Amaç: Bu çalışmanın amacı, hafif ve orta düzeydeki karpal tünel sendromlu hastalarda, splinttedavisi ile buna ek olarak verilen sinir ve tendon kaydırma egzersizlerinin median sinirmorfolojisi ve hastaların fonksiyonel durumları üzerine etkisini ve bu tedavi yöntemlerininbirbirine üstünlüğü olup olmadığını araştırmaktır.Gereç ve Yöntem: Çalışmaya, elektrodiagnostik olarak hafif ve orta düzey karpal tünelsendromu tanısı olan, splint tedavisi önerilen 19 hasta dahil edildi. 8 hastanın bilateral, 11hastanın da unilateral KTS tanısı vardı. 27 el bileği randomize edilerek, nötral el bileği splintitedavisi grubu (n=8), nötral el bileği splinti ile sinir kaydırma egzersizleri grubu (n=10) venötral el bileği splinti ile sinir ve tendon kaydırma egzersizleri grubu (n=9) olmak üzere üçgruba ayrıldı. Hastaların median sinir enine kesit alanı (ultrasonografi), fonksiyonel durumları(Boston Karpal Tünel Sorgulama Anketi, Hastaya Spesifik Fonksiyonel Skala), uyuşuklukşiddeti (görsel analog skala), elektrodiagnostik bulguları tedavi öncesinde ve tedavi sonrası 6.haftada değerlendirildi.Bulgular: Her üç grubun fonksiyonel durumları anlamlı derecede gelişirken (p˂0,05), sadecesinir kaydırma egzersizlerinin uygulandığı grupta median sinirin enine kesit alanı anlamlıderecede azaldı (p˂0,05). Hiçbir tedavi yönteminin de median sinirin duyusal ve motor sinirileti hızı ile duyusal latansı üzerine anlamlı etkisi olmadığı görüldü (p>0,05). Splint tedavisininuygulandığı grupta median sinirin distal motor latansının anlamlı derecede azaldığı saptandıSonuç: 6 haftalık splint tedavisi ile buna ek olarak verilen sinir ve tendon kaydırmaegzersizlerinin, hastaların semptomlarını iyileştirmede, fonksiyonel durumlarını geliştirmedeve uyuşukluk şiddetini azaltmada etkili olduğu görüldü. Ancak intranöral ödemi azaltmadasplint tedavisine ek olarak verilen sinir kaydırma egzersizlerinin etkili olduğu görüldü. Bunedenle, hafif ve orta düzeydeki karpal tünel sendromlu hastalarda fonksiyonel durumugeliştirmek ve intranöral ödemi azaltmak için, sadece geceleri kullanılan, hastaya özgü tasarlanan nötral el bileği splinti tedavisi ile birlikte sinir kaydırma egzersizlerinin etkiliolabileceği düşünülmektedir.Anahtar kelimeler: Elektrodiagnoz, karpal tünel sendromu, sinir kaydırma egzersizleri, splint,tendon kaydırma egzersizleri, ultrasonografi. Aim: The aim of this study was to investigate the effects of splint and additional nerve andtendon gliding exercises on median nerve morphology and patients' functional status in patientswith mild to moderate carpal tunnel syndrome and whether these treatment methods aresuperior to each other.Material and Methods: Nineteen patients with mild to moderate carpal tunnel syndrome(CTS) who were diagnosed based on electrodiagnostic criteria and offered splint treatment wereincluded in the study. Eight patients had bilateral CTS and eleven patients had unilateral CTS.Twenty-seven wrists were randomized in to three groups as neutral wrist splint alone (n=8),combination of splint and nerve gliding exercises (n=10), combination of splint andnerve/tendon gliding exercises (n=9). The cross sectional area of the median nerve(ultrasonography), functional status (Boston Carpal Tunnel Syndrome Questionnaire andPatient Spesific Functional Scale), numbness intensity (visual analogue scale) andelectrodiagnostic findings were evaluated before treatment and after treatment at 6th week.Results: While the functional status of all groups improved significantly (p˂0.05), the crosssectional area of the median nerve decreased significantly in combination of splint and nervegliding exercises group (p˂0.05). There was no significant effect of the three treatments onsensory and motor nerve conduction velocity and sensory latency of the median nerve (p>0.05).Distal motor latency of the median nerve was significantly decreased in the splint group(p˂0.05). Conclusion: The 6-week splint treatment and additional nerve and tendon gliding exerciseswere found to be effective in improving patients' symptoms, functional status and reducingnumbness intensity. However, in addition to splint treatment, nerve gliding exercises wereeffective in reducing intranural edema. Therefore, neural gliding exercises combined withcustom molded neutral wrist splint, which is used only at night, may be effective to improve viiifunctional status and reduce intraneural edema in patients with mild to moderate carpal tunnelsyndrome.Keywords: Carpal tunnel syndrome, electrodiagnosis, nerve gliding exercises, splint, tendongliding exercises, ultrasonography. 123

