Abstract Background Ischemia-reperfusion injury (I/R) for skeletal muscle usually results from vascular injuries or trauma. Sitagliptin (STG) is an effective member of the dipeptidyl peptidase-4 (DPP-4) inhibitors drug family that plays roles in oxidative stress regulation, inflammation, and autophagy control. In this study, we evaluated the protective roles of STG against I/R of gastrocnemius muscle and the underlying mechanisms. Materials and methods Forty-eight mice were randomly allocated into three groups: Group I (n = 24): control group which was subdivided equally into subgroup IA; negative control, subgroup IB; sitagliptin (STG), Group II (n = 12): ischemia–reperfusion injury (I/R), and Group III (n = 12): sitagliptin pretreatment (300 mg/kg/ day; p.o.) for two weeks followed by ischemia–reperfusion injury (STG + I/R). We measured SOD activity and MDA level to assess oxidative stress. Moreover, GLP-1/p-PI3K/ p-AKT expression levels were investigated. Autophagy was estimated by assessing lncRNA H19, Beclin-1 and ATG7 expression by RT-qPCR analysis. Inflammatory markers were assessed by iNOS and NF‐κB expression using immunohistochemistry. Results Our results revealed that STG pretreatment significantly attenuated oxidative stress and inflammation and upregulated GLP-1, p-PI3K, and p-AKT protein levels. Also, LnRNA H19, Becline-1, and ATG7 mRNA expression were downregulated as well as decrease the expression of the inflammatory markers iNOS and NF‐κB with STG pretreatment. Conclusion Our results highlighted the useful effects of Sitagliptin during hind limb I/R that could be mediated by antioxidant, anti-inflammatory effects, and attenuation of excessive autophagy.

Abstract Background Hyperhidrosis (HH), characterized by excessive sweating, poses a significant challenge to patients’ quality of life. This meta-analysis evaluates the safety and efficacy of topical glycopyrronium bromide (GBP) in treating primary hyperhidrosis, a chronic condition affecting various body regions. Despite its prevalence, primary axillary hyperhidrosis is often undertreated due to a lack of awareness and social stigma. Methods Following PRISMA guidelines, we conducted a systematic review and meta-analysis of randomized controlled trials comparing GBP to a placebo in primary hyperhidrosis patients. Eligibility criteria included outcomes related to perspiration suppression and symptom improvement. Results Four RCTs involving 1401 patients were included. Abstract Background Osteoarthritis OA is a common progressive disabling disease. Current research aims at finding therapies to prevent its progression. In this work, we assessed the therapeutic role of intra-articular injection of stromal vascular fraction SVF in collagenase induced knee OA in rats. Results Post right Knee OA induction in 42 Wistar rats, histopathological examination and quantification of articular cartilage degeneration using Mankin’s score revealed degenerative changes were significantly higher in untreated Group II compared to SVF treated Group III at 1 month (10.75 ± 0.50 and 2.50 ± 0.53, P = 0.001) and 2 months (8.50 ± 0.58, 0.50 ± 0.53, P = 0.001), respectively. Abstract Background Radiotherapy in head and neck cancer management causes degeneration of the salivary glands (SG). This study was designed to determine the potential of gingival mesenchymal stem cells (GMSCs) as a cell-based therapy to regenerate irradiated parotid SG tissues and restore their function using a murine model. Methods Cultured isolated cells from gingival tissues of 4 healthy guinea pigs at passage 3 were characterized as GMSCSs using flow cytometry for surface markers and multilineage differentiation capacity. Twenty-one Guinea pigs were equally divided into three groups: Group I/Test, received single local irradiation of 15 Gy to the head and neck field followed by intravenous injection of labeled GMSCs, Group II/Positive control, which received the same irradiation dose followed by injection of phosphate buffer solution (PBS), and Group III/Negative control, received (PBS) injection only. Body weight and salivary flow rate (SFR) were measured at baseline, 11 days, 8-, 13- and 16-weeks post-irradiation. At 16 weeks, parotid glands were harvested for assessment of gland weight and histological and immunohistochemical analysis. Results The injected GMSCs homed to degenerated glands, with subsequent restoration of the normal gland histological acinar and tubular structure associated with a significant increase in cell proliferation and reduction in apoptotic activity. Subsequently, a significant increase in body weight and SFR, as well as an increase in gland weight at 16 weeks in comparison with the irradiated non-treated group were observed. Conclusion The study provided a new potential therapeutic strategy for the treatment of xerostomia by re-engineering radiated SG using GMSCs.

