Here, we retrospectively evaluated the informational yield of 338 post-reperfusion kidney transplant biopsies (including 95 living donations) assessed according to BANFF for the histological characteristics interstitial fibrosis and tubular atrophy (IF/TA), glomerulosclerosis, arteriosclerosis, and acute tubular injury (ATI). Associations with delayed graft function (DGF) and death-censored graft survival were explored through Cox-regression analyses. The maximum follow-up time was 11.4 years, with DGF observed in 108 (32%) cases. After deceased donation there was no association between DGF and histologic parameters. Univariable Cox-regression unveiled an association of IF/TA and glomerulosclerosis with long-term death-censored graft survival (HR per 10% increase: IF/TA 1.63; 95% CI 1.17–2.28; p = 0.003; glomerulosclerosis 1.19; 95% CI 1.01–1.39; p = 0.031). In multivariable Cox regression analyses, adjusted for recognized clinical risk variables like expanded criteria donor-status, donor age, history of diabetes, and HLA-mismatches, only IF/TA maintained association over the total observation period in deceased donations and in the total cohort. Arteriosclerosis and ATI were not associated with clinical outcome after deceased donation. Especially ATI did not affect delayed graft function if only deceased donations were considered. Background: Insulin resistance (IR) is involved in the pathogenesis and progression of obstructive sleep apnea (OSA) and coronary heart disease (CHD). The triglyceride glucose(TyG) index, an alternative indicator of IR with high reliability, is linked to CHD development and severity. However, the association of TyG index with CHD in OSA cases remains undefined. This study explores the association of TyG index with CHD risk and coronary artery sclerosis severity in the OSA population. Methods: OSA cases with suspected CHD, enrolled at the Department of Hypertension, The First Affiliated Hospital of Xinjiang Medical University between February 2020 and June 2024, were assigned to the OSA and OSA + CHD groups, followed by TyG index assessment. The t-test, Mann-Whitney U-test and one-way analysis of variance were utilized to compare continuous data, while comparisons of categorical data utilized the chi-square test or Fisher's exact test. Logistic regression analysis was conducted to determine factors independently predicting OSA with CHD and Gensini scores. Restrictive cubic spline (RCS) was used to assess potential non-linear associations of TyG index with CHD risk and Gensini score in OSA patients. Results: Totally 1059 OSA patients were included, with 514 diagnosed with CHD (48.54%). Multivariable logistic regression analysis upon adjustment for age, gender, hypertension, diabetes, and smoking history revealed TyG index as a risk factor for CHD in OSA cases. CHD risk in the high-TyG index group was 1.977 fold higher versus the low-TyG index group (OR = 1.977, 95% CI 1.424-2.800, P < 0.001). In addition, TyG index had a linear relationship with CHD (P for nonlinearity = 0.0709). In moderate to severe OSA cases, the high-TyG index group had a significantly higher CHD risk (OR = 2.430, 95% CI 1.601–3.690, P < 0.001). Gensini score and TyG index (P for non-linearity = 0.0033) had a non-linear relationship, while high TyG index was a risk factor for high Gensini score. Conclusion: TyG index is a risk factor for CHD in OSA cases and reflects the severity of coronary atherosclerosis. TyG, a surrogate indicator for IR evaluation, may help predict CHD in OSA cases, especially in moderate to severe OSA. [ABSTRACT FROM AUTHOR]

Background: Mesenchymal stem cells (MSCs) have drawn significant attention for their regenerative potential and therapeutic applicability across a range of conditions, including cardiovascular diseases and age-related frailty. Despite extensive preclinical studies, there remain gaps in understanding the long-term safety and efficacy of MSC therapy in humans. This study aimed to assess the safety of intravenous MSC administration, evaluate the mean major adverse cardiac and cerebrovascular event (MACCE)-free period, and identify potential risk factors for MACCE development in patients receiving MSC therapy for various indications. Methods: A retrospective observational study was conducted on 2504 patients (mean age: 54.09 ± 11.65 years) who received intravenous adipose-derived MSC (AD-MSC) therapy between October 2014 and December 2023 at the Omotesando Helene Clinic, Tokyo, Japan. Patients received MSC doses ranging from 100 million to 2 billion cells, with the majority receiving 1–2 billion cells per treatment. Statistical analyses included multivariate Cox proportional hazards regression and Kaplan–Meier survival analysis to evaluate MACCE risk factors and event-free duration. Results: Over the follow-up period, the MACCE rate was exceptionally low at 0.2%. Multivariate analysis identified age as a significant risk factor for MACCE (hazard ratio: 1.127; 95% CI: 1.0418–1.219; p = 0.0029), while sex and MSC dose showed no significant association. Minor adverse events occurred in 0.8% of patients, with no severe adverse events reported. The study found MSC therapy to be safe, with a low adverse event rate and minimal risk of MACCE. Conclusions: This