Over the last decade, the issue of treatment of acute necrotizing pancreatitis has been requiring special attention because of the growth of this pathology worldwide and high mortality from it, especially with extended infected types of the disease. The aim of the research was to improve results of treatment of patients with nonbiliary necrotizing pancreatitis by optimizing of the surgical tactics, which depend on the extent of necrosis in the pancreas and peripancreatic fat. Materials and methods. The study analyzed results of treatment of 220 patients with acute nonbiliary necrotizing pancreatitis, who were hospitalized in the clinic from 2014 to 2016. The age of the patients ranged from 18 to 80 years, including patients under the age of 50, who accounted for 54 %. Comparison of qualitative features was carried out through analysis of conjugacy tables, using the χ² Pearson criterion (Pearson Chi-square). For four-field tables (for 1 degree of freedom), the Yates's correction was used. When comparing the groups, the level of statistical significance (P) was calculated. At P < 0.05, the differences were considered statistically significant. Results. The research argues that mortality depends not only on the extent of pancreatic necrosis, but also on the choice of method of surgical intervention. Use of puncture-draining operations under ultrasound control and lumbotomy made it possible to reduce postoperative mortality from lesser sac abscess from 6.5 % to 3.9 %, from retroperitoneal phlegmon – from 31.6 % to 15.8 %, and from their combination – from 60 % to 33.3 %. In cases of diffuse purulent peritonitis, mortality from laparostomy combined with the programmed sanitation of the abdominal cavity was 40 %; it included cases when carbapenem antibiotics (imipenem/cilastatin, and meropenem) were used in the complex treatment, accounting for 33.3 %, and cases of implementing laparostomy, sanitation, and drainage of the abdominal cavity, making 50 %. Conclusions. The more extensive use of puncture-draining operations under ultrasound control and lumbotomy, as well as laparostomy in combination with the programmed sanitation of the abdominal cavity, allowed to reduce mortality from 26.1 % to 12.1 %.

The aim of the study was to conduct spatio-temporal retrospective and operational genetic analysis of the tularemia agent population in the Rostov Region, Donetsk People’s Republic (DPR), Lugansk PR and Ukraine in the period of 1943–2023. Materials and methods. We used Francisella tularensis strains isolated in the Rostov Region (RR), DPR, LPR and Ukraine between 1943 and 2023. VNTR genotyping was performed by 5 loci. Cluster analysis and construction of a phylogenetic tree were carried out using the GrapeTree software (NJ and MST algorithms). Whole-genome sequencing was performed on the MiSeq Illumina platform. The assembly of genomes in the form of reads was carried out using the Spades program. Results and discussion. A comparative analysis of the VNTR genotypes in 72 F. tularensis strains has been conducted: 35 strains isolated from various natural foci of Russia and the Rostov Region in particular from 1928 to 2023, 11 strains from the DPR and LPR (2022–2023) and 26 strains from the collection of the Rostov-on-Don Anti-Plague Institute, isolated on the territory of Ukraine from 1949 to 1990. In the studied strains, 20 individual genotypes have been identified, distributed among three clonal complexes (CC1 – CC3). Molecular-genetic analysis of F. tularensis strains has made it possible to identify three clonal complexes of tularemia agent (CC1 – CC3), circula ting in the studied areas. All of them are present on the territory of the RR and only complex CC2 shows a geographic confidedness to the southern areas of the RR. Strains isolated in western Ukraine before the 1990s belong exclusively to the CC3 complex. Judging by the range of distribution and diversity of genotypes, the population of strains of the CC3 and CC1 complexes has been existing for a long time in the studied areas. Strains of the same genotype can be found in endemic areas for up to 80 years and at a distance of about 1000 km from each other. On the contrary, the population of the CC2 complex is the most recent in time of origin and is geographically limited to the territory of RR. Geographic data on the origin of the CC1 and CC3 progenitor clones of the tularemia agent clonal complexes suggest an expansion of the genetic diversity of tularemia agent population from east to west.