Amaç: Servikal radikulopatili hastalarda stabilizasyon egzersiz eğitiminin ağrı, boyun özürlülüğü, kavrama kuvveti, postüral durum ve yaşam kalitesine etkisini araştırmaktır.Yöntem: Çalışmaya, servikal radikulopati tanısı almış ve yaşları 20-65 arasında olan 32 hasta dahil edildi. Hastalar randomize edilerek stabilizasyon egzersiz eğitim grubu (Grup 1; n=18) ve standart tedavi grubu ( Grup 2; n=14) olmak üzere iki gruba ayrıldı. Grup 1' deki hastalara 15 seans elektroterapi (TENS ve US) ve 12 hafta servikal stabilizasyon egzersizlerinden oluşan kombine bir program uygulandı. Grup 2 hastalarına ise aynı elektroterapi uygulaması ile 12 haftalık standart servikal bölge egzersizleri (dirençli egzersizler, esneklik egzersizleri ve postür egzersizleri) verildi. Hastaların, ağrı şiddeti (VAS), boyun özürlülük düzeyi (Boyun Özür Göstergesi), yaşam kalitesi (SF-36 Kısa-Form), postür (Corbin) ve kavrama kuvveti değerlendirmeleri yapıldı. Değerlendirmeler tedavi öncesi yapıldı, tedavi sonundaki 4. haftada ve 3. ayda tekrarlandı.Bulgular: Her iki grupta da tedavi sonrası ve 3. ay ölçümlerinde ağrı şiddeti ve boyun özürlülük düzeyi istatistiksel olarak anlamlı şekilde azalırken (p0.05). Sonuç: Servikal radikulopatili hastalarda hastaya özel programlanan 12 haftalık stabilizasyon egzersizleri, ağrının azaltılmasında, hastaların fonksiyonelliklerinin ve yaşam kalitesinin gelişmesinde etkili olabilmektedir. Bununla birlikte, sonuçlarımız stabilizasyon egzersizlerinin standart servikal bölge egzersiz programlarına benzer bir etkiye sebep olduğunu da göstermiştir. Objective: The purpose of the study was to investigate the effect of stabilization exercise training on pain and functional status in patients with cervical radiculopathyMethod: Thirty-two patients who was diagnosed with cervical radiculopathy and their age was between 20-65 was included in this study. The patients were randomized to two groups as follows: Group 1 (n=18) - physical therapy agents (for 15 treatment sessions) including transcutaneous electrical nerve stimulation, continuous ultrasound, hot-pack and neck stabilization exercises described by Ylinen (for 12 weeks) and Group 2 (n=14) - physical therapy agents (for 15 treatment sessions), resistance exercises, flexibility and postural exercises. The patients were evaluated with a visual analog scale, Neck Disability Index, SF-36 (Short-Form), posture and hand grip at baseline, after treatment fourth weeks and 3rd month. Results: At baseline, pain severity, functional level, hand grip, Corbin Postural Assessment Scale and SF-36 scores were similar and there were no statistically differences between groups (p˃0.05). After treatment and 3 month later, pain and Neck Disability Index were decreased; Corbin Postural Assessment, hand grip and SF-36 scores were improved statistically in both groups (p˂0.05). Otherwise changes of the measurements both groups were similar and there were no significant differences between Group 1 and Group 2 of measurements at 4th week and 3rd month (p˃0.05).Conclusion: This study demonstrates that stabilization exercise training could be an effective intervention for decreasing pain and improving quality of life in patients with cervical radiculopathy. Nevertheless, the results of our study suggest that there is a similar effect of between stabilization exercises and general neck exercises.Key words: cervical radiculopathy, pain, stabilization exercise, quality of life. 98

The major aim of this paper is to provide a solution to one warehouse and N retailers problem with uncertain demand. The 'static-dynamic uncertainty' strategy is used. The objective function is to minimize expected average total costs incurred by the total system comprising of retailers' replenishment cost, retailers' inventory holding cost, warehouse's replenishment cost, warehouse's inventory holding cost and lost-sale cost. We use integer-ratio policies. We generated 15 samples for each 5 retailers by using ProModel simulation software for Heuristic I and Heuristic II. The results of stochastic model compared with the deviation from optimum solution for these different heuristics.

