2. An Analysis of Autism as a Contingency-Shaped Disorder of Verbal Behavior

Abstract

This paper analyzes autism as a contingency-shaped disorder of verbal behavior. Contingencies of reinforcement in effect during the first to third year of a child's life may operate to establish and maintain those behaviors that later result in a diagnosis of autism. While neurobiological variables may, in some cases, predispose some children to be more or less responsive to environmental variables than others, our analysis suggests that reliance on neurobiological variables as causal factors in autism is unnecessary. We present six paradigms that may play critical etiologic roles in the development of behaviors labeled as autistic. Recognizing these contingencies and their resulting behaviors during the first two years of a child's life may contribute substantially to earlier identification, more effective treatment and, quite possibly, to the development of Applied Behavior Analysis programs for the prevention of autism that could be implemented immediately. Conceptualizing autism as a contingency-shaped disorder of verbal behavior may provide a new and potentially more effective paradigm for behavioral research and treatment in autism.

3. Is Autism a Preventable Disorder of Verbal Behavior? A Response to Five Commentaries

Abstract

This paper presents a response to five commentaries on our article "An Analysis of Autism as a Contingency-Shaped Disorder of Verbal Behavior" (Drash & Tudor, 2004). One of the principal objectives of that article is to provide the behavior analysis community and the autism community with a conceptual basis for analyzing autism as a behavioral disorder rather than a neurological disorder. This analysis provides a logical and testable behavioral answer to the question of the etiology of autism, a question that has baffled scientists and researchers for more than half a century. Elements of the original article with which reviewers expressed concern include: need for more data, need for greater emphasis on neurological and epidemiological factors in autism, the relative importance of verbal behavior as a core deficit of autism, and disruptive and avoidant behavior as a primary variable in the etiology of autism. We provide a behavioral response to each of these concerns. We also show how our analysis will provide a conceptual foundation for behavior analysis to begin developing urgently needed programs for prevention and earlier intervention in autism. 4. Treatment of Hyperactive Two-Year-Old Children.

Abstract

Examined with five preschool male children (1 year, 11 months to 2 years, 6 months old) was the effectiveness of a behaviorally oriented treatment program to reduce hyperactivity. Ss were enrolled in a behavior modification class which met for 2 hours per day, 3 days per week, and parents were enrolled in a parent training program. Among findings were that hyperactivity and distractibility fell from the 99th percentile to the 58th percentile, total disturbed behavior fell from the 99th percentile to the 77th percentile upon completion of the program, compliance behavior increased in all settings (home, institute, and classroom), and all parents made marked progress in their ability to use contingent positive reinforcement in control of their child's behavior. 5. Hyperactivity in Preschool Children as Non-Compliance: A New Conceptual Basis for Treatment.

Abstract

A multifaceted behavioral treatment program designed to modify noncompliance in five hyperactive preschoolers and to train their parents in techniques of maintaining compliant behavior was implemented. Results indicated that hyperactivity and noncompliance were reduced to normal limits. Parents' use of behavior modification techniques were equivalent to that of trained child behavior specialists. Data supported the hypothesis that hyperactivity in the preschool child and noncompliance are operationally equivalent terms, and that a treatment program based upon modification of compliance behavior is an effective means for remediating hyperactivity in preschool children. Abstract

