Abstract Gastrin is secreted following a rise in gastric pH, leading to gastric acid secretion. Sleeve gastrectomy (SG), a bariatric surgery where 80% of the gastric corpus is excised, presents a challenge for gastric pH homeostasis. Using histology, and single-cell RNA sequencing of the gastric epithelium in 12 women, we observed that SG is associated with an increase in a sub-population of acid-secreting parietal cells that overexpress respiratory enzymes and an increase in histamine-secreting enterochromaffin-like cells (ECLs). ECLs of SG-operated patients overexpressed genes coding for biosynthesis of neuropeptides and serotonin. Mathematical modeling showed that pH homeostasis by gastrin is analogous to non-linear proportional and integral control, that drives adaptation of the epithelium to acid-secretion demand. Quantitative model predictions were validated in patients. The results demonstrate human gastric epithelium remodeling following SG at the molecular and cellular levels, and more generally how trophic hormones enable robust adaptation of tissue function to meet physiological demand.

3. Prophylactic barrage laser in patients with previous vitreous loss during cataract surgery: a randomized clinical trial

Abstract Background One of the significant challenges that can arise during cataract surgery is the occurrence of posterior capsule rupture (PCR) and vitreous loss. It may result in different complications including cystoid macular edema, endophthalmitis, glaucoma, and considerably retinal detachment which consequently contributes to poor functional outcomes. Methods This study was a prospective double blinded randomized clinical trial including 42 patients with previous complicated cataract surgery including posterior capsular rupture and consequently vitreous loss that anterior vitrectomy was done for them. The patients were randomly categorized in to two groups by simple randomization. One group (25 patients) received sham laser and the other group (17 patients) received prophylactic 360° barrage laser 4 weeks after cataract surgery. After eighteen months, rate of retinal detachment was compared between two groups. Results In terms of gender, age, axial length, maturity, pseudophakia v/s aphakia, type of IOL and type of cataract surgery, there was no statistically significant difference between intervention and control groups. Distribution of RRD was clinically different between 2 groups (intervention and control group). Sandfly-borne Toscana virus (TOSV) is an enveloped tri-segmented negative single-strand RNA Phlebovirus. It is an emerging virus predominantly endemic in southwestern Europe and Northern Africa. Although TOSV infection is typically asymptomatic or results in mild febrile disease, it is neurovirulent and ranks among the three most common causes of summer meningitis in certain regions. Despite this clinical significance, our understanding of the molecular aspects and host factors regulating phlebovirus infection is limited. This study characterized the early steps of TOSV infection. Our findings reveal that two members of the Numb-associated kinases family of Ser/Thr kinases, namely adaptor-associated kinase 1 (AAK1) and cyclin G-associated kinase (GAK), play a role in regulating the early stages of TOSV entry. FDA-approved inhibitors targeting these kinases demonstrated significant inhibition of TOSV infection. This study suggests that AAK1 and GAK represent druggable targets for inhibiting TOSV infection and, potentially, related Phleboviruses.

Background and Aim: Kidney failure is a common and increasing problem in Iran and worldwide. Kidney transplantation is recognized as a preferred treatment method for patients with end-stage renal disease (ESRD). Machine learning, as one of the most valuable branches of artificial intelligence in the field of predicting patient outcomes or predicting various conditions in patients, has significant applications. The purpose of this research was to predict kidney transplant outcomes in patients using machine learning. Materials and Methods: Since CRISP is one of the strongest methodologies for implementing data mining projects, it was chosen as the working method. In order to identify the factors affecting the prediction of kidney transplant outcomes, a researcher-created checklist was sent to some of nephrologists nationwide to determine the importance of each factor. The results were analyzed and examined. Then, using Python language and different algorithms such as random forest, SVM, KNN, deep learning, and XGBoost the data was modeled. Results: The final model was multilabel, capable of predicting various kidney transplant outcomes, including rejection probability, diabetic reactions, malignant reactions, and patient rehospitalization. After modeling the input data features, the model was able to predict the four kidney transplant outcomes such as rejection, diabetes, malignancy and readmission with an error rate of less than 0.01. Conclusion: The high level of accuracy and precision of the random forest model demonstrates its strong predictive power for forecasting kidney transplant outcomes. In this study, the most influential factors contributing to patient susceptibility to the mentioned outcomes were identified. Using this machine learning-based system, it is possible to predict the probability of these outcomes occurring for new cases.