Bir ambar ve N adet perakendeciden oluşan bir dağıtım sistemi incelenmektedir. Bu tezin amacı bütün sistemde oluşan tedarik zinciri maliyetlerini minimize etmektir. Bu amacı gerçekleştirmek için yeni bir sezgisel yöntem geliştirilmiştir. Geliştirilen yeni sezgiselde talebin sürekli ve sabit olduğu; temin sürelerinin ihmal edilerek yok sayıldığı; yok satmaya müsaade edilmediği, ambar siparişlerini harici bir tedarikçiden sipariş ederken perakendecilerin siparişlerini ambardan karşıladığı varsayılmıştır. Toplam sistem maliyetinin elde bulundurma ve sipariş maliyetlerinden oluştuğu varsayılmaktadır. Algoritmanın geliştirilmesinde temel fikir envanter elde bulundurma maliyetleri ve sipariş maliyetlerinin karşılaştırılmasıdır. Yeni sezgiselin verdiği olumlu sonuçları gözlemlemek için 6000 adet veri seti 5, 10, 20, 35 ve 50 perakendecili sistemler için rassal olarak üretilmiş ve Yeni Sezgisel Yöntem, Abdul Jalbar(2009) Sezgisel Yöntemi, Roundy(1985) Sezgisel Yöntemi ve optimum çözümler elde edilmiştir. Yeni sezgisel, Abdul Jalbar(2009) sezgiseline göre ortalama % 1,9922; Roundy(1985) sezgiseline göre ortalama % 1,2575 daha iyi sonuçlar vermiştir. N=5, N=10 ve N=20 perakendecili sistemler için elde edilen optimum sonuçlara göre Yeni Sezgisel ortalama % 0,317; Abdul Jalbar(2009) sezgiseli ortalama % 2,563 ve Roundy(1985) sezgiseli ortalama %1,441, oranında sapmıştır. Optimum sonuçların doğruluğunu kanıtladığı varsayılan rs oranı N=5, N=10 ve N=20 perakendecili sistemlerde ideal değer ?1? `den ortalama 2,4404x10-6 oranında saparken, çözüm için kullanılan yazılımın saptanabilen hatası 3,0882x10-6 oranında olmuştur. The objective of this thesis is to minimize supply chain costs of the whole system. To achieve this objective a new heuristic algorithm was developed. We assume that demand is constant and deterministic, shortages are not allowed, lead times are negligible and warehouse replenishes its orders from external supplier and supplies all orders for retailers. It is also assumed that total system cost consists of replenishment and inventory holding costs. The main idea of new heuristic is to compare replenishment and inventory holding costs. To observe the performance of new heuristic we randomly generated 6000 datasets for the system containing 5, 10, 20, 35 and 50 retailers and we obtained the New Heuristic, Heuristic of Abdul Jalbar(2009), Heuristic of Roundy(1985) and optimal solutions. The New Heuristic gives averagely % 1,9922; % 1,2575 better solutions according to Heuristic of Abdul Jalbar(2009) and Heuristic of Roundy(1985), respectively. According to optimal solutions of N=5, N=10 and N=20 retailers distribution systems the New Heuristic averagely % 0,317; the Heuristic of Abdul Jalbar(2009) averagely % 2,563, and the Heuristic of Roundy(1985) averagely % 1,441 deviated. The rate rs of N=5, N=10 and N=20 retailers? systems which is assumed to prove the optimal solution has deviated from ideal value ?1? averagely 2,4404x10-6. Failure of the software used to find solutions could be detected around 3,0882x10-6. İki aşamadan oluşan ve bir ambar ile N adet perakendecinin olduğu bir envanter/dağıtım sistemi üzerine yapılan bu çalışma da sistemin tamamı bir bütün olarak ele alınarak maliyetlerin minimize edilmesi hedeflenmiştir. Ambar stoklarını, dışarıdan bir tedarikçiden sipariş etmektedir. Perakendecilerde siparişlerini merkez dağıtıcıdan ikmal etmektedirler. Her perakendecinin sabit ve bilinen talebi vardır. Bu talep yok satmaya müsaade edilmeden ambar tarafından karşılanmaktadır. Teslimatların anında yapıldığı ve tedarik sürelerinin ihmal edildiği varsayılmaktadır. Her işletmede sipariş maliyetleri ve elde bulundurma maliyetleri sabittir. Literatürde bu problemin çözümü için birçok yöntem geliştirilmiştir ancak bir maliyet fonksiyonu olan amaç fonksiyonundaki elde bulundurma maliyeti hesaplama koşulu yüzünden optimal sonuç elde edilememiştir. Bu yöntemlerden performansı en iyi olan Abdul-Jalbar sezgisel yöntemidir. Belirtilen bu yönteme alternatif yeni bir yöntem geliştirilmiş çalışmada sunulmuştur. Çalışmanın amacı, 1 ambar N perakendeci probleminin ve yeni yöntemin sunulmasıdır., The objective of this study, which considered an inventory/distribution system has just two echelons, containing one warehouse and N retailers, is to minimize supply chain costs of the whole system. The warehouse replenishes its stock from an outside supplier. The retailers face a constant and deterministic demand and replenish their stocks from the central warehouse. . It is assumed that shortages are not allowed. There are a lot of methods to solve this problem but the optimum solution of it can not be found by the reason of the calculation condition of holding cost in objective function which is a cost function. One of the best methods is Abdul Jalbar heuristic. A new heuristic is developed as an alternative method and is presented in this study. The goal of this study is the presentation of one warehouse and N retailers problem and new heuristic.

