer3-0.4.0/) and optimized in the Butali lab using a gradient polymerase chain reaction to determine the annealing temperature for each primer set (forward and reverse). We measured the DNA concentration using Qubit and XY genotyping done for quality control. A concentration of 5 ng/μL of DNA was used for Sanger sequencing. Results A total of 150 subjects were sequenced (66 cases; 84 controls). Mutations in GJB2 gene were detected in 2 individuals with cleft palate. We found p.Arg165Trp variant in 1 case and p.Leu81Val variant in the second case. Although p.Arg165Trp was predicted to be either benign or tolerated by SIFT/POLYPHEN, the single nucleotide change from C>T, that is, CGG>TGG leads to a premature stop codon preventing the protein formation. The p.Leu81Val variant was predicted to be probably damaging/ deleterious. Conclusions The present study implicates variants in the GJB2 gene in the etiology of hearing defects in nonsyndromic cleft lip and palate in the Nigerian population. Screening for variations in GJB2 gene is important for genetic counseling especially in high-risk families.

Objectives Despite treatment, the pre-traumatic facial appearance of patients with maxillofacial fractures might not be able to be restored, and this difference can affect the person's quality of life (QoL). This study was designed to evaluate changes in QoL of people with maxillofacial fractures. Materials and Methods The study population was comprised of participants with maxillofacial fracture and age- and sex-matched healthy controls without history of such fracture. QoL was measured using the World Health Organization Quality of Life BREF (WHOQOL-BREF) questionnaire, which was administered to the patients before treatment (Time 1), at 6 weeks post-treatment (Time 2), and at 12 weeks postoperatively (Time 3). The values were compared with those of healthy controls. The QoL was compared between closed reduction group and open reduction and internal fixation group. Results The QoL scores of people with maxillofacial fracture before treatment were significantly lower (P=0.001) than those of healthy controls in all domains of the WHOQOL-BREF. The QoL scores in the psychological and social domains of patients with maxillofacial fracture at Time 3 were still lower than those of healthy controls (P=0.001). Conclusion The QoL of patients with maxillofacial fracture was significantly reduced before treatment in all domains and remained reduced in both psychological and social domains weeks after treatment. Therefore, clinicians must be aware of and manage the residual psychosocial issues that can accompany the post-treatment period of maxillofacial injury.

Introduction:primary maxillofacial tumors are uncommon in pediatric patients. When they do occur, the tissue damage caused directly alters facial growth, development as well as psycho-social evolution. This study was carried out to determine the pattern, sociodemographic characteristics and histologic peculiarities of paediatric jaw tumors in our environment. Methods:a retrospective hospital-based study where the case notes of children below the age of 14 years who presented with jaw tumors and tumor-like lesions from January 2014 to December 2018 were studied. Results:eighty-two patients were studied; patients aged 10-14 years had the highest representation. Mean time of presentation was 8 months with jaw swelling being the commonest presentation (84.1%). Majority of the fathers were in their 4thdecade of life while most of the mothers were in their third decade of life and both parents possessed primary school certificate as their highest level of educational attainment. Fathers were mostly traders, while mothers were mostly full-time housewives. The maxilla and mandible were most commonly affected with the left side showing higher preponderance. Burkitt lymphoma (19 (23.2%)) and adenomatoid odontogenic tumor (14 (17.1%)) were the commonest lesions. When the tumor involved both the maxilla and the mandible, the tumor was most likely malignant. Conclusion:in our center, paediatric jaw tumors are commonest in male children with the 10-14 years´ age group most commonly affected. Burkitt lymphoma and adenomatoid odontogenic tumors were the commonest tumors. Early presentation must be encouraged since these tumors if presented early can be successfully treated.

OBJECTIVES The aim of this study was to compare the quality of life (QoL) of parents/caregivers of children with cleft lip and/or palate before and after surgical repair of an orofacial cleft. MATERIALS AND METHODS Families of subjects who required either primary or secondary orofacial cleft repair who satisfied the inclusion criteria were recruited. A preoperative and postoperative health-related QoL questionnaire, the 'Impact on Family Scale' (IOFS), was applied in order to detect the subjectively perceived QoL in the affected family before and after surgical intervention. The mean pre- and postoperative total scores were compared using paired t-test. Pre- and postoperative mean scores were also compared across the 5 domains of the IOFS. RESULTS The proportion of families whose QoL was affected before surgery was 95.7%. The domains with the greatest impact preoperatively were the financial domain and social domains. Families having children with bilateral cleft lip showed QoL effects mostly in the social domain and 'impact on sibling' domain. Postoperatively, the mean total QoL score was significantly lower than the mean preoperative QoL score, indicating significant improvement in QoL (P

Different dosage protocols may be advocated by different clinicians for the same keloid lesion. The aim of this study was to determine the optimal frequency of injection of triamcinolone for the purpose of reducing the size of keloid, by monitoring volume change in lesions. Volume of all lesions was measured, and 40 mg of triamcinolone was injected once. Lesion volume was thereafter monitored weekly for 6 weeks. Mean pretreatment volume was 6.4 ml. Following triamcinolone injection, mean lesion volume became 5.1, 3.7, 3.6, and 3.6 ml at 1, 2, 3, and 4 weeks postinjection, respectively. The mean lesion volume was 3.7 and 3.9 ml at 5 and 6 weeks postinjection. Mean lesion volume was 1.29 ml lower at 1 week than at pretreatment stage (SD ± 0.8797), 1.35 ml lower in volume at 2 weeks than 1 week (SD ± 1.0386), and 0.138 ml lower in volume at 3 weeks than 2 weeks (SD ± 0.159). Mean lesion volume was 0.0250 ml lower at 4 weeks than at 3 weeks (SD ± 0.3215), 0.1000 ml greater in volume at 5 weeks than 4 weeks (SD ± 0.1713), and 0.2000 ml greater in volume at 3 weeks than 2 weeks (SD ± 0.0418). There is a statistically significant difference between the mean volume at 1 week postinjection and that at pretreatment stage, between 2 and 1 week, and between 3 and 2 weeks (p ≤ 0.05). Reduction in volume was found to be most profound and statistically significant within the first 2 weeks postinjection. This study finds that the optimal frequency of intralesional injection of triamcinolone involves a 2-week injection interval. Level of Evidence: Level IV, therapeutic study