Background Many CpGs become hyper or hypo-methylated with age. Multiple methods have been developed by Horvath et al. to estimate DNA methylation (DNAm) age including Pan-tissue, Skin & Blood, PhenoAge, and GrimAge. Pan-tissue and Skin & Blood try to estimate chronological age in the normal population whereas PhenoAge and GrimAge use surrogate markers associated with mortality to estimate biological age and its departure from chronological age. Here, we applied Horvath’s four methods to calculate and compare DNAm age in 499 subjects with type 1 diabetes (T1D) from the Diabetes Control and Complications Trial/Epidemiology of Diabetes Interventions and Complications (DCCT/EDIC) study using DNAm data measured by Illumina EPIC array in the whole blood. Association of the four DNAm ages with development of diabetic complications including cardiovascular diseases (CVD), nephropathy, retinopathy, and neuropathy, and their risk factors were investigated. Results Pan-tissue and GrimAge were higher whereas Skin & Blood and PhenoAge were lower than chronological age (p < 0.0001). DNAm age was not associated with the risk of CVD or retinopathy over 18–20 years after DNAm measurement. However, higher PhenoAge (β = 0.023, p = 0.007) and GrimAge (β = 0.029, p = 0.002) were associated with higher albumin excretion rate (AER), an indicator of diabetic renal disease, measured over time. GrimAge was also associated with development of both diabetic peripheral neuropathy (OR = 1.07, p = 9.24E−3) and cardiovascular autonomic neuropathy (OR = 1.06, p = 0.011). Both HbA1c (β = 0.38, p = 0.026) and T1D duration (β = 0.01, p = 0.043) were associated with higher PhenoAge. Employment (β = − 1.99, p = 0.045) and leisure time (β = − 0.81, p = 0.022) physical activity were associated with lower Pan-tissue and Skin & Blood, respectively. BMI (β = 0.09, p = 0.048) and current smoking (β = 7.13, p = 9.03E−50) were positively associated with Skin & Blood and GrimAge, respectively. Blood pressure, lipid levels, pulse rate, and alcohol consumption were not associated with DNAm age regardless of the method used. Conclusions Various methods of measuring DNAm age are sub-optimal in detecting people at higher risk of developing diabetic complications although some work better than the others.

Abstract

Background Hyperglycemia leading to increased oxidative stress is implicated in the increased risk for the development of macrovascular and microvascular complications in patients with type 1 diabetes mellitus. Methods and Results A random subcohort of 349 participants was selected from the DCCT / EDIC (Diabetes Control and Complications Trial/Epidemiology of Diabetes Interventions and Complications) cohort. This included 320 controls and 29 cardiovascular disease cases that were augmented with 98 additional known cases to yield a case cohort of 447 participants (320 controls, 127 cases). Biosamples from DCCT baseline, year 1, and closeout of DCCT , and 1 to 2 years post‐ DCCT ( EDIC years 1 and 2) were measured for markers of oxidative stress, including plasma myeloperoxidase, paraoxonase activity, urinary F 2α isoprostanes, and its metabolite, 2,3 dinor‐8 iso prostaglandin F 2α . Following adjustment for glycated hemoblobin and weighting the observations inversely proportional to the sampling selection probabilities, higher paraoxonase activity, reflective of antioxidant activity, and 2,3 dinor‐8 iso prostaglandin F 2α , an oxidative marker, were significantly associated with lower risk of cardiovascular disease (−4.5% risk for 10% higher paraoxonase, P iso prostaglandin F 2α , P =0.0092). In contrast, the oxidative markers myeloperoxidase and F 2α isoprostanes were not significantly associated with cardiovascular disease after adjustment for glycated hemoblobin. There were no significant differences between DCCT intensive and conventional treatment groups in the change in all biomarkers across time segments. Conclusions Heightened antioxidant activity (rather than diminished oxidative stress markers) is associated with lower cardiovascular disease risk in type 1 diabetes mellitus, but these biomarkers did not change over time with intensification of glycemic control. Abstract

This paper explores ethical considerations for active studies of circumcision, i.e., the amputation of the foreskin, in the form of a case study of three major trials performed in African countries in the early 2000s. The paper outlines the function of the foreskin and method and history of its amputation as well as its current use in attempting to combat the global AIDS crisis. These trials are then interrogated in accordance with the Declaration of Helsinki. In particular, the irreversible nature of amputation is given great consideration. The case studies are found to have serious failures in terms of the modern iteration of the Declaration of Helsinki. This paper calls for extreme scrutiny in accordance with international norms relating to the ethical treatment of human subjects in a research context of studies of circumcision if and when they are proposed again.