Bu çalısmada Türk öğrencilerin ?ngilizcedeki önek ve sonekleri kullanarakparçadan bilinmeyen kelimelerin anlamlarını nasıl ve hangi ölçüde tahmin ettikleriarastırılmıstır. Ayrıca, bu çalısma Türk öğrencilerin ?ngilizce önek ve sonekleri ayrıayrı nasıl kullandıklarını arastırmıstır.Çalısma Gaziosmanpasa Üniversitesi ?ngilizce Hazırlık Okulunda ortadüzey?ngilizce bilgisine sahip 10 öğrenci ile gerçeklestirilmistir. Veriler, öğrencilerindüsüncelerini sesli söyleme yöntemiyle toplanmıstır. Öğrencilerden bir okumaparçasını okumaları ve içinde ?ngilizce önek, sonek veya her ikisininde bulunduğu 13hedef kelimeyi tahmin etmeleri istenmistir. Sesli düsünce söyleme prosedürüsüresinde, öğrencilerin sesleri bir ses kaydediciye kaydedilmistir.Verileri elde etmek için ses kayıtları kağıda dökülmüstür. In addition, this study also examined Turkish learners’ use of prefixes and suffixes separately. The study was conducted with the participation of 10 pre-intermediate students at the English Language Preparatory School of Gaziosmanpaşa University. The data was gathered through think aloud procedures. The participants were asked to read a reading passage and try to infer the meanings of 13 target words that included prefixes, suffixes, or both. The participants were tape-recorded during the think aloud procedures. The tape recordings were transcribed in order to provide the data. The data analysis involved reading and rereading of the tape scripts. This study implies that English affixes are effective knowledge sources in determining the meanings of unknown words. Amaç: Çalışmanın amacı; total diz artroplastisi (TDA) geçirmiş olgularda diz protezinin, ağrı, yürüme ve diz fonksiyonlarınaolan etkilerini araştırmaktı.Yöntem: Çalışmaya 10 gönüllü dahil edildi. Bireyler görsel ağrı skalası, diz eklemi hareket açıklığı ölçümü ve süreli kalk yürü(SKY) testi ile değerlendirildi. Yürüyüşün zaman-mesafe parametrelerini değerlendirmek için Zebris™ FDM-2 cihazı kullanıldı.Değerlendirmeler cerrahi öncesi ve cerrahi sonrası 15. günde alındı.Bulgular: Bireylerin ağrı ve diz eklemi hareket açıklığı cerrahi sonrası anlamlı olarak azaldı (p=0,01, p=0,07). Sağlam tarafadım zamanı ve duruş fazı yüzdesi ile adım genişliği parametrelerinde cerrahi sonrası anlamlı artış bulundu (p=0,02, p=0,04,p=0,01). Diğer yürüme parametrelerinde anlamlı değişiklik görülmedi (p>0,05). SKY testi ile adım uzunluğu, çift destek fazıyüzdesi, tempo ve hız arasında anlamlı ilişki bulundu (p0,05).Sonuç: Sonuç olarak TDA cerrahisi geçiren olgularda SKY testi ile yürüme parametreleri arasında anlamlı bir ilişkibulunmaktadır. Gelecekte daha yüksek örneklem büyüklüğüne sahip ve uzun dönem takip sonuçlarının araştırıldığıçalışmalara ihtiyaç duyulmaktadır.