The objective of the study was to assess the association between parental age and risk of orofacial cleft. Materials and Methods: This was a case–control study among 110 parents of children with orofacial cleft (case group) and 110 parents of children without orofacial cleft (control group). Background: Orofacial clefts (OCs) are one of the most common craniofacial anomalies and are reported to be associated with congenital cardiovascular anomalies (CCAs). However, there is paucity of data in African populations on the risk of CCAs in OC patients compared to the general population. Aims: This study aims to determine the odds of congenital cardiovascular anomalies in patients with OC compared to the general population. Subjects and Methods: A case-control study design was used. Case subjects were non-syndromic OC subjects, while controls were non-syndromic subjects without OC. All subjects were thoroughly assessed by a pediatric cardiologist for CCAs; and grouped by OC phenotypic type (cleft lip and/or alveolus, cleft lip and palate, cleft palate only and Tessier cleft). Statistical analysis was done using STATA version 14 (College Station, Texas), and significance was placed at P value ≤0.05. Results: A total of 120 subjects (60 cases and 60 controls) were enrolled in the study. In total, 23.3% of the subjects had CCAs. Among the case group, 40% had CCAs compared to 6.7% in the control group. Patent foramen ovale (18.3%) and atrial septal defects (10.0%) were the most common type of CCAs in cases, respectively. Further, cases had significantly higher odds of CCAs compared to controls (OR: 9.3; CI: 2.8, 39.4). Conclusions: Our finding reveals that the odds of CCAs are significantly higher in patients with OC than the general population. Future studies could assess the effect of CCAs on surgical outcome.

Orofacial clefts are the most common malformations of the head and neck. In Africa, orofacial clefts are underascertained, with little or no surveillance system in most parts for clefts and other birth defects. A Nigerian craniofacial anomalies study, NigeriaCRAN, was established in 2006 to support cleft research specifically for epidemiological studies, treatment outcomes, and studies into etiology and prevention. We pooled data from seven of the largest Smile Train treatment centers in the six geopolitical zones in Nigeria. Data from September 2006 to June 2011 were analyzed and clefts compared between sides and genders using the Fisher exact test. A total of 2197 cases were identified during the study period, with an estimated prevalence rate of 0.5 per 1000. Of the total number of orofacial clefts, 54.4% occur in males and 45.6% in females. There was a significant difference ( P=.0001) between unilateral left clefts and unilateral right clefts, and there was a significant difference ( P=.0001) between bilateral clefts and clefts on either the left or right side. A significant gender difference ( P=.03) was also observed for cleft palate, with more females than males. A total of 103 (4.7%) associated anomalies were identified. There were nine syndromic cleft cases, and 10.4% of the total number of individuals with clefts have an affected relative. The significant difference between unilateral clefts and the gender differences in the proportion of cleft palate only are consistent with the literature. The present study emphasizes the need for birth defects registries in developing countries in order to estimate the exact prevalence of birth defects including orofacial clefts.

Background Various conflicting claims of superiority of particular treatment modalities or regimens abound in the literature for the treatment of keloid. This study was designed to determine the more reliable method to flatten keloid between intralesional triamcinolone injection and excision combined with radiotherapy.

Intralesional injection of steroid is a reported treatment method for keloids. There are no prior studies comparing the response of sessile and pedunculated keloids to triamcinolone injections. Fifty-two patients who attended the Maxillofacial Unit of the National Hospital Abuja between March 2006 and February 2010 for treatment of facial keloids were the subjects of this study. They were distributed into two groups. Twenty-six people in group A had sessile keloid scars, and 26 in group B presented with pedunculated keloid scars. Fortnightly injection of 10 mg of triamcinolone per centimetre of keloid lesion was administered on both groups for a maximum of 6 months. Values of proportion of flattened lesions in each group were estimated. Flattening of lesion was achieved in 23 (88.46 %) cases in group A, with no such change observed in 3 (11.54 %). None (0 %) of the lesions in group B showed complete flattening after therapy. Intralesional triamcinolone injection caused flattening of sessile keloid lesions, but does not have such effect on pedunculated lesion. Level of Evidence: Level IV, therapeutic study.

Background: Van der Woude syndrome (VWS), an autosomal dominant condition associated with clefts of the lip and/or palate and lower lip pits and is caused by mutations in interferon regulatory factor six gene. It isreported to be the most common syndromic cleft worldwide. Non-penetrance for the lip pit phenotype is found in at least 10% of affected individuals and those without the pits are phenocopies for non-syndromic clefting. The aim of this study is to present the phenotypiccharacteristic of VWS seen at the Lagos University Teaching Hospital (LUTH) cleft clinic.Materials and Methods: A review of cases of patients with VWS that attended the cleft lip and palate clinic at the LUTH Idi-Araba, Lagos, from January 2007 to December 2012 was conducted. Data analyses included sex of affected patients, types of cleft, presence of lower lip pits andhistory of lower lip pits/cleft in the family.Results: A total of 11 cases were seen during the period (male = 4; female = 7). Age at presentation ranged between 1 week and 12 years, with majority (n = 10) less than 2 years of age. Bilateral cleft lip and palate (BCLP) was seen in six patients, isolated soft palatal cleft (n = 3)and unilateral cleft lip and alveolus (n = 1) and cleft of hard and soft palate (n = 1). Bilateral lower lips were presented in 10 out of the 11 cases. The mother of the only patient without lip pits presented with bilateral lower lip pits. No family history of cleft/lip pits was elicited in 10 other cases. Conclusion: Most of the cases of VWS presented with BCLP and lower lippits. Non-penetrance for the lip pits was seen in one out of 11 cases. Our study emphasizes the need to screen family members in all cleft cases, especially clinically diagnosed non-syndromic cases who may be VWS with no lip pits. Future studies are required to investigate the genetic causes of this syndrome in our population.Key words: Cleft lip, cleft palate, lip pits, Van der Woude syndrome

Orofacial clefts (OFC) are complex genetic traits that are often classified as syndromic or nonsyndromic clefts. Currently, there are over 500 types of syndromic clefts in the Online Mendelian Inheritance in Man (OMIM) database, of which Van der Woude syndrome (VWS) is one of the most common (accounting for 2% of all OFC). Popliteal pterygium syndrome (PPS) is considered to be a more severe form of VWS. Mutations in the IRF6 gene have been reported worldwide to cause VWS and PPS. Here, we report studies of families with VWS and PPS in sub-Saharan Africa. We screened the DNA of eight families with VWS and one family with PPS from Nigeria and Ethiopia by Sanger sequencing of the most commonly affected exons in IRF6 (exons 3, 4, 7, and 9). For the VWS families, we found a novel nonsense variant in exon 4 (p.Lys66X), a novel splice-site variant in exon 4 (p.Pro126Pro), a novel missense variant in exon 4 (p.Phe230Leu), a previously reported splice-site variant in exon 7 that changes the acceptor splice site, and a known missense variant in exon 7 (p.Leu251Pro). A previously known missense variant was found in exon 4 (p.Arg84His) in the PPS family. All the mutations segregate in the families. Our data confirm the presence of IRF6-related VWS and PPS in sub-Saharan Africa and highlights the importance of screening for novel mutations in known genes when studying diverse global populations. This is important for counseling and prenatal diagnosis for high-risk families.

