The primary objective of this research is to explore how the level of Mental Health Literacy influences the motivation for treatment among individuals dealing with alcohol or substance addiction. The study involved 128 subjects undergoing inpatient treatment at the Alcohol and Substance Abuse Treatment Center. To gather data, researchers utilized a Personal Information Form, Mental Health Literacy Scale, and Treatment Motivation (TM) survey. The scores obtained from the Treatment Motivation Questionnaire and Mental Health Literacy Scale were 94.21±12.2 and 108.03±11.7, respectively. The analysis revealed a statistically significant, albeit weak, positive correlation between the participants' Mental Health Literacy Scale-Knowledge of How to Seek Mental Health Information subscale mean score and both their total Treatment Motivation Questionnaire score and their Interpersonal Help-Seeking subscale mean scores (r=0.284). Conversely, a statistically significant weak negative correlation emerged between the participants' Mental Health Literacy Scale-Knowledge of Professional Help Available subscale mean score and their Treatment Motivation Questionnaire-Distrust in Treatment subscale mean score (r=−0.230). Conclusions drawn from the study indicate that a lack of knowledge regarding addiction and accessing professional assistance, coupled with concerns surrounding stigma, pose as significant barriers to motivation for seeking treatment.

Amaç: Üçüncü basamak bir doğum hastanesinde bir yılda gerçekleşen sezaryen doğumların Robson On Gruplu Sınıflandırma Sistemi kullanılarak analiz edilmesi amaçlanmıştır. Gereç ve Yöntem: 1 Ocak 2019 - 31 Aralık 2019 tarihleri arasında hastanemiz doğum salonuna kabul edilip, hastanemizde doğum yapmış olan ve herhangi bir obstetrik risk faktö rü olmayan gebeler çalışmaya dahil edilmiştir. Çalışmamız retrospektif olarak dizayn edilmiştir. Bulgular: Çalışmada 10781 doğuma ait veriler incelendi. Bu doğumların 4391’i sezaryen doğum ile gerçekleşmiştir. Tüm doğumlar içerisinde sezaryen doğum oranı %40,7’dir. Sezaryen doğum yapan gebelerin %9,3’lük kısmı Robson sınıflandırmasına göre birinci grupta, %13,6’sı Robson sınıflandırılmasına göre ikinci grupta yer almıştır. Sezaryen doğum yapan gebelerin %12,1’lik kısmı Robson sınıflandırmasına göre üçüncü grupta, %10,8’i dördüncü gruptadır. Grup 5, grup 6 ve grup 7’nin tüm sezaryen doğumlardaki oranları sırasıyla % 47,7, %2,5 ve %2,6’dır. Robson sınıflandırmasına göre Grup 9 ’un tüm sezaryen doğumlar arasındaki oranı ise %1,4’tür. Sonuç: Sonuç olarak, hastanelerin yıllık Robson On Gruplu Sınıflandırma Sistemi Formundaki yüzdelere gö re sezaryen doğumlarının izlenmesi ve değerlendirilmesi, kendi sezaryen eylem planlarını hazırlamaları ve belirledikleri sezaryen hedefine gö re kalite notlarının değerlendirilmesi de sezaryen oranının azaltılması hedefine katkı sağlayacaktır.

Background Provoked anger is associated with an increased risk of cardiovascular disease events. The underlying mechanism linking provoked anger as well as other core negative emotions including anxiety and sadness to cardiovascular disease remain unknown. The study objective was to examine the acute effects of provoked anger, and secondarily, anxiety and sadness on endothelial cell health. Methods and Results Apparently healthy adult participants (n=280) were randomized to an 8‐minute anger recall task, a depressed mood recall task, an anxiety recall task, or an emotionally neutral condition. Pre−/post‐assessments of endothelial health including endothelium‐dependent vasodilation (reactive hyperemia index), circulating endothelial cell‐derived microparticles (CD62E+, CD31+/CD42−, and CD31+/Annexin V+) and circulating bone marrow‐derived endothelial progenitor cells (CD34+/CD133+/kinase insert domain receptor+ endothelial progenitor cells and CD34+/kinase insert domain receptor+ endothelial progenitor cells) were measured. There was a group×time interaction for the anger versus neutral condition on the change in reactive hyperemia index score from baseline to 40 minutes (P=0.007) with a mean±SD change in reactive hyperemia index score of 0.20±0.67 and 0.50±0.60 in the anger and neutral conditions, respectively. For the change in reactive hyperemia index score, the anxiety versus neutral condition group by time interaction approached but did not reach statistical significance (P=0.054), and the sadness versus neutral condition group by time interaction was not statistically significant (P=0.160). There were no consistent statistically significant group×time interactions for the anger, anxiety, and sadness versus neutral condition on endothelial cell‐derived microparticles and endothelial progenitor cells from baseline to 40 minutes. Conclusions In this randomized controlled experimental study, a brief provocation of anger adversely affected endothelial cell health by impairing endothelium‐dependent vasodilation.