Background. The study of the phylogeography of the tularemia microbe is not only practical, but also of scientific interest, as it allows us to evaluate the evolutionary processes of Francisella tularensis in modern conditions.Aim. Phylogenetic analysis of strains of the causative agent of tularemia in the Rostov region and identification of erythromycin-sensitive genovariants.Results. A representative collection of 122 cultures, isolated from various sources from 1945 to 2022, was studied. As it was established, all the studied strains of the causative agent of tularemia had typical cultural, morphological, and biological characteristics. The results of the study of bacteria antibiograms showed that all strains were resistant to β-lactams, polymyxin, clindamycin, and sensitive to aminoglycosides, rifampicin, and fluoroquinolones. In relation to erythromycin, only 2 erythromycin-sensitive strains were found. Phylogenetic analysis of 34 European and Russian strains of F. tularensis made it possible to determine whether these strains belong to subgroup B.7 of the main genetic group B.6.Conclusion. For the first time, in the Tselinsky district of the Rostov region, the presence of «atypical» genovariants of the tularemia microbe belonging to subgroup B.7 of the main group B.6 of the subspecies holarctica was registered, according to the modern scheme of genetic typing of F. tularensis subsp. holarctica.

Six cultures of tularemia microbe from fallen and captured live animals were isolated during epizootiological monitoring in the steppe focus in the south-east of the Rostov Region in 2020 against the background of extensive epizootics in the populations of the common vole Microtus arvalis obscurus and the public vole Microtus socialis.The aim of the work was to develop an SNP-typing scheme and to conduct a comparative study of the phylogenetic relations between Francisella tularensis strains isolated in the Rostov Region (2020) and strains from other regions.Materials and methods. Genome-wide sequencing was performed on the MiSeq Illumina platform. The author’s software GeneExpert, PrimerM and VirtualPCR, written in the Java programming language, were used for the analysis.Results and discussion. The strains of tularemia agent, isolated on the territory of the Rostov Region in 2020, can be allocated to two different clusters. It is established that two strains of tularemia pathogen (F0884 and F0889) isolated in Turkey are genetically close to some isolates circulating in the Rostov Region. A unique INDEL marker characteristic of this group of strains has been identified. The comparison of our proposed typing scheme with the scheme of “canonical” SNPs has showed a fairly good consistency and convergence of results within large clusters, meanwhile using a set of 6626 SNPs allows for differentiating the strains within one canSNP type. It is revealed that the vaccine strain has a common canSNP type with clinical and natural strains. A set of SNP markers has been selected for comparative analysis. A new INDEL marker that enables intraspecific typing of F. tularensis has been discovered and the possibility of its application in vitro and in silico has been comfirmed.