Background: The central third of the face is distorted by the bilateral cleft of the lip and palate and restoring the normal facial form is one of the primary goals for the reconstructive surgeons. The history of bilateral cleft lip repair has evolved from discarding the premaxilla and prolabium and approximating the lateral lip elements to a definitive lip and primary cleft nasal repair utilising the underlying musculature. The aim of this study was to review surgical outcome of bilateral cleft lip surgery (BCLS) done at the Lagos University Teaching Hospital. Materials and Methods: A review of all cases of BCLS done between January 2007 and December 2012 at the Lagos University Teaching Hospital was done. Data analysis included age and sex of patients, type of cleft deformity and type of surgery (primary or secondary) and whether the cleft deformity was syndromic and non-syndromic. Techniques of repair, surgical outcome and complications were also recorded. Results: A total of 39 cases of BCLS involving 21 males and 18 females were done during the period. This constituted 10% (39/390) of all cases of cleft surgery done during the period. There were 5 syndromic and 34 non-syndromic cases. Age of patients at time of surgery ranged between 3 months and 32 years. There were 24 bilateral cleft lip and palate deformities and 15 bilateral cleft lip deformities. Thirty-one of the cases were primary surgery, while 8 were secondary (revision) surgery. The most common surgical technique employed was modified Fork flap (Millard) technique, which was employed in 37 (95%) cases. Conclusion: Bilateral cleft lip deformity is a common cleft deformity seen in clinical practice, surgical repair of which can be a challenge to an experienced surgeon. A modified Fork flap technique for repair of bilateral cleft lip is a reliable and versatile technique associated with excellent surgical outcome.

Purpose To compare the effect of total and partial wound closure techniques on immediate postoperative tissue reactions and complications after mandibular third molar surgery. Patients and Methods This prospective, randomized, controlled study was carried out at the Department of Oral and Maxillofacial Surgery, National Hospital, Abuja, Nigeria. Patients were randomly allocated to partial (group I) and total (group II) wound closure groups. Data were collected on pain, trismus, swelling, and complications experienced by the patients. Data analysis was carried out with SPSS software for Windows (version 13.0; SPSS, Chicago, IL) and GenStat (Discovery Edition, 2005; VSN International Ltd, Hemel Hempstead, UK). Results We studied 82 patients, consisting of 54 female and 28 male patients, with mandibular third molar teeth impaction. There were 40 patients (48.8%) in group I and 42 patients (51.2%) in group II. A statistically significant reduction in facial swelling ( P = .001) was found in group I compared with group II, but no difference was recorded for pain and trismus. Group I presented with significant postoperative reactionary bleeding ( P = .007), but no difference was recorded between the 2 groups regarding dry socket and socket infection. Conclusion The partial wound closure technique after third molar surgery was considered to be associated with more postoperative morbidity, in view of the distressing nature of reactionary bleeding that was found to be associated with it. However, patients should be adequately informed about the possibility of attendant facial swelling each time the technique of total wound closure is used.

Cleft lip surgery is a common procedure performed by surgeons worldwide. The aim of the study was to determine blood transfusion requirements and factors influencing blood transfusion in cleft lip surgery.Transfusion rate in 100 consecutive patients who had cleft lip surgery was prospectively evaluated at the Lagos University Teaching Hospital, Nigeria. Data collected included age and sex of patients, type of cleft defects, type of surgery done, preoperative haematocrit, duration of surgery, amount of blood loss during surgery, the number of units of blood cross-matched and those used. Cardiovascular parameters at the point of transfusion were also recorded. Each patient was made to donate a unit of homologous blood prior to surgery.Mean estimated blood loss during surgery was 26.5 ± 47.1 ml. Most patients (92%) lost between 2 and 50 ml of blood. Mean estimated blood loss in unilateral cleft lip surgery was not significantly differently from that of bilateral cleft lip surgery (P=0.46). Only five patients (5%) required blood transfusion. The mean blood transfused was 50.0 ± 16.9 ml. All the patients transfused had a preoperative haematocrit of30% (23-27%). The cross-match-to-transfusion ratio for cleft surgery was 20. The transfusion index was 0.05 and overall blood-ordering quotient was 20.Cleft lip surgery is a low volume blood loss surgery. Homologous blood donation prior to cleft lip surgery in patients with preoperative haematocrit of 30% or more is not necessary. For patients with preoperative haematocrit of less than 30%, type and screen of donated blood should be adequate.

Purpose To obtain a national profile on the prevalence and management of ameloblastic carcinoma in Nigerians. Materials and Methods Data were collected from the case files of patients with a histologic diagnosis of ameloblastic carcinoma from 4 tertiary referral centers in Nigeria from January 1980 to December 2008. Results Twenty patients were seen within the study period. There were 11 male and 9 female patients, with a male-to-female ratio of 1.2:1. Their ages ranged from 16 to 85 years (mean ± SD, 41.63 ± 19.8 years). The duration of the lesion before presentation was 6 months to 4 years. Twelve cases occurred in the posterior mandible alone, 1 case occurred in the anterior mandible alone, and 4 cases involved the anterior and posterior mandible. The posterior part of the maxilla was involved in 3 cases. A majority of the cases (17) occurred de novo, and 3 patients presented with carcinoma ex-ameloblastoma. Treatment included surgical resection with or without neck dissection. Eight patients declined treatment after diagnosis. Surgery was planned for 12 patients, but 2 patients died of intractable bleeding episodes before surgery. Mandibulectomies and maxillectomies were performed for 10 patients. Follow-up was carried out for 5 patients. Recurrence ranged from 6 to 96 months after the first surgery. Overall deaths recorded involved 6 patients. Three patients died within 3 years after the initial surgery and 1 patient died about 8 years after the initial surgery. One patient is still alive and well 1 year after surgery. Conclusion Ameloblastic carcinoma is an uncommon malignancy. Most cases occur in the mandible and arise de novo. Early diagnosis and radical local excision remain the mainstay of treatment.