Background: Skin diseases are usually chronic in nature but not life-threatening. They affect the well-being and pose a threat to the general health of the affected animals. Aim: This study aimed to investigate epidemiological, clinical and therapeutic aspects of ectoparasitic infestations in dogs in a number of Egyptian governorates. Methods: Ninety dogs (58 males and 32 females) aged from one month to eleven years from six Egyptian governorates were clinically examined during the years 2022 and 2023. Skin scraping samples were taken from all examined dogs, and deep ear swab specimens from five dogs suspected to have ear mites were obtained and parasitologically examined. Different ectoparasites were classified according to their morphological features. Twenty dogs were treated in four different patterns of administration of local, systemic, and supportive medications. Results: The prevalence of ectoparasite infestation in examined dogs was 64% (58/90). The majority of ectoparasitic infestations (50/58) were single, while the rest (8/58) were mixed. Ninespecies of ectoparasites of fourtaxa were identified: a tick species (Rhipicephalus sanguineus); which had the highest prevalence among isolated ectoparasites from dogs (29%, 26/90), three flea species (Ctenocephalides canis, Ctenocephalides orientis and Ctenocephalides felis) isolated from 18 out of 90 cases (20%), two types of dog chewing louse species (Trichodectes canis and Heterodoxus spiniger) isolated from 2/90 (2.2%) and three mite species: Demodex canis (18/90, 20%), Sarcoptes scabei var. canis (5/90, 6%) and Otodectes cynotis (2/90, 2.2%). Background: The utilization of optical coherence tomography [OCT] has demonstrated its efficacy in the identification and assessment of glaucomatous impairment.The Aim of the work: The objective of this study is to evaluate and compare the alterations in Optical Coherence Tomography measurements, specifically the Optic Nerve Head, Retinal Nerve Fiber Layer Thickness, and Ganglion Cell Complex, in patients diagnosed with primary open-angle glaucoma before and after undergoing subscleral trabeculectomy surgery.Patients and Methods: This study included 25 eyes having primary open angle glaucoma scheduled for subscleral trabeculectomy at Al-Azhar university hospital, Damietta. Ethical approval was obtained from ethics committee of Al-Azhar university, faculty of medicine. A written informed consent was obtained from all patients included in the study after nature and possible consequences of the procedure has been explained. The patients were evaluated by Topcon 3D OCT 2000 device. Standard resolution scans captured the temporal and nasal quadrants [nasal-temporal 0° -180°] before subscleral trabeculectomy operation and was evaluated at 1 week, 1 month and 3 months post operativelyResults: According to the total RNFL, it was 60.7 ± 13.9 µm preoperatively, and significantly increased to 71.9 ± 14.3 µm at 1 month postoperative and also, increased to 74.7 ± 13.6 µm at 3 months postoperative [P value = 0.001 for both]. According to the total GCC, it was 65.5 ± 14.9 µm preoperatively, and significantly increased to 74.1 ± 13.9 µm at 1 month postoperative and also, increased to 76.2 ± 13.5 µm at 3 months postoperative [P value = 0.001 for both]. In terms of IOP, it was significantly decreased from 35.5 ± 5.5 mmHg to 13.7 ± 1.4 at 1 month postoperative [P value = 0.001], and also, decreased to 13.9 ± 1 mmHg at 3 months postoperative [P value = 0.001].Conclusion: Sub scleral trabeculectomy is an effective surgical procedure for the treatment of Open angle glaucoma.