Introduction and purpose: The members of the Islamic Council contribute greatly to the enactment of elder laws. Therefore, the view of these people toward the elderly can play a decisive role in this matter. Therefore, the present study aimed to investigate the attitude of the representatives of the Islamic Council towards the elderly. Methods: This cross-sectional descriptive-analytical research was conducted on 274 representatives of the Islamic Council who were selected by the whole number method in 2018. The needed data were collected through interviews and questionnaires. The research tools in this study included demographic information questionnaires and Kogan's attitude questionnaire on aging. The data were analyzed in SPSS software (version 22) using correlation tests (Pearson, Spermman), independent t-test, and one-way ANOVA. Results: The mean age of the participants was 53.49±8.02 years. The majority of MPs (93.8%) were male. The mean score of the attitude towards the elderly was 145.20±15.05, and the mean score of the negative attitude towards the elderly among parliamentarians was higher (62.28±12.36). Abstract Background Genetic risk factors for chemotherapy-induced peripheral neuropathy (CIPN), a major dose-limiting side-effect of paclitaxel, are not well understood. Methods We performed a genome-wide association study (GWAS) in 183 paclitaxel-treated patients to identify genetic loci associated with CIPN assessed via comprehensive neuropathy phenotyping tools (patient-reported, clinical and neurological grading scales). Bioinformatic analyses including pathway enrichment and polygenic risk score analysis were used to identify mechanistic pathways of interest. Results In total, 77% of the cohort were classified with CIPN (n = 139), with moderate/severe neuropathy in 36%. GWAS was undertaken separately for the three measures of CIPN. GWAS of patient-reported CIPN identified 4 chromosomal regions that exceeded genome-wide significance (rs9846958, chromosome 3; rs117158921, chromosome 18; rs4560447, chromosome 4; rs200091415, chromosome 10). rs4560447 is located within a protein-coding gene, LIMCH1, associated with actin and neural development and expressed in the dorsal root ganglia (DRG). Abstract Demand forecasting is a key parameter to achieve optimal supply chain management at different levels. The basic metals industry is one of the most energy‐intensive industries in the electricity supply chain. The impossibility of large‐scale energy storage, reservation constraints, and its high costs, limitations on electricity transmission lines capacity, real‐time response to high priority strategic demands, and variety of energy rates at different hours and seasons are issues that challenge the electricity supply chain. A hybrid approach is presented in this paper to improve the accuracy of long‐term demand forecasting in the electricity supply chain. The proposed approach uses wavelet decomposition and long short‐term memory (LSTM) neural networks to produce new predictors for demand forecasting in case of defective data. The data used in this study consists of the recorded hourly demand of Espidan Iron Stone (EIS) company and data of Mobarakeh Steel (MS) company in Isfahan Province. The understudy time series includes a lot of interruptions due to non‐production of the factory or power outages and spikes which cause uncertainty that makes the forecasting more difficult in comparison with continuous time series. Multiple machine learning models including decision trees (DTs), boosted and bagged trees (BGTs), support vector regression (SVR) models, extreme learning machines (ELMs), and a LSTM neural network model are employed to evaluate the effectiveness of the proposed approach in this paper. A comparison of the results obtained using the hybrid proposed approach and training machine learning (ML) methods shows a notable reduction in forecasting error.

