Proprioception has long been linked with emotional dysregulation in neurotypical adults. Neuropediatric disorders such as autism spectrum disorder (ASD) and cerebral palsy (CP) are distinct entities and yet both present with deficits and challenges in sensory processing and the regulation of emotions. This study aimed to explore the relationship between proprioception and emotional–social performance in children and to compare proprioception and emotional–social performance in different underlying neurodevelopmental conditions. For this purpose, this cross-sectional study included 42 children with ASD, 34 children with CP and 50 typically developing peers. Proprioceptive acuity, proprioceptive reactive behavior as well as emotion regulation and social responsiveness were assessed. The results show a significant correlation between proprioceptive deficits and emotional difficulties in this pediatric sample, with distinct proprioceptive impairment patterns according to the underlying neurological disorder. Children with CP showed significant emotional knowledge deficits, while children with ASD predominantly showed challenges in social responsiveness. These data thus suggest a differentiated impact of proprioception on emotional–social performance in neurodevelopmental disorders and highlight proprioception as a potential therapeutic target for balancing emotion regulation in children with neurodevelopmental conditions.

Abstract Background Lumbar back pain in an adolescent is generally musculoskeletal, that is, due to a muscle strain or scoliosis. This case describes a young individual without any previous health issues who rapidly developed a life-threatening condition, though initially presenting with “only” back pain. Case presentation A 16-year-old Caucasian male patient was admitted twice to the emergency department with debilitating lumbar pain without neurological or vascular symptoms. Imagery showed an extensive thrombosis of the inferior vena cava. No external cause, structural abnormality, or any systemic disease were found that predisposed the patient to this highly unusual vaso-occlusive incident. Conclusion Thrombosis of the inferior vena cava is a rare but life-threatening entity. It is underrecognized and associated with serious short- and long-term morbidity and mortality. Increased awareness is essential because deep vein thrombosis in children seems to cause atypical symptoms, such as spinal pain or the absence of edema of the lower limbs, as in the present case.

Africa is rich with an abundance of renewable energy sources that can help meeting the continent’s demand for electricity to promote economic growth and meet global targets for CO2 reduction. Green Hydrogen is considered one of the most promising technologies for energy generation, transportation, and storage. In this paper, the prospects of green hydrogen production potential in Africa are investigated along with its usage for future implementation. Moreover, an overview of the benefits of shifting to green Hydrogen technology is presented. The current African infrastructure and policies are tested against future targets and goals. Furthermore, the study embraces a detailed theoretical, environmental, technological, and economic assessment putting the local energy demands into consideration.

Solar energy has become crucial in providing the world with an opportunity to reduce its carbon footprint, as well as fighting the consequences of climate change. Furthermore, Africa is at the forefront of the regions affected by climate change with a future expectancy of worsening consequences, in addition to being a region with various electricity complications, energy demands, and global greenhouse emissions contributions. Therefore, not only is Africa in major need of adopting efficient renewable energy technologies for their apparent future concerns but also Africa is already considered a region that has significant solar energy potential due to its abundant natural and renewable resources. Solar energy’s potential in Africa could not only be a solution to many of the continent’s complications, but it could also help the continent’s economy thrive The potential of solar energy in Africa as well as techniques to efficiently implement it has piqued literary interest a lot recently however, this paper aims to highlight through literature not only the potential and the technologies available for adoption but also the challenges complicating the implementation. This paper is split into four literary sections, first section puts forward the challenges facing Africa in the uptake of solar energy technologies including financial, technological, human resources, and environmental challenges. Then, in-depth discussions of the various solar energy technologies for potential adoption. Third, the biggest solar energy projects implemented in Africa are put forward. Finally, a thorough study of the aforementioned challenges, their current state, and the actions required to overcome them is presented in order to fully realize the potential of solar energy deployment across the continent.

