Objective: The reverse radial forearm flap has been used for soft-tissue hand defect surgical procedures worldwide. One of the major drawbacks of this flap, however, is donor site morbidity, as the donor site is closed with a skin graft. Problems with skin graft donor areas include adhesion, contracture, and wound-healing complications. In this study, only the adipofascial component of a reverse radial forearm flap was used to prevent these problems; in addition, a skin graft was applied over the flap instead of over the donor site. Methods: Between January 2011 and December 2013, a total of 13 hand defects were reconstructed with a reverse adipofascial radial forearm flap. Patients were evaluated for functional results using total active motion criteria and disability of the arm, shoulder, and hand scores, operation time, hospitalization time, and patient satisfaction. Results: All flaps and grafts placed on flaps survived completely and donor sites healed without complications. The total active motion criteria and the disability of the arm, shoulder, and hand score demonstrated that the functional outcomes were successful. Patient satisfaction scores using the visual analog scale had a mean of 88.3 (SD = 2.95) mm. Operation time for the flap surgery was 126.1 (SD = 21.80) minutes, and patients were discharged at an average of 6.3 (SD = 1.44) days. Conclusion: Use of an adipofascial component in reverse radial forearm flap surgery is appropriate for reducing problems with donor site skin grafts. Patients' functional outcomes denoted that the reverse adipofascial radial forearm flap is a reliable and effective method to cover soft-tissue defects of the hand. Level of Evidence: IV

Amaç: Üst gözkapağına altın ağırlık yerleştirilmesi, lagoftalmi tedavisinde genel kabul görmüş bir yöntemdir. İngilizce literatürde "custom-made" olarak geçen şekilde kişiye özel yapılabileceği gibi, hazır ticari form olarak veya kuyumcu aracılığıyla da temin edilebilir. İstenmeyen postoperatif sonuçları önlemek için kliniğimizde uygulanan algoritmanın paylaşılması amaçlanmıştır.Gereç ve Yöntemler: 2012-2014 yılları arasında lagoftalmi için altın implantla tedavi planlanan yedi hasta için preoperatif ağırlık testi, oturur pozisyonda bekletme, delikler açıldıktan sonraki ağırlığın saptanması, altın ağırlığın iki kez denenmesi ve sterilizasyon işlemleri uygulandı. Erken ve geç komplikasyonlar, altın gramajı ve maliyet değerlendirildi.Bulgular: Hastaların hiçbirinde erken veya geç komplikasyona rastlanmadı. Gözkapağında tam kapanma elde edildi. Hastaların estetik değerlendirmeleri, hekim değerlendirmesinden daha iyi olarak saptandı.Sonuç: Altın implant tedavisinin başarısını belirleyen en önemli etken altının ağırlığıdır. Maliyet etkin ve hasta memnuniyeti yüksek olan kuyumcu aracılığı ile temin yönteminde basit preoperatif kriterlere dikkat edilerek postoperatif morbiditeyi azaltmak ve başarıyı artırmak mümkündür, Objective: Placing gold implants in the upper eyelids is a widely accepted method in lagophthalmos treatment. Gold can be obtained as custom-made or from medical firms, as well as from jewelers. This review intends to share the algorithm applied in our clinic, to help avoid undesirable postoperative results. Material and Methods: Between 2012 and 2014, seven patients were planned to undergo gold implant surgery for lagophthalmos. In each of the seven cases preoperative weight tests were performed in upright-seated position, incision site and implant weights were measured, and the determined weights were tested twice, sterilization was applied.Results: None of the patients showed any early or late complications. Complete closure of the eyelids was achieved. Aesthetic evaluations of the patients were found to be better than the physician’s evaluations.Conclusion: The weight of the gold is the most important factor that determines the success of the implant treatment. It is possible to reduce postoperative morbidity and increase success by adhering to simple preoperative criteria when gold is obtained from jewelers, a cost-effective method that yields high patient satisfaction