لقد كان القرن الثالث الهجري قرناً حافلاً بالتقدم النوعي في المنجزات العلمية، حيث تميزت تلك المنجزات العلمية بالاتساع في المادة العلمية المجموعة، والتنظيم الدقيق والفريد من نوعه لها، ومن جملة ذلك التراث العلمي علمُ التفسير المأثور، الذي تم جمعه من الأجزاء والمدوَّنات التفسيرية التي كُتِبت في القرنين الأول والثاني، ثم كان عصر التصنيف في القرن الثالث. وقد كان الإمام البخاري متميزًا عن سائر علماء القرن الثالث بمنهج منضبط في شأن التعامل مع التفسير بشكل عامٍّ؛ فلا نجده يحفَلُ كثيرًا في صحيحه بالتفسير بالرأي المجرّد، إلا إذا خَلَتِ الآيةُ عن تفسير مأثور. والتفسير المأثور لدى البخاري يشتمل على الأقسام الأربعة المشهورة، وهي: تفسير القرآن بالقرآن، وتفسير القرآن بالسُّنة، ثم تفسير القرآن بأقوال الصحابة، وتفسير القرآن بأقوال التابعين. ولكن المعتمد في تفسير الصحابي والتابعي عند البخاري أن يكون في أمور لا مجال للاجتهاد فيها. الكلمات المفتاحية: التفسير، المأثور، البخاري، القرن الثالث، الصحابة، التابعون.

Aim:Familial Mediterranean Fever (FMF) is the most frequent monogenetic autoinflammatory disorder. It is characterized by fever and serositis. The first line treatment of FMF is colchicine. Adherence to colchicine is one of the main factors affecting colchicine response. In this study, we aimed to evaluate drug adherence in children with FMF using the medication adherence scale in FMF (MASIF). We also assessed the clinical characteristics of drug-adherent patients and factors affecting drug adherence.Materials and Methods:Eighty-two children with FMF under colchicine therapy were included in this cross-sectional observational study. The patients were divided into two groups according to medication adherence and compared according to their demographic and clinical data.Results:According to MASIF, 31 (38%) patients had non-adherence to colchicine. There was a significant difference between the colchicine-adherent and non-adherent groups in terms of age, disease severity according to the International severity score for FMF, attack rate, colchicine dosage, M694V homozygosity, and family type (p=0.005, p=0.04, p=0.025, p=0.045, p=0.04, and p=0.046, respectively).Conclusion:Patients with FMF should be questioned about their medication adherence at every visit, and children with a high risk of colchicine non-adherence should be followed up more closely.

Objective: In this study, we created an animal model to demonstrate the effects of thiamine on the hearing pathways of new-borns during pregnancy and lactation by inducing a dietary thiamine deficiency in the mother. Methods: The study included 16 female Wistar albino rats. The animals were separated into four groups and provided the appropriate amounts of dietary thiamine according to their groups during pre-pregnancy, pregnancy, and lactation periods. Three pups from each mother were included in the study, and 12 pups were selected from each group. On the fortieth day after birth, the auditory pathways of 48 pups in the 4 groups were examined electro physiologically and ultra-structurally. Results: In Group N-N, morphology of hair cells stereocilia degeneration was not obtained in all turns of cochlea. In Group N-T, Inner Hair Cells (IHCs) and Outher Hair Cells (OHCs) stereocilia didn’t show degeneration in all turns of cochlea but had rupture inrows of HCs stereocilia. In group T-N IHCs stereocilia less degeneration was observed in all turns of cochlea. OHC stereocilia partial loss was observed only in basal turn of cochlea. In Group T-T IHCs stereocilia was observed less degeneration and rupture in all turns of cochlea. Conclusion: Thiamine is vital for the development of cochlear hair cells during both prenatal and postnatal periods. Even partial deficiency of thiamine causes significant degeneration to the auditory pathway. Level of evidence: The level of evidence of this article is 5. Objectives: The aim is to use three-dimensional transvaginal ultrasonography (3-D TVUS) to evaluate the success of hysteroscopic metroplasty for the uterine septum and to compare the pregnancy outcomes. Materials and Methods: Thirty-eight patients with uterine septum who had hysteroscopic uterine septum resection were recruited. Preoperative 3-D TVUS measurements of the septal apex to the uterine fundus (s1), septal apex to internal os distance (s2), and intercornual distance (s3) were compared with the postoperative values. The pregnancies of the patients were followed up for a year postoperative period. Results: Out of the 38 patients, thirty-five had partial uterine septum (class U2a), while 3 patients had complete uterine septum (class U2b). Eighteen (47.36%) of the patients who underwent uterine septum resection achieved pregnancy, and thirteen of these pregnancies were (72.2%) term pregnancies, and all term pregnancies resulted in a live birth. Natural conception was achieved in 77.7% (14 of 18) of the patients. Term pregnancy occurred in 68.7% (11 of 16) of the patients with a partial septum and in 66.6% (2 of 3) of the patients with a complete uterine septum. A comparison of the 3-D TVUS measurements of the uterus pre- and postoperatively showed a decrease in s1 and an increase in s2 (P < 0.05). The uterine cavity length of pregnant patients was found to be higher than nonpregnant patients (P < 0.05). Conclusion: Reproductive results of hysteroscopic metroplasty were favorable in achieving live and term birth. three-dimensional TVUS can be preferred as a noninvasive effective method in objective evaluation of the success of the hysteroscopic surgery.

