8. Redo Heart transplantation in a high-risk patient due to severe aortic regurgitation and accompanying right ventricular failure after LVAD implantation and temporary RVAD support

- Abstract
Background: We report a 62-year-old patient who received redo-orthotopic Heart transplantation due to worsening severe aortic regurgitation after 19 months of continuous flow LVAD (cf-LVAD) and temporary RVAD support for one month. Case Report: The patient received a heartware LVAD (HVAD) and annuloplasty of the tricuspid valve due to end-stage heart failure (as a consequence of dilated cardiomyopathy) and severe tricuspid regurgitation in addition to right-sided ECMO implantation. Postoperatively due to the inability to wean the implanted ECMO, a temporary RVAD was implanted after which the patient’s condition improved so that it had been explanted later and the patient was discharged after nine-month. In immediate post-operative echo, minimal aortic regurgitation was noted but in the follow-up transthoracic echocardiograms, there was a gradual increase in the severity of aortic regurgitation with worsening both right and left ventricular functions. TAVI was not an option due to unfavourable anatomical issues. That’s why the patient was listed for urgent heart transplantation, performed 19 months after the LVAD implantation. The postoperative course was complicated due to acute renal failure. After recompensation, dialysis, and intensive physiotherapy, the patient could be discharged home after three months. Conclusion: severe aortic regurgitation is a recognizable complication after cf-LVAD implantation which in our case was managed successfully with orthotopic heart transplantation in this high-risk patient.

Background and Objective: The silent brain infarctions are common in first-ever stroke. Some recent studies show the possible role of silent brain infarction in ischemic stroke. The aim of this study was to evaluate the frequency of silent brain infarction in acute first-ever ischemic stroke and its association with stroke risk factors. Materials and Methods: This descriptive - analytical study was carried out on 203 (94 males, 109 females) patients with acute first-ever ischemic stroke in Shafa hospital in Kerman, Iran during 2010. The diagnosis of ischemic stroke was made by physical examination and was confirmed by MRI and CT Scan. A questionnaire containing demographic information and risk factors including hypertension, diabetes, hyperlipidemia, cigarette smoking, ischemic and non-ischemic heart disease for stroke as well as presence of silent stroke was completed for each patient. Data were analyzed using SPSS-16 and logistic regression test. Results: The mean age of patients was 62.56±17.35 years. Among our patients history of hypertension, hyperlipidemia, cigarette smoking, diabetes, history of ischemic heart disease, history of other cardiac disease and silent stroke were observed in 66 (32.5%), 26 (12.8%), 16 (7.9%), 40 (19.7%), 27 (13.3%), 16 (7.9%) and 31 (15.3%), respectively. Background Lack of awareness of cognitive decline (ACD) is common in late-stage Alzheimer’s disease (AD). Recent studies showed that ACD can also be reduced in the early stages. Methods We described different trends of evolution of ACD over 3 years in a cohort of memory-complainers and their association to amyloid burden and brain metabolism. We studied the impact of ACD at baseline on cognitive scores’ evolution and the association between longitudinal changes in ACD and in cognitive score. Results 76.8% of subjects constantly had an accurate ACD (reference class). 18.95% showed a steadily heightened ACD and were comparable to those with accurate ACD in terms of demographic characteristics and AD biomarkers. 4.25% constantly showed low ACD, had significantly higher amyloid burden than the reference class, and were mostly men. We found no overall effect of baseline ACD on cognitive scores’ evolution and no association between longitudinal changes in ACD and in cognitive scores. Conclusions ACD begins to decrease during the preclinical phase in a group of individuals, who are of great interest and need to be further characterized. Introduction The longitudinal trajectories of functional brain dynamics and the impact of genetic risk factors in individuals at risk for Alzheimer's disease are poorly understood. Methods In a large-scale monocentric cohort of 224 amyloid stratified individuals at risk for Alzheimer's disease, default mode network (DMN) resting state functional connectivity (FC) was investigated between two serial time points across 2 years. Results Widespread DMN FC changes were shown in frontal and posterior areas, as well as in the right hippocampus. There were no cross-sectional differences, however, apolipoprotein E e4 (APOE e4) carriers demonstrated slower increase in FC in frontal lobes. There was no impact of individual brain amyloid load status. Discussion For the first time, we demonstrated that the pleiotropic biological effect of the APOE e4 allele impacts the dynamic trajectory of the DMN during aging. Dynamic functional biomarkers may become useful surrogate outcomes for the development of preclinical targeted therapeutic interventions.