29. Differential Effects of Regulatory T Cells in the Meninges and Spinal Cord of Male and Female Mice with Neuropathic Pain

Immune cells play a critical role in promoting neuroinflammation and the development of neuropathic pain. However, some subsets of immune cells are essential for pain resolution. Among them are regulatory T cells (Tregs), a specialised subpopulation of T cells that limit excessive immune responses and preserve immune homeostasis. In this study, we utilised intrathecal adoptive transfer of activated Tregs in male and female mice after peripheral nerve injury to investigate Treg migration and whether Treg-mediated suppression of pain behaviours is associated with changes in peripheral immune cell populations in lymphoid and meningeal tissues and spinal microglial and astrocyte reactivity and phenotypes. Treatment with Tregs suppressed mechanical pain hypersensitivity and improved changes in exploratory behaviours after chronic constriction injury (CCI) of the sciatic nerve in both male and female mice. The injected Treg cells were detected in the choroid plexus and the pia mater and in peripheral lymphoid organs in both male and female recipient mice. Nonetheless, Treg treatment resulted in differential changes in meningeal and lymph node immune cell profiles in male and female mice. Moreover, in male mice, adoptive transfer of Tregs ameliorated the CCI-induced increase in microglia reactivity and inflammatory phenotypic shift, increasing M2-like phenotypic markers and attenuating astrocyte reactivity and neurotoxic astrocytes. Contrastingly, in CCI female mice, Treg injection increased astrocyte reactivity and neuroprotective astrocytes. These findings show that the adoptive transfer of Tregs modulates meningeal and peripheral immunity, as well as spinal glial populations, and alleviates neuropathic pain, potentially through different mechanisms in males and females.

A simplified formalism of first quantized massless fields of any spin is presented. The angular momentum basis for particles of zero mass and finite spin s of the D^(s-1/2,1/2) representation of the Lorentz group is used to describe the wavefunctions. The advantage of the formalism is that by equating to zero the s-1 components of the wave functions, the 2s-1 subsidiary conditions (needed to eliminate the non-forward and non-backward helicities) are automatically satisfied. Probability currents and Lagrangians are derived allowing a first quantized formalism. A simple procedure is derived for connecting the wave functions with potentials and gauge conditions. The spin 1 case is of particular interest and is described with the D^(1/2,1/2) vector representation of the well known self-dual representation of the Maxwell's equations. This representation allows us to generalize Maxwell's equations by adding the E_0 and B_0 components to the electric and magnetic four-vectors. The global population is increasingly, urbanizing and growing wealthier, yet it is also becoming more wasteful. Each year, over two billion tons of municipal solid waste (MSW) is generated globally, a figure projected to nearly double by 2050, resulting in significant economic costs and hidden impacts such as resource depletion, pollution, and health challenges. This paper examines the distinct roles and potential collaborations between the informal and formal waste sectors in advancing circular economy principles and generating value from waste, using a comparative analysis of Denmark and India. Denmark's formalized, technologically advanced system exemplifies structured waste management driven by government policies, public-private partnerships, and waste-to-energy programs. In contrast, India relies heavily on an informal sector of waste pickers and small-scale recyclers, who play a critical role in resource recovery despite limited recognition and support with recent initiative of formalizing waste pickers in Swachh Bharat Mission. This study assesses how both sectors contribute to reuse, recycling, and environmental sustainability in different socioeconomic and regulatory contexts. Key findings reveal that while Denmark excels in waste segregation, advanced recycling, and energy recovery, India's informal sector demonstrates high material recovery efficiency but faces unsafe and inequitable conditions. Furthermore, Denmark faces challenges in extending product lifespans and complying with EU mandates for Preparation for Reuse (PfR), while India's informal sector struggles for recognition and safe working conditions. By analyzing the dynamics of formalization and collaboration, and identifying challenges posed by infrastructure and policy gaps, this study explores opportunities and barriers to integrate informal waste workers into formal systems. The findings contribute to understanding how diverse waste management approaches can foster circular economy goals and provide insights for policymakers seeking to optimize collaboration and improve sustainability in both high- and low-to-middle-income countries. Equally important, fostering consumer responsibility and awareness is vital, as focusing solely on waste sorting, reuse, and recycling will not suffice. Background The COVID-19 pandemic in 2020 posed significant communication challenges, especially in the healthcare sector. Telepathology provides a valuable means for healthcare providers to communicate. This study investigated the key challenges and benefits of telepathology in education through a systematic review of relevant studies conducted during this period. Methods This systematic review was conducted in 2022. We utilized databases, including PubMed, Google Scholar and ScienceDirect. Our search was performed from 7 February 2022 to 13 February 2022. We selected articles based on inclusion criteria and used the Critical Appraisal Skills Program checklist to assess study strengths and limitations. We extracted data using a checklist and synthesized the results narratively. Results We initially identified 125 articles, and after screening, 15 were included in the study. These studies reported various challenges, including cost, technology, communication problems, educational difficulties, time wasting, legal issues and family distraction problems. Conversely, studies mentioned benefits, such as care improvement, better education, time efficiency, proper communication, cost and technology advancement. Conclusions The results of this study will help future efforts and investigations to implement and set up telepathology. Based on our review, despite the challenges, the benefits of telepathology in education are greater than these obstacles, indicating its potential for future use. [ABSTRACT FROM AUTHOR]

