1. A model to predict the population size of the dengue fever vector based on rainfall data

Abstract

According to the World Health Organization, dengue fever is the most important mosquito-borne disease of humans, and it is currently estimated that there may be 50 - 100 million yearly dengue infections worldwide. For the purpose to provide new techniques to public health policies in course, we introduce a predictive non-linear population dynamics model to describe the population size of four stages of the development of \emph{Aedes aegypti}, having the coefficients set to be dependent on the rainfall index data. In spite of the population dynamics of the \emph{Ae. aegypti} be mainly ruled by the rainfall regime, most models are dedicated exclusively to effects of temperature and only few models are dedicated to influence of rainfall. Vector control actions are also implemented in many periods of the year in order to compare relative efficiency of public health policies. The analysis of equilibrium and stability was performed. Field rainfall time series data from the City of Lavras (Minas Gerais, Brazil) was used for the model evaluation. The model was validated in a comparison with experimental mosquito abundance data acquired by field health agents. We evaluated and validated an entomological conjecture that claims that control actions should be performed during the dry season, instead of the common procedure adopted by vector control programs, in which those are mainly applied in the rainy season., Comment: 21 pages, 8 figures. Abstract

Constrained Differential Renormalization (CDR) and the constrained version of Implicit Regularization (IR) are two regularization independent techniques that do not rely on dimensional continuation of the space-time. These two methods which have rather distinct basis have been successfully applied to several calculations which show that they can be trusted as practical, symmetry invariant frameworks (gauge and supersymmetry included) in perturbative computations even beyond one-loop order. In this paper, we show the equivalence between these two methods at one-loop order. We show that the configuration space rules of CDR can be mapped into the momentum space procedures of Implicit Regularization, the major principle behind this equivalence being the extension of the properties of regular distributions to the regularized ones., Comment: 16 pages

Abstract

We reassess the problem of renormalization in finite temperature field theory (FTFT). A new point of view elucidates the relation between the ultraviolet divergences for T=0 and $T \not= 0$ theories and makes clear the reason why the ultraviolet behavior keeps unaffected when we consider the FTFT version associated to a given quantum field theory (QFT). The strength of the derivation one lies on the H\"ormander's criterion for the existence of products of distributions in terms of the wavefront sets of the respective distributions. The approach allows us to regard the FTFT both imaginary and real time formalism at once in a unified way in the contour ordered formalism., Comment: New title. 4. Asymptotic Conformal Invariance in a Non-Abelian Chern-Simons-Matter Model

Abstract

One shows here the existence of solutions to the Callan-Symanzik equation for the non-Abelian SU(2) Chern-Simons-matter model which exhibits asymptotic conformal invariance to every order in perturbative theory. The conformal symmetry in the classical domain is shown to hold by means of a local criteria based on the trace of the energy-momentum tensor. By using the recently exhibited regimes for the dependence between the several couplings in which the set of $\beta$-functions vanish, the asymptotic conformal invariance of the model appears to be valid in the quantum domain. By considering the SU(n) case the possible non validity of the proof for a particular n would be merely accidental., Comment: Latex2e 8 pages

Abstract

The general method of reduction in the number of coupling parameters is applied in a Chern-Simons-matter model with several independent couplings. We claim that considering the asymptotic region, and expressing all dimensionless coupling parameters as functions of the Chern-Simons coupling, it is possible to show that all $\beta$-functions vanish to any order of perturbative series. 11. A Model for Aedes aegypti Infestation According to Meteorological Variables: Case of Caratinga (Minas Gerais - Brazil)