Objective:Acute pain management after open abdominal surgeries is an essential goal in perioperative management.. Recently, serratus-intercostal plane block (SIPB) was suggested as an analgesic technique for upper abdominal surgeries.Methods:This prospective, randomized, controlled study included sixty adult patients scheduled for open upper abdominal surgeries. Patients were allocated into two equal groups: SIPB group (S group, n = 30) and control group (the C group, n = 30). In the S group, SIPB was performed in the midaxillary line at the eighth rib level followed by continuous infusion of local anaesthetic for the first postoperative day. In the C group, no block was done. The primary objective of the study was to control postoperative pain on the first postoperative day as assessed by the numerical rating scale (NRS). Secondary outcomes included perioperative hemodynamics, total postoperative analgesic consumption, number of analgesic requests, and incidence of postoperative nausea and vomiting.Results:The mean postoperative NRS reported in group S was statistically lower than that in group C (2.4±0.7, 3.9±0.31, P < 0.001). The postoperative morphine consumption was lower in the S group than in the C group [(0 (0-4), 3 (1-4), respectively, P < 0.001]. The incidence of PONV was significantly lower in the S group than in the C group (16.7% and 40%, P < 0.045).Conclusion:SIPB was associated with a better analgesic profile compared with the control group after upper abdominal surgeries. Further studies are recommended to determine block safety in special patient groups, including bariatric and laparoscopic surgeries.

Background: Small ruminants have a socioeconomic impact on Egypt's production of meat, milk and wool. Hence, every effort should be taken to prevent infections. Aim: To elucidate the prevalence and serogrouping of Escherichia coli (E. coli) strains from diarrheic lambs and kids, determine their antibiotic susceptibility and associated risk factors affecting the occurrence of the disease and establish the most common virulence genes marker and major antimicrobial resistance (AMR) genes. Methods: A total of 150 diarrheic animals (95 lambs and 55 kids) at different age and seasons were subjected to clinical examination. Rectal swabs were collected from 150 diarrheic animals for isolation and biochemical identification of E. coli. Results: The bacteriological examination revealed that 62/95 lambs and 26/55 kids with percentages of 65 % and 47 %, respectively showed infection with E. coli. Serotyping of 88 isolates of E. coli revealed the strains belonging to O2(8), O55(17), O84(5), O17(4), O6(8), O91(17), O26(9), O103(5), O126(5) and O124(6), O159(4). A total of 21 isolates were examined by multiplex Polymerase Chain Reaction (PCR) assay for detection of virulence and resistance genes. All examined isolates possessed a combination between intimin gene (eaeA) and heat stable toxin (STa) (100%), the serine protease (pic) gene on 8/21 isolates of O55, O2, O6 (38%) and α-hemolysin (hlyA) gene on 8/21 isolates of O26, O91(38%) while adherent invasive gene (invA) gene on 3/21 isolates of O124, O159 (14%) which divided diarrheagenic E. coli into four types assigned to be atypical enteropathogenic E. coli (aEPEC) (48%), atypical enterohemorrhagic E. coli (aEHEC) (35%), atypical enterotoxigenic E. coli (aETEC) (6%) and atypical enteroinvasive E. coli (aEIEC) (11%). On other hand, the results of antimicrobial susceptibility testing (AST) revealed high resistance to ampicillin, erythromycin and tetracycline (100%) and amoxicillin/clavulanic acid (92%) but were highly sensitive to gentamicin, imipenem, norfloxacin, ciprofloxacin, chloramphenicol and amikacin (100%). Concerning to ß lactams antibiotic resistance genes of examined isolates had blaSHV (100%) and BlaCTX-M (43%). For tetracycline, we detected the tetA in all examined isolates. Conclusion: The wide spread of atypical E. coli strains among diarrheic lambs and kids with marked resistance to several antibiotic of interest and detection of major resistance genes assess the potential risk of this pathogen to animal and public health. Background The shivering effect after spinal anesthesia in total knee arthroplasty (TKA) is challenging for anesthesiologists. This study aimed to compare two administration routes of dexmedetomidine as a post-neuraxial shivering prevention measure and an adjunctive analgesic and sedative agent. Methods Fifty-six patients were randomly allocated into two equal groups. The intravenous dexmedetomidine (IV dex) group received an IV infusion of 0.5 µg/kg dexmedetomidine diluted in 20 ml saline and an adductor canal block (ACB) consisting of 20 ml of 0.25% levobupivacaine and 1 ml saline. The adductor canal block dexmedetomidine (ACB dex) group received a 20 ml IV infusion of saline and an ACB consisting of 20 ml 0.25% levobupivacaine and 1 ml of 0.5 µg/kg dexmedetomidine. Results The incidence of shivering 1 h post spinal anesthesia was equal in both groups (50%); however, the shivering grade was significantly lower in the IV dex group 1 h postoperatively. The onset of sensory block was significantly later in the IV dex group (22.14 ± 2.52 min) than in the ACB dex group (12 ± 3.31 min). Postoperative analgesic duration (h) was significantly longer in the ACB dex group (12.28 ± 4.47) compared to the IV dex group (9.28 ± 1.90). The sedation scores were also significantly higher in the IV dex group in the preoperative, intraoperative, and immediate postoperative periods. Conclusions While perineural ACB dexmedetomidine had similar intraoperative anti-shivering with less sedative effects as IV dexmedetomidine, it was associated with both less shivering control and superior analgesia post-TKA under spinal anesthesia.

