Abstract

Viral infections remain a major risk in immunocompromised pediatric patients, and virus-specific T cell (VST) therapy has been successful for treatment of refractory viral infections in prior studies. We performed a phase II multicenter study (NCT03475212) for the treatment of pediatric patients with inborn errors of immunity and/or post allogeneic hematopoietic stem cell transplant with refractory viral infections using partially-HLA matched VSTs targeting cytomegalovirus, Epstein-Barr virus, or adenovirus. Primary endpoints were feasibility, safety, and clinical responses (>1 log reduction in viremia at 28 days). Secondary endpoints were reconstitution of antiviral immunity and persistence of the infused VSTs. Suitable VST products were identified for 75 of 77 clinical queries. Clinical responses were achieved in 29 of 47 (62%) of patients post-HSCT including 73% of patients evaluable at 1-month post-infusion, meeting the primary efficacy endpoint (>52%). Secondary graft rejection occurred in one child following VST infusion as described in a companion article. Corticosteroids, graft-versus-host disease, transplant-associated thrombotic microangiopathy, and eculizumab treatment correlated with poor response, while uptrending absolute lymphocyte and CD8 T cell counts correlated with good response. This study highlights key clinical factors that impact response to VSTs and demonstrates the feasibility and efficacy of this therapy in pediatric HSCT.

Abstract

We studied 1110 patients with β-thalassemia major aged ≤25 years who received transplants with grafts from HLA-matched related (n = 677; 61%), HLA-mismatched related (n = 78; 7%), HLA-matched unrelated (n = 252; 23%), and HLA-mismatched unrelated (n = 103; 9%) donors between 2000 and 2016. Ninety percent of transplants were performed in the last decade. Eight-five percent of patients received ≥20 transfusions and 88% were inadequately chelated. All patients received myeloablative-conditioning regimen. Overall and event-free survival were highest for patients aged ≤6 years and after HLA-matched related and HLA-matched unrelated donor transplantation. The 5-year probabilities of overall survival for patients aged ≤6 years, 7 to 15 years, and 16 to 25 years, adjusted for donor type and conditioning regimen were 90%, 84%, and 63%, respectively (P < .001). The corresponding probabilities for event-free survival were 86%, 80%, and 63% (P < .001). Overall and event-free survival did not differ between HLA-matched related and HLA-matched unrelated donor transplantation (89% vs 87% and 86% vs 82%, respectively). Corresponding probabilities after mismatched related and mismatched unrelated donor transplantation were 73% vs 83% and 70% vs 78%. In conclusion, if transplantation is considered as a treatment option it should be offered early (age ≤6 years). An HLA-matched unrelated donor is a suitable alternative if an HLA-matched relative is not available.

Abstract

Background: Intravenous iron administration is routinely used to treat moderate anemia during pregnancy. Iron sucrose has the disadvantage of repeated doses and increased hospital visits. Ferric carboxymaltose provides rapid replacement of iron storage in a single dose. In this study, we compare the safety and efficacy of the two parenteral iron complexes in the treatment of iron deficiency anemia in pregnancy from gestation age of 14-38 weeks. Methodology: The Obstetrics and Gynecology Department at Rajshree Medical Research Institute in Bareilly conducted this comparative investigation. In order to conduct the study, a total of 100 pregnant women who were diagnosed with moderate anemia between the ages of 14 and 36 weeks of gestation were divided into two groups at random (n = 50) (Group A: Iron sucrose complex (ISC); Group B: Ferric carboxymaltose). Result: In group A of the study, the majority of the patients were women between the ages of 26 and 30 (38%) and between 31 and 35 (44%), respectively. In group A, 8% of patients and 6% in group B had nausea. In group A, 1% of patients reported abdominal pain, whereas in group B, there were no complaints. Conclusion: There aren't many randomized controlled trials comparing ferrous carboxymaltose and ISC in pregnant women between 14 and 28 weeks along with very few prospective studies on FCM. The present study concludes Ferric carboxymaltose to be more efficient with no life-threatening reaction compared with ISC for treatment of moderate iron deficiency anemia in 14-36 weeks of gestation. Abstract

BACKGROUND: Assay of T-cell receptor excision circles (TRECs) in dried blood spots obtained at birth permits population-based newborn screening (NBS) for severe combined immunodeficiency (SCID). OBJECTIVE: We sought to report the first 2 years of TREC NBS in California. METHODS: Since August 2010, California has conducted SCID NBS. A high-throughput TREC quantitative PCR assay with DNA isolated from routine dried blood spots was developed. Samples with initial low TREC numbers had repeat DNA isolation with quantitative PCR for TRECs and a genomic control, and immunophenotyping was performed within the screening program for infants with incomplete or abnormal results. Outcomes were tracked. RESULTS: Of 993,724 infants screened, 50 (1/19,900 [0.005%]) had significant T-cell lymphopenia. Fifteen (1/66,250) required hematopoietic cell or thymus transplantation or gene therapy; these infants had typical SCID (n = 11), leaky SCID or Omenn syndrome (n = 3), or complete DiGeorge syndrome (n = 1). Survival to date in this group is 93%. Other T-cell lymphopenic infants had variant SCID or combined immunodeficiency (n = 6), genetic syndromes associated with T-cell impairment (n = 12), secondary T-cell lymphopenia (n = 9), or preterm birth (n = 8). All T-cell lymphopenic infants avoided live vaccines and received appropriate interventions to prevent infections. TREC test specificity was excellent: only 0.08% of infants required a second test, and 0.016% required lymphocyte phenotyping by using flow cytometry. CONCLUSIONS: TREC NBS in California has achieved early diagnosis of SCID and other conditions with T-cell lymphopenia, facilitating management and optimizing outcomes. Furthermore, NBS has revealed the incidence, causes, and follow-up of T-cell lymphopenia in a large diverse population.

Abstract

Postpartum cardiomyopathy also called peripartum cardiomyopathy (PPCM) remains an entity of poorly understood aetiology and is essentially a diagnosis of exclusion. Preeclampsia is one of the most common complications observed during pregnancy. The overlap between PPCM and preeclampsia is clinically significant as patient can present with similar findings and diagnosis of PPCM can be missed. Coexistence of both these conditions should be kept in mind during patient work-up. We present a case of a 35 year old P4L3, who reported on sixth day postpartum, and had delivered at home (stilbrith) She presented to obstetric casualty with complaints of dry cough, chest pain, weakness since delivery, vomi blurring of vision and mild breathlessness since past one month. At the time of admission her blood pressure (BP) was 140/100 mm Hg. A diagnosis of severe preeclampsia was made and treatment was started accordingly. Component of COVID-19 was also suspected which was later confirmed to be negative. On further work-up her LVEF was found to be reduced along with moderate MR and LV diastolic dysfunction.