Aim: The study aims to determine the frequency of homologous blood transfusion in patientsundergoing cleft lip and palate surgery at the Lagos University Teaching Hospital, Nigeria. Setting and Design: A prospective study of transfusion rate in cleft surgery conducted at the Lagos University Teaching Hospital, Nigeria. Material and Methods: One hundred consecutive patients who required cleft lip and palate surgery were recruited into the study. Data collected included age, sex and weight of patients, type of cleft defects, type of surgery done, preoperative haematocrit, duration of surgery, amount of blood loss during surgery, the number of units of blood cross-matched and those used. Each patient was made to donate a unit of homologous blood prior to surgery. Results: There were 52 females and 48 males with a mean age of 64.4 ± 101.1 months (range, 3-420 months). The most common cleft defect was isolated cleft palate (45%) followed by unilateral cleft lip (28%). Cleft palate repair was the most common procedure (45%) followed by unilateral cleft lip repair (41%). The mean estimated blood loss was 95.8 ± 144.9 ml (range, 2-800ml). Ten (10%) patients (CL=2; CP=5, BCL=1; CLP=2) were transfused but only two of these were deemed appropriate based on percentage blood volume loss. The mean blood transfused was 131.5 ± 135.4ml (range, 35-500ml). Six (60%) of those transfused had a preoperative PCV of < 30%. Only 4.9% of patients who had unilateral cleft lip surgery were transfused as compared with 50% for CLP surgery, 11% for CP surgery, and 10% for bilateral cleft lip surgery. Conclusions: The frequency of blood transfusion in cleft lip and palate surgery was 10% with a cross-match: transfusion ratio of 10 and transfusion index of 0.1. A “type and screen” policy is advocated for cleft lip and palate surgery.

AimProphylactic surgical extraction of impacted third molars is a common practice throughout the world justified on the presumption that the risk of surgical morbidity increases with increasing age, among other reasons. The aim of this study was to analyze and compare surgical morbidity associated with third-molar extractions in young and aging populations.Methods and MaterialsA review of records for all patients who underwent the surgical extraction of impacted third molars between April 2001 and June 2006 at the Lagos University Teaching Hospital was carried out.ResultsA total of 506 patients had surgical extractions of impacted third molars under local anaesthesia during the period of the study. Of these, 470 (92.9 percent) patients were below the age of 40 years (Group A) and 36 (7.1 percent) patients were 40 years of age and older (Group B). No incidences of severe intraoperative complications (excessive bleeding or mandibular fractures) were recorded in either group, but other postoperative complications were reported in 70 (13.8 percent) patients. Of these 70 patients, 65 (92.9 percent) were from Group A and 5 (7.1 percent) were from Group B, and their complications included infected socket, dry socket, paraesthesia, and buccal space abscess.ConclusionsNo significant difference in postoperative complications following surgical removal of mandibular third molars was found between patients 40 years old and greater and those below age 40. This study was designed to establish the current demographic and treatment patterns of mandibular fractures in two urban centers (Lagos University Teaching Hospital, Lagos, and National Hospital, Abuja) in Nigeria. All cases of mandibular fractures diagnosed and treated at the Department of Oral and Maxillofacial Surgery, Lagos University Teaching Hospital, Lagos (1998–2007) and Department of Oral and Maxillofacial Surgery, National Hospital, Abuja, Nigeria (2001–2007) were reviewed. Data collected included age, sex, etiology of fracture, anatomic site of fracture, associated maxillofacial fracture, types of treatment, and postoperative complications. The highest incidence of mandibular fractures (49.3%) occurred in the age group 21–30 years and the lowest in the age group 0–10 years, with male preponderance in nearly all age groups. Road traffic crashes (RTC) were the leading cause (67.5%), followed by assault (18.8%), and gunshot. Of the RTC cases, 85 (40%) were sustained from motorcycle-related crashes. The commonest site of fracture was the body of the mandible (n = 137), followed by the angle (n = 114). The majority (83.1%) were treated by closed reduction using intermaxillary fixation, 13.1% by open reduction and internal fixation, and 3.8% had conservative treatment. Mandibular fractures are commonest during the third decade of life and in men, with almost half of the cases due to of road traffic crashes. RTC was the leading cause of mandibular fractures in all age groups. Motorcycle-related mandibular fractures seem to be increasing in Nigeria. There is a need to enforce legislation designed to prevent RTC to reduce maxillofacial fractures in Nigeria.

Aim: The aim of this study is to review the pertinent literature and determine the most appropriate method of treatment for ameloblastomas. Methods: A computerized literature search using MEDLINE was conducted for published articles on treatment of ameloblastomas. Mesh phrases used in the search were: ameloblastoma AND treatment; ameloblastoma AND surgical management. An attempt was made to conduct a systematic review on the subject, but due to inconsistency in terminology, treatment protocol, lack of randomised controlled trial and inadequate follow-up assessment in most of the articles assessed, a narrative critical review of selected relevant literature regarding treatment of ameloblastoma was undertaken. Results: It is widely reported that the recurrence of an ameloblastoma in large part reflects the inadequacy or failure of the primary surgical procedure. Recent studies have unequivocally showed that when a diagnosis of ameloblastoma is made, the treatment must be aggressive and radical to avoid recurrence. The recurrence rates of 55% to 90% for solid or multicystic lesions treated by conservative approach (enucleation or curettage) and even metastases have been reported. Regarding unicystic ameloblastoma, systematic review of the literature has shown that radical approach to treatment resulted in lowest recurrence rate. For ameloblastomas, the first operation (especially radical) affords the best chance for cure. Conclusions: There is a lack of consensus on the most appropriate treatment modality for ameloblastomas. However, a more radical approach (whenever possible) appears to be the best method for the management of these benign, but locally aggressive, lesions with propensity for multiple recurrences.