High temperatures in late spring, summer, and autumn are one of the main elements limiting cucumber production. Heat stress in cucumber plants leads to significant changes in sex expression and negative effects at the physiological stage, including decreased absorption of water and nutrients, decreased photosynthetic functions, and increased respiration, which in turn affects growth and production. Therefore, it is very difficult for cucumber plants exposed to heat stress to improve their productivity, especially in arid and semi-arid regions. Cucumber plants bear a variety of flower forms, including staminate, pistillate, and hermaphrodites that occur in different arrangements. In addition, maleness is considered the main problem in cucumber production, as it significantly reduces fruit yield. Recently, the growth, flowering, and productivity of plants have been controlled with the help of growth-regulating substances, and one of these substances is naphthalene acetic acid (NAA), which is of great importance in modifying sex in cucumber and various cucurbit crops. Two experiments were conducted during 2022 and 2023 during the summer season to study the effect of foliar spraying of two levels of potassium nitrate, 500 mg/L and 1000 mg/L, and two levels of NAA, 50 mg/L and 100 mg/L, individually and together, and the comparison treatment in nine treatments on growth traits, sex ratio, fruit traits and yield of cucumber (Cucumis sativas L.) plants in three replicates in a completely randomized block design. The results indicated that applying the treatment containing 1000 mg/L KNO3 + 100 mg/L NAA led to obtaining the highest value in most vegetable growth, flowering, and production parameters, with a significant increase compared to the control and all other treatments. Followed by treatments consisting of potassium nitrate and NAA, which gave high values in the traits of sex ratio %, fruit setting %, fruit yield, and most other traits. This study showed the importance of spraying with both NAA and potassium nitrate, individually or in combination, and the high combined effect of both in the treatments that contained each of them in the vegetative growth traits and floral traits such as the number of female flowers, the sexual ratio, fruit set percentage, weight fruit, length fruit, and the fruit yield in cucumber plants grown in the summer season with high temperatures

Abstract Background Acute kidney injury (AKI) is a common complication after liver transplantation, which is associated with increased morbidity and mortality. Therefore, this study investigated mannitol as an oxygen-free radical scavenger and its role in the prevention of early AKI after living donor liver transplantation (LDLT). Methods A total of 84 adult patients who underwent LDLT were randomly assigned to two equal groups: the M group, where patients received 1 g/kg mannitol 20%, or the S group, where patients received an equal volume of saline. The primary outcome was the incidence of early AKI, defined as a 0.3 mg/dl increase in the serum creatinine 48 h postoperatively. Laboratory assessments of the graft and creatinine were recorded until 3 months after transplantation besides the post-reperfusion syndrome and the intraoperative hemodynamic measurements. Results The AKI incidence was comparable between groups (relative risk ratio of 1.285, 95% CI 0.598–2.759, P = 0.518). Moreover, AKI stages and serum creatinine 3 months after transplantation, P = 0.23 and P = 0.25, respectively. The incidence of the post-reperfusion syndrome was comparable in both groups, 29/39 (74.4%) and 31/41 (75.6%) in M and S groups, respectively, P = 0.897. The intraoperative hemodynamic parameters showed no significant difference between groups using the area under the curve. Conclusion The current LDLT recipient sample was insufficient to demonstrate that pre-reperfusion 1 g/kg mannitol infusion would reduce the risk of early AKI or post-reperfusion syndrome. Unmanned Aerial Vehicles (UAVs) have been widely used both in military and civil across the world in recent years. Nevertheless, their design always involves complex design optimization variables and decisions. Therefore, this paper aims to guide through the designing, manufacturing, and testing of an electrically powered radio-controlled aircraft for achieving a take-off, cruise, safe landing and carrying the highest payload possible. The whole process involves several phases, design phase, structural analysis, performance analysis, materials used, manufacturing, and finally aircraft testing. The final aircraft was designed with an empty weight and maximum take-off weight of 15.43 Ibs and 33.07 Ibs respectively while the wingspan, cruising speed and maximum speed were 70.1 in., 46 ft/s and 78 ft/sec respectively with a total take-off distance of 100 ft.

