Abstract Background Investigator-initiated clinical studies (IITs) are crucial to generate reliable evidence that answers questions of day-to-day clinical practice. Many challenges make IITs a complex endeavour, for example, IITs often need to be multinational in order to recruit a sufficient number of patients. Recent studies highlighted that well-trained study personnel are a major factor to conduct such complex IITs successfully. As of today, however, no overview of the European training activities, requirements and career options for clinical study personnel exists. Methods To fill this knowledge gap, a survey was performed in all 11 member and observer countries of the European Clinical Research Infrastructure Network (ECRIN), using a standardised questionnaire. Three rounds of data collection were performed to maximize completeness and comparability of the received answers. The survey aimed to describe the landscape of academic training opportunities, to facilitate the exchange of expertise and experience among countries and to identify new fields of action. Results The survey found that training for Good Clinical Practice (GCP) and investigator training is offered in all but one country. A specific training for study nurses or study coordinators is also either provided or planned in ten out of eleven countries. A majority of countries train in monitoring and clinical pharmacovigilance and offer specific training for principal investigators but only few countries also train operators of clinical research organisations (CRO) or provide training for methodology and quality management systems (QMS). Minimal requirements for study-specific functions cover GCP in ten countries. Only three countries issued no requirements or recommendations regarding the continuous training of study personnel. Yet, only four countries developed a national strategy for training in clinical research and the career options for clinical researchers are still limited in the majority of countries. Conclusions There is a substantial and impressive investment in training and education of clinical research in the individual ECRIN countries. But so far, a systematic approach for (top-down) strategic and overarching considerations and cross-network exchange is missing. Exchange of available curricula and sets of core competencies between countries could be a starting point for improving the situation.

Abstract Background Dorsomedian nasopharyngeal masses with benign macroscopic appearance are frequently observed during retrograde nasopharyngoscopy, particularly in brachycephalic breeds, but are not well described. Hypothesis/Objectives To characterize these masses, assess their frequency, and identify the potential factors associated with their presence. Animals Dogs that underwent retrograde nasopharyngoscopy at a private hospital. Methods Medical records between November 2019 and October 2022 were retrospectively reviewed. Dogs were included if suitable nasopharynx images were available for review. Univariable and multivariable logistic regression analyses were performed to determine the factors associated with these masses. Results One‐hundred ninety‐eight dogs met the inclusion criteria of which 47.9% (95/198) had a dorsal nasopharyngeal mass. The masses measured 30% of the nasopharyngeal height in 64.2% (61/95), 28.4% (27/95), and 7.4% (7/95) of cases, respectively. Background The 'birthday effect' refers to increased mortality around birthdays, although evidence remains mixed, with some studies even indicating reduced mortality. While the effect has a minimal impact on overall mortality, lacks prevention potential, and faces methodological challenges, it offers seminal insights into psychogenic health effects. Methods Major causes of death were analyzed using data from the Swiss Federal Statistical Office (1987-2022), excluding leap years and cases with imputed dates. The relationship between day of birth and death was assessed using a Poisson count model with overdispersion, and mortality rates (MRs) per 100,000 deaths per year were calculated for ±182 days around birthdays. Mortality rate ratios (MRRs) compared these periods to a reference period. Cluster analyses explored mortality rate patterns across age and diagnostic groups. Age was not adjusted for in the Poisson model to avoid zero inflation, with bias from age-related mortality changes assessed separately through sensitivity analyses. Results In the analysis of 1,702,865 deaths over 27 years, an eight percent increase in birthday mortality was observed, equivalent to 22 additional deaths per 100,000 deaths per year. There were 889 excess deaths in the 28 days after the birthday compared to 206 in the 28 days before. Excess birthday mortality was particularly pronounced in men, unmarried individuals, and those with religious affiliations. In individuals aged 85 and older, a pre-birthday dip followed by post-birthday mortality excess was noted. Cardiovascular causes, suicides, and accidents had the highest birthday-related mortality, while diabetes, respiratory diseases, and other conditions showed varied pre- and post-birthday patterns. Conclusions Administrative and age-related biases were ruled out, suggesting the birthday effect is psychogenic. While behavior-linked causes like suicides and accidents had clear birthday patterns, other main causes of death also showed birthday-period effects. Further research, including psychometric and biomarker studies, is needed to elucidate the underlying mechanisms.