Aim: This study presents a review of the clinicopathological features of excised submandibular glands over a 16 year period of time at the Lagos University Teaching Hospital, Nigeria. Materials and methods: A retrospective review of excised submandibular glands over a period of 16 years (January 1990 to December 2005) was conducted. Parameters studied included: age and gender of patients, symptoms and duration of symptoms and histologic diagnosis. Results: A total of 47 patients had their submandibular gland excised during the period of the study. There were 28 men and 19 women (men : women ratio of 1.5:1). Mean age [standard deviation (SD)] of patients at presentation was 40.5 ± 18.1 years (range, 13–84 years). There were 34 (72%) histologically diagnosed neoplasms and 13 (28%) non-neoplastic pathologies. Patients with neoplastic pathologies were significantly older than those with non-neoplastic pathologies (P = 0.004). The most common neoplasm was pleomorphic adenoma (35%), followed by adenocystic carcinoma (12%) and anaplastic carcinoma (12%). Chronic sialadenitis with or without sialolithiasis (85%) was the most common non-neoplastic pathology. Conclusion: The most common submandibular salivary gland pathology that necessitated surgical excision, in our institution, was neoplasia, predominately pleomorphic adenomas. The most common non-neoplastic pathology was chronic sialadenitis with or without sialolithiasis.

The aim of this study was to evaluate the changes in oral health-related Quality of Life (QoL) following third molar surgery with either oral administration or submucosal injection of prednisolone. Subjects were randomly distributed into three groups of 62 subjects each: Group A consisted of subjects who received 40 mg oral prednisolone; group B received 40 mg submucosal injection of prednisolone, while group C did not receive prednisolone. Preoperative and postoperative quality of life evaluations of all subjects were done using the 14-item Oral Health Impact Profile (OHIP-14) questionnaire. Postoperatively, quality of life evaluation was done on postoperative days 1, 3, and 7. Subjects were categorized as either affected (OHIP score ≤28) or not affected (OHIP score >28). Mean OHIP scores between preoperative and postoperative periods, as well as between the three groups, were compared. A significant increase in the mean total and subscale scores was found in all the groups postoperatively. This was most marked on the first postoperative day, and it gradually improved throughout the immediate postoperative period. Subjects who received prednisolone experienced a statistically significant better quality of life than those who did not. Subjects who received submucosal injection of prednisolone showed statistically significant less deterioration in QoL than those who received oral prednisolone (P = 0.001). Administration of prednisolone was significantly associated with less deterioration in quality of life and earlier recovery when compared with subjects who did not receive prednisolone. Submucosal injection of prednisolone 40 mg (which offers a simple, safe, painless, and cost-effective therapeutic option) is an effective therapeutic strategy for improving the quality of life after surgical removal of impacted lower third molars.

Purpose The aim of this prospective study was to investigate radiologic and clinical factors associated with increased difficulty in the removal of impacted mandibular third molars. We also aimed to form an index to measure the difficulty of removal of the impacted molars preoperatively. Patients and Methods A total of 87 patients who required 90 surgical extractions of impacted mandibular third molars from November 2003 to May 2004 were involved in the study. Radiologic and clinical data were taken preoperatively. All extractions were performed under local anesthesia by a single operator. Surgical difficulty was measured by the total intervention time. Results Increased surgical difficulty was associated with increasing age and body mass index. Aim Extraction healing complications have been attributed to several factors. The influence of trans-operative complications on an extraction site wound healing was the focus of this investigation. Methods and Materials This prospective study was conducted at the Oral Surgery Clinic of the Department of Oral and Maxillofacial Surgery of the Lagos University Teaching Hospital (LUTH) in Nigeria . Subjects selected were those referred for one or two adjacent extractions and who satisfied the inclusion criteria for the study. The relevant pre-operative information recorded for each patient were age and sex of patient, indications for extraction, time taken to extract the tooth, tooth/teeth removed, and any trans-operative complications. Extractions were performed with dental forceps, elevators, or both under local anaesthesia. Patients were blindly evaluated on the third and seventh post-operative day for socket healing assessment without reference to preoperative information on the patients. Results Seventy-three (24.25%) of 301 teeth considered for socket healing assessment had various transoperative complications due to accidental crown, root, or alveolar bone fractures. Of the 73 extractions with trans-operative complications during extraction, 18 developed a socket healing complication, while 17 of the 228 extractions without trans-operative complications developed socket healing complications (p = .000). The mean (SD) time taken to extract teeth developing healing complications was also found to be significantly longer than those without healing complications (p < .01). Conclusions The study demonstrated the combination of tooth/bone fragments in the socket and increased time of extraction due to trans-operative complications and accidents predispose to the development of extraction site wound healing disturbance. Aim The aim of this prospective study was to evaluate the clinical pattern of post-extraction wound healing with a view to identify the types, incidence, and pattern of healing complications following non-surgical tooth extraction. Study Design A total of 311 patients, who were referred for non-surgical (intra-alveolar) extractions, were included in the study. The relevant pre-operative information recorded for each patient included age and gender of the patient, indications for extraction, and tooth/teeth removed. Extractions were performed under local anesthesia with dental forceps, elevators, or both. Patients were evaluated on the third and seventh postoperative days for alveolus healing assessment. Data recorded were: biodata, day of presentation for alveolus healing assessment, day of onset of any symptoms, body temperature (°C) in cases of alveolus infection, and presence or absence of pain. Results Two hundred eighty-two patients (282) with 318 extraction sites were evaluated for alveolus healing. Healing was uneventful in 283 alveoli (89%), while 35 alveoli (11%) developed healing complications. These complications were: localized osteitis 26 (8.2%); acutely infected alveolus 5 (1.6%); and an acutely inflamed alveolus 4 (1.2%). Females developed more complications than males (p=0.003). Most complications were found in molars (60%) and premolars (37.1%). Localized osteitis caused severe pain in all cases, while infected and inflamed alveolus caused mild or no pain. Thirty patients (12%) among those without healing complications experienced mild pain. Conclusions Most of the post-extraction alveoli healed uneventfully. Apart from alveolar osteitis (AO), post-extraction alveolus healing was also complicated by acutely infected alveoli and acutely inflamed alveoli. This study also demonstrated a painful alveolus is not necessarily a disturbance of post-extraction site wound healing; a thorough clinical examination must, therefore, be made to exclude any of the complications. Objective The purpose of this article is to review cases of osteogenic sarcoma of the jaw bones diagnosed in our institution during a 21-year period. Study design Records of patients seen at the Lagos University Teaching Hospital between January 1983 and December 2003 with histologic diagnosis of osteogenic sarcoma were analyzed for age and gender of patients, as well as sites and histologic types of tumor. Results Osteogenic sarcoma of the jaws constituted 0.6% of all the biopsies of oral and jaw lesions seen within the period under study. The mean (±SD) age of patients at presentation was 27.2 ± 13.6 years (range 11-70 years). The mean age of patients with maxillary lesions, 30.7 ± 15.8 years (range 17-70 years), was higher than that of patients with mandibular lesions, 23.4 ± 10.1 years (range 11-40 years); however, no statistical significance difference was found (P = .08) There were 12 male and 5 female patients, a ratio of 2.4:1. Nine (53%) tumors were found in the maxilla and 8 (47%) in the mandible, a ratio of 1.1:1. The mandibular lesions were predominant in female patients, while the maxillary lesions were predominant in male patients. The most common histologic subtype was chondroblastic (47%), followed by fibroblastic (35.3%) and osteoblastic types (17.7%). Conclusion Osteogenic sarcoma of the jaws is rare in patients who presented with tumor and tumor-like lesions of the jaws in our center, in agreement with previous reports from Africa and other centers around the world. However, the mean age at presentation of our patients was lower than reported in other parts of the world.