Introduction Motivation for treatment is an important socio-psychological characteristic of patients, which is subject to the joint influence of various factors, each of which may require specific rehabilitation interventions. Objectives To analyze and evaluate the cumulative influence of adverse childhood experiences (ACE), internal stigma, social characteristics on the intensity of treatment motivation in patients with mental disorders. Methods 102 patients with mental disorders were examined using Adverse Childhood Experience Questionnaire (ACEQ), Russian-language validated Internalized Stigma of Mental Illness (ISMI) scale and Treatment Motivation Assessment Questionnaire (TMAQ). Results As a result of regression analysis (table 1), a model was obtained that predicted an increase in the chances of high patient’s motivation for treatment with an increase in the total score of ACEs (ACEQ total score) and with higher education. The overall severity of internal stigma (ISMI total score) did not show a significant effect on the chances of developing intense motivation in patients. Table 1. The role of social and psychological characteristics of patients with mental disorders in the psychology of the treatment process should be considered systemically, rather than discretely. Introduction It is known that many psychopharmacological drugs have anti-inflammatory, as well as antibacterial and antiviral effects. Objectives To investigate the association between the severity and duration criteria of COVID-19 with psychopharmacotherapy in double-diagnosed patients. Methods A total of 169 case histories from a specialized infectious psychiatric department (May 2020 to January 2021) were evaluated. Progression indicators of severe and mild COVID-19, along with the duration of persistent SARS-CoV-2 viral shedding, were assessed in correlation with the administration of antidepressants, antipsychotics, and acid sphingomyelinase inhibitors (FIASMA-active drugs). Results The use of any psychotropic agents was associated with a 0.9% increase in the risk of severe course of COVID-19 for each unit increase in the systemic inflammation index PLR, specifically in patients with intellectual disability (ICD-10 codes F70-79), when compared to patients with schizophrenia (ICD-10 codes F20-29): R²McF=0.138; AIC=181; χ²=25.8; df=9; p=0.002. High PLR values and the use of FIASMA-active drugs were associated with prolonged COVID-19 duration, while antidepressant therapy and elevated C-reactive protein levels were associated with a reduced predicted duration of viral shedding in 13.8% of variance: R²=0.0864; AIC=1299; F=5.2(3), p=0.002. Including the nosology of psychiatric disorders in the regression model increased the proportion of explained variance to 22.8%. Conclusions Thymoanaleptic therapy for individuals with psychiatric disorders may act as a protective factor against COVID-19. There is no evidence suggesting adverse effects of antipsychotics on the severity and duration of COVID-19. Further research is necessary to investigate the effects of FIASMA-active psychopharmacological agents within nosologically homogeneous groups. Disclosure of Interest None Declared

Introduction Personality neuroscience employs a broad range of methods to identify the neurobiological mechanisms of complex psychological phenomena. The role of the insula is often associated with its involvement in emotion processing. Objectives The study aims to identify the associations between neural activity in the anterior insula cortex (AInC) and self-stigma (or its components) in a scoping review. Methods We searched in PubMed (MEDLINE), PsychINFO, EMBASE via the Ovid platform through September 21st, 2022. Included studies had to use fMRI to assess neurophysiological markers in AInC, and to include a measure of association between fMRI results and a measure of self-stigma and/or its components as assessed by a scale or questionnaire in participants aged 18-65 y.o. The PRISMA-ScR checklist was used. Results After full-text screening 10 of 206 original researches were chosen for the final analysis (Table 1). Figure 1 illustrats the regions of local activity change maxima according to MNI-space coordinates based on the results of the included studies in the analysis. Neural activation in the regions of the AInC was positively associated with greater levels of social rejection sensitivity and other components of self-stigma in 9 studies. Reduced activity was observed in only one study (Lindner et al., PLoS One. 2014; 9(1): e85014) among highly self-stigmatized patients with schizophrenia. This finding may reflect a biological manifestation of deficits in self-awareness and affective processing in schizophrenia. Image: Conclusions Associations between neural activity changes in specific brain regions and levels of self-stigma and/or its components, as reported in included neuroimaging studies, have the potential to shed light on the neurobiological mechanisms underlying such a complex psychological phenomenon as stigma. Background. Helicobacter pylori-associated chronic gastritis (CG), being a very heterogeneous group, still does not have a divisin based on Helicobacter pylori (Hp) genotyping data, which could predict the clinical form of CG.The aim of the study is to search for the prevalence of the cagA gene or any allelic combination of the vacA gene, or stable combinations of cagA and any allelic combination of vacA genes in Hp isolates from patients with mild and severe CG, as well as patients with peptic ulcer disease (PUD).Methods. Hp isolates from gastrobiopsy specimens were genotyped for cagA and vacA allelic combinations (s1m1, s2m1, s1m2, s2m2). The difference in the occurrence of vacA allelic combinations was assessed by Mann–Whitney U test; the conjunction of cagA and vacA allelic combinations was assessed by the Spearman's rank correlation coefficient (rs).Results. The cagA gene was found in more than half of all cases, both in patients with ulcer and in patients with CG (mild and severe). The incidence of vacAs1m1 (the most virulent allelic combination) showed no significant differences in all forms of gastritis and in PUD; the correlation between cagA and vacAs1m1 was significant in all groups of patients, rs ranged from 0.57 to 0.72. In patients with mild CG, an abundance of non-virulent allelic combination vacAs2m2 was observed, which was significantly different from its occurrence both in patients with severe CG and in patients with ulcer; the joint occurrence of vacAs2m2 and cagA in patients with mild CG was chaotic (rs=-0.13; P=0.40).Conclusion. In mild CG, despite the absence of significant differences in cagA and vacAs1m1 (when compared with severe CG and ulcer disease), strains with a non-virulent allelic combination vacAs2m2 were significantly dominant; therefore, the detection of this particular allelic combination of vacA speaks in favor of a mild course of CG.