Abstract

Aedes aegypti mosquitoes are the vector of diseases such as dengue, zika, chikungunya, and yellow fever among others. All the stages of development, eggs, larvae, pupa, and the adult of the species have its population modulated by meteorological variables, such as precipitation and temperature through affecting the productivity of breeding sites, metabolic processes, and others. Since adult females are responsible for transmitting the virus, the population of females becomes a direct indicator of the risk of infection. For this reason, some ongoing vector surveillance programs are based on adult female capture. In turn, all the stages of development have its population modulated by meteorological variables, such as precipitation and temperature, through productivity of breeding sites, metabolic processes and others. In this work, field data of capture of females was used to evaluate if a population dynamics model of Aedes aegypti under the effect of weather would be able to forecast field population. The nonlinear dynamic system model comprises: (1) four equations for the populations of the stages of development of the mosquito, designed for the ongoing surveillance program; (2) parametric dependencies of the rates of development on mean temperature and weekly accumulated precipitation. The dependencies on temperature and precipitation are modelled with aim of simplicity with the fewer number of parameters as possible. Temperature dependence is modelled based on values of the related literature under the assumption of existence of a optimum temperature for the rates, getting worse for extreme temperatures. The dependence on precipitation which is barely treated in experiments is modelled under the assumption of a monotonic dependence described by a power law with values estimated in orders of magnitude from data in the literature. By comparison with field data of an entomological indicator based on the number of Ae. aegypti females captured by a public health program in the city of Caratinga (Minas Gerais, Brazil), the model showed a significant correlation (R = 0.75). The result shows that the approach, if refined, can provide forecasting for of the population size. RESUMO Os mosquitos Aedes aegypti são vetores de doenças como dengue, zika, chikungunya, febre amarela, entre outras. Como as fêmeas adultas são responsáveis pela transmissão do vírus, sua população torna-se um indicador direto do risco de infecção. Por esse motivo, alguns programas de vigilância de vetores baseiam-se na captura de fêmeas adultas. Por sua vez, todas as fases de desenvolvimento (ovos, larvas, pupas e o vetor adulto) têm sua população modulada por variáveis meteorológicas, como precipitação e temperatura, através da produtividade dos criadouros, processos metabólicos e outros. Neste trabalho, dados de captura de fêmeas em campo foram utilizados para avaliar se um modelo de dinâmica populacional de Aedes aegypti , sob o efeito de variáveis meteorológicas, seria capaz de prever a população de campo. O modelo de sistema dinâmico não linear compreende: quatro equações para as populações dos estágios de desenvolvimento do mosquito, projetadas para um programa de vigilância em andamento; (2) dependências paramétricas das taxas de desenvolvimento em relação a` temperatura média e precipitação acumulada semanalmente. As dependências de temperatura e precipitação são modeladas, por simplicidade, com o menor número de parâmetros possível. A dependência da temperatura é modelada com base em valores da literatura relacionada sob o pressuposto da existência de uma temperatura ótima para as taxas, piorando para temperaturas extremas. A dependência da precipitação, que mal é tratada em experimentos, é modelada sob o pressuposto de uma dependência monotônica descrita por uma lei de potência com valores estimados em ordens de magnitude a partir de dados da literatura. Por comparação com dados de campo de um indicador entomológico baseado no número de fêmeas Ae. aegypti capturadas por um programa de saúde pública na cidade de Caratinga (Minas Gerais, Brasil), o modelo apresentou correlação significativa (R = 0,75). O resultado mostra que a abordagem, se refinada, pode fornecer previsões para o tamanho da população.

Abstract

Objective: To investigate the rate of colorectal neoplasms (CRNs) in patients who have Enterococcus faecalis infective endocarditis (EFIE) with available colonoscopies and to assess whether this is associated with the identification of a focus the infection. Patients and Methods: Retrospective analysis of data from a prospective multicenter study involving 35 centers who are members of the Grupo de Apoyo para el Manej o de la Endocarditis en Espana [Support Group for the Management of Infective Endocarditis in Spain] cohort. A specific set of queries regarding information on colonoscopy and histopathology of colorectal diseases was sent to each participating center. Four-hundred sixty-seven patients with EFIE were included from January 1, 2008, to December 31, 2017, from whom data on colonoscopy performance and results were available in 411 patients. Results: One hundred forty-two (34.5%) patients had a colonoscopy close to the EFIE episode. The overall rate of colorectal diseases was 70.4% (100 of 142), whereas the prevalence of CRN (advanced adenomas and colorectal carcinoma) was 14.8% (21 of 142), with no significant differences between the group of EFIE of unknown focus and that with an identified focus. Conclusion: Our study adds to prior evidence suggesting a much higher rate of CRN among patients with EFIE than in the general population of the same age and sex. In addition, our findings suggest that this phenomenon might take place both in EFIE with an unknown and an identified source of infection. Abstract

Objectives: Tropheryma whipplei has been detected in 3.5% of the blood culture-negative cases of endocarditis in Spain. Experience in the management of T. whipplei endocarditis is limited. Here we report the long-term outcome of the treatment of previously reported patients who were diagnosed with infective endocarditis (IE) caused by T. whipplei from the Spanish Collaboration on Endocarditis-Grupo de Apoyo al Manejo de la Endocarditis Infecciosa en Espana (GAMES) and discuss potential options for antimicrobial therapy for IE caused by T. whipplei. Patients and methods: Seventeen patients with T. whipplei endocarditis were recruited between 2008 and 2014 in 25 Spanish hospitals. Patients were classified according to the therapeutic regimen: ceftriaxone and trimethoprim/sulfamethoxazole, doxycycline + hydroxychloroquine and other treatment options. Results: Follow-up data were obtained from 14 patients. The median follow-up was 46.5months. All patients completed the antibiotic treatment prescribed, with a median duration of 13months. Six patients were treated with ceftriaxone and trimethoprim/sulfamethoxazole (median duration 13months), four with doxycycline + hydroxychloroquine (median duration 13.8months) and four with other treatment options (median duration 22.3months). The follow-up after the end of the treatments was between 5 and 84months (median 24months). Conclusions: All treatment lines were effective and well tolerated. Therapeutic failures were not detected during the treatment. None of the patients died or experienced a relapse during the follow-up. Only six patients received antibiotic treatment in accordance with guidelines. These data suggest that shorter antimicrobial treatments could be effective.