Objective. This study sought to determine the relative frequency of odontogenic tumors in a Nigerian population and to compare these data with previous reports. Study design. Records of patients seen at the Lagos University Teaching Hospital between January 1980 and December 2003, with histologic diagnosis of odontogenic tumors (based on World Health Organisation classification, 1992), were analyzed. Results. Odontogenic tumors constituted 9.6% of all the biopsies of oral and jaw lesions seen within the period under study. Three hundred and eight (96.6%) were intraosseous, and 11 (3.4%) were peripheral (peripheral odontogenic fibroma = 7; peripheral myxoma = 3; peripheral ameloblastoma = 1). The mean age of patients was 29.9 6 15.6 years (range, 4-85 years). Among these cases, 96.6% of the tumors were benign and 3.4% were malignant. Ameloblastoma with predilection for the mandible was the most frequent odontogenic tumor (63%), followed by adenomatoid odontogenic tumor (AOT) (7.5%), myxoma (6.5%), calcifying epithelial odontogenic cyst (5.3%), and odontogenic fibroma (5.3%). More cases of malignant odontogenic tumors were seen than cases of calcifying epithelial odontogenic tumor and odontomas. The mean ages of patients with AOT, ameloblastic fibroma, and odontoma were significantly lower than those with ameloblastoma (P \ .05). No significant difference was found between the mean ages of patients with benign odontogenic tumors and those with malignant odontogenic tumors (P = .058). Conclusions. Odontogenic tumors, especially ameloblastoma, are not considered rare among Nigerians, whereas odontoma, regarded as the most frequent odontogenic tumor in North and South America, is rare. Background: The aims of this study were to determine the pattern and frequency of oral lesions and to compare the prevalence of HIV‑related oral lesions in paediatric Nigerian patients on HAART with those not on HAART. Materials and Methods: All patients aged 15 years and below attending the Infectious Disease Clinic of Aminu Kano Teaching Hospital with a diagnosis of HIV were consecutively examined in a cross‑sectional study over a 2‑year period. Nonsyndromic clefts of the lip and palate (NSCLP) are complex genetic traits. Together, they are classified as one of the most common birth defects with a prevalence of 1/700 live births. Genome-wide association studies (GWAS) for nonsyndromic cleft lip with or without cleft palate (NSCL[P]) revealed significant association for common single nucleotide polymorphisms near genes involved in craniofacial development i.e., MAFB, PAX7, VAX1, ARHGAP29 (ABCA4 locus), and IRF6. Sequencing of protein coding regions of the NSCL(P) GWAS candidate genes or adjacent genes suggest a role for rare functional variants. Replication studies in the African population did not observe any significant association with the GWAS candidate genes. On the other hand, the role of rare functional variants in GWAS candidate genes has not been evaluated in the African population. We obtained saliva samples from case triads in Nigeria and Ethiopia for Sanger sequencing of the GWAS candidate genes (MAFB, PAX7, VAX1, ARHGAP29, and IRF6) in order to identify rare functional variants. A total of 220 African samples (140 Nigerians and 80 Ethiopians) were sequenced and we found the following new rare variants— p.His165Asn in the MAFB gene, p.Asp428Asn in the PAX7, a splice-site variant that creates a new donor splice-site in PAX7. We also found three previously reported missense variants p.Gly466Ser in PAX7; p.Leu913Ser and Arg955His in ARHGAP29. No de novo mutations were found. Future genome-wide association and sequencing studies should be conducted using samples from Africa in order to identify new molecular genetic factors that contribute to the etiology of NSCLP. © 2014 Wiley Periodicals, Inc.

The aim of the study was to determine the prevalence and bacteriology of bacteremia associated with cleft lip and palate (CLP) surgery. Three venous blood samples were obtained from 90 eligible subjects who presented for CLP surgery: before surgical incision, 1 minute after placement of the last suture, and 15 minutes thereafter. The samples were injected into an Oxoid Signal blood culture and transported to the laboratory for gram-positive/negative and aerobic/anaerobic bacteria analysis. Prevalence of bacteremia associated with cleft surgery was 38.1%. Prevalence rates of bacteremia in cleft lip surgery, cleft palate surgery, and alveoloplasty were 40.9%, 33.3%, and 50%, respectively. There was no significant difference in prevalence rate of positive blood culture in cleft lip surgery, cleft palate surgery, and alveoloplasty (P = 0.69). Positive blood culture was detected most frequently (47%) 1 minute after placement of the last suture. Of the 23 subjects who had positive blood culture at 1 minute, bacteremia persisted in 8 (35%) of them after 15 minutes. The most common bacteria isolated were coagulase-negative staphylococcus, Acinetobacter lwoffii, and coagulase-positive Staphylococcus aureus. Sex and age of the subjects, duration of surgery, blood loss, and type of cleft surgery were not significantly associated with positive blood culture. Bacteremia associated with CLP surgery is polymicrobial and persisted for at least 15 minutes after surgery in 35% of cases. This may reinforce the need for prophylactic antibiotics to protect at-risk patients from developing focal infection of the heart by oral flora.

