Trial matches are frequently used for team preparation in rugby league competitions, making it essential to understand the demands experienced to assess their specificity to actual competition. Consequently, this study aimed to compare the activity demands between pre-season trial matches and early in-season rugby league matches. Following a repeated-measures observational design, 39 semi-professional, male rugby league players from two clubs were monitored using microsensors during two trial matches and the first two in-season matches across two consecutive seasons. Total distance, average speed, peak speed, absolute and relative highspeed running (HSR; > 18 km·h -1 ) and low-speed running (LSR; < 18 km·h -1 ) distance, as well as absolute and relative impacts, accelerations (total and high-intensity > 3 m·s -2 ), and decelerations (total and highintensity < -3 m·s -2 ) were measured. Linear mixed models and Cohen’s d effect sizes were used to compare variables between match types. Playing duration was greater for in-season matches (p < 0.001, d = 0.64). Likewise, higher (p < 0.001, d = 0.45–0.70) activity volumes were evident during in-season matches indicated via total distance, HSR distance, LSR distance, total accelerations, high-intensity accelerations, total decelerations, and high-intensity decelerations. Regarding activity intensities, a higher average speed (p = 0.008, d = 0.31) and relative LSR distance (p = 0.005, d = 0.31) only were encountered during in-season matches. Despite players completing less volume, the average activity intensities and impact demands were mostly similar between trial and early in-season matches. These findings indicate trial matches might impose suitable activity stimuli to assist players in preparing for early in-season activity intensities.

The aim of this study was to compare knee extensor and flexor strength recovery following anterior cruciate ligament (ACL) reconstruction between bone-patellar tendon-bone (BPTB) and hamstring tendon (HT) grafts in international male soccer players undergoing comparable 6-month rehabilitation programmes. Seventeen players underwent ACL reconstruction with either an autogenous BPTB graft or HT graft. Knee extensor and flexor peak torques were measured at 3 months and 6 months in the injured and contralateral legs following surgery using isokinetic dynamometry. The moderate –large asymmetries in knee extensor peak torque between legs at 3 months across graft types (BPTB: p = 0.002, g = -0.94; HT: p = 0.02, g = -0.55) were reduced to trivial asymmetries at 6 months (BPTB: p = 0.30, g = -0.19; HT: p = 0.40, g = -0.16), with a non-significant difference in limb symmetry index (LSI) between grafts at 6 months (p = 0.62, g = -0.24). Similarly, moderate–large asymmetries in knee flexor peak torque between legs at 3 months across graft types (BPTB: p = 0.13, g = -0.50; HT: p = 0.01, g = -0.97) were reduced to trivial-small asymmetries at 6 months (BPTB: p = 0.25, g = 0.18; HT: p = 0.01, g = -0.47); however, a superior LSI was evident with BPTB compared to HT grafts at 6 months (p = 0.007, g = 1.43, large ). Strength and conditioning professionals working with soccer players who are rehabilitating from ACL reconstruction after receiving a HT graft should give adequate attention to delivering suitable hamstring exercises that ensure optimal strength restoration.

Despite the increased popularity and number of international competitions for 3×3 basketball, the precise physical demands of this sport are not well described. Therefore, this study aimed to quantify the physical demands of 3×3 basketball games according to game outcome and competition phase. Following an observational design, video footage from 27 games were analysed from 104 international 3×3 basketball players (n = 52 male and n = 52 female players) belonging to 26 national teams (n = 13 male and n = 13 female teams) during the 2019 FIBA 3×3 World Cup. Manual frame-by-frame time-motion analyses were conducted to determine the relative frequency (n·min−1) and duration (% of live playing time) for several physical demand variables to make comparisons according to game outcome (win vs. loss) and competition phase (group games vs. finals games). Linear mixed models for repeated measures and effect size analyses revealed non-significant, trivial-small differences in physical demands between games that were won and lost. Regarding competition phase, male players completed more high-intensity activity (sprinting, high-intensity specific movements, and jumping) but spent a greater proportion of playing time jumping and performing recovery activities (standing/ walking) during finals games than group games (P < 0.05, small), while female players performed more low intensity activity (jogging and low-intensity specific movements) during group games than finals games (P < 0.05, small). These findings indicate that the physical capabilities of male and female 3×3 basketball players may not be the determining factor for team success in games and players can mostly maintain activity outputs across phases of tournament play conducted at the highest international standard.

To quantify and compare the external peak demands (PD) encountered according to game result (win vs. loss), quarter result (win vs. tie vs. loss), and quarter point difference (± difference in score) in under-18 years (U18), male basketball players. Thirteen basketball players had external load variables monitored across 9 games using local positioning system technology, including distance covered, distance covered in different intensity zones, accelerations, decelerations, and PlayerLoad™. PD were calculated across 30-s, 1-min, and 5-min time windows for each variable. Linear mixed models were used to compare PD for each variable according to game result (win vs. loss), quarter result (win vs tie vs loss), and quarter point difference (high vs. low). External PD were comparable between games that were won and lost for all variables and between quarters that were won and lost for most variables (p > 0.05, trivial-small effects). In contrast, players produced higher (p < 0.05, small effects) 1-min high-speed running distance and 5-min PlayerLoadTM in quarters that were won compared to quarters that were lost. Additionally, high quarter point differences (7.51±3.75 points) elicited greater (p < 0.05, small effects) external PD (30-s PlayerLoadTM, 30-s and 5-min decelerations, and 1-min and 5-min high-speed running distance) than low quarter point differences (-2.47±2.67 points). External PD remain consistent (trivial small effects) regardless of game result, quarter result, and quarter point difference in U18, male basketball players. Accordingly, external PD attained during games may not be a key indicator of team success.

