Introdução: A cirurgia estética facial e os seus resultados continuam a ser um desafio, tanto para o doente como para o cirurgião. O Rhinoplasty Outcome Evaluation (ROE) é um questionário de fácil aplicação que abrange os principais aspetos que influenciam a satisfação do doente submetido a rinoplastia. O objetivo deste estudo foi proceder à tradução, adaptação cultural e validação deste questionário da língua inglesa para o Português de Portugal. Material e Métodos: Estudo retrospetivo com 40 doentes (22 mulheres e 18 homens) no pós-operatório de rinoplastia e 30 voluntários (18 mulheres e 12 homens) sem indicação ou vontade para efetuar rinoplastia ou qualquer outro tipo de cirurgia nasal. Resultados: O score médio no pós-operatório de 6 meses foi de 19.63 pontos no grupo de doentes, tendo sido de 22.51 pontos nos controles mostrando a validade do questionário (p < 0.05). Foi testada a reprodutibilidade teste-reteste intraentrevistador com 96h de intervalo (r=0,81) e a coerência interna foi alta (coeficiente α de Cronbach de 0,824). Assim, resultou a versão final do ROE, constituído por 6 questões que avaliam os principais fatores que influenciam o contentamento do doente em relação à rinoplastia. Conclusões: A versão Portuguesa do ROE é um instrumento válido na avaliação de doentes submetidos a rinoplastia, permitindo objetivar a sua satisfação. info:eu-repo/semantics/publishedVersion

Tonsillar tumors are rare (1.45 per 100000) and mostly primary, usually of epithelial or lymphoproliferative origin. However, 0.8% are metastatic. The majority of the 100 cases found in the literature refers to lung, breast and renal cancer and melanoma, with no report of a lower limb sarcoma metastasis so far. This article describes a case of a 72-yearsold woman, with hypertrophy, induration and ulceration of the right palatine tonsil. Tonsillectomy led to the diagnosis of a pleomorphic sarcoma. The primary lesion, in the left thigh, had been diagnosed two years earlier. A small minority of tonsillar tumors is metastatic. Metastization from a lower limb sarcoma to the tonsil was not reported in literature but, as in other histologic types, occurred in a late stage of the disease with a poor prognosis. Os tumores das amígdalas palatinas são raros (1.45 casos:100000 indivíduos/ ano) e maioritariamente primários, geralmente de origem epitelial ou linfoproliferativa. Contudo, estima-se que 0.8% sejam secundários. A maioria dos cerca de 100 casos descritos na literatura é referente a carcinomas pulmonares, da mama, de células renais e melanomas, não havendo referência a nenhum caso de metastização por sarcoma do membro inferior. Este artigo descreve o caso de uma paciente de 72 anos que apresentava hipertrofia, endurecimento e ulceração da amígdala palatina direita. A amigdalectomia direita levou ao diagnóstico de sarcoma pleomórfico. A neoplasia primária, na face interna da coxa esquerda, tinha sido diagnosticada dois anos antes. Uma pequena minoria dos tumores da amígdala palatina é metastática. A metastização da amígdala por um sarcoma do membro inferior não se encontrava descrita na literatura, mas como noutros tipos histológicos, surgiu numa fase tardia da doença e com prognóstico muito reservado.