IntroductionAutistic children may have abnormal sensory perception, emotion dysregulation and behavior problems. The aim of this cross-sectional study was to explore the relationship between skin-mediated somatosensory signals and emotion/behavior difficulties in autistic children and adolescents, in comparison typically developing peers (TDP).MethodsThirty-eight autistic children and adolescents and 34 TDP completed a multidimensional assessment consisting of the measurement of somatosensory thresholds of touch, pain and temperature, a task on emotion knowledge and parent-reported questionnaires on sensory reactivity, emotion regulation and behavior.ResultsAutistic children had higher pain sensitivity, less sensory reactive behaviors and more behavior problems than their TDP. In contrast to TDP, several somatosensory thresholds of autistic children correlated with emotion regulation and behavior problems.DiscussionSensory dysfunction may affect the development of emotional processing and behavior in autistic children and adolescents. This knowledge can lay the foundation for future studies on co-occurring alterations in corresponding neural networks and for the implementation of early interventions, including sensory rehabilitation therapy, for promoting regulated behaviors in autistic children and adolescents.

Abstract Background Obesity is caused by the enlargement of the white adipose tissue (WAT) depots, characterized by the hypertrophic enlargement of malfunctioning adipocytes within WAT which increases the storage of triglycerides (TG) in the lipid droplets (LD). Adipogenesis pathways as well as the expression and activity of some extracellular matrix receptors integrins are upregulated. Integrinβ1 (INTB1) is the main isoform involved in WAT remodeling during obesity and insulin resistance-related diseases. We recently described Integrin Linked Kinase (ILK), a scaffold protein recruited by INTB1, as an important mediator of WAT remodeling and insulin resistance. As the few approved drugs to fight obesity have brought long-term cardiovascular side effects and given that the consideration of INTB1 and/or ILK modulation as anti-obesogenic strategies remains unexplored, we aimed to evaluate the anti-obesogenic capacity of the clinically approved anticoagulant Tirofiban (TF), stated in preclinical studies as a cardiovascular protector. Methods Fully differentiated adipocytes originating from C3H10T1/2 were exposed to TF and were co-treated with specific INTB1 blockers or with siRNA-based knockdown ILK expression. Lipid-specific dyes were used to determine the TG content in LD. The genetic expression pattern of ILK, pro-inflammatory cytokines (MCP1, IL6), adipogenesis (PPARγ, Leptin), thermogenesis (UCP1), proliferation (PCNA), lipid metabolism (FASN, HSL, ATGL), and metabolite transporters (FABP4, FAT, AQP7) were detected using quantitative PCR. Cytoskeletal actin polymerization was detected by confocal microscopy. Immunoblotting was performed to detect INTB1 phosphorylation at Thr788/9 and ILK activity as phosphorylation levels of protein kinase B (AKT) in Ser473 and glycogen synthase kinase 3β (GSK3β) at Ser9. TF was intraperitoneally administered once per day to wildtype and ILK knockdown mice (cKDILK) challenged with a high-fat diet (HFD) or control diet (STD) for 2 weeks. Body and WAT weight gains were compared. The expression of ILK and other markers was determined in the visceral epididymal (epi) and inguinal subcutaneous (sc) WAT. Results TF reduced TG content and the expression of adipogenesis markers and transporters in adipocytes, while UCP-1 expression was increased and the expression of lipases, cytokines or PCNA was not affected. Mechanistically, TF rapidly increased and faded the intracellular phosphorylation of INTB1 but not AKT or GSK3β. F-actin levels were rapidly decreased, and INTB1 blockade avoided the TF effect. After 24 h, ILK expression and phosphorylation rates of AKT and GSK3β were upregulated, while ILK silencing increased TG content. INTB1 blockade and ILK silencing avoided TF effects on the TG content and the transcriptional expression of PPARγ and UCP1. In HFD-challenged mice, the systemic administration of TF for several days reduced the weight gain on WAT depots. TF reduced adipogenesis and pro-inflammatory biomarkers and increased lipolysis markers HSL and FAT in epiWAT from HFD, while increased UCP1 in scWAT. In both WATs, TF upregulated ILK expression and activity, while no changes were observed in other tissues. In HFD-fed cKDILK, the blunted ILK in epiWAT worsened weight gain and avoided the anti-obesogenic effect of in vivo TF administration. Conclusions ILK downregulation in WAT can be considered a biomarker of obesity establishment. Via an INTB1-ILK axis, TF restores malfunctioning hypertrophied WAT by changing the expression of adipocyte-related genes, increasing ILK expression and activity, and reducing TG storage. TF prevents obesity, a property to be added to its anticoagulant and cardiovascular protective advantages.

