Abstract

Current quantum key distribution (QKD) networks focus almost exclusively on transporting secret keys with the highest possible rate. Consequently, they are built as mostly fixed, ad hoc, logically, and physically isolated infrastructures designed to avoid any penalty to the quantum channel. This architecture is neither scalable nor cost-effective and future, real-world deployments will differ considerably. The structure of the MadQCI QKD network presented here is based on disaggregated components and modern paradigms especially designed for flexibility, upgradability, and facilitating the integration of QKD in the security and telecommunications-networks ecosystem. These underlying ideas have been tested by deploying many QKD systems from several manufacturers in a real-world, multi-tenant telecommunications network, installed in production facilities and sharing the infrastructure with commercial traffic. Different technologies have been used in different links to address the variety of situations and needs that arise in real networks, exploring a wide range of possibilities. Finally, a set of realistic use cases have been implemented to demonstrate the validity and performance of the network. The testing took place during a period close to three years, where most of the nodes were continuously active.

Abstract

A Bell test is a randomized trial that compares experimental observations against the philosophical worldview of local realism. A Bell test requires spatially distributed entanglement, fast and high-efficiency detection and unpredictable measurement settings. Although technology can satisfy the first two of these requirements, the use of physical devices to choose settings in a Bell test involves making assumptions about the physics that one aims to test. Bell himself noted this weakness in using physical setting choices and argued that human `free will' could be used rigorously to ensure unpredictability in Bell tests. Here we report a set of local-realism tests using human choices, which avoids assumptions about predictability in physics. We recruited about 100,000 human participants to play an online video game that incentivizes fast, sustained input of unpredictable selections and illustrates Bell-test methodology. The participants generated 97,347,490 binary choices, which were directed via a scalable web platform to 12 laboratories on five continents, where 13 experiments tested local realism using photons, single atoms, atomic ensembles, and superconducting devices. Over a 12-hour period on 30 November 2016, participants worldwide provided a sustained data flow of over 1,000 bits per second to the experiments, which used different human-generated data to choose each measurement setting. The observed correlations strongly contradict local realism and other realistic positions in bipartite and tripartite scenarios. Project outcomes include closing the `freedom-of-choice loophole' (the possibility that the setting choices are influenced by `hidden variables' to correlate with the particle properties), the utilization of video-game methods for rapid collection of human generated randomness, and the use of networking techniques for global participation in experimental science., Comment: This version includes minor changes resulting from reviewer and editorial input. Abstract

For more than 80 years, the counterintuitive predictions of quantum theory have stimulated debate about the nature of reality. In his seminal work, John Bell proved that no theory of nature that obeys locality and realism can reproduce all the predictions of quantum theory. Bell showed that in any local realist theory the correlations between distant measurements satisfy an inequality and, moreover, that this inequality can be violated according to quantum theory. This provided a recipe for experimental tests of the fundamental principles underlying the laws of nature. In the past decades, numerous ingenious Bell inequality tests have been reported. However, because of experimental limitations, all experiments to date required additional assumptions to obtain a contradiction with local realism, resulting in loopholes. Here we report on a Bell experiment that is free of any such additional assumption and thus directly tests the principles underlying Bell's inequality. We employ an event-ready scheme that enables the generation of high-fidelity entanglement between distant electron spins. Efficient spin readout avoids the fair sampling assumption (detection loophole), while the use of fast random basis selection and readout combined with a spatial separation of 1.3 km ensure the required locality conditions. We perform 245 trials testing the CHSH-Bell inequality $S \leq 2$ and find $S = 2.42 \pm 0.20$. A null hypothesis test yields a probability of $p = 0.039$ that a local-realist model for space-like separated sites produces data with a violation at least as large as observed, even when allowing for memory in the devices. This result rules out large classes of local realist theories, and paves the way for implementing device-independent quantum-secure communication and randomness certification., Comment: Raw data will be made available after publication