Sport-specific skills display diurnal variation across various team sports such as badminton and tennis serving accuracy and soccer dribbling, volleying, and chipping execution. However, the effects of athlete chronotype on in-game sport-specific skill performance according to time of day across team sports is not well understood. Therefore, the aim of this study was to identify the effect of player chronotype on in-game basketball performance during evening games. Professional male basketball players (n = 11) completed a morningness–eveningness questionnaire and were categorized according to chronotype (morning-type: n = 4; neither-type: n = 6; evening-type: n = 1). Box score data from the 2019/20 season were utilized to determine individual in-game performance during evening games played after 18:00 h. Composite metrics (i.e., effective field goal percentage, offensive rating, defensive rating, and player efficiency) were used as indicators of player performance. Non-significant (p ≥ 0.21) differences were evident between M-types and N-types for most performance measures. Small to very large effects were observed in the number of rebounds favoring M-types, and three-point shots attempted and made, assists, and steals favored N-types. In-game performance appeared to not be affected by chronotype (i.e., M-type vs. N-type) in evening games among professional male basketball players. The lack of observed effect between chronotype and in-game performance suggest coaching staff may not need to consider player chronotype when developing a match strategy or assigning player roles if largely dealing with M-types and N-types. However, to ensure the greatest specificity, coaching staff may endeavor to schedule habitual training times in line with that of competition in an effort to align player circadian rhythms to games.

Understanding the game demands encountered in basketball provides useful insight for developing specific, individualized and team-based training sessions. This study quantified and compared the game activity demands encountered by basketball players of different playing positions: i) strictly when in possession of the ball and ii) overall during live playing time (irrespective of ball possession). The activity demands encountered by 44 (22 guards, 14 forwards, 8 centres) adult, professional, male basketball players were assessed across 10 official games. Time-motion analysis was used to determine the frequency and proportion (%) of playing time performing recovery (REC), low- (LIA), moderate- (MIA), and high- (HIA) intensity activities. Linear mixed models were constructed to examine differences in dependent variables between playing positions, accounting for repeated measures. Guards, forwards, and centres spent 11.9±5.9%, 3.5±1.3%, and 2.9±1.1% of live playing time in possession of the ball, respectively. Guards performed more activities at all intensities (total movements, REC, LIA, MIA, and HIA) than forwards (P < 0.05) and centres (P < 0.05) when in possession of the ball. The proportion of time spent performing HIA in possession of the ball was greater for forwards (P = 0.001) and centres (P = 0.001) than guards. During live playing time overall across games, centres performed more HIA per minute (P = 0.049) and spent a greater proportion of time performing HIA (P = 0.047) than guards. Activities performed when in possession of the ball and during live playing time across basketball games are affected by playing position. These data highlight the need to develop position-specific training drills, particularly with ball possession.

The aim of this study was to examine the impact of team size on acute physiological, perceptual, and activity demands of recreational handball to provide a better understanding for the potential prescription of recreational handball to achieve health benefits. Active, male college students (N=22) completed 3-, 4-, and 5-a-side handball game formats across three separate sessions following a repeated-measures, crossover design. Heart rate (HR), blood lactate concentration (BLa), rating of perceived exertion (RPE), distance covered, and frequency of accelerations/decelerations were monitored during games. Each game format elicited vigorous intensities with a mean HR in the range 82–85%HRmax and post-game BLa in the range 3.9–4.4 mmol·L-1. No significant differences (P>0.05) in absolute (ηp2=0.40), relative mean HR (ηp2=0.43), BLa (ηp2=0.16), total distance (ηp2=0.32), total accelerations (ηp2=0.23), or total decelerations (ηp2=0.23) were observed between game formats. A significant effect was observed for RPE (ηp2=0.51), where 3-a-side games elicited a higher RPE than 5-a-side games (P=0.03, large). Modifying player number has a negligible effect on the physiological and activity demands encountered during recreational handball games. Recreational handball consisting of 3–5 players imposes similar intermittent workloads, resulting in vigorous physiological responses concomitant with those recommended for overall health improvements as part of regular training.