Due to their early brain lesion, children with unilateral spastic cerebral palsy (USCP) present important changes in brain gray and white matter, often manifested by perturbed sensorimotor functions. We predicted that type and side of the lesion could influence the microstructure of white matter tracts. Using diffusion tensor imaging in 40 children with USCP, we investigated optic radiation (OR) characteristics: fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity (AD) and radial diffusivity (RD). First, we compared the OR of the lesional and non-lesional hemisphere. Then we evaluated the impact of the brain lesion type (periventricular or cortico-subcortical) and side in the differences observed in the lesional and non-lesional OR. Additionally, we examined the relationship between OR characteristics and performance of a visuospatial attention task. We observed alterations in the OR of children with USCP on the lesional hemisphere compared with the non-lesional hemisphere in the FA, MD and RD. These differences were influenced by the type of lesion and by the side of the lesion. A correlation was also observed between FA, MD and RD and the visuospatial assessment mainly in children with periventricular and right lesions. Our results indicate an important role of the timing and side of the lesion in the resulting features of these children’s OR and probably in the compensation resulting from neuroplastic changes.

Abstract Introduction Obesity is a common chronic disease, representing a major health hazard. Obesity has several delirious effects on the respiratory functions. Aim of the study To study the effect of obesity on pulmonary functions among our local population of obese adults and to assess the correlation between the severity of lung function impairment and the degree of obesity. Patients and methods Healthy nonsmoker adult patients were recruited in our cross-sectional study. After full medical evaluation, measurement of height and weight, and calculation of BMI, patients underwent spirometry tests with measurement of forced expiratory volume in first second (FEV1), forced vital capacity (FVC), forced mid-expiratory flow, and peak expiratory flow rate. Then, they were classified according to their BMI into five groups. Results The study included 293 patients divided into five groups according to their BMI. Significant statistical differences were noticed between nonobese patients and patients with classes II and III obesity regarding FVC, FEV1, and forced mid-expiratory flow, but no differences regarding peak expiratory flow rate and FEV1/FVC ratio. Overall, 28.9% of the total obese patients presented with restrictive pattern of spirometry, 2.8% with obstructive, and 2.4% with mixed patterns. Conclusion Obesity of especially marked degrees with BMI of more than 35 kg/m2 negatively affects the spirometric parameters. Restrictive pattern was the commonest abnormality observed in the spirometry of obese patients.

Background Respiratory viral and atypical bacterial infections data in Egyptian patients are sparse. This study described the epidemiological pattern of viral and atypical bacteria as causes for severe acute respiratory infections (SARI) in hospitalized patients in Egypt. Patients and methods SARI surveillance was carried out at a Teaching University Hospital during the period 2010–2014. All hospitalized adults and pediatric patients meeting the WHO case definition criteria for SARI were enrolled. Nasopharyngeal/oropharyngeal swabs were collected and samples were tested using reverse transcription-PCR for influenza A, B, respiratory syncytial virus, human metapneumovirus, parainfluenza viruses 1, 2, 3, 4, adenovirus, human bocavirus, coronavirus, enterovirus, rhinovirus, and atypical bacteria (Mycoplasma spp., Chlamydia spp., and Legionella spp.). Results Overall, 1075/3207 (33.5%) cases had a viral etiology, and included 912 (84.4%) women and 163 (15.6%) men, with a mean age of 5.74±13.87 years. The highest rates were reported for respiratory syncytial virus (485 cases, 45.2%), parainfluenza virus (125, 11.6%), and adenovirus (105, 9.8%). Single viral etiology was reported in 901 (83.3%), while 174 (16.7%) cases had multiple etiologies. Children had a higher rate (981, 91.2%) compared with 94 (8.8%) cases in adults. Only three and one cases were positive for Mycoplasma spp. and Chlamydia spp. infections, respectively. Neither coronavirus nor Legionella spp. were detected. Conclusion Viral infections were encountered in one-third of hospitalized Egyptian adult and pediatric patients with SARI. Atypical bacteria had a minor role in SARI in our locality. Ongoing surveillance programs will better describe the epidemiology of SARI and will provide specific data to enable decision makers to take appropriate prevention measures.