6. Ultra-fast quantum randomness generation by accelerated phase diffusion in a pulsed laser diode

Abstract

We demonstrate a high bit-rate quantum random number generator by interferometric detection of phase diffusion in a gain-switched DFB laser diode. Gain switching at few-GHz frequencies produces a train of bright pulses with nearly equal amplitudes and random phases. An unbalanced Mach-Zehnder interferometer is used to interfere subsequent pulses and thereby generate strong random-amplitude pulses, which are detected and digitized to produce a high-rate random bit string. Using established models of semiconductor laser field dynamics, we predict a regime of high visibility interference and nearly complete vacuum-fluctuation-induced phase diffusion between pulses. These are confirmed by measurement of pulse power statistics at the output of the interferometer. Using a 5.825 GHz excitation rate and 14-bit digitization, we observe 43 Gbps quantum randomness generation.

Abstract

More than 50 years ago (1), John Bell proved that no theory of nature that obeys locality and realism (2) can reproduce all the predictions of quantum theory: in any local-realist theory, the correlations between outcomes of measurements on distant particles satisfy an inequality that can be violated if the particles are entangled. Numerous Bell inequality tests have been reported (3-13); however, all experiments reported so far required additional assumptions to obtain a contradiction with local realism, resulting in 'loopholes' (13-16). Here we report a Bell experiment that is free of any such additional assumption and thus directly tests the principles underlying Bell's inequality. We use an event-ready scheme (17-19) that enables the generation of robust entanglement between distant electron spins (estimated state fidelity of 0.92 ± 0.03). Efficient spin read-out avoids the fair-sampling assumption (detection loophole (14,15)), while the use of fast random-basis selection and spin read-out combined with a spatial separation of 1.3 kilometres ensure the required locality conditions (13). We performed 245 trials that tested the CHSH-Bell inequality (20) S ≤ 2 and found S = 2.42 ± 0.20 (where S quantifies the correlation between measurement outcomes). A null-hypothesis test yields a probability of at most P = 0.039 that a local-realist model for space-like separated sites could produce data with a violation at least as large as we observe, even when allowing for memory (16,21) in the devices. Our data hence imply statistically significant rejection of the local-realist null hypothesis. This conclusion may be further consolidated in future experiments; for instance, reaching a value of P= 0.001 would require approximately 700 trials for an observed S = 2.4. Abstract

Quality management systems (QMS) are tools that serve to structure, control and improve the usual activities that take place in an organization or service. The ISO 9001:2015 is an internationally recognized standard, which provides the necessary resources to help an organization to improve its performance, based on the principle of plan-do-control-act, in order to obtain continuous improvement. In the field of health, it is an essential tool for the management of the services offered to patients. The ISO quality certification allows to demonstrate compliance, according to established quality standards. The process of implementing a QMS follows several phases that culminate with the completion of an external audit, which once passed, allows obtaining the quality certification ISO 9001:2015. 21. Littératie en santé : Situation des personnes incarcérées dans le canton de Vaud