Abstract

Insulin-dependent diabetes mellitus (IDDM) is a disease in which destruction of the insulin-secreting pancreatic beta cells results in the inability to regulate blood glucose levels. The resulting high blood glucose (hyperglycemia) is deleterious to a several organs, and must be controlled with injections of insulin. The inability to completely compensate for the pancreatic deficiency inevitably leads to complications in long-term diabetes. These complications include retinopathy (disorders of the retina of the eye), nephropathy (kidney disease), microalbuminurea (the presence of protein in the urine, often a sign of kidney malfunction), cardiovascular disease, and neuropathy (impaired function of the nerves). It is thought that these disorders may not be equally represented in males and females, and that this asymmetry may vary with the duration of the diabetic state. To further investigate this issue, the records of 657 patients diagnosed with IDDM between 1950 and 1980 were studied. The patients ranged in age from 8 to 48 years, and males and females were equally represented. All disorders showed an increase in incidence with increasing disease duration. The most prevalent finding was mild retinopathy, which was present in almost 100 percent of the patients who had been diabetic for over 20 years; no sex difference observed. Severe retinopathy was found in 70 percent of the subjects after 30 years of IDDM. However, it was more prevalent in females with short-duration IDDM and males with longer-duration (greater than 25 years) IDDM. Kidney dysfunction was more common among females with short disease duration; by 30 years the prevalence was about 45 percent in both sexes. Males experienced more severe microalbuminurea; after 30 years of IDDM, the prevalence of both microalbuminurea and nephropathy was 84 percent in males, but only 59 percent in females. Neuropathy and cardiovascular disease showed very minor sex differences, and attained a prevalence of 70 percent and 10 percent, respectively, after 30 years of IDDM. These findings demonstrate that the natural course of IDDM affects the sexes differently, and that the prevalence of these complications is somewhat higher than has been previously reported. 34. Acceleration of Cognitive, Linguistic and Social Development in the Normal Infant.

Abstract

This pilot study trained parents in behavior modification techniques and attempted to evaluate the impact of this training on the rate of cognitive, linguistic and social development in infants. A second goal was to identify significant areas in infant development which might be influenced by the teaching strategies used. Subjects were four first-born Caucasian infants whose parents were married at the time of involvement with the program. Parents participated in weekly parent-training sessions. The regular 3-hour schedule included a review of observed changes in motor, intellectual and linguistic behaviors, the modeling of behavioral procedures by the staff and lectures and discussions on aspects of infant development. Measures of intellectual, speech and social development were administered prior to discharge from the treatment program and one year later. Results indicated that infants' intellectual and social growth were enhanced by the program. Developmental areas amenable to the parent teaching program were intellectual skills, motor skills, expressive and receptive speech, social skills, and compliance with verbal commands. (SB)

35. Three Procedures for Increasing Vocal Response to Prompt and Initial Imitation in Down Syndrome Children: A Comparison.

Abstract

The study sought to identify effective procedures for facilitating initial imitative vocal behavior in mentally retarded children, thus accelerating the initial stages of language acquisition. Three groups of Down syndrome children ages 6 months to 4 years (N=15) served as subjects in an examination of the relative effectiveness of three procedures for increasing vocal response to experimenter prompt: (1) positive reinforcement only; (2) light-out (room lights dimmed) combined with positive reinforcement; (3) visual screening (silk handkerchief placed across the face) combined with positive reinforcement. No more than three prompts were delivered under the screen for light-out and visual screening conditions, and a response to prompt removed the screen. Results showed that light-out and visual screening were equally effective in increasing the frequency of vocalizations to the therapist's prompt, and that both were significantly more effective than positive reinforcement alone. A clear advantage of light-out and visual screening was a rapid increase in the percentage response to prompt observed between baseline and the first 40 minutes of training. Combining positive social reinforcement for responses with light-out or visual screening for nonresponses increased the probability of vocalization. (JW)

39. Intellectual Acceleration in Normal and Down's Syndrome Children Through Infant Stimulation and Language Training.

Abstract

The effects of stimulation and language training on the intellectual acceleration of seven normal and Down's Syndrome infants were studied. Intervention consisted of the operant conditioning of linguistic, social, self-help, and motor skills, and the training of parents in child management and child development topics. Among other things, it was found that, by the age of 1 year, all Ss were between two and three standard deviations above the expected mean IQ scores for their respective reference groups. The continued acceleration of intellectual development was demonstrated in all Ss by increasing IQ scores in the first year while in treatment, and through a full year of followup for the normal Ss. Abstract