38. Sandfly Fever Viruses Attenuate the Type I Interferon Response by Targeting the Phosphorylation of JAK-STAT Components

Sandfly fever viruses are emerging Phleboviruses typically causing mild febrile illness. Some strains, however, can cause severe and occasionally fatal neuro-invasive disease. Like most viruses, Phleboviruses have devised various strategies to inhibit the type I interferon (IFN) response to support a productive infection. Still, most of the strategies identified so far focus on inhibiting the sensing arm of the IFN response. In contrast, the effect of sandfly virus infection on signaling from the IFN receptor is less characterized. Therefore, we tested the effect of sandfly fever virus Naples (SFNV) and Sicily (SFSV) infection on IFN signaling. We found that infection with either of these viruses inhibits signaling from the IFN receptor by inhibiting STAT1 phosphorylation and nuclear localization. We show that the viral nonstructural protein NSs mediates these effects, but only NSs from SFNV was found to interact with STAT1 directly. Thus, we tested the upstream IFN signaling components and found that Janus kinase 1 (Jak1) phosphorylation is also impaired by infection.Furthermore, the NSs proteins from both viruses directly interacted with Jak1. Last, we show that IFN inhibition by SFNV and SFSV is most likely downstream of the IFN receptor at the Jak1 level. Overall, our results reveal the multiple strategies used by these related viruses to overcome host defenses.

This research studied the effect of electrolyte composition NaCl, Na
2 SO4 and CTAB (cetyltrimethylammonium bromide, C19 H42 BrN) on the electrochemical synthesis of Cu2 O nanoparticles. The NaCl concentration was varied in three levels of 58.5, 117 and 175.5 (g/L); the Na2 SO4 concentration was changed in three levels of 24, 32 and 48 (g/L) and the CTAB concentration was adjusted in four levels of 2, 4, 6 and 15 (g/L). The current density and temperature were kept constant at 0.4 (A/cm2 ) and 70 °C, respectively and the cathode-anode distance was set at 2.5 cm. The morphology, composition, particle size and distribution of the samples were examined by FESEM, XRD and DLS. The results indicated that increasing the concentration of electrolyte NaCl and Na2 SO4 enhanced the purity of Cu2 O. The Cu2 O crystals were smaller than 100 nm and formed as polycrystalline. Moreover, higher concentration of NaCl than Na2 SO4 led to a better result. CTAB effectively inhibited the growth of Cu2 O nanoparticles and the particle size decreased with increasing CTAB. The optimal result was achieved in the sample with 4 (g/L) CTAB and 175 (g/L) NaCl. Cu2 O nanoparticles with high purity and crystal size of 27.87 nm were produced. [ABSTRACT FROM AUTHOR]- Published