Abstract

La première définition internationalement acceptée de la littératie a été définie par l’UNESCO en 1958. Elle indique qu’une personne « lettrée » est une personne qui peut, en en comprenant le sens, lire ou écrire un énoncé simple dans sa vie de tous les jours(1). Cette définition, très réductrice, n’a pas beaucoup évolué dans le temps en dehors de littératures spécifiques au sujet. Elle est définie aujourd’hui selon l'OCDE comme étant « l'aptitude à comprendre et à utiliser l'information écrite dans la vie courante, à la maison, au travail et dans la collectivité en vue d'atteindre des buts personnels et d'étendre ses connaissances et ses capacités ».(2) Ces définitions nous montrent que l’idée commune de ce terme est celle d’un ensemble de compétences, en particulier les compétences cognitives de lire et d’écrire(3).Cependant, lorsque l’on parle d’individus présentant une mauvaise littératie, il est important de ne pas confondre ce terme avec ceux de l’illettrisme ou de l’analphabétisme : - L’illettrisme désigne d’une part « la situation de jeunes et d'adultes qui parlent la langue du pays ou de la région dans laquelle ils vivent, qui ont fréquenté l'école obligatoire, mais qui maîtrisent mal les compétences de base que sont la lecture et l'écriture »(4). - D’autre part, on parle d’analphabétisme pour désigner « des personnes qui n’ont jamais été scolarisées. Il s’agit pour elles d’entrer dans un premier niveau d’apprentissage »(5). Alors que les termes d’illettrisme et d’analphabétisme se concentrent sur les compétences de lecture et d’écriture, la littératie est un concept bien plus étendu. Elle peut non seulement se mesurer sur les critères de lecture, d’écriture, de langage ou de calcul(6) mais également sur ceux de l’information, de la science, de la nutrition, de la santé etc.(3)(7) Une faible ou mauvaise littératie dans un domaine précis n’implique donc pas une déficience générale en littératie. Il s’agit plutôt d’une lacune spécifique visant à être comblée par l’intérêt d’un sujet ou par son besoin dans un contexte donné. Nous pouvons ici la comparer au terme de « capital culturel »(8) relatif à la santé qui sera déterminé par les ressources d’un individu et définira donc des inégalités face à la santé. Dans ce travail, nous avons choisi de nous focaliser sur la littératie en santé qui, bien qu’elle soit souvent corrélée au niveau d’éducation scolaire des individus(9), relève également d’autres problématiques que nous développerons par la suite.

25. Acute Fibrinous and Organizing Pneumonia (AFOP) in Lung Transplant Recipient: A Case Report of Successful Treatment with Infliximab® and Tocilizumab®.

Abstract

Acute fibrinous and organizing pneumonia (AFOP) is an uncommon yet severe complication after lung transplantation (LT). We discuss the case of an 18-year-old LT recipient at 5-years post-transplantation with suboptimal therapeutic adherence to immunosuppressive therapy who developed AFOP following a viral infection. Following a positive naso-pharyngeal PCR for Adenovirus and Rhinovirus , the patient rapidly developed dyspnea at rest. Pulmonary functions showed rapid harmonious loss of both forced expiratory volume in 1 second (FEV1) and forced vital capacity. Thoracic CT-scan revealed multifocal ground glass opacities associated with lower lobes peribronchial condensations (Figure 1C and E). Despite high dose corticosteroids (CS) (Methylprednisolone 500 mg IV daily for 3 days), no clinical improvement was achieved. Cryobiopsies showed intra-alveolar fibrin balls (arrows in Figure 1A and B) and both bronchial aspiration and bronchoalveolar lavage PCR were positive for Aspergillus spp (286000 and 41000 copies/ml, respectively) with no hyphae at histopathology. Her condition rapidly deteriorated with severe hypoxemic respiratory failure necessitating orotracheal intubation (OTI). IV Infliximab® 250 mg (based on cases report), Tocilizumab® 8mg/kg (based on IL-6 elevation on serum cytokinic panel) and CS (prednisone 1 mg/kg) were administered. Aspergillus spp was treated with IV Voriconazole. The patient was extubated after 11 days. A second dose of Infliximab® 150 mg was administered 3 weeks later. Repeat CT-scan showed resolution of ground glass opacities (Figure 1D and F). The patient was discharged at home 42 days post-OTI with 1L/min O2. An 18-year-old LTx recipient developed AFOP with severe respiratory failure necessitating OTI. Infliximab®, Tocilizumab® and high dose CS allowed weaning of mechanical ventilation on day 11 and home discharge on day 42 after OTI. [ABSTRACT FROM AUTHOR]

We report an experimental violation of a Bell inequality with strong statistical significance. Our experiment employs polarization measurements on entangled single photons and closes the locality, freedom-of-choice, fair-sampling, coincidence-time, and memory loopholes simultaneously.