Implementation of multicenter and/or longitudinal studies requires an effective quality assurance program to identify trends, data inconsistencies and process variability of results over time. The Diabetes Control and Complications Trial (DCCT) and the follow-up Epidemiology of Diabetes Interventions and Complications (EDIC) study represent over 30 years of data collection among a cohort of participants across 27 clinical centers. The quality assurance plan is overseen by the Data Coordinating Center and is implemented across the clinical centers and central reading units. Each central unit incorporates specific DCCT/EDIC quality monitoring activities into their routine quality assurance plan. The results are reviewed by a data quality assurance committee whose function is to identify variances in quality that may impact study results from the central units as well as within and across clinical centers, and to recommend implementation of corrective procedures when necessary. Over the 30-year period, changes to the methods, equipment, or clinical procedures have been required to keep procedures current and ensure continued collection of scientifically valid and clinically relevant results. Pilot testing to compare historic processes with contemporary alternatives is performed and comparability is validated prior to incorporation of new procedures into the study. Details of the quality assurance plan across and within the clinical and central reading units are described, and quality outcomes for core measures analyzed by the central reading units (e.g. biochemical samples, fundus photographs, ECGs) are presented.

Abstract

The discovery of islet cell antibodies (ICAs) was the prelude to the understanding that type 1 diabetes mellitus (T1DM) is a chronic autoimmune disease. The issue regarding whether or not the measurement of ICAs should be completely replaced by biochemical markers detecting islet autoantibodies (AAs) for the prediction of T1DM has been the subject of endless international debates. In light of this controversy, we assessed the current role of ICAs as a predictive marker for T1DM progression. We examined a cohort of 1484 first-degree relatives (FDRs) of T1DM probands from the Children's Hospital of Pittsburgh Registry. These relatives were consecutively enrolled between 1979 through 1984 and followed up to 22 yr. Serum obtained at the time of enrollment was assayed for ICAs, glutamic acid decarboxylase (GAD)65, insulin A (IA)-2 AA, and insulin AAs (IAAs). In FDRs who had ICAs in addition to GAD65 and IA-2 AAs, the cumulative risk of developing insulin-requiring diabetes was 80% at 6.7 yr of follow-up, whereas this risk in those with GAD65 and IA-2 AAs without ICAs was only 14% at 10 yr of follow-up (log rank: P

Abstract

This paper presents a response to five commentaries on our article An Analysis of Autism as a Contingency-Shaped Disorder of Verbal Behavior (Drash & Tudor, 2004). One of the principal objectives of that article is to provide the behavior analysis community and the autism community with a conceptual basis for analyzing autism as a behavioral disorder rather than a neurological disorder. This analysis provides a logical and testable behavioral answer to the question of the etiology of autism, a question that has baffled scientists and researchers for more than half a century. Elements of the original article with which reviewers expressed concern include: need for more data, need for greater emphasis on neurological and epidemiological factors in autism, the relative importance of verbal behavior as a core deficit of autism, and disruptive and avoidant behavior as a primary variable in the etiology of autism. We provide a behavioral response to each of these concerns. We also show how our analysis will provide a conceptual foundation for behavior analysis to begin developing urgently needed programs for prevention and earlier intervention in autism.

Abstract

OBJECTIVE—To quantify total diabetes care received (generalist or specialist) from diagnosis onward and its association with the incidence of diabetes complications in a representative cohort of patients with type 1 diabetes. RESEARCH DESIGN AND METHODS—A total of 429 subjects from the Pittsburgh Epidemiology of Diabetes Complications Study, a prospective follow-up study of childhood-onset type 1 diabetic subjects first seen between 1986 and 1988 (mean age 28 years, mean duration 19 years), followed biennially for up to 10 years were studied. Specialist care was defined as care received from a board-certified endocrinologist, diabetologist, or diabetes clinic and quantified as the percent of diabetes duration spent in specialist care. RESULTS—There was a significant trend for a higher incidence of neuropathy, overt nephropathy, and coronary artery disease with lower use of specialist care. Multivariate analyses controlling for diabetes duration, demographic characteristics, health care practices, and physiological risk factors demonstrated that higher past use of specialist care was found to be significantly protective against the development of overt nephropathy (risk ratio 0.43, 95% CI 0.21–0.88) and neuropathy (0.54, 0.35–0.83) and weakly protective against coronary artery disease (0.65, 0.37–1.1). CONCLUSIONS—A higher proportion of diabetes duration spent in specialist care may result in delayed development of certain diabetes complications independent of other risk factors. This study thus supports the concept that the benefits of specialist care should be available to all patients with type 1 diabetes.