Introduction Several neurodegenerative brain proteinopathies, including Alzheimer's disease (AD), are associated with cerebral deposition of insoluble aggregates of α-synuclein. Previous studies reported a trend toward increased cerebrospinal fluid (CSF) α-synuclein (α-syn) concentrations in AD compared with other neurodegenerative diseases and healthy controls. Methods The pathophysiological role of CSF α-syn in asymptomatic subjects at risk of AD has not been explored. We performed a large-scale cross-sectional observational monocentric study of preclinical individuals at risk for AD (INSIGHT-preAD). Results We found a positive association between CSF α-syn concentrations and brain β-amyloid deposition measures as mean cortical standard uptake value ratios. We demonstrate positive correlations between CSF α-syn and both CSF t-tau and p-tau 181 concentrations. Discussion Animal models presented evidence, indicating that α-syn may synergistically and directly induce fibrillization of both tau and β-amyloid. Our data indicate an association of CSF α-syn with AD-related pathophysiological mechanisms, during the preclinical phase of the disease.

International audience; Background:Subjective cognitive decline (SCD) may result from many conditions, including Alzheimer’s disease (AD). Objective:In this study, we searched for a specific pattern of SCD in asymptomatic individuals at risk for AD. Methods:Cognitively normal older adults (N = 318) reporting SCD and their informants were enrolled in the INSIGHT-PreAD cohort. We examined the relationship between six SCD measures and both cognitive scores and AD neuroimaging markers (amyloid burden, hippocampal atrophy and brain hypometabolism). An awareness of cognitive decline index (ACDI) has been introduced based on the subject-informant discrepancy in a questionnaire of SCD and participants with low versus high awareness were compared. Results:Scores in the INSIGHT-PreAD SCD questionnaires did not correlate with AD neuroimaging markers. As well, no correlation has been found between SCD measures and cognitive scores. Comparing subjects with a low (n = 19) and high (n = 86) level of awareness, no significant difference in terms of demography, neuropsychiatric symptoms, autonomy, quality of life, cognition, and hippocampal volume was found. However, the “low awareness” group showed greater amyloid burden and lower cortical metabolism, compared to the “high awareness” group. Conclusion:This study provided additional evidence that reporting SCD by itself is not a specific symptom of preclinical AD. Conversely, a low cognitive awareness (namely, when subjects report fewer difficulties than their relatives do) may represent a very early form of anosognosia and serve as a specific indicator of preclinical AD. This finding is of key importance as an enrichment factor to consider in both clinical practice and research trials.

for the INSIGHT-preAD study group; International audience; The accumulation of amyloid plaques is one of the earliest pathological changes in Alzheimer's disease (AD) and may occur 20 years before the onset of symptoms. Examining associations between amyloid pathology and other early brain changes is critical for understanding the pathophysiological underpinnings of AD. Alterations in gray matter networks might already start at early preclinical stages of AD. In this study, we examined the regional relationship between amyloid aggregation measured with positron emission tomography (PET) and gray matter network measures in elderly subjects with subjective memory complaints. Single-subject gray matter networks were extracted from T1-weigthed structural MRI in cognitively normal subjects (n = 318, mean age 76.1 ± 3.5, 64% female, 28% amyloid positive). Degree, clustering, path length and small world properties were computed. Global and regional amyloid load was determined using [ 18 F]-Florbetapir PET. Associations between standardized uptake value ratio (SUVr) values and network measures were examined using linear regression models. We found that higher global SUVr was associated with lower clustering (β = −0.12, p < 0.05), and small world values (β = −0.16, p < 0.01). Associations were most prominent in orbito-and dorsolateral frontal and parieto-occipital regions. Local SUVr values showed less anatomical variability and did not convey additional information beyond global amyloid burden. In conclusion, we found that in cognitively normal elderly subjects, increased global amyloid pathology is associated with alterations in gray matter networks that are indicative of incipient network breakdown towards AD dementia.