International audience; The aim of irradiation monitoring during a treatment in ion therapy is to control in real time the agreement between the delivered dose and the planned treatment. In fact, the discrepancies might come from uncertainties such as the positioning or the anatomical changes of the patient can lead to ion-range variations of a few millimeters. Several devices are under development over the world to detect secondary radiations, which are correlated to the dose deposited by incident ions. Compton cameras are in particular investigated for their potential high efficiency. The present work aims at discussing the clinical applicability of a Compton camera design by means of Monte Carlo simulations validated against measurements of single and coincidence rates.

Indeed, prompt-gamma longitudinal profiles are correlated to the ion range.

Abstract

In this paper the Front End electronics of the Scintillator Pad Detector (SPD) is outlined. The SPD is a sub-system of the Calorimeter of the LHCb experiment designed to discriminate between charged and neutral particles for the first level trigger. The system design is presented, describing its different functionalities implemented through three different cards and several ASICs. These functionalities are signal processing and digitization, data transmission, interface with control and timing systems of the experiment, low voltage power supply distribution and monitoring. Special emphasis is placed on installation and commissioning subjects such as cabling, grounding, shielding and power distribution.

Abstract

An integrated test environment for the data acquisition electronics of the Scintillator Pad Detector (SPD) from the calorimeter of the LHCb experiment is presented. It allows to test separately every single board or to perform global system tests, while being able to emulate every part of the system and debug it. This environment is foreseen to test the production of spare electronic boards and help the maintenance of the SPD electronics along the life of the detector. The heart of the system is an Altera Stratix II FPGA while the main board can be controlled over USB, Ethernet or WiFi.

Abstract

The state of the art in the development of photocatalytic membrane reactors, for the treatment of persistent organic pollutants in water and wastewater, was identified through a documentary investigation. The review covered the following aspects: characteristics of the membranes used in the photocatalytic process, influence of the operational parameters in the photocatalytic process, configurations and designs of the photocatalytic membrane reactors, and the treatment of persistent organic pollutants with the help of these reactors. A comparison of the main types of membranes, which are used in photocatalytic reactors, was made; it was shown that membrane fouling is a critical phenomenon in the use of these materials. The main variables that influence the operation of photocatalytic membrane reactors were analyzed: reactor design, wavelength and light intensity, photocatalyst load, initial concentration of the reactant, temperature and pH, among others. The advantages and disadvantages of the photocatalytic reactors with immobilized TiO2 in the membrane were shown, being this design one of the most used. Finally, successful examples were shown, both at laboratory scale and at pilot scale, of the use of photocatalytic membrane reactors for the treatment of persistent organic pollutants, present in water and wastewater. Abstract

&beta, cyclodextrin (&beta, CD) and hydroxypropyl-&beta, cyclodextrin (HP-&beta, CD) were used to prepare insoluble polymers using epichlorohydrin as a cross-linking agent and the azo dye Direct Red 83:1 was used as target adsorbate. The preliminary study related to adsorbent dosage, pH, agitation or dye concentration allowed us to select the best conditions to carry out the rest of experiments. The kinetics was evaluated by Elovich, pseudo first order, pseudo second order, and intra-particle diffusion models. The results indicated that the pseudo second order model presented the best fit to the experimental data, indicating that chemisorption is controlling the process. The results were also evaluated by Freundlich, Langmuir and Temkin isotherms. According to the determination coefficient (R2), Freunlich gave the best results, which indicates that the adsorption process is happening on heterogeneous surfaces. One interesting parameter obtained from Langmuir isotherm is qmax (maximum adsorption capacity). This value was six times higher when a &beta, CDs-EPI polymer was employed. The cross-linked polymers were fully characterized by nuclear magnetic resonance (NMR), Fourier transform infrared spectroscopy (FTIR), thermal gravimetric analysis (TGA). Also, morphology and particle size distribution were both assessed. Under optimized conditions, the &beta, CDs-EPI polymer seems to be a useful device for removing Direct Red 83:1 (close 90%), from aqueous solutions and industrial effluents. Complementarily, non-adsorbed dye was photolyzed by a pulsed light driven advanced oxidation process. The proposed methodology is environmental and economically advantageous, considering the point of view of a sustainable recycling economy in the textile dyeing process.