The aim of this study is to establish the pattern and actiology of mid-facial fractures, of patients seen and treated at the Lagos University Teaching Hospital, Lagos, Nigeria over a 5-year period.All cases of midfacial fractures diagnosed and treated at the Department of Oral and Maxillofacial Surgery, Lagos University Teaching Hospital (LUTH), Lagos between January 2004 and December 2008 were reviewed. Data collected from case records of patients included age, sex, aetiology of fracture, anatomic site of fracture, associated maxillofacial fracture, types of treatment and postoperative complications.The leading cause of mid-facial fractures was road traffic crashes (RTC) which occurred in 93% of highest incidence of mid-facial fractures (36.5%) occurred in the age group of 21-30 years and the lowest in the age group of 60 years and above (2.6%); with male preponderance in nearly all age groups. Female patients were significantly younger than their male counterparts (P=0.000). The most common site of mid-facial bone fractures was the zygomatic complex (n=89, 44.5%), followed by dento-alveolar (n=26, 13%), and Le fort II (n=23, 12%). Conservative management of the fractures was employed in 2.6% (n=4) of the cases while 97.4% (n=152) of the patients required surgical, interventionMid-facial fractures were most common in the 3rd decade of life; with a male-to-female ratio of 3.7: 1. Over 93% of the cases were due to road traffic crashes. Zygomatic complex was the most commonly fractured site. The fact that road traffic crashes still remain the major cause of mid-facial fracture in the studied environment is an indication that the enforcement of legislation to prevent road traffic crashes needs to be re-emphasised.

Background Alloplastic materials are increasingly being used in augmentation of craniofacial defects because of its ready availability, good aesthetic outcome and absence of donor site morbidity. This paper highlights experience in the use of heat-cured acrylic in augmentation cranioplasty. Subjects and methods The management of three patients with anterior skull defect who presented at the Dental and Maxillofacial Surgery Clinic of the Aminu Kano Teaching Hospital over a five-year period is presented. Results There was good aesthetic outcome in all the patients and no complications were recorded. Conclusion Augmentation of craniofacial defects using customized prefabricated heat-cured acrylic provides patients with a durable, stable and structural repair of craniofacial defects with good aesthetic outcome.

This study was undertaken to compare the healing outcome of a short period (2 weeks) of intermaxillary fixation (IMF) with conventional (4-6 weeks) IMF in the management of fractures of the mandibular tooth-bearing area.This was a randomized controlled study conducted at the Lagos University Teaching Hospital, Lagos, Nigeria, between November 2007 and January 2009. Subjects with minimally displaced mandibular fractures in the tooth-bearing area were randomly allocated into 2 treatment groups: IMF for 2 weeks (study group) or IMF for 4 to 6 weeks (control group). For the purpose of study analysis, the primary predictor variable was the treatment (IMF for 2 weeks vs IMF for 4-6 weeks). Other predictor variables were the age and gender of subjects. The primary healing outcome was considered either satisfactory or unsatisfactory. The following outcome variables that described the healing process were also compared in the 2 groups: healing time, postoperative infection, paresthesia, and maximal interincisal opening. Loss of body weight and oral hygiene status at the end of treatment were compared in the 2 groups. A value of P.05 was considered significant.Satisfactory healing was observed in all cases in both groups. However, satisfactory healing was observed earlier (5.4 ± 0.9 weeks) in the control group than in patients with the short IMF period (7.2 ± 0.9 weeks) (P.001). Malocclusion that was amenable to selective grinding was the only complication seen in both groups (n = 2 in study group and n = 1 in control group) (P = .492). Subjects in the control group lost more weight after treatment than those in the study group (P.001). The recovery of interincisal mouth opening was also better in the study group than in the control group (P.001). The study group had better oral hygiene than the control group at the end of treatment.The healing outcome was comparable in both groups. However, the healing time was significantly longer in the group with the short IMF period. The recovery of maximal mouth opening, oral hygiene status, and loss of weight body in the study group were significantly better than those in the control group. This study suggests that a short period (2 weeks) of IMF in the management of minimally displaced mandibular fractures of the tooth-bearing area in young adults is a suitable alternative to the conventional method in terms of the healing outcome.

The submental / transmylohyoid intubation technique, as an alternative technique of airway management in oral and maxillofacial surgery, and its modifications, have been widely reported in the literature since it was first described by Altemir in 1986. However, the technique is not yet popular in Nigeria and Africa in general. A report of two cases in which this technique was used in orofacial reconstruction is presented here. The surgical / anesthetic outcome was satisfactory. No complication was seen in the two cases, and healing of the submental wound was uneventful. Submental intubation is a reliable technique of alternative airway management in oral and maxillofacial surgery. The submental / transmylohyoid technique should be considered by both the anesthetist and the maxillofacial surgeon in challenging cases, where an alternative airway technique is required for maxillofacial surgery.

A rare case of horizontal fracture of mandibular symphysis caused by a sharp matchet cut is presented. Under local anaesthesia with sedation, the fracture site was exposed through the sutured laceration on the left submandibular region. The fracture was adequately reduced and fixed with two circum-mandibular wires. Healing was uneventful, but postoperative radiograph after six weeks revealed a bony defect in the anterior part of the lower border of the mandible, resulting from shifting of the fractured segment posteriorly during healing. Complete recovery of left lower lip sensation was found 4 months postoperatively.

Pregnancy tumour is a clinical term used to identify a pyogenic granuloma that occurs in pregnant women. Pregnancy tumour is not uncommon, but severe bleeding associated with the lesion necessitating blood transfusion is rare. We report a rare case of mandibular gingival pregnancy tumour with a life-threatening haemorrhage in a 25-year-old Nigerian woman. The patient had lost an estimated 2 litres of blood before presentation. Her packed cell volume was 8% at presentation. The patient was managed by infusion of fluid, transfusion of 4 units of blood and ligation of the facial artery; and excision of the lesion. Following an uneventful healing, she was discharged 10 days later with a PCV of 40.5%. Following subsequent review there was a recurrence 2 months later that regress after 1 month. She was subsequently lost to further follow up.

