Abstract

Intra-abdominal infections (IAI) are an important cause of morbidity and are frequently associated with poor prognosis, particularly in high-risk patients. The cornerstones in the management of complicated IAIs are timely effective source control with appropriate antimicrobial therapy. Empiric antimicrobial therapy is important in the management of intra-abdominal infections and must be broad enough to cover all likely organisms because inappropriate initial antimicrobial therapy is associated with poor patient outcomes and the development of bacterial resistance. The overuse of antimicrobials is widely accepted as a major driver of some emerging infections (such as C. difficile), the selection of resistant pathogens in individual patients, and for the continued development of antimicrobial resistance globally. The growing emergence of multi-drug resistant organisms and the limited development of new agents available to counteract them have caused an impending crisis with alarming implications, especially with regards to Gram-negative bacteria. An international task force from 79 different countries has joined this project by sharing a document on the rational use of antimicrobials for patients with IAIs. The project has been termed AGORA (Antimicrobials: A Global Alliance for Optimizing their Rational Use in Intra-Abdominal Infections). Abstract

Background The Jordan Cancer Registry was established in 1996, since which time all cases of cancer have been reported and registered. We have used this registry to perform the first analysis of skin cancer in northern Jordan and to compare our findings with those of published reports from other regions. Methods All histopathologically proven cases of skin cancer, reported during the years 1997 through 2001, were reviewed. Information regarding tumor type, age, gender, and anatomical location was collected. Results A total of 272 cases of malignant skin tumors were diagnosed between the years 1997 and 2001. Basal cell carcinoma (BCC) was the commonest type, representing 52.9% of all skin cancers. Females were more frequently affected than males, with age-adjusted incidence rates of 23.3 and 19.7 per 100,000 of population, respectively. Squamous cell carcinoma (SCC) comprised 26.4% of the total, its age-adjusted incidence rate per 100,000 of population being 14.2 for males and 6.18 for females. the incidence rate increased in males and decreased in females during the study period. The incidence of both BCC and SCC increased with age. The head and neck region was the commonest site affected by both types of cancer. Malignant melanoma (MM) comprised 11.39% of all skin cancer cases, with a female to male ratio of 1.2 : 1. The median age at onset for female patients was 49 years while that for males was 70 years, and the commonest site affected was the lower limbs, followed by the trunk. Conclusions In Jordan, sun-related skin cancers have relatively low incidences and a rather stable pattern, compared with other areas with similar climate and skin phenotypes.

3. Abdominal wall endometriosis. An overlooked diagnosis

Abstract

To study the incidence of abdominal wall endometriosis after cesarean section and its presentation to the general surgeon.Fourteen patients were treated for abdominal wall endometriosis during the period June 1997 to May 2002 at Princess Basma Teaching Hospital and King Abdulla University Hospital, Irbid, Jordan. The patient's files were reviewed to see their way and time of presentation after cesarean section, provisional diagnosis made and operative procedures performed. Symptoms suggestive of and investigations carried out to detect pelvic endometriosis were also looked for and recorded.Fourteen patients were treated within 5 years; all had painful scar-related mass. The pain was exacerbating during menstruation in 5. The clinical diagnosis was stitch granuloma in 3; incisional hernia in 3, abdominal wall tumor in 3 and abdominal wall endometrioma in 5 patients. The mean time for the mass to be noticed by the patient was 2 years. They were treated with wide local excision. Histopathological examination proved the diagnosis of abdominal wall endometriosis. None had evidence of pelvic endometriosis and none of them had recurrence. The incidence of the disease is around 0.2% of the cesarean sections performed during the same period.The treating physician should keep in mind abdominal wall endometriosis as a possible cause of post cesarean section scar-related masses.

4. Surgery and postoperative mebendazole in the treatment of hydatid disease

Abstract

Surgery was virtually the only curative method for hydatid disease until the early 1970s when studies have shown the safety and effectiveness of benzomidazole compounds for the treatment of hydatid cysts. Our aim is to establish the outcome of chemotherapy in the postoperative period on secondary hydatid cyst.Between 1987 to 1996 inclusive, all patients who were diagnosed with hepatic hydatid disease and who underwent surgery and received a post operative course of chemotherapy at Princess Basma Teaching Hospital, Irbid, North Jordan, were retrospectively reviewed with special referral to side effects of chemotherapy and recurrence rate of hydatid disease.There were 78 patients (41 females, 37 males). All age groups were involved with an age range of 8-70 years. The main clinical presentation was upper abdominal discomfort, heaviness and mild pain. Ultrasonography and indirect hemagglutination test diagnosed more than 90% of cases. All patients were operated upon and received a postoperative prophylactic course of mebendazole. We reported a recurrence rate of less than 3%. Excessive loss of hair as a side effect to mebendazole therapy occurred in 2 young female patients.The postoperative prophylactic course of mebendazole is reliable, safe and with minor side effects and the recurrence rate of the disease was reduced to the lowest possible levels.