Giriş: Renal hücreli karsinom, böbrek korteksinden kaynaklanan malign bir tümördür. Tümör yan ağrısı, makroskopik hematüri ve palpe edilebilen abdominal kitle bulguları vermektedir. Bir grup hastada nadir de olsa paraneoplastik sendromlara neden olabilen metastazlar yapmaktadır. Renal hücreli karsinomların cilt metastazları nadir görülmektedir. Olgu: Parotis tümörünü taklit eden ve paraneoplastik sendromlara neden olan nadir bir renal hücreli karsinom metastazı vakası tartışılacaktır. Sonuçlar : Parotis kitlesinin patolojik incelemesi ile böbrek tümörü tanısı konulan hasta, primer tümör tedavisi için üroloji kliniğine yönlendirildi. Tartışma: Tümör ile uğraşan kliniklerde alınan kitlelerin patolojik incelemesinin yapılması çok önemlidir. Hastanın sistemik olarak değerlendirilmesi, cerrahi işlem ile beraber eşlik edecek diğer noktalara da odaklanılması önem arzetmektedir., Introduction: Renal cell carcinoma is a malignant tumor which arising from the renal cortex. Tumor has some symptoms such as flank pain, macroscopic hematuria, and a palpable abdominal mass. Rarely a group patients have metastasizes which cause paraneoplastic syndromes. Cutaneous metastases are quite rare. Case: In this article we will be discussed a rare renal cell carcinoma metastases case which mimicking parotid tumor. Results: After pathological examination, patient were diagnosed with renal cell carcinoma and for the primary tumor treatment, he was referred to the urology clinic. Conclusion: Clinics which are dealing with tumor, are very important to the pathological examination. Patients should be considered as a systemic and the surgeon should focus on other systemic point.

Endoscopic treatment of maxillofacial fractures is a minimally invasive method and which is the application of a different way of open reduction internal fixation (ORIF) procedure. Between April 2013-December 2013, 13 patients admitted with maxillofacial trauma were treated with this method. All patients were evaluated clinically and radiographically. In the course of postoperative evaluation, two minor and one major complications were observed in 3 patients in total. In other patients, satisfactory results were obtained. In our study, in accordance with the data obtained from these patients, operative techniques have been described in detail and the results were compared with literature. We suppose that endoscopic surgery, which we consider as a different and innovative method for maxillofacial trauma surgery, will take part of standard treatment protocols with appropriate patient, equipment and experience., Maksillofasiyal kırıkların endoskopik yolla tedavisi minimal invaziv bir yöntemdir ve açık redüksiyon internal fiksasyon (ARİF) yönteminin farklı bir yolla uygulamasıdır. Nisan 2013-Aralık 2013 tarihleri arasında maksillofasiyal travma nedeni ile başvuran 13 hasta bu yöntemle tedavi edildi. Hastalar preoperatif klinik ve radyolojik olarak değerlendirildi. Postoperatif değerlendirmelerde toplam 3 hastada iki minör, bir majör komplikasyonlar gözlendi. Diğer hastalarda memnun edici sonuçlar elde edildi. Çalışmamızda bu hastalardan elde edilen veriler doğrultusunda ayrıntılı operasyon teknikleri anlatılmış ve sonuçlar literatürle karşılaştırılmıştır. Maksillofasiyal travma cerrahisinde farklı ve yenilikçi bir yöntem olduğunu düşündüğümüz endoskopik cerrahinin uygun hasta ekipman ve tecrübe ile standart tedavi protokolleri arasında yer alacağı kanaatindeyiz.

Ayak bileği ve topuk bölgesinde; diyabet, osteomyelit, travma, tümör eksizyonu gibi sebeplerle doku defektleri oluşabilmekte ve bunların flepler ile kapatılması gerekmektedir. Ters akımlı sural ada flebi bu bölge defektlerinin tamirinde kullanılan güvenilir bir seçenektir. Travmaya bağlı doku defekti olan, sigara içicisi veya diyabetli 11 hastaya geciktirme prosedürü ile ters akımlı sural flep uygulaması yapıldı. Hiçbir hastada flep kaybı görülmezken 2 hastada oluşan minör problemler dışında sorun yaşanmadı. Klinik sonuçların yanında hastaların memnuniyetleri, 100 mm’lik ‘hasta memnuniyet skalası’ ile değerlendirildi. Bu yazıda ayak bileği ve topuk bölgesi defektlerine serbest flebe alternatif olarak uyguladığımız geciktirme yapılmış ters akımlı sural ada flebi ile rekonstrüksiyon olgularımız ve memnuniyet dereceleri sunulmuştur., In ankle and heel region, tissue defects can be occurred due to diabetes, osteomyelitis, trauma and tumor excisions and these defects should be repaired with flaps. Reverse-flow sural island flap is a reliable alternative for repairing these regions’ defects. Delayed reverse-flow sural island flap procedure was applied for 11 smoker or diabetic patients who have tissue defects due to trauma. While we haven’t seen flap failure in any patient, we also didn’t have any difficulty except 2 minor problems. Besides clinical results, patient satisfaction was evaluated with “patient satisfaction rate scale”. In this article, it is reported our ankle and heel reconstruction cases with delayed reverse-flow sural island flap as an alternative of free flap and their satisfaction rate.