BackgroundThe increasing use of smartphones, wearables, and connected devices has enabled the increasing application of digital technologies for research. Remote digital study platforms comprise a patient-interfacing digital application that enables multimodal data collection from a mobile app and connected sources. They offer an opportunity to recruit at scale, acquire data longitudinally at a high frequency, and engage study participants at any time of the day in any place. Few published descriptions of centralized digital research platforms provide a framework for their development. ObjectiveThis study aims to serve as a road map for those seeking to develop a centralized digital research platform. We describe the technical and functional aspects of the ehive app, the centralized digital research platform of the Hasso Plattner Institute for Digital Health at Mount Sinai Hospital, New York, New York. We then provide information about ongoing studies hosted on ehive, including usership statistics and data infrastructure. Finally, we discuss our experience with ehive in the broader context of the current landscape of digital health research platforms. MethodsThe ehive app is a multifaceted and patient-facing central digital research platform that permits the collection of e-consent for digital health studies. An overview of its development, its e-consent process, and the tools it uses for participant recruitment and retention are provided. Data integration with the platform and the infrastructure supporting its operations are discussed; furthermore, a description of its participant- and researcher-facing dashboard interfaces and the e-consent architecture is provided. ResultsThe ehive platform was launched in 2020 and has successfully hosted 8 studies, namely 6 observational studies and 2 clinical trials. Approximately 1484 participants downloaded the app across 36 states in the United States. The use of recruitment methods such as bulk messaging through the EPIC electronic health records and standard email portals enables broad recruitment. Light-touch engagement methods, used in an automated fashion through the platform, maintain high degrees of engagement and retention. The ehive platform demonstrates the successful deployment of a central digital research platform that can be modified across study designs. ConclusionsCentralized digital research platforms such as ehive provide a novel tool that allows investigators to expand their research beyond their institution, engage in large-scale longitudinal studies, and combine multimodal data streams. Crypto assets succeeded in making their name known to large masses with Bitcoin, which emerged as a result of the creation of the first genesis block in 2008. Until 2010, the aforementioned recognition showed itself mostly in areas such as games, but over time it managed to enter the portfolios of individual investors. Especially as of end of 2017, the rapid increases in monetary value quickly attracted the attention of corporate companies and then the (Central Banks). These assets have created different alternatives (also know as altcoins) by working and have managed to become one of the important financial instruments today. This study has examined in detail the techniques (Chaos theory, Onchain analysis and Sentiment analysis) developed on the price predictions of crypto assets, which are very important in terms of the number and quality of investors. In the study, findings were obtained that new techniques such as onchain and sentiment are more prominent in estimating crypto asset prices compared to traditional asset price estimation methods of crypto assets and that these techniques can make consistent estimations.