Aim. This observational study aimed at assessing the prevalence of visuospatial attention deficits in children with unilateral spastic cerebral palsy (USCP), taking into consideration the affected hemibody and the localization of the brain lesion. Method. Seventy-five children with USCP were assessed with four visuospatial attention tests: star cancellation, Ogden figure copy, line bisection, and proprioceptive pointing. Results. A majority (64%) of children with USCP presented a deficit in at least one test compared to the reference values. The alterations observed in children with left or right USCP were related to egocentric or allocentric neglect, respectively. Children with cortico/subcortical lesion presented more often visuospatial attention deficits than children with periventricular lesion. Visuospatial attention deficits were not associated with brain lesion locations. Interpretation. Visuospatial attention deficits are prevalent in children with USCP and should be taken into account during their rehabilitation process. The present results shed new light on the interpretation of motor impairments in children with USCP as they may be influenced by the frequent presence of visuospatial deficits.

The aim of the present study is to investigate the development of visuospatial attention in typically developing children and to propose reference values for children for the following six visuospatial attention tests: star cancellation, Ogden figure, reading test, line bisection, proprioceptive pointing and visuo-proprioceptive pointing. Data of 159 children attending primary or secondary school in the Fédération Wallonie Bruxelles (Belgium) were analyzed. Results showed that the children's performance on star cancellation, Ogden figure and reading test improved until the age of 13 years, whereas their performance on proprioceptive pointing, visuo-proprioceptive pointing and line bisection was stable with increasing age. These results suggest that the execution of different types of visuospatial attention tasks are not following the same developmental trajectories. This dissociation is strengthened by the lack of correlation observed between tests assessing egocentric and allocentric visuospatial attention, except for the star cancellation test (egocentric) and the Ogden figure copy (ego- and allocentric). Reference values are proposed that may be useful to examine children with clinical disorders of visuospatial attention.

Stroke is one of the leading causes for disability worldwide. Motor function deficits due to stroke affect the patients’ mobility, their limitation in daily life activities, their participation in society and their odds of returning to professional activities. All of these factors contribute to a low overall quality of life. Rehabilitation training is the most effective way to reduce motor impairments in stroke patients. This multiple systematic review focuses both on standard treatment methods and on innovating rehabilitation techniques used to promote upper extremity motor function in stroke patients. A total number of 5712 publications on stroke rehabilitation was systematically reviewed for relevance and quality with regards to upper extremity motor outcome. This procedure yielded 270 publications corresponding to the inclusion criteria of the systematic review. Recent technology-based interventions in stroke rehabilitation including non-invasive brain stimulation, robot-assisted training and virtual reality immersion are addressed. Finally, a decisional tree based on evidence from the literature and characteristics of stroke patients is proposed.At present, the stroke rehabilitation field faces the challenge to tailor evidence-based treatment strategies to the needs of the individual stroke patient. Interventions can be combined in order to achieve the maximal motor function recovery for each patient. Though the efficacy of some interventions may be under debate, motor skill learning and some new technological approaches give promising outcome prognosis in stroke motor rehabilitation.

Children with autism spectrum disorders (ASD) often display an abnormal reactivity to tactile stimuli, altered pain perception, and lower motor skills than healthy children. Nevertheless, these motor and sensory deficits have been mostly assessed by using clinical observation and self-report questionnaires. The present study aims to explore somatosensory and motor function in children with ASD by using standardized and objective testing procedures. Methods. Tactile and pressure pain thresholds in hands and lips, stereognosis, proprioception, and fine motor performance of the upper limbs were assessed in high-functioning children with ASD (n=27) and compared with typically developing peers (n=30). Results. Children with ASD showed increased pain sensitivity, increased touch sensitivity in C-tactile afferents innervated areas, and diminished fine motor performance and proprioception compared to healthy children. No group differences were observed for stereognosis. Conclusion. Increased pain sensitivity and increased touch sensitivity in areas classically related to affective touch (C-tactile afferents innervated areas) may explain typical avoiding behaviors associated with hypersensitivity. Both sensory and motor impairments should be assessed and treated in children with ASD.