study demonstrates the safety of intravenous MSC therapy in a large cohort of patients, with a low incidence of MACCE and minimal adverse effects. Age was the only significant predictor of MACCE risk. Further prospective randomized studies are needed to validate these findings and explore the potential of MSC therapy in reducing MACCE risk and improving clinical outcomes across diverse indications. Among deaths related to cardiovascular disease (CVD), arteriosclerosis accounts for three-quarters of the total. The most common treatment for arteriosclerosis is revascularization through the implantation of endovascular scaffolds. However, most metal scaffolds currently available are non-biodegradable, which can lead to rejection reactions. In this study, we have developed a CMCS-PEI/miR polysaccharide nucleic acid nanodrug loaded with miR-199a-5p for the first time. The CMCS-PEI/miR polysaccharide nucleic acid nanodrug exhibits good blood compatibility, successfully transfects cells, and induces apoptosis of vascular endothelial cells. Additionally, PCL/GO composite scaffolds with excellent mechanical properties and biocompatibility were fabricated using 3D printing technology. This technology allows for the customisation of scaffolds to meet the needs of individual patients. The 3D-printed PCL/GO composite scaffolds loaded with CMCS-PEI/miRNA polysaccharide nucleic acid nanodrug effectively induce apoptosis of vascular endothelial cells, showing great potential in promoting the functional repair of arteriosclerosis and preventing vascular restenosis. Background: The Renal Pathology Society developed a universal pathological classification of diabetic nephropathy in 2010. Some research has been conducted to validate this classification's ability to predict the outcome. However, the clinical implications of these pathological abnormalities are still being investigated. Objectives: In this study, we aimed to demonstrate the clinical reflections of these lesions to better understand the underlying mechanisms. Methods: Data of 119 patients with biopsy proven diabetic nephropathy from a single center were included in the study. Pathology specimens were reclassified according to 2010 criteria suggested by the RPS. Results: Diabetic retinopathy was more frequently present in patients with advanced glomerular class, IFTA score, interstitial inflammation score, arteriolar hyalinosis score, arteriosclerosis score, and in patients with exudative lesions present (p < 0.05). The proteinuria levels of patients with advanced glomerular classes and exudative lesions were significantly higher, and serum albumin levels were lower (p < 0.05). Hematuria occurrence was more frequent in glomerular class III and IV patients and in patients with advanced arteriolar hyalinosis (p < 0.05). Conclusion: This large, single center, retrospective study reveals that diabetic retinopathy is associated with glomerular and arteriolar lesions but not with interstitial lesions. Proteinuria and hematuria were independent predictors of glomerular lesions, but not other renal lesions. Nevertheless, prospective studies which include all the confounding clinical factors are required to reach a conclusion on the relationship of hematuria and renal lesions. [ABSTRACT FROM AUTHOR]

Aging plays a pivotal role in the pathogenesis of cerebral small vessel disease (CSVD), contributing to the onset and progression of vascular cognitive impairment and dementia (VCID). In older adults, CSVD often leads to significant pathological outcomes, including blood–brain barrier (BBB) disruption, which in turn triggers neuroinflammation and white matter damage. This damage is frequently observed as white matter hyperintensities (WMHs) in neuroimaging studies. There is mounting evidence that older adults with atherosclerotic vascular diseases, such as peripheral artery disease, ischemic heart disease, and carotid artery stenosis, face a heightened risk of developing CSVD and VCID. This review explores the complex relationship between peripheral atherosclerosis, the pathogenesis of CSVD, and BBB disruption. It explores the continuum of vascular aging, emphasizing the shared pathomechanisms that underlie atherosclerosis in large arteries and BBB disruption in the cerebral microcirculation, exacerbating both CSVD and VCID. By reviewing current evidence, this paper discusses the impact of endothelial dysfunction, cellular senescence, inflammation, and oxidative stress on vascular and neurovascular health. This review aims to enhance understanding of these complex interactions and advocate for integrated approaches to manage vascular health, thereby mitigating the risk and progression of CSVD and VCID. As a common disease, cardiovascular and cerebrovascular diseases pose a great harm threat to human wellness. Even using advanced and comprehensive treatment methods, there is still a high mortality rate. Arteriosclerosis, as an important factor reflecting the severity of cardiovascular and cerebrovascular diseases, is imperative to detect the arteriosclerotic retinopathy. However, the detection of arteriosclerosis retinopathy requires expensive and time-consuming manual evaluation, while end-to-end deep learning detection methods also need interpretable design to