Background and Aims: The homozygous PiZ mutation (PIZZ genotype) constitutes the predominant cause of severe alpha‐1 antitrypsin (AAT) deficiency and leads to liver disease via hepatocellular AAT aggregation. We systematically analysed the composition of AAT aggregates and studied the impact of bile acids. Methods: AAT inclusions were isolated from livers of PiZ overexpressing mice and PIZZ humans via fluorescence‐activated and immunomagnetic sorting (FACS/MACS), while insoluble proteins were obtained via Triton‐X extraction. Inclusion composition was evaluated through mass‐spectrometry (MS), immunoblotting and immunostaining. Hepatocytes with versus without AAT aggregates were obtained via microdissection. Serum bile acids were assessed in 57 PIZZ subjects and 19 controls. Mice were administered 2% cholic acid (CA)–supplemented chow for 7 days. Results: MS identified the key endoplasmic reticulum chaperone 78 kDa glucose‐regulated protein (GRP78) in FACS/MACS pulldowns. GRP78 was also enriched in insoluble fractions from PiZ mice versus wild types and detected in insoluble fractions/MACS isolates from PIZZ liver explants. In cultured cells/primary hepatocytes, PiZ overexpression was associated with increased GRP78 mRNA/protein levels. In human livers, hepatocytes with AAT aggregates had higher GRP78 levels than hepatocytes without. PIZZ subjects displayed higher serum bile acid levels than controls and the highest levels were seen in individuals with liver injury/fibrosis. In PiZ mice, CA‐mediated bile acid challenge resulted in increased liver injury and translocation of GRP78 into the aggregates. Conclusions: Our results demonstrate that GRP78 is sequestered within AAT inclusions. Bile acid accumulation, as seen in PIZZ subjects with liver disease, may promote GRP78 segregation and thereby augment liver damage. Trial Registration: NCT02929940 [ABSTRACT FROM AUTHOR]

Background: Low-grade appendiceal mucinous neoplasms (LAMNs) are rare lesions of the vermiform appendix and characterized by mucinous epithelial proliferation, extracellular mucin, and the absence of destructive invasion. Appendiceal diverticulum (AD) is also an uncommon condition that may be challenging to differentiate from acute appendicitis when it is superimposed by diverticulitis or perforation. Some recently published studies emphasized that complicated AD with mucosal hyperplasia can be confused with LAMNs, leading to overdiagnosis. The present study aimed to determine the histopathological features which can be used in the differential diagnosis of LAMNs and ADs, particularly complicated diverticula, in a large cohort. Methods: Cases comprising LAMNs and ADs diagnosed between 2011 and 2021 were included in the study. All cases were evaluated for the epithelial lining, the wall of the lesions, and the presence of cellular or acellular mucin, with its localization in terms of level and site of involvement within the appendix also recorded. Results: The hypermucinous epithelium characteristic of LAMNs, fibrosis, and calcification in the wall and the absence of lamina propria and muscularis mucosa proved to be the most discriminatory features in the differential diagnosis of LAMNs and ADs. Conclusions: The distinction between mucinous neoplasia and its mimics is critically important, since mucinous neoplasia requires surveillance imaging and potential surgery or chemotherapy depending on the extent of the disease, whereas non-neoplastic lesions are treated by an appendectomy and require no future intervention or surveillance. Objective: To examine the SCUBE1 level, a biomarker in vascular biology that could determine the prognosis of cardiovascular events during OSA treatment. Methods: In total, 129 patients were included in the study. Thirty were diagnosed with simple snoring and 99 with OSA. Results: In males, significant correlation was determined between SCUBE1 non-REM AHI, hypopnea index, total apnea index, mean SO2, minimum SO2, and < 90% saturation duration. Conclusion: Serum SCUBE1 levels increased more in male patients with severe OSA compared to other OSA levels, and high serum SCUBE1 levels were found to be associated with lower oxygen levels in OSA patients. The SCUBE1 biomarker can correlate with severe OSA in males. There was a statistically significant difference between OSA groups in terms of SCUBE1 score for male patients (p = 0.002) but not for females (p = 0.498). In this study, a two-stage approach for developing a Semantic-Based Image Retrieval system supported by Ontology is proposed. In the initial stage, the Object Detection process is employed to identify objects within the image. Subsequently, a predicate describing the relationship between these two objects is determined using the developed Bi-directional Recurrent Neural Network (Bi-RNN) model. In the second stage, relations defined in the form of are transformed into Ontologies and utilized to search for images that are semantically similar. In addressing the primary challenge of Semantic Gap within the Semantic-Based Image Retrieval approach, the proposed solution involves measuring the number of similar relationships between two images through the utilization of entropy. The Semantic Gap between two images was computed using the Joint Entropy method, leveraging the number of relationships (X) identified in the query image and the total number of relationships (Y) in the image with similar relationships obtained as a query result. The proposed approach exhibits characteristics of a novel method within this field, distinct from other similar methods employed in Semantic-Based Image Retrieval through the utilization of Ontologies. In the performance measurement of the developed model, 91% accuracy was obtained according to the Recall@100 (Top-5 accuracy) result. [ABSTRACT FROM AUTHOR]