A sinusite é uma patologia comum que afecta aproximadamente 20 % da população. Existe controvérsia acerca da possibilidade da existência de fungos na mucosa sinusal, sem causar doença. A sinusite fúngica ainda é considerada uma patologia rara, apesar do aumento da incidência nas duas últimas décadas. Contudo, desconhece-se a prevalência exacta desta patologia. Segundo a classificação mais recente, a sinusite fúngica é dividida nas formas não-invasiva e invasiva. A sinusite fúngica não-invasiva é subdividida em bola fúngica e sinusite fúngica alérgica. A forma invasiva está categorizada nas formas sinusite fúngica fulminante aguda, sinusite fúngica invasiva crónica e sinusite fúngica invasiva granulomatosa. O desenvolvimento das diferentes formas depende do estado imunológico do doente, sendo que as formas invasivas ocorrem geralmente em doentes imunocomprometidos. Os autores relatam o caso de uma doente de 62 anos de idade, imunocompetente, com queixas de cefaleias e amaurose. O estudo imagiológico revelou ocupação do seio esfenoidal com erosão óssea adjacente. Foi submetida a intervenção cirúrgica onde se constatou ocupação do seio esfenoidal por material amarelo-esverdeado. O estudo microbiológico desse material revelou a presença de Aspergillus fumigatus. Foi medicada com variconazol, com resolução completa das queixas em uma semana. Os tumores do espaço parafaríngeo são raros e correspondem a menos de 1% de todas as neoplasias da cabeça e pescoço. Os tumores, benignos ou malignos, podem ter origem em qualquer uma das estruturas contidas no espaço parafaríngeo e geralmente apresentam-se como uma tumefacção indolor no pescoço ou orofaringe, geralmente o diagnóstico definitivo é anatomo-patológico; contudo é importante reconhecer a possibilidade de ocorrência de lesões vasculares que obrigam à realização de estudos imagiologicos desta região antes de se proceder a biópsia ou excisão da lesão. Os autores relatam um caso de uma doente com uma volumosa tumefacção parafaringea esquerda com 3 meses de evolução, indolor e causadora de disfagia. 11. Labyrinthine Fistula in Chronic Otitis Media Surgery: Management and Outcomes.

- Abstract
The objective of this retrospective study was to present the experience on the management of labyrinthine fistula secondary to chronic otitis media in a tertiary center. 263 patients, who underwent tympanomastoidecomy, in Centro Hospitalar Universitário do Porto, between 2015 and 2020 were reviewed, to select only those with labyrinthine fistulas. 26 patients (9.89%) had cholesteatoma complicated by fistula of the lateral semicircular canal. Most frequent symptoms were unspecific, such as otorrhea, hearing loss and dizziness. Preoperative high-resolution computed tomography predicted fistula in 54%. Using the Dornhoffer and Milewski classification, 10 cases (38.46%) were identified as stage I, 15 (57.69%) at stage II, and 1 (3.85%) as stage III. The choice between open or closed surgical procedure was independent of the type of fistulae. The cholesteatoma matrix was completely removed from the fistula and immediately covered by autogenous material. In one patient matrix was left over the fistula. After surgery, hearing (bone conduction) was preserved or improved in 73% of the patients. There was no statistically significant relationship between the extent of the labyrinthine fistula, type of material used in fistula repair and the hearing outcome. Also, we didn't find a statistically significant relationship between extent of the labyrinthine fistula and the presence of facial nerve canal dehiscence, tegmen timpani erosion, sigmoid sinus exposure or ossicular bone erosions. 12. Clues for facial canal dehiscence in chronic otitis media.

Introduction and Objectives: The goal of this study was to determine the impact of external and middle ear findings in the incidence of facial canal dehiscence (FCD) during mastoidectomy for chronic otitis media., Material and Methods: We examined the operative details of 186 patients who underwent primary tympanomastoidectomy for chronic otitis media between January 2015 and January 2020 retrospectively. In this study we only evaluated the second portion of the facial nerve canal., Results: The global prevalence of FCD was 22.6% (42/186 patients) with a higher incidence, of 38.7% (36/93), in patients with chronic otitis media with cholesteatoma (C-COM). Associations were found between facial canal dehiscence, labyrinthine fistula (p˂ .001) and facial nerve paralysis (p˂ .001). Ossicular erosions were observed at a significant level in patients with facial canal dehiscence, the incidence of FCD was significantly higher (p=.005, Odds ratio 5.489) when malleus and incus were eroded, incus plus stapes were eroded (p=.014; OR 4.059) and malleus, incus, and stapes together were eroded (p=.002; OR 4.929)., Conclusions: This study revealed an incidence of facial canal dehiscence of 22.6%. It also revealed that the presence of lateral semicircular canal fistula is associated with a higher prevalence of facial canal dehiscence. The same was noted in the case of some ossicular erosions, especially the combinations of eroded malleus and incus, incus and stapes, and all 3 ossicles. - Abstract
Henneguya quelen n. sp. (Myxosporea) is described from the kidney of the silver catfish, Rhamdia quelen (Quoy & Gaimard, 1824), from Marajó Island. In all, 40% of the 57 silver catfish collected from the Paracauari River in the municipality of Salvaterra presented cysts in the kidney. The cysts were whitish, spherical, and 0.45 mm in diameter. The histological analyses revealed tubular renal compression and cellular degeneration in the area adjacent to the xenomas. The spores are ellipsoid, with well-marked valves, caudal projection, and two elongated polar capsules of equal size. The mature spores measure approximately 15.6 μm in length, with the caudal appendage split posteriorly; 24.3 μm in length, for a total spore length of 40.0 μm; and a width of 4.1 μm. The polar capsules are pyriform, 5.5 μm long and 1.7 μm wide. A phylogenetic analysis based on Bayesian inference confirmed that the specimens represented a new species, which was denominated Henneguya quelen n. sp.