This paper considers a multi-period and product inventory-routing problem. This model involves two levels, namely, a distributor and several retailers. This problem is modeled with the aim of minimizing bi-objectives, namely, the total cost of the system (including start-up, distribution, and maintenance costs) and risk-based transportation. Products are delivered to retailers by some heterogeneous vehicles with specific capacities through a direct delivery strategy, and inventory shortage is assumed to be impermissible. To validate this new bi-objective model, the ε-constraint method is used to solve problems. Because problems without distribution planning are very complex to solve optimally, the problem considered in this paper is also Non-deterministic Polynomial (NP-hard). Therefore, a nondominated sorting genetic algorithm (NSGA-II) and a multi-objective imperialist competitive algorithm (MOICA) are used and developed to solve a number of test problems. Background This review aims to explore the healthcare needs and access to healthcare services among Syrian refugees in Jordan. Furthermore, it identifies possible ways of responding to such needs within the framework of the structural challenges faced in this particular context. Methods Through qualitative techniques, this study systematically reviewed nine articles identified in academic and international organizations' databases. Six articles are from scholarly sources, while three are published reports or commentary articles; thus, there are variations in methodological rigour. An analysis of the articles was performed based on the Critical Appraisal Skills Programme check tool to systematically assess the trustworthiness, relevance and results of the included papers. Results The overall access to healthcare is relatively good for most registered Syrian refugees. However, some groups lack access due to financial and structural barriers, such as not possessing the appropriate civil documentation and/or not having the means to pay out-of-pocket expenses to access adequate services. A key challenge relates to access of inpatient care. Accessing care for non-communicable/chronic diseases requires not only legal documentation but also long-term access and continuity of care in an already overburdened health care system. Conclusion The healthcare needs of the refugee, as well as the host population in Jordan, cannot be adequately met without the international society acknowledging a collective responsibility, including a financial commitment. Olive and carnation absolutes are new forms extracted from the concrete produced after extraction of oil. Repellency and fumigation bioassays are of the best methods which used to evaluate the absolutes of olive and carnation against T. castaneum and R. dominica adults. Results obtained showed that the repellent and fumigant activities of carnation had the strongest effect to the two tested insects in comparison with that of olive. Moreover, the lowest concentration of carnation (5 ppm) achieved 50, 100% repellent activity compared to that of olive which caused zero and 35% after 24 h against T. castaneum and R. dominica, respectively. Additionally, the affection of oil increased with increasing of exposure period. However, the repellent of the two absolutes against the two insects started to decrease after 24 h post treatment. The time needed for oil to cause LT50 was also estimated at the highest concentration which achieved nearly 100 % mortality (449.95 mg/170 cm3) for both carnation and olive. Basedon LT 50 values the carnation oil was the premier compared to the olive with LT 50 values of 0.166, 0.01 and 0.688, 50.166 h against R. dominica and T. castaneum , respectively. Finally, the present study suggests application the absolute oils against stored product insects where they are very cheap, available and easy affordable and they may introduce the program of integrated pest management. We compared the release of C and N from untreated and anaerobically digested plant biomasses and animal manures. Based on losses of C during the plant biomass (feed) passage in cattle, during anaerobic digestion (AD), and during incubation with soil, the retention of C in soil was estimated. When related to the amount of C originally present in the feed, the digestion by cattle and/or ADdid not significantly affect the longer-term retention of C in soil whereas the contribution to the labile soil C pool differed. Digestion by cattle and AD increased the release of inorganic N but with no simple relationship between N release and C retention. The increase in N availability due to AD was equivalent to 10-35% of total N in slurry, but after AD of plant biomass and faeces the Navailability increased significantly more.

Two field experiments were carried out at Sidi Salem Directorate, Kafr EI-Sheikh Governorate, Egypt, during the two successive winter seasons of 2011/2012 and 201212013 to study the effect offour nitrogen fertilization rates (0, 37.5, 75 and 112.5 kg N/fed) and three phosphorous fertilization rates (0, 15 and 30 kg P
2 O5 /fed) on the growth characters, yield, its component and some chemical properties of three bread wheat cultivars (Gemmeiza 9, Sakha 93 and Giza 168). The experimental design was split split plot design in four replications. The obvious results of this investigation can be summarized as follows: All traits of wheat under stUdy, i.e. No. of tillers/m2 , No. of spike/m2 , plant height, spike length, No. of kernels/spike, 1000-kernel weight, kernels weigh/spike, straw yield/fed, grain yield/fed, biological yield/fed, harvest index, kernels nitrogen content (%) and grain protein yield/fed showed significantly increased by increasing nitrogen fertilizer rates, except kernels phosphorous content (%). Application of 112.5 kg N /fed significantly gave the maximum values of all traits. Increasing phosphorous fertilizer rates up to 30 Kg P2 O5 /fed significantly increased No. of tillers/m2 , No. of spike/m2 , plant height, spike length, straw yield/fed, grain yield/fed, biological yieldlfed and kernels phosphorous content (%) in the two seasons. While, No. of kernels/spike, 1000-kernel weight, kernels weight/spike, harvest index, kernels nitrogen content % and grain protein yield/fed were not affected by increasing phosphorous fertilizer rates. The results showed that wheat cultivars were significantly differed in all traits studied under study in the both seasons, except biological yield kg/fed, kernels nitrogen content (%) and kernels phosphorous content (%). Gemmeiza 9 cultivar significantly surpassed Sakha 93 and Giza 168 cultivars in plant height, spike length, 1000-kernel weight, kernels weight/spike, grain yield/fed, harvest index and kernels protein yield/fed in the both seasons, while, Sakha 93 recorded the highest values of No. of kernels/spike. Morever, Giza 168 surpassed the other two cultivars in No. of tillers/m2 , No. of spike/m2 and straw yield/fed. The first order interactions between (112.5 kg N/fed X 30 Kg P2 O5 /fed), (112.5 kg N/fed X Gemmeiza 9) and (30 Kg P2 O5 /fed X Gemmeiza 9) as well as the second order interaction between 112.5 kg N/fed X 30 Kg P2 O5 /fed X Gemmeiza 9 were significantly recorded the greatest grain yield/fed as compared with the others interactions in the both seasons. In this paper, first, we investigate the construction of compact sets of Ck and C0k by proving "Ck, C0k - version" of Arzelà-Ascoli theorem, and then introduce new measures of noncompactness on these spaces. Finally, as an application, we study the existence of entire solutions for a class of the functional integral-differential equations by using Darbo's fixed point theorem associated with these new measures of non- compactness.