In this paper, we derive the well-defined solutions to a θ-dimensional system of difference equations. We show that, the well-defined solutions to that system are represented in terms of Fibonacci and Lucas sequences. Moreover, we study the global stability of the solutions to that system. Finally, we give some numerical examples which confirm our theoretical results.

Background: Various rejuvenation surgeries, including hyaluronic acid (HA) fillers, aim to address mid-face volume loss. However, literature on the comparative efficacy and safety of different HA fillers for the zygomatic area remains limited. Methods: This systematic review and network meta-analysis (NMA), adhering to NMA PRISMA 2020 and Cochrane guidelines. Searches were conducted on different bases. Eligible studies included patients with mid-face volume loss undergoing HA interventions. The primary outcome was the Global Aesthetic Improvement Scale (GAIS) at different time points and the incidence of adverse events. Results: Among 1742 articles, six randomized controlled trials (RCTs) met the inclusion criteria with a total of 579 patients. The studies evaluated Giselleligne, VYC-20, Neuramis Lidocaine and ART-Filler. Results showed that Giselleligne demonstrated superior safety compared to VYC-20, ART-Filler and Neuramis-Lidocaine (Risk Ratios (RR) = 0.27, 95% Confidence interval (CI) [0.14; 0.54]), (RR = 0.36, 95% CI [0.17; 0.75], & RR = 0.22, 95% CI [0.07; 0.67]) respectively However, there were no significant differences in the Global Aesthetic Improvement Scale Assessment (GAIS) scores between HA fillers at 1, 3, and 6 months. Conclusion: While no significant differences were observed in efficacy among HA fillers, Giselleligne emerged as a potentially safer option for the cheeks volume restoration. The study emphasizes the need for further well-designed RCTs to explore the long-term safety and durability of HA fillers. These findings contribute valuable insights for clinicians and patients in making evidence-based decisions regarding mid-face rejuvenation options. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate the safety and efficacy of Lotilaner ophthalmic solution 0.25% in the treatment of demodex blepharitis. Methods: PubMed, Web of Science, Scopus, and Embase databases were searched. RCTs comparing lotilaner with placebo or any other standard treatments were included. Outcomes of mean collarette grade (MCG), mite density (Md), meaningful collarette reduction (MCR), mite eradication (ME), were pooled as mean difference (MD), and the outcomes of erythema cure (EC), collarette cure (CC) adverse events (AE) as risk ratio (RR) with their 95% confidence interval (CI) between the two groups from baseline to the endpoint. Review Manager (Version 5.4.1) software was used to conduct all statistical analyses. Results: Four RCTs (947 patients) were included in this study. The overall effect favored the lotilaner group in terms of mean collarette grade upper lid (MD −0.99, 95% CI [−1.26, −0.72]), MCG lower lid (MD −0.57, 95% CI [−1.03, −0.11]), Md (MD −1.13, 95% CI [−1.47, −0.79]), MCR (MD 2.07, 95% CI [2.27, 3.21]), ME (MD 3.46, 95% CI [2.96, 4.04]). EC (RR 3.16, 95% CI [2.18 to 4.59]) and CC (RR 4.17, 95% CI [2.97 to 5.85]). No significant difference between the two groups in terms of AE (RR 1.25, 95% CI [0.75 to 2.06]). However, these findings are limited by significant heterogeneity in some of the reported outcomes. Conclusions: Our findings show that lotilaner might effectively treat Demodex blepharitis. However, further RCTs with larger and more diverse populations are needed to confirm these findings as some outcomes show significant heterogeneity. [ABSTRACT FROM AUTHOR]