Objective of the study was to investigate biological properties and genetic characteristics of tularemia agent strains isolated from natural foci of the Rostov Region in 2020.Materials and methods. Field material from natural foci of the Rostov Region was examined by serological, bacteriological, biological, and molecular-genetic methods. Cultural-morphological, biochemical, antigenic and pathogenic properties of isolated cultures were studied. Protein profles were obtained through MALDI-TOF MS using mass spectrometer Autoflex speed III Bruker Daltonics and Flex Control of Biotyper software. The genetic characteristics of the strains were determined by VNTR and INDEL typing and SNP analysis.Results and discussion. Six strains of tularemia pathogen were isolated from mouse-like rodents using biological method. The investigation of their biological features and data of PCR analysis and INDEL typing with canonical markers showed that all strains are typical representatives of the Francisella tularensis subsp. holarctica biovar EryR. VNTR typing by six genetic loci revealed that all strains belong to four individual genotypes. The strain isolated in 2020 in the Salsky district was identical to the strain which was isolated in the same area in 1989. Based on the whole genome sequencing of two strains, we established that they are closest to the cultures isolated in Turkey (2009, 2012) and Khanty-Mansiysk (2013) by the studied set of SNP markers. Thus, we found that both identical (or closely related) clones of the tularemia agent and new strains with unique genotypes which previously were not described for the Rostov Region can circulate in natural foci of this region for a long period of time.

Relevance. If we reduce the treatment exclusively to the eradication of Helicobacter pylori (Hp), the question of monitoring the results of anti-Hp therapy is controversial, given the widely known data on the detection of Hp, including virulent strains of Hp, in most healthy individuals.The aim of the study: to track the dynamics of stomach colonization with various Hp strains in patients with gastric ulcer and duodenal ulcer (PUD) immediately after the use of standard three-component anti-Hp therapy (AHT) and 1.5–2 months after AHT.Methods. Genotyping of Hp strains was carried out by the VNTR method together with the determination of the cagA gene.Results. Assessment of the results of AHT in the form of «eradication of Hp — re-detection of Hp» (i. e., without taking the determination of Hp strains into account) showed less reliability in the differences than the «eradication — preservation of the strain or change of the strain» score in patients with PUD; in addition, the differences between the initial bacteriological picture and that observed immediately after AHT were on the verge of reliability, while the differences between the initial bacteriological picture and that observed after 1.5–2 months had a high degree of reliability. The number of eradications increased (eradication achieved immediately after AHT was not preserved, except for one) and the number of cagA-containing strains decreased (due to new eradication detected at late follow-up periods and due to a change in strains) in long-term follow-up in patients with PUD.Conclusion. Since all patients with PUD achieved clinical remission, which lasted for the next 1.5–2 months, the success of AHT should not be unambiguously associated with the eradication of Hp; the restoration of the organism's colonization resistance to Hp after a course of therapy is more likely.