Background: Low back Myofascial pain syndrome is a disorder triggering pain at myofascial trigger points and appears to be the most frequent source of pain in clinical practice. Active release technique and myofascial release both help in improving low back myofascial pain. Purpose: This study aimed to compare the effect of active release against trigger point release technique in low back myofascial pain syndrome. Methods: Forty-five male and female participants with low back myofascial pain syndrome participated in the current study had been divided into three groups randomly. Group A obtained active release technique for myofascial trigger points of Iliocostalis Lambourum and Gluteus Medius muscle, and traditional physiotherapy. Group B obtained myofascial release for trigger point of Iliocostalis and Gletus Medius, and traditional physiotherapy. Group C (control group) obtained traditional physiotherapy only in the form of stretching and strengthening exercises. The treatment was conducted two sessions per week for one month. The Wagner FPX25 digital Algometer was utilized to evaluate pain pressure threshold and Ronald Morris disability Questionnaire to measure function disability of back. It is a self-reported questionnaire. Results: no substantial difference has been detected among groups pre-treatment (p > 0.05). Comparison among groups, post treatment, showed a substantial improvement in the PPT of trigger point of Iliocostalis and Gluteus Medius muscle and a significant decrease of Ronald Morris disability Questionnaire score in group A as compared to that of group B (p > 0.01) and group C (p > 0.01). Conclusion: The active release technique improves tolerance of trigger points of Iliocostalis and Gluteus Medius more than myofascial release and improves back function more than myofascial release in low back myofascial pain syndrome. OBJETIVO: Avaliar a correlação e concordância entre medidas ecocardiográficas das dimensões cardíacas, obtidas através do aplicativo Echo off-line (programa para obtenção de medidas de imagens digitalizadas em estação de trabalho dedicada), com as realizadas convencionalmente. MÉTODOS: Estudo transversal, contemporâneo, sendo randomizados 56 pacientes. Através do programa Echo off-line foram mensuradas as medidas ao modo M e 2D ao nível dos ventrículos, do átrio esquerdo e da aorta. Estas medidas foram comparadas às realizadas por outro profissional, através do teste de correlação de Pearson (r), com alfa crítico de 0,05 e pela análise de concordância (Bland e Altman). RESULTADOS: As mensurações realizadas no sistema Echo off-line demonstraram r de 0,85 a 0,98. A análise de concordância mostrou que, para a maioria das medidas, a diferença média entre os métodos foi aproximadamente zero. A variação de valores absolutos não apresentou, em média, significância clínica. O aplicativo Echo off-line permite uma redução de, aproximadamente, 30% no tempo para realização das medidas. CONCLUSÃO: Este trabalho demonstrou a acurácia do programa Echo off-line para mensurar as dimensões cardíacas em estação de trabalho dedicada, podendo ser utilizado rotineiramente nos laboratórios de ecocardiografia.OBJECTIVE: To evaluate the correlation and concordance between the measurements of echocardiographic analysis of cardiac dimensions obtained through the Echo off-line applicative (software for obtaining digitized image measurements in a dedicated workstation) available to public domain, and those obtained through the conventional method. METHOD: Transversal contemporary study, of 56 randomized patients whose images were obtained during examina-tions. The measurements of the M mode and 2D, were done in the ventricles, left atrium, and aorta by the Echo off-line program. These measurements were compared to those obtained by another professional, through Pearson's correlation test (r), alpha = 0.05 and by concordance analysis (Bland and Altman). RESULT: The measurements carried out by the Echo off-line system showed r that varied from 0.85 to 0.98. The analysis of concordance showed that for most measu-rements, the mean difference between the methods was approximately zero. The variation of absolute values did not show, in average, a clinical significance. The Echo off-line applicative allows a reduction of approximately 30% in the time spent to obtain the measurements. CONCLUSION: This study demonstrated the accuracy of the Echo off-line program to measure cardiac dimensions in a dedicated workstation, showing that it can be routinely used in echocardiography labs.