Abstract

IntroductionInfective endocarditis (IE) still carries high morbidity and mortality and frequently requires surgery. The benefit of mitral valve repair (MVr) in the setting of IE is yet to be proven. The goal of this study was to assess the results of MVr in patients with IE after a minimum follow-up of 1year.MethodsThis study is based on a Spanish nationwide prospective registry that included patients operated on for native mitral valve IE. The collaborating Institutions pooled their pre-, peri-, and postoperative data into the database of the GAMES group [Grupo de Apoyo al Manejo de la EndocarditiS (Group for support and management of infective endocarditis)].ResultsData from 27 hospitals were recorded and 3524 cases of active IE identified between 2008 and 2016. There were 1513 cases of mitral IE, of which 898 involved native valves. Of these, 437 patients underwent surgical treatment, and 369 completed the 1-year follow-up. The valve was repaired in 68 cases (18.4%). Preoperative groups were comparable (EuroSCORE MVr 7.7 vs MVR 8.0; p=ns). Mortality in the repair group was inferior to that in the replacement group (16.2% vs 27.2%, p=0.058). At 1year, mortality remained higher in the replacement group: 3.7% vs 2.9%. Relapse of the infection was slightly more frequent in the repair group (7.1% vs 3.7%; p=ns), although this did not lead to higher rates of reintervention (MVr/MVR: 2.9% vs 4.9%).ConclusionMVr is an attractive option for specific patients with IE and does not seem to negatively impact on relapses.

Abstract

Background. Outpatient parenteral antibiotic treatment (OPAT) has proven efficacious for treating infective endocarditis (IE). However, the 2001 Infectious Diseases Society of America (IDSA) criteria for OPAT in IE are very restrictive. We aimed to compare the outcomes of OPAT with those of hospital-based antibiotic treatment (HBAT). Methods. Retrospective analysis of data from a multicenter, prospective cohort study of 2000 consecutive IE patients in 25 Spanish hospitals (2008-2012) was performed. Results. A total of 429 patients (21.5%) received OPAT, and only 21.7% fulfilled IDSA criteria. Males accounted for 70.5%, median age was 68 years (interquartile range [IQR], 56-76), and 57% had native-valve IE. The most frequent causal microorganisms were viridans group streptococci (18.6%), Staphylococcus aureus (15.6%), and coagulase-negative staphylococci (14.5%). Median length of antibiotic treatment was 42 days (IQR, 32-54), and 44% of patients underwent cardiac surgery. One-year mortality was 8% (42% for HBAT; P < .001), 1.4% of patients relapsed, and 10.9% were readmitted during the first 3 months after discharge (no significant differences compared with HBAT). Charlson score (odds ratio [OR], 1.21; 95% confidence interval [CI], 1.04-1.42; P = .01) and cardiac surgery (OR, 0.24; 95% CI, .09-.63; P = .04) were associated with 1-year mortality, whereas aortic valve involvement (OR, 0.47; 95% CI, .22-.98; P = .007) was the only predictor of 1-year readmission. Failing to fulfill IDSA criteria was not a risk factor for mortality or readmission. Conclusions. OPAT provided excellent results despite the use of broader criteria than those recommended by IDSA. OPAT criteria should therefore be expanded.