Introduction The European Psychiatric Association (EPA) Summer School allows psychiatric trainees and early career psychiatrists (ECPs) from all over Europe to meet, network, and learn together. After the 2020 edition being cancelled due to COVID-19, the 10th edition in 2021 focused for the first time on research and was conducted remotely. Objectives To provide an overview and feedback about the first Virtual EPA Research Summer School as a new way to encourage international networking during COVID-19. Methods The School was organized by the EPA Secretary for Education, and 4 Faculty members. It started with a “breaking the ice session” one week before and then a two-days meeting on 23-24 September 2021 using an online video-platform. This was preceded by all the 21 participants (from 18 different countries) recording a short 4-minute video presentation, which was uploaded and shared with other participants and Faculty. Results Participants were divided on a voluntary basis into three working groups: 1) “Drug repurposing: overcoming challenges in pharmacoepidemiology” 2) “Psychopathological research in psychiatry”; 3) “How to conduct a cross-sectional survey?”. The Summer School program was composed of plenary sessions with lectures by the Faculty members, discussion sessions, and working groups time. At the end, each group presented a summary of the work done to the rest of the participants. Conclusions Although the remote format limits social interactions during the Summer School, overall participants’ high satisfaction and productivity indicate that not only online formats, but also the topic of research might be covered in future editions. Introduction Psychiatric patients often are self-stigmatized and hardly involve in the treatment. Objectives Associations of self-stigmatizing beliefs in psychiatric inpatients and their treatment motivation. Methods 63 inpatients; ICD-10: F2–65%, F3–13%, F4+F6–14%, F06–8%; mean age 34±13, illness duration 12±11 years. Treatment Motivation Assessment Questionnaire (TMAQ), Internalized Stigma of Mental Illness scale (ISMI); K-mean cluster analysis; dispersion analyses; p≤0.05. Results 18 patients of cluster 1 (C1) demonstrated explicit self-stigmatization. In comparison with 25 subjects from cluster 3 (C3) stigmatized patients (C1) had higher levels of overall ISMI scores (2.9±0.3) caused by alienation (3.1±0.5), stereotype endorsement (2.5±0.5), social withdrawal (2.7±0.4), and discrimination experience (2.7±0.4). 20 patients of cluster 2 (C2) had an implicit stigma. They were more self-stigmatized (ISMI score 2.7±0.3) in contrast with subjects from cluster 3 (1.9±0.2) due to a lower level of stigma resistance (C2: 3.8±0.5 and C3 3.1±0.6 – reverse scores). Patients with implicit self-stigma (C2) had the lowest intensity of treatment motivation (Z-scores -1.2±0.6) compering with others (C1 and C3) due to the lowest TMAQ factor 1 (reliance on own knowledge and skills to cope with the disorder: -1.0±0.6) and factor 4 (willingness to cooperate with doctor: -0.9±1.0). Differences between explicitly and implicitly stigmatized patients manifested also in lower TMAQ factor 3 for the second group (awareness of the psychological mechanism of maladaptation: -0.5±0.9). Conclusions Despite alienation, stereotype endorsement, social withdrawal, discrimination experience some patients could sustain stigma due to cooperation with doctors and reliance on their own knowledge and skills to cope with illness.

Introduction The health care workers have extremely high risks of adverse psychological reactions from COVID-19 pandemic. On the other hand, patients with respiratory syndromes face stigmatization due to their possible contagiousness of SARS-Cov-2. Objectives To study the association of behavior, psychological distress in health care workers, and their stigmatization attitudes to the patients. Methods The online-survey of 1800 health care workers performed during different lockdown periods in Russia: the first week and the last (30/Mar-5/Apr/20 and 4-10/May/20). The Psychological stress scale (PSM-25), modified Perceived devaluation-discrimination scale (Cronbach’s α=0.74) were used. Dispersion analysis with p-value=0.05 and Cohen’s d, Cramer’s V calculation (ES) performed. Results In the 2nd phase medical stuff more often wore masks (64% vs. 89%; χ2=98.7, p=0.000, df=1; ES=0.23) and gloves (30% vs. 57%; χ2=57.6, p=0.000, df=1; ES=0.18), continued perform hand hygiene (94-95%) and physical distancing (73-74%), but was restricted in most effective protective measure: self-isolation (49% vs. 36%; χ2=16.0, p=0.000, df=1; ES=1.0). The psychological stress levels decreased in the 2nd phase (ES=0.13), while the stigma levels (ES=0.33) increased. Physicians experienced more stress compared with nurses and paramedical personnel (ES=0.34; 0.64) but were less likely to stigmatize SARS-CoV-2 infected individuals (ES=0.43; 0.41). The highest rates of contacts with COVID-19 patients (83%) were reported by physicians (χ2=123.0; p = 0.00, df=4; ES=0.28). Conclusions Direct contact with SARS-Cov-2 is associated with a significant increase in stress among medical personnel. However, the stigmatizing reactions are not directly associated with the risks of infection and are most prevalent among nurses and paramedical personnel.