The aim of this study was to analyze the effects of different factors on the external peak demands (PD) encountered by elite, junior, male basketball players in games, including the (1) total playing time during games and (2) playing time accumulated directly prior to each PD episode. Workload variables included the PD for total distance, distance covered in different intensity zones, accelerations >2 m·s -2 (ACC), decelerations 18 km·h -1 ) distance (2-min window), and ACC (30-s, 45-s, 1-min, 2-min, and 5-min windows) were significantly ( p < .05) higher for players who completed lower total playing times (16.6 ± 2.4 min) than players who completed higher total playing times (25.0 ± 3.4 min). The PD for total distance (30-s, 45-s, 1-min, and 2-min windows), high-speed running distance (30-s and 5-min windows), and PlayerLoad (1-min and 2-min windows) were significantly ( p < .05) higher for players who accumulated lower playing times before each PD episode than players who accumulated higher playing times before each PD episode. Players who undertake less playing time overall and prior to each PD episode can reach higher peak external loads aggregated across varied time windows. These findings can inform tactical coaching decisions during games for high external loads to be accomplished during important passages of play.

Neurocognitive impairment (NCI) associated with HIV infection of the brain impacts a large proportion of people with HIV (PWH) regardless of antiretroviral therapy (ART). While the number of PWH and severe NCI has dropped considerably with the introduction of ART, the sole use of ART is not sufficient to prevent or arrest NCI in many PWH. As the HIV field continues to investigate cure strategies, adjunctive therapies are greatly needed. HIV imaging, cerebrospinal fluid, and pathological studies point to the presence of continual inflammation, and the presence of HIV RNA, DNA, and proteins in the brain despite ART. Clinical trials exploring potential adjunctive therapeutics for the treatment of HIV NCI over the last few decades have had limited success. Ideally, future research and development of novel compounds need to address both the HIV replication and neuroinflammation associated with HIV infection in the brain. Brain mononuclear phagocytes (MPs) are the primary instigators of inflammation and HIV protein expression; therefore, adjunctive treatments that act on MPs, such as immunomodulating agents, look promising. In this review, we will highlight recent developments of innovative therapies and discuss future approaches for HIV NCI treatment.

PurposeThe aims of this study were to (1) quantify the multidimensional attributes of male and female basketball athletes under 16 years of age (U16) and under 18 years of age (U18), and (2) identify attributes that distinguish selection into a talent pathway according to sex and age group.Methods67 male and 71 female athletes competing in U16 and U18 selection trials for a state based Australian basketball talent pathway completed a multidimensional testing battery. The test battery consisted of anthropometric, physical (20- linear sprint, countermovement jump height, Yo-Yo Intermittent Recovery Test Level 2), technical (Basketball Jump Shooting Accuracy Test), tactical (video decision making), and psychological (Sports Orientation Questionnaire, Psychological Performance Inventory-Alternative) assessments. Mean differences and independent t-tests were used to assess comparative differences between selected and non-selected athletes within each age and sex cohort. Stepwise discriminant analyses were used to identify attributes that were the strongest discriminators of selection in each group (male U16, male U18, female U16, and female U18).ResultsThe discrimminant models showed for male U16 athletes smaller height (ES = -0.18) and greater shooting accuracy (ES = 0.52) was most discriminant of selection. Results were largely homogenous for male U18 athletes with lower visualisation score (ES = -0.62) most discriminant of selection. In female cohorts, faster 20-m sprint time (ES = -0.66) and taller height (ES = 0.58) was most discriminant of selection in U16 athletes while greater shooting accuracy (ES = 0.67), countermovement jump height (ES = 1.04), and height (ES = 0.65) was most discriminant of selection in U18 athletes.ConclusionsThese results emphasise the differing selection priorities within adolescent basketball cohorts according to sex and age group. The testing of anthropometric, physical and technical attributes may hold particular utility in adolescent female basketball given their identified importance to selection across U16 and U18 cohorts.

Anti-retroviral therapy (ART) has been highly successful in controlling HIV replication, reducing viral burden, and preventing both progression to AIDS and viral transmission. Yet, ART alone cannot cure the infection. Even after years of successful therapy, ART withdrawal leads inevitably to viral rebound within a few weeks or months. Our hypothesis: effective therapy must control both the replicating virus pool and the reactivatable latent viral reservoir. To do this, we have combined ART and immunotherapy to attack both viral pools simultaneously. The vaccine regimen consisted of DNA vaccine expressing SIV Gag, followed by a boost with live attenuated rubella/gag vectors. The vectors grow well in rhesus macaques, and they are potent immunogens when used in a prime and boost strategy. We infected rhesus macaques by high dose mucosal challenge with virulent SIVmac251 and waited three days to allow viral dissemination and establishment of a reactivatable viral reservoir before starting ART. While on ART, the control group received control DNA and empty rubella vaccine, while the immunotherapy group received DNA/gag prime, followed by boosts with rubella vectors expressing SIV gag over 27 weeks. Both groups had a vaccine "take" to rubella, and the vaccine group developed antibodies and T cells specific for Gag. Five weeks after the last immunization, we stopped ART and monitored virus rebound. All four control animals eventually had a viral rebound, and two were euthanized for AIDS. One control macaque did not rebound until 2 years after ART release. In contrast, there was only one viral rebound in the vaccine group. Three out of four vaccinees had no viral rebound, even after CD8 depletion, and they remain in drug-free viral remission more than 2.5 years later. The strategy of early ART combined with immunotherapy can produce a sustained SIV remission in macaques and may be relevant for immunotherapy of HIV in humans.