Abstract

Allergic bronchopulmonary aspergillosis (ABPA) is a complex hypersensitivity reaction to Aspergillus spp. ABPA diagnosis may be challenging due to its non-specific presentation. Standard ABPA treatment consists of systemic corticosteroids and antifungal agents. Mepolizumab, a monoclonal antibody against interleukin-5 seems to be a promising treatment for ABPA. Data about ABPA following lung transplantation (LuTx) are scarce. LuTx recipients are at higher risk for adverse effects of ABPA treatment compared to the general population. Here we present a case of a LuTx recipient who was successfully treated with mepolizumab for ABPA following LuTx. Prolonged administration of high dose prednisone was thus avoided. To our knowledge, this is the first case describing mepolizumab administration following LuTx. Abstract

A growing body of evidence suggests that extrathoracic vascular accesses for transcatheter aortic valve replacement (TAVR) yield favorable outcomes and can be considered as primary alternatives when the gold-standard transfemoral access is contraindicated. Data comparing the transcaval (TCv) to supra-aortic (SAo) approaches (transcarotid, transsubclavian, and transaxillary) for TAVR are lacking. We aimed to compare the outcomes and safety of TCv and SAo accesses for TAVR as alternatives to transfemoral TAVR. A systematic review with meta-analysis was performed by searching PubMed/MEDLINE and EMBASE databases for all articles comparing TCv-TAVR against SAo-TAVR published until September 2023. Outcomes included in-hospital or 30-day all-cause mortality (ACM) and postoperative complications. A total of three studies with 318 TCv-TAVR and 179 SAo-TAVR patients were included. No statistically significant difference was found regarding in-hospital or 30-day ACM (relative risk [RR] 1.04, 95% confidence interval [CI] 0.47-2.34, p = 0.91), major bleeding, the need for blood transfusions, major vascular complications, and acute kidney injury. TCv-TAVR was associated with a non-statistically significant lower rate of neurovascular complications (RR 0.39, 95%CI 0.14-1.09, p = 0.07). These results suggest that both approaches may be considered as first-line alternatives to transfemoral TAVR, depending on local expertise and patients' anatomy. Additional data from long-term cohort studies are needed.

Abstract

Alternative vascular accesses to transfemoral access for transcatheter aortic valve replacement (TAVR) can be divided into intrathoracic (IT)-transapical and transaortic- and extrathoracic (ET)-transcarotid, transsubclavian, and transaxillary. This study aimed to compare the outcomes and safety of IT and ET accesses for TAVR as alternatives to transfemoral access. A systematic review with meta-analysis was performed by searching PubMed/MEDLINE and EMBASE databases for all studies comparing IT-TAVR with ET-TAVR published until April 2023. Outcomes included in-hospital or 30-day all-cause mortality (ACM), 1-year ACM, postoperative and 30-day complications. A total of 18 studies with 6,800 IT-TAVR patients and 5,032 ET-TAVR patients were included. IT accesses were associated with a significantly higher risk of in-hospital or 30-day ACM (relative risk 1.99, 95% confidence interval 1.67 to 2.36, p <0.001), and 1-year ACM (relative risk 1.31, 95% confidence interval 1.21 to 1.42, p <0.001). IT-TAVR patients presented more often with postoperative life-threatening bleeding, 30-day new-onset atrial fibrillation or flutter, and 30-day acute kidney injury needing renal replacement therapy. The risks of postoperative permanent pacemaker implantation and significant paravalvular leak were lower with IT-TAVR. ET-TAVR patients were more likely to be directly discharged home. There was no statistically significant difference regarding the 30-day risk of stroke. Compared with ET-TAVR, IT-TAVR was associated with higher risks of in-hospital or 30-day ACM, 1-year ACM and higher risks for some critical postprocedural and 30-day complications.