This study presents a clinicopathologic review of cases of submandibular salivary gland tumours at the Lagos University Teaching Hospital, Nigeria, over a period of 17 years.The records of all patients with histological diagnosis of submandibular salivary gland tumours at the Lagos University Teaching Hospital over a period of 17 years (January 1990 to December 2006) were retrospectively reviewed. Parameters studied were; age and gender of patients, symptoms and duration of symptoms, and histological diagnosis.A total of 36 patients with submandibular gland tumours were seen during the period. Male-to-female ratio was 1.8:1 (male=23, female=13). Mean age (+/- SD) at presentation was 43 (+/- 19) years (age range, 17-84 years). There were 19 malignant tumours and 17 benign ones. Pleomorphic adenoma (36.1%) was the most frequent tumours, followed by adenoid cystic carcinoma (11.1%), anaplastic carcinoma (11.1%) and malignant lymphoma (11.1%). Patients with histological diagnosis of malignant tumours were significantly older than those with benign tumours (p = 0.01). Most patients (80.6%) presented with painless swelling.Malignant submandibular salivary gland tumours were slightly more than the benign ones in the studied population. A prospective study was conducted between March 2007 and September 2009, to identify the reasons and treatment challenges of delayed presentation of cleft lip and palate deformities at the Lagos University Teaching Hospital, Nigeria. Out of a total of 150 patients with cleft defects during the period, 43 (28.7%) were adults and children aged over six years. The mean age of these patients at the time of presentation was 17.3 years. The most common reasons for late presentation were lack of money (56.7%), lack of health care services nearby (18.4%), and lack of awareness of treatment availability (13.3%). Common challenges in these patients included surgical, orthodontic, speech, anesthetic, and psychological. Although adult clefts were significantly enlarged in three dimensions the anatomic landmarks were easier to discern than in an infant. However, extensive soft tissue dissection in adult cleft lip repair resulted in significant postoperative edema. Closure of wide palatal cleft often required the use of adjunct intraoral flaps. Despite late presentation, surgical outcome of these patients was satisfactory and comparable to cleft repair in infants.Keywords: cleft deformities, adults, adolescents, late presentation, management, challenges

OBJECTIVES: The aim of this prospective study was to investigate the role of pre and Intra operative variables in predicting postoperative complications after removal of impacted Mandibular third molars. PATIENTS AND METHODS: 87 patients who required 90 surgical extractions of impacted Mandibular teeth were involved in the study. Radiological and clinical data were taken preoperatively. Intra operative difficulty and complications were recorded. Postoperative complications were also recorded. RESULTS: Increasing body mass index (BMI), depth from point of elevation and occlusal level of teeth were significantly associated (P < 0.05) with the occurrence of postoperative complications. The Intra operative difficulty and Intra operative complications were both statistically significantly associated with postoperative complications (P < 0.05). CONCLUSIONS: The BMI, depth of point of elevation and occlusal level of teeth are important preoperative predictors of postoperative complications. Also important are the frequency of intra operative complication and the duration of surgery in the removal of impacted teeth. Specific intra operative complications were however not as helpful in predicting specific postoperative complications in this study.

To determine the incidence and pattern of presentation of dry socket as a complication of intra-alveolar dental extraction at the Lagos University Teaching Hospital.Patients who were referred for intra-alveolar dental extractions under local anaesthesia at the exodontia clinic of the Department of Oral and Maxillofacial Surgery of the Lagos University Teaching Hospital between July and December 2002 were included in the study. The following data were recorded for each patient: biodata, reason for extraction, number and type of teeth extracted, time taken for the extraction to be done, smoking habit, and use of oral contraceptives. Onset of symptoms and type of teeth involved for those with diagnosis of dry socket were also recorded.Thirty-one teeth (5.6%) of a total of 554 teeth extracted during the period of the study developed dry socket. The mean age (SD) of the 27 patients who developed dry socket was 32.2 +/- 13.0 years, (m; f ratio of 1: 4.4). Most (44.5%) of the patients who presented with dry socket were in the 3rd decade of life, with more than half (59%) of them presenting on the 3rd day following extraction. Mandibular and maxillary teeth were equally affected. Molars and premolars were exclusively affected. Difficulty of extraction was significantly associated with the development of dry socket (p.05).The incidence of dry socket in our centre is similar to previous reports. Female patients were significantly affected. Difficulty of extraction was significantly associated with the development of dry socket. Non-dressing treatment provided satisfactory relief of the symptoms and subsequent granulation of the socket.

Background: The aim of the study was to evaluate the effect of preoperatively administered submucosal and oral prednisolone on postoperative pain, facial swelling, and trismus following third molar surgery. Patients and Methods: This was a randomized controlled trial in which subjects were randomly distributed into three groups. Group A consisted of subjects who received 40 mg oral prednisolone; Group B consisted of subjects who received 40 mg submucosal injection of prednisolone while Group C consisted of subjects who did not receive prednisolone. Each group had 62 subjects. Measurements for facial width/facial swelling, pain, and mouth opening were recorded preoperatively and postoperatively. The postoperative evaluation points were postoperative days 1, 3, and 7. These measurements were compared with the preoperative values both within and among the groups. Results: Most of the subjects were in their third decade of life. A considerable increase in the mean postoperative values for pain, facial width and trismus was observed. Notably, subjects who did not receive prednisolone showed comparatively higher values for the measured parameters throughout the postoperative evaluation period. Subjects who received submucosal injection of prednisolone showed overall lower values compared to those who received oral prednisolone. Conclusion: The results of this study indicate that the administration of prednisolone has a significantly beneficial effect in ameliorating the postoperative sequelae of the third molar surgery. In addition, the effect of submucosally injected prednisolone is comparable to the orally administered prednisolone; indeed it shows superiority to the latter in a number of dimensions. Submucosal injection of prednisolone offers a simple, effective, easy, safe, and minimally invasive option to existing therapeutic methods of reducing these postoperative sequelae.

Prophylactic surgical extraction of impacted third molars is not an uncommon practice in Europe and America. This has been justified on the basis that the risk of surgical morbidity increases with increasing age among other reasons. The purpose of this study is to report the prevalence of impacted third molar extraction and associated morbidity in patients = 40 years of age in our institution. A retrospective review of patients = 40 years of age who required surgical extraction of their impacted third molars between April 2001 and March 2006 at the Lagos University Teaching Hospital was carried out. Data collected included patients' age and sex, tooth/teeth extracted, indication(s) for tooth/teeth extraction, types of impaction, and surgical morbidity (intra-and postoperative complications). A total of 6.3% of patients requiring third molar surgical extraction were 40 years or older. No intraoperative complications occurred in any of the patients. Only 3 patients (9.7%) developed minor postoperative complications (infected socket, dry socket) which were reversible and of short duration). Less than 7% of patients requiring surgical extraction of impacted third molars in our institution were 40 years and above. In addition, minor postoperative complications were seen in only 3 patients. Our result does not support prophylactic surgical extraction of third molars based on the assumption that surgical morbidity increases with age.