- Abstract
A new myxozoan was found parasitizing the freshwater catfish, Rhamdia quelen (Quoy Gaimard), in the Marajó island, Amazon region, Brazil. The new species is described based on the results of morphological and molecular analyses. The parasite is approximately 1.5 mm in diameter and develops in the musculature of the host in the form of spherical, whitish cysts, which are visible macroscopically between the epaxial and hypaxial layers. When ruptured, these cysts produced ellipsoidal spores with a mean length of 11.4 μm (10.7-12.6) and width of 7.2 μm (6.4-7.9). Anomalous spores with a caudal elongation, vesicles in the peripheral portion of the spore and ornamentation of the valves were also observed. The results of the phylogenetic analysis of sequences of the 18S rDNA gene using Bayesian Inference indicated clear differences among the Myxobolus species that reinforced the taxonomic position of the parasite, confirming its status as a new species, denominated Myxobolus arariensis n. sp.

This study provides morphological and molecular data of a new parasite species found in the muscle layer of the intestinal tract of the South American silver catfish, Rhamdia quelen from Marajó Island region (Pará State, Brazil), an important fishery resource with recognized potential for fish farming. The morphology of these parasites was reanalyzed and phylogenetic analyses were run on their 18S rDNA gene sequences. The spores were morphologically distinct from those of other Myxobolus species described previously. The obtained partial sequence of the 18S rDNA gene sequences of the new species were compared to those of 24 other Myxobolus and Henneguya species available in GenBank. The results of morphological and molecular analyses indicated clearly the existence of a new species, Myxobolus marajoensis sp. n.

- Abstract
Introduction: Evaluation of surgery outcome measured by patient satisfaction or quality of life is very important, especially in plastic surgery. There is increasing interest in self-reporting outcomes evaluation in plastic surgery., Objective: The aim of our study was to determine patient satisfaction in regard to nose appearance and function with the use of a validated questionnaire, before and after rhinoplasty surgery., Methods: A prospective study was realized at a tertiary centre. All rhinoplasty surgeries performed in adults between February 2013 and August 2014 were included. Many patients underwent additional nasal surgery such as septoplasty or turbinoplasty. The surgical procedures and patients' characteristics were also recorded., Results: Among 113 patients, 107 completed the questionnaires and the follow-up period. Analysis of pre-operative and post-operative Rhinoplasty Evaluation Outcome showed a significant improvement after 3 and 6 months in functional and aesthetic questions (p<0.01). In the pre-operative, patients anxious and insecure had a worse score (p<0.05). Difference in improvement of scores was not significant when groups were divided on basis of other nasal procedures, primary or revision surgery and open versus closed approach., Conclusion: We found that patients with lower literacy degree were more satisfied with the procedure. Hereditary hemorrhagic telangiectasia (HHT), also known by the eponym Osler-Weber-Rendu syndrome, is an autosomal dominant disorder characterised by the presence of multiple arteriovenous malformations (AVMs) affecting multiple organs. Many procedures have been used for epistaxis control in patients with this disorder. The objective of this study was to report the treatment of severe HHT-related epistaxiswith the modified Young's procedure., Materials and Methods: We describe the treatment of 4 patients with severe blood-transfusion-dependent epistaxis who underwent a modified Young's procedure in a tertiary hospital. The nasal closure was bilateral and complete in all cases. - Abstract
Porencephalic cyst expanding into the ear is a very rare complication of temporal bone fracture. We report a case of a 20-year-old male who developed a traumatic porencephalic cyst of the temporal lobe expanding into the ear through a tegmen fracture in association with a cholesteatoma. The clinical presentation was otitic meningitis. This occurrence was not encountered in any of the cases reviewed in the literature. The diagnosis, pathogenesis, treatment and prognosis of this condition are reviewed.
