Forming a key source of the regulatory framework for the provision of medical care to the population since 2022, Russian National Guidelines have given rise to a complex set of legal relations in the financial and economic activities of medical organizations and constituent entities of the Russian Federation, associated with ensuring the necessary amount of medical care. This article analyzes the scope of opportunities for a regional health authority in providing the most effective system for financing oncological care in the region and analyzes examples of effective adaptation of the regional financing system to the requirements of clinical guidelines.

Reconstruction of ice conditions and climate of the East Siberian Sea during the Late Middle Holocene was implemented for the first time using a transfer function method based on marine sedimentation proxies. Transfer functions were developed based on a hydrometeorological time series of changes in mean annual air temperature (ΔT10) and the ice-free period (IF10) for the last 70–90 years and a geochemical time series of bottom sediments accumulated during this time, which were obtained using a submillimeter scanning of the core in the X-ray fluorescence analysis of synchrotron radiation. Geochemical time series of Holocene deposits were obtained from X-ray fluorescence analysis of the core in a step of 1–2 cm. Reconstruction of ice conditions and average annual air temperature revealed a decreasing trend of ΔT10 and IF10, which started from the middle Holocene up to the Little Ice Age due to an orbitally determined decrease in insolation. A synchronous periodicity of changes in ice cover and air temperature was revealed and approximated by periods of 1600 years for IF10 and 1740 years for ΔT10, which are comparable to Bond’s climatic cycles. These results indicates the telecommunication of the Atlantic processes, climate of the East Siberian Sea, and predominance of the cyclonic type of atmospheric circulation during the Holocene.

We model the electrolysis of aluminum in specific electrolysis baths. A model of the physical process is proposed, allowing for the specific bath design, including the state of the bottom surface. The mathematical model utilizes the information capabilities of the Navier–Stokes equations written for the metal and the electrolyte media. The multidimensional system of equations enables us to consider the magnetohydrodynamic processes in two media and the interaction of the media. The problem is solved numerically. The calculations simulate the interface dynamics and the distribution of velocities and currents in the middle layer in both media. The numerical results are compared with physical experiments.

The clinical and radiological manifestations of suboccipital tuberculosis have been analyzed in 7 patients aged 7 to 35 years. A case of this condition accompanied by extensive vertebral destruction, abscesses, and basilar impression is described in a 7.5-year-old child. A radical stabilizing operation was successfully performed using the currently available technologies.

Thirteen of 84 children with tuberculous ostitis of the proximal tibial portion were repeatedly operated on 1.5 to 12 years after radical reconstructive surgery due to developed orthopedic sequels. Deformities of the tibia were found to develop in the earliest period and tibial shortening formed significantly later. An association of the development of anatomic and functional disorders with the values obtained in the quantitative analysis of the baseline X-ray films of the diseased extremity. The findings permits one to consider it possible to significantly predict orthopedic sequels in children operated on for tuberculous ostitis and accordingly to plan repeated medical examinations.