PurposeMethodsResultsConclusionCataract surgery is a common operation, typically carried out on older adults who have many comorbid medical conditions. Many of these patients may be undergoing aspirin therapy for diverse cardiovascular causes. Nevertheless, there is ongoing controversy regarding the administration of aspirin therapy during the perioperative period, as there are concerns regarding the risk of bleeding problems compared to thromboembolic events. Although aspirin is commonly used, there is no agreement on whether to continue or stop taking it before cataract surgery. This systematic review and meta-analysis attempt to assess the safety of continuing or discontinuing aspirin in the setting of cataract surgery.We conducted a systematic review and meta-analysis following PRISMA guidelines. Four databases were searched until January 25, 2024, for studies assessing the safety and efficacy of aspirin continuation or discontinuation before cataract surgery. The risk ratios (RR) of the extracted data and their 95% confidence interval (95% CI) were pooled using RevMan 5.4 software. We registered our protocol in the International Prospective Register of Systematic Reviews (PROSPERO) (registration number: CRD42024529986).We included nine studies with a total of 65,196 patients comprising various study designs and global populations. The analysis revealed a significant increase in subconjunctival hemorrhage risk with aspirin continuation (RR: 1.74, 95% CI: 1.22, 2.50, p = .002). However, the risk of hyphema, retrobulbar hemorrhage, vitreous hemorrhage, intraocular pressure spike, corneal edema, posterior capsule rupture, and visual acuity changes did not significantly differ between aspirin continuation and discontinuation or aspirin and placebo groups.Although the use of aspirin before cataract surgery raises the likelihood of subconjunctival bleeding, it does not increase the risk of potentially sight-threatening bleeding events. The primary objectives were to isolate and identify Klebsiella pneumoniae (K. pneumoniae), and determine the antimicrobial resistance patterns and biofilm formation abilities of the isolates. Additionally, the study aimed to investigate the antimicrobial and anti-biofilm effects of cinnamon oil against K. pneumoniae isolates. A cross-sectional study was conducted from March 2022 to April 2023 to collect 200 samples (including 156 nasal swabs and 44 lung specimens) from pneumonic sheep and goats admitted to the Veterinary Teaching Hospital of Zagazig University, Egypt. K. pneumoniae was isolated from a total of 72 (36%) samples, with 53 (73.6%) isolates recovered from nasal swabs and 19 (26.4%) from lung samples. Among the samples, 52 (36.9%) were from sheep and 20 (33.9%) were from goats. Antimicrobial susceptibility testing of the 72 K. pneumoniae isolates to 18 antimicrobials revealed that all isolates were resistant to ampicillin, amoxicillin/clavulanic acid, cefotaxime, ceftriaxone, tetracycline, colistin, fosfomycin, and trimethoprim/sulphamethoxazole. None of the isolates were resistant to amikacin, imipenem, and norfloxacin. Multidrug resistance (MDR) was observed in all K. pneumoniae isolates recovered from sheep and goats. The average MAR index was 0.71, ranging from 0.50 to 0.83. Regarding biofilm formation, among the K. pneumoniae isolates with a high MAR index (n = 30), 10% exhibited strong formation, 40% showed moderate formation, 43.3% displayed weak formation, and 6.7% did not form biofilms. Additionally, the biofilm-forming genes treC and fimA were present in all 28 biofilm-forming K. pneumoniae isolates, while the mrkA gene was detected in 15 (53.6%) of the 28 isolates. MDR K. pneumoniae isolates with strong biofilm formation abilities were treated with cinnamon oil at varying concentrations (100%, 75%, 50%, and 25%). This treatment resulted in inhibition zone diameters ranging from 35 to 45 mm. Cinnamon oil exhibited lower minimum inhibitory concentration and minimum bactericidal concentration values compared to norfloxacin for all isolates. Additionally, cinnamon oil significantly reduced the expression of biofilm-associated genes (treC, fimA, and mrkA) when compared to isolates treated with norfloxacin or untreated. In conclusion, this study identified a high level of MDR K. pneumoniae with strong and moderate biofilm formation abilities in pneumonic sheep and goats in Sharika Governorate, Egypt. Although cinnamon oil demonstrated potential antibacterial and anti-biofilm properties against K. pneumoniae, further research is required to investigate its effectiveness in treating K. pneumoniae infections in pneumonic sheep and goats.