Abstract

Aim: To evaluate the effect of the type of surgical indication on mortality in infective endocarditis (IE) patients who are rejected for surgery. Methods and results: From January 2008 to December 2016, 2714 patients with definite left-sided IE were attended in the participating hospitals. One thousand six hundred and fifty-three patients (60.9%) presented surgical indications. Five hundred and thirty-eight patients (32.5%) presented surgical indications but received medical treatment alone. The indications for surgery in these patients were uncontrolled infection (366 patients, 68%), heart failure (168 patients, 31.3%) and prevention of embolism (148 patients, 27.6%). One hundred and thirty patients (24.2%) presented more than one indication. The mortality during hospital admission was 60% (323 patients). The in-hospital mortality of patients whose indication for surgery was heart failure, uncontrolled infection or risk of embolism was 75.6%, 61.4% and 54.7%, respectively (p < 0.001). Surgical indications due to heart failure (OR: 3.24; CI 95%: 1.99-5.9) or uncontrolled infection (OR: 1.83; CI 95%: 1.04-3.18) were independently associated with a fatal outcome during hospital admission. Mortality during the first year was 75.4%. The mortality during the first year in patients whose indication for surgery was heart failure, uncontrolled infection or risk of embolism was 85.9%, 76.7% and 72.7%, respectively (p = 0.016). Surgical indication due to heart failure (OR: 3.03; CI 95%: 1.53-5.98) were independently associated with fatal outcome during the first year. Purpose: To analyze the influence of adding gentamicin to a regimen consisting of beta-lactam or vancomycin plus rifampicin on survival in patients suffering from Staphylococcal prosthetic valve endocarditis (SPVE). Methods: From January 2008 to September 2016, 334 patients with definite SPVE were attended in the participating hospitals. Ninety-four patients (28.1%) received treatment based on beta-lactam or vancomycin plus rifampicin and were included in the study. Variables were analyzed which related to patient survival during admission, including having received treatment with gentamicin. Results: Seventy-seven (81.9%) were treated with cloxacillin (or vancomycin) plus rifampicin plus gentamicin, and 17 patients (18.1%) received the same regimen without gentamicin. The causative microorganism was Staphylococcus aureus in 40 cases (42.6%) and coagulase-negative staphylococci in 54 cases (57.4%). Overall, 40 patients (42.6%) died during hospital admission, 33 patients (42.9%) in the group receiving gentamicin and 7 patients in the group that did not (41.2%, P = 0.899). Worsening renal function was observed in 42 patients (54.5%) who received gentamicin and in 9 patients (52.9%) who did not (p = 0.904). Heart failure as a complication of endocarditis (OR: 4.58; CI 95%: 1.84-11.42) and not performing surgery when indicated (OR: 2.68; CI 95%: 1.03-6.94) increased mortality. Gentamicin administration remained unrelated to mortality (OR: 1.001; CI 95%: 0.29-3.38) in the multivariable analysis. Conclusions: The addition of gentamicin to a regimen containing vancomycin or cloxacillin plus rifampicin in SPVE was not associated to better outcome. Abstract

Summary: What is known and objective: Enterococcus faecalis is the third most common causal agent of infective endocarditis. Currently, the treatment recommended is a combination of ampicillin (2 g/4 h) plus ceftriaxone (2 g/12 h), so patients must remain hospitalized for almost 6 weeks to receive the treatment. They are not generally included in outpatient parenteral antimicrobial therapy programs because 2 different electronic pumps are required to administer these 2 antibiotics. To enable the treatment of patients with E. faecalis IE at home, we designed a continuation combination regimen of ceftriaxone 4 g once daily in a short infusion plus ampicillin 2 g/4 h using a programmable pump. Methods: We analyzed a cohort of patients attended in an outpatient parenteral antimicrobial therapy program that has been working since 2012 in 2 tertiary hospitals. We selected patients attended in this program for E. faecalis IE treated with a continuation regimen of ampicillin 12 g daily (2 g/4 h) and ceftriaxone 4 g every 24 hours between July 2012 and March 2017. Results and discussion: Of the 720 patients included in the outpatient parenteral antimicrobial therapy program, 42 had infective endocarditis, and 4 (9.52%) were treated using the combination regimen described above. All patients were men, and all had left‐sided native‐valve infective endocarditis. All 4 patients received ampicillin 2 g every 4 hours and ceftriaxone 2 g every 12 hours in hospital, for a median duration of 25 days (IQR 15‐32). Thereafter, in the program, they received the following regimen: a 30‐minute infusion of ceftriaxone 4 g in 250 mL of saline solution, followed by ampicillin 12 g daily in 500 mL of saline solution delivered by a pump programmed to administer 2 g every 4 hours. Patients received this treatment at home for a median of 22.5 days (IQR 13‐32). All patients achieved clinical and microbiological cure with no recurrences or complications after a lengthy follow‐up period (median 365 days, IQR 221‐406). No drug‐related adverse events or problems with the pump system were reported. What is new and conclusions: Use of ceftriaxone 4 g in a single dose yields a mean plasma concentration of 30 μg/mL. Ceftriaxone also has a high plasma protein binding capability, and once this binding is saturated, there is no reason to administer higher doses. Therefore, it seems reasonable to use a dose of 4 g of ceftriaxone once daily to have a synergist effect with ampicillin within the vegetation, and enable the treatment of patients with E. faecalis infective endocarditis at home. In conclusion, the administration of ampicillin (2 g/4 h) plus ceftriaxone (4 g/24 h) as a continuation regimen in an outpatient parenteral antimicrobial therapy program may be as effective and safe as the usual lengthy in‐hospital regimen (ampicillin 2 g/4 h and ceftriaxone 2 g/12 h) in patients with E. faecalis infective endocarditis.