BackgroundMotor imagery (MI) has emerged as a promising therapeutic approach for Parkinson’s disease (PD). MI entails mentally rehearsing motor actions without executing them. This cognitive process has garnered attention due to its potential benefits in aiding motor function recovery in patients. The purpose of this review was to highlight the findings observed in motor symptoms, balance, gait, and quality of life.MethodsA literature search was carried out in Medline, Embase, Cochrane, and Physiotherapy Evidence Database (PEDro), from the first publication to February 2024. Studies with at least one keyword to PD and MI in the title were included.ResultsThe analysis included 53 studies out of the 262 identified. These comprised 12 randomized controlled trials (RCTs) with an average PEDro score of 6.6 out of 10, as well as 41 non-RCT studies. In this study, the removal of benzotriazole (BTA), a pervasive aquatic contaminant widely used for its anti-corrosion, UV-stabilizing, and antioxidant properties, by nanomagnetite, biochar, and nanomagnetite–biochar composite is investigated. Nanomagnetite and nanomagnetite–biochar composite were synthesized under anoxic conditions and tested for BTA removal efficiency at neutral pH under both oxic and anoxic conditions at different time scales. Within the short time scale (up to 8 h), the removal of BTA by nanomagnetite–biochar composite was shown to be due to BTA deprotonation by the nanomagnetite surface. Through proton liberation, Fe²⁺ is released in accordance with the reaction Fe₃O₄ + 2H⁺ → Fe₂O₃ + Fe²⁺ + H₂O, which likely influences BTA complexation and its possible redox degradation. On the longer time scale, biochar achieved higher removal efficiency: 50% BTA removed within 48 h, due to formation of a ternary complex with surface Ca2+ ions, or 75% BTA removed after HCl biochar acid wash followed by Ca2+ surface saturation. As BTA presents significant environmental risks due to its extensive industrial applications, the present study offers critical insights into the mechanisms of BTA removal by nanomagnetite–biochar composite, and highlights the potential of such materials for water treatment applications.

Les auteur·e·s démontrent les formations continues nécessaires pour que des psychologues soient apte à effectuer de telles expertises.

Many post-secondary institutions have implemented mandatory reporting (MR) requiring employees to report knowledge of sexual misconduct. Understanding employee perceptions (e.g., benefits/drawbacks) and experiences (e.g., reporting disclosure) is important given this responsibility. Accordingly, this study analyzes responses from a recent survey that polled faculty and staff concerning MR ("N" = 125). Findings indicate that most employees fall under a MR policy, are aware of their obligations, and will comply with them. While majority approval was evident for MR, employees also identified drawbacks (e.g., reduced victim autonomy). One in five employees in this sample have made a report. Additional perceptions, experiences, and attitudes toward MR varied, based on institutional-level factors and socio-demographic characteristics.

Fungi of the genus Aspergillus are widespread in the environment, where they produce large quantities of airborne conidia. Inhalation of Aspergillus spp. conidia in immunocompromised individuals can cause a wide spectrum of diseases, ranging from hypersensitivity responses to lethal invasive infections. Upon deposition in the lung epithelial surface, conidia encounter and interact with complex microbial communities that constitute the lung microbiota. The lung microbiota has been suggested to influence the establishment and growth of Aspergillus spp. in the human airways. However, the mechanisms underlying this interaction have not yet been sufficiently investigated. In this study, we aimed to enrich and isolate bacterial strains capable of inhibiting the germination and growth of A. fumigatus conidia from bronchoalveolar lavage fluid (BALF) samples of lung transplant recipients using a novel enrichment method. This method is based on a soft agar overlay plate assay in which bacteria are directly in contact with conidia, allowing inhibition to be readily observed during enrichment. We isolated a total of five clonal bacterial strains with identical genotypic fingerprints, as shown by random amplified polymorphic DNA PCR (RAPD–PCR). All strains were identified as Pseudomonas aeruginosa (strains b1–b5). The strains were able to inhibit the germination and growth of Aspergillus fumigatus in a soft agar confrontation assay, as well as in a germination multiplate assay. Moreover, when compared with ten P. aeruginosa strains isolated from expectoration through standard methods, no significant differences in inhibitory potential were observed. Additionally, we showed inhibition of A. fumigatus growth on Calu-3 cell culture monolayers. However, the isolated P. aeruginosa strains were shown to cause significant damage to the cell monolayers. Overall, although P. aeruginosa is a known opportunistic lung pathogen and antagonist of A. fumigatus, we validated this novel one-step enrichment approach for the isolation of bacterial strains antagonistic to A. fumigatus from BALF samples as a proof-of-concept. This opens up a new venue for the targeted enrichment of antagonistic bacterial strains against specific fungal pathogens.
