5. Quality of life at older ages and marital status : gender and welfare regime variation

Abstract

Background: Evidence of an association between marital status and well-being has been demonstrated, with married people reporting higher levels of well-being. However, the strength of this relationship in later life may be influenced by both societal context and gender. This thesis will examine the association between marital status and quality of life in older people and consider if this relationship varies by welfare arrangements and if gender moderates these associations. This research will also explore how quality of life is experienced within married couples. Methods: Quality of life was measured using CASP-12. The relationship with both current and past marital status was examined. Analyses were conducted using data from The Survey of Health, Ageing and Retirement in Europe (SHARE) and the English Longitudinal Study of Ageing (ELSA). A welfare state regime approach was used to examine societal context and due to feminist criticisms of these approaches, two further methods of comparison were used. A dyadic data technique was also used to examine the interdependence of quality of life for married people. Results: Current marital status was found to be a predictor of quality of life at older ages. However, variation in this association was observed across the welfare state regimes. When health and socio-economic circumstances were taken into account, the advantage of marriage for quality of life was often attenuated. Gender differences in this relationship were also observed; especially when gender focused methods were examined. Spousal interdependence of quality of life was also observed within married couples. Conclusions: This research expands our understanding of the association between marriage and well-being by suggesting that societal context is important and that marriage may offer men and women different kinds of protection as they age. It also suggests that for older married couples, quality of life is not just an individual experience.

Abstract

Background: Falls in later life that require admission to hospital have well-established consequences for future disability and health. The likelihood and severity of a fall will result from the presence of one or more risk factors. The aim of this study is to examine risk factors identified for their ability to prevent falls and to assess whether they are associated with hospital admission after a fall.Methods: Analyses of data from the English Longitudinal Study of Aging (ELSA), a prospective cohort study. In a sample of 3783 men and women older than 60 years old, a range of potential risk factors measured at Wave 4 (demographic, social environment, physical, and mental functioning) were examined as predictors of fall-related hospitalizations, identified using International Classification of Diseases, 10th Revision (ICD-10) code from linked hospital records in the United Kingdom. Subdistribution hazard models were used to account for competing risk of death.Results: Several risk factors identified by previous work were confirmed. Suffering from urinary incontinence (subdistribution hazard ratio = 1.49; 95% CI: 1.14, 1.95) and osteoporosis (subdistribution hazard ratio = 1.48; 95% CI: 1.05, 2.07), which are not commonly considered at an early stage of screening, were found to be associated with hospital admission after a fall. Both low and moderate levels of physical activity were also found to somewhat increase the risk of hospital admission after a fall.Conclusions: Several predictors of having a fall, severe enough to require hospital admission, have been confirmed. Logistic regression was used to test associations between parental absence (0-7 years) and reports of smoking and alcohol consumption at age 11.Results: Children who experienced parental absence were more likely to have smoked (OR=2.58, 95% CI 1.88 to 3.56) and consumed alcohol (OR=1.46, 95% CI 1.25 to 1.72). No differences were found by child sex or age, or parent absent. Children who experienced parental death were less likely to have drunk alcohol but those who had were more likely to have consumed enough to feel drunk.Conclusions: Parental absence was associated with early uptake of risky health behaviours in a large, nationally representative UK cohort. Abstract

• Summary: This study examines the early impact on care coordinators’ (care managers’) work activity patterns of implementing the current personalization agenda within English local authorities. The Individual Budget (IB) pilots operated between 2005 and 2007 and provided a basis for personalization that, ultimately, sought to give personal care budgets to every eligible service user in England. Of particular interest was how the pilots impacted upon the roles, responsibilities and activity of care coordinators, who are expected to play a key role in this transformation of social care. A self-administered diary schedule was completed by 249 care coordinators, including teams directly involved in delivering IBs and a comparative sample of teams not involved in the pilots. These data were supplemented by semi-structured interviews with 48 care coordinators and 43 team managers.• Findings: The study found that on most measures there were no differences in working patterns between care managers with and without IB holders on their caseload. However, the results do show that – contrary to expectations – more time was spent assessing needs, and that more time generally was required to conduct support planning activities.• Application: The findings are necessarily dependent upon the early experiences of the pilot phase of IBs. As personal budgets are rolled out across all eligible service users, it will be interesting to examine whether the time-use of frontline staff, and indeed the wider organization, structure and function of local authority frontline teams, changes further. Abstract

Throughout the history of political research, both public opinion and citizen knowledge have been extensively studied, examined and surveyed. In my paper, I will evaluate the role of the public and the source of their decisions as it relates to the controversial issue of Capital Punishment. I will review the importance of legal sanctions and events that have occurred during the modern era of capital punishment. I will also evaluate, by surveying undergraduate students, the decision-making process and opinions of students and whether or not exposure to informational material influences those decisions. Media influence, personal or religious beliefs and ignorance contribute to the support of a policy or candidate. By utilizing a pre and posttest survey, I can evaluate decisions based on feelings versus academic and educational stimuli. Historically, research has shown that Americans are largely ill-informed and the public is typically ignorant on the specific intricacies of policies and other politically related issues. Exposure to information will likely change the policy preferences and opinions of Americans. The relationship between public opinion, candidate support and policy issues is crucial in the field of politics and the study of political science. The issues and policies that the public passionately supports or opposes has the potential to affect its priority among political leaders.