Introduction The psychological stress associated with the COVID-19 pandemic has a complex multifactorial nature. Objectives The aim: to evaluate the level of stress during the COVID-19 pandemic in the Russian-speaking population with(-out) affective disorders in different cities of residence. Methods The data obtained from an online survey of 4803 Russian-speaking respondents (age over 18) from March 30 to May 18, 2020. The survey included social, demographic and the history of affective disorders data. The anxiety distress level was evaluated with the Psychological Stress Measure (PSM-25). Objective: to study the infection of the adult and children’s population of the Rostov and Astrakhan regions by the bacteria Helicobacter pylori, to determine the frequency of H. pylori pathogenicity factors in different age groups of the population.Materials and methods: 118 adults and 112 children were examined. Te presence of H. pylori DNA and the pathogenicity factors of CagA and VacA in the biopsy material of the mucous membrane of the antrum of the stomach was determined by PCR.Results: a signifcantly smaller percentage of H. pylori–positive patients is characteristic of the child population. Different groups of non-steroidal anti-inflammatory remedies are analyzed in respect of its efficacy and safety. The information concerning gastric and intestinal sequelae is presented. The approaches in treatment of NSAI-gastropathy are shown in dependence of Helicobacter pylori involvement into the affection.

Creation of new liquid crystal displays and their adaptation for different external environments are impossible without correct diagnosing of wide range of electro-optical effects inherent to nematic, smectic and cholesteric liquid crystal materials. The modern universal display measuring complexes allow to solve this problem. Among different display measuring complexes those are wide used in the world for scientific centers and enterprises in Russia, Belorussia and Ukraine the complex CM-100, which has been developed in Institute of Semiconductor Phisics of NAS of Ukraine, is the most suitable.

The influence of variations in conductivity and external electric current variations in the lower atmosphere on DC electric field over a seismic region is investigated. The external current is formed with the occurrence of convective upward transport of charged aerosols and their gravitational sedimentation in the atmosphere. This effect is related with the occurrence of ionization source due to seismic-related emanation of radon and other radioactive elements into the lower atmosphere. An increase in atmosphere radioactivity level results in the appearance of additional sources of ionization, and altitude dependence of the ion formation rate is calculated. Ionization source varies the atmospheric conductivity and the external current through appearance of ions with equilibrium number density and their adhesion to aerosols. We have calculated the perturbation of conductivity and external electric current as a function of altitude. Variation of conductivity and external current in the lower atmosphere leads to a perturbation of electric current which flows in the global atmosphere-ionosphere circuit. Finally, perturbations of DC electric field both on the Earth's surface and in the ionosphere are estimated.

DC electric field in the ionosphere above seismically active regions can be formed in a process of external current insertion into the atmosphere-ionosphere electric circuit. This current arises as a result of convective upward transport of charged aerosols and their gravitational sedimentation. Aerosols are injected into the atmosphere by soil gases intensified in the zones of active faults. In general case the horizontal distribution of injected aerosols in such zones is asymmetric. In this report we propose the method for computation of DC electric field generated in the ionosphere and the atmosphere by external electric current with arbitrary spatial distribution. Oblique magnetic field and the conjugate ionosphere effects are taken into consideration.