Abstract

Refractive error (RE) is a complex, multifactorial disorder characterized by a mismatch between the optical power of the eye and its axial length that causes object images to be focused off the retina. The two major subtypes of RE are myopia (nearsightedness) and hyperopia (farsightedness), which represent opposite ends of the distribution of the quantitative measure of spherical refraction. We performed a fixed effects meta-analysis of genome-wide association results of myopia and hyperopia from 9 studies of European-derived populations: AREDS, KORA, FES, OGP-Talana, MESA, RSI, RSII, RSIII and ERF. One genome-wide significant region was observed for myopia, corresponding to a previously identified myopia locus on 8q12 (p = 1.25×10-8), which has been reported by Kiefer et al. as significantly associated with myopia age at onset and Verhoeven et al. as significantly associated to mean spherical-equivalent (MSE) refractive error. We observed two genome-wide significant associations with hyperopia. These regions overlapped with loci on 15q14 (minimum p value = 9.11×10-11) and 8q12 (minimum p value 1.82×10-11) previously reported for MSE and myopia age at onset. We also used an intermarker linkage- disequilibrium-based method for calculating the effective number of tests in targeted regional replication analyses. We analyzed myopia (which represents the closest phenotype in our data to the one used by Kiefer et al.) and showed replication of 10 additional loci associated with myopia previously reported by Kiefer et al. This is the first replication of these loci using myopia as the trait under analysis. "Replication-level" association was also seen between hyperopia and 12 of Kiefer et al.'s published loci. For the loci that show evidence of association to both myopia and hyperopia, the estimated effect of the risk alleles were in opposite directions for the two traits. This suggests that these loci are important contributors to variation of refractive error across the distribution.

Abstract

The dopamine transporter (DAT) was localized in normal human brain tissue by light microscopic immunocytochemistry by using highly specific monoclonal antibodies. Regional distribution of DAT was found in areas with established dopaminergic circuitry, e.g., mesostriatal, mesolimbic, and mesocortical pathways. Mesencephalic DAT-immunoreactivity was enriched in the dendrites and cell bodies of neurons in the substantia nigra pars compacta and ventral tegmental area. Staining in the striatum and nucleus accumbens was dense and heterogeneous. Mesocortical DAT immunoreactivity in motor, premotor, anterior cingulate, prefrontal, entorhinal/perirhinal, insular, and visual cortices was detected in scattered varicose and a few nonvaricose fibers. Varicose fibers were relatively enriched in the basolateral and central subnuclei of amygdala, with sparser fibers in lateral and basomedial subnuclei. Double-labeling studies combining DAT and tyrosine hydroxylase (TH) immunostaining in the ventral mesencephalon showed two subpopulations of dopaminergic neurons differentiated by the presence or absence of DAT-immunoreactivity in the A9 and A10 cell groups. In other dopaminergic cell groups (All, A13-A15), TH-positive hypothalamic neurons showed no detectable DAT-immunoreactivity. However, fine DAT-immunoreactive axons were scattered throughout the hypothalamus, particularly concentrated along the medial border, with more coarse axons present along the lateral border. These findings demonstrate that most mesotelencephalic dopamine neurons of human brain express high levels of DAT throughout their entire somatodendritic and axonal domains, whereas a smaller subpopulation of mesencephalic dopamine cells and all hypothalamic dopamine cell groups examined express little or no DAT. These data indicate that different subpopulations of dopaminergic neurons use different mechanisms to regulate their extracellular dopamine levels.