high light task-related features. Considering the importance of automatic arteriosclerotic retinopathy grading, we propose a segmentation and classification interaction network (SCINet). We propose a segmentation and classification interaction architecture for grading arteriosclerotic retinopathy. After IterNet is used to segment retinal vessel from original fundus images, the backbone feature extractor roughly extracts features from the segmented and original fundus arteriosclerosis images and further enhances them through the vessel aware module. The last classifier module generates fundus arteriosclerosis grading results. Specifically, the vessel aware module is designed to highlight the important areal vessel features segmented from original images by attention mechanism, thereby achieving information interaction. The attention mechanism selectively learns the vessel features of segmentation region information under the proposed interactive architecture, which leads to reweighting the extracted features and enhances significant feature information. Extensive experiments have confirmed the effect of our model. SCINet has the best performance on the task of arteriosclerotic retinopathy grading. Objective. Pulse pressure waves contain information about human physiology. There is a need for a simple, accurate way to know cardiovascular health in the clinic, so as to realize the implementation of convenient and effective early health monitoring for patients with arteriosclerosis. Approach. This study proposes an arteriosclerosis assessment method based on fitting a lognormal function, along with improving a conventional electronic sphygmomanometer. During the deflation phase of blood pressure measurement, the cuff pressure was kept constant (40 mmHg) and an additional 10 s of pulse signal was acquired. To derive the pulse pressure waveforms for a single cycle, the acquired pulse data of 101 cases were preprocessed in this study, including filtering for noise removal, onset point identification, removal of baseline drift, and normalization. In this study, an improved pulse resolution algorithm is proposed for the multimodal problem of the pulse wave, combining waveform matching and threshold setting, and finally obtaining the resolution parameters of the lognormal function with an average error less than 1.5%. Main results. According to the correlation analysis, the resolved parameters A 1, W 2, C 2, W 3, and C 3 were significantly correlated with brachial-ankle Pulse Wave Velocity, and the absolute correlation range in 0.17–0.53, which can be used as a reference index for arteriosclerosis. An arteriosclerosis assessment model was constructed based on the support vector mechanism, and the prediction accuracy was 91.1%. Significance. There are comorbidities and complications in atopic dermatitis and psoriasis that often occur after the appearance of skin inflammation. Statistically, data show that patients with psoriasis and atopic dermatitis have a shorter life expectancy than patients without psoriatic dermatitis, due to the occurrence of arteriosclerosis, myocardial infarction, and cerebral infarction. Many types of skin inflammation are treated with various antibody preparations, and marked improvement in patients' quality of life can be achieved. The next theme is to understand the pathogenesis of arteriosclerosis, myocardial infarction, stroke, and other complications associated with dermatitis and to find treatments and drugs to reduce their occurrence. The skin, a crucial immune organ, generates large amounts of inflammatory cytokines in response to various stimuli, leading to systemic inflammation and potential damage to internal organs. The link between inflammatory skin conditions like psoriasis and atopic dermatitis with serious health complications such as vascular disorders and systemic amyloidosis has been increasingly recognized. In psoriasis, biological treatments targeting Interleukin (IL)-17A, a key cytokine, have shown promise in reducing cardiovascular risks. Recent developments include treatments that target both IL-17A and IL-17F in the psoriasis field, though each cytokine's impact on internal organ damage is still under debate. Among visceral complications secondary to dermatitis, systemic amyloidosis and atherosclerosis have been reported to be controlled by suppressing IL-17 in the early stages of dermatitis. Still, it remains unclear whether suppressing IL-17 prevents organ damage in the late stages of persistent severe dermatitis. A study using a long-lasting dermatitis mouse model that overexpressed human caspase-1 in keratinocytes (Kcasp1Tg) investigated the effects of deleting IL-17A and IL-17F on visceral complications. Cross-mating Kcasp1Tg with IL-17A-, IL-17F-, and IL-17AF-deficient mice assessed the skin and visceral organs histologically, and RT-PCR analysis of aortic sclerosis markers was performed. Despite less improvement in dermatitis, deletion of IL-17A in Kcasp1Tg mice showed promising results in reducing multiple organ amyloidosis. On the other hand, the effect was observed in both IL-17A and IL-17F deleted mice for aortic sclerosis. The inhibition of IL-17A and IL-17F was suggested to reduce the risk of developing comorbidities in internal organs.
