Abstract

Objective: To describe the clinicopathological features of 3 new cases of soft-tissue keratocyst (STK) of the buccal mucosa from Brazil. Materials and methods: Clinical data from 3 patients diagnosed with STK were obtained from the archives of the Laboratory of Oral Pathology at the Federal University of Rio de Janeiro– Brazil from 2020 to 2023. Two oral pathologists reviewed conventional hematoxylin and eosin-stained slides of each case. Immunohistochemical stainings for CK19, Bcl-2, CD138, D2-40, EMA, and Ki-67 were performed in all cases. Results: Case 1 was a 58-year-old man with a 2-year history of painless swelling of the right buccal mucosa, measuring approximately 5 cm. Case 2 was of a 44-year-old man with a painful swelling in the left buccal mucosa lasting 3 years with drainage to the oral cavity. Case 3 was of a 74-year-old woman with a painful swelling in the left buccal mucosa of unknown duration. Microscopic evaluation of all 3 cases revealed a cyst lined with a few cell layers composed of columnar palisading basal cells and a corrugated parakeratin surface. The capsule comprised fibrous connective tissue with variable amounts of blood vessels with hemorrhage, nerve bundles, fat tissue, striated muscle fibers, and the absence of pilosebaceous units. No recurrence was observed after complete surgical removal. All epithelial layers of the 3 cases expressed positivity for CK19 and CD138, the basal cells were positive for Bcl-2 and D2-40, and the superficial epithelial layer was positive for EMA. All cases demonstrated a low proliferation index by Ki-67. Conclusion: This study represents a series of 3 cases of STK that affected the posterior buccal mucosa of middle-aged patients from Brazil, sharing histopathological and immunohistochemical features with odontogenic keratocyst. Abstract

According to the International Agency for Research on Cancer (2020), oral cancer is the 11th most common cancer among men in the world. The best way to fight cancer is to prevent it. Its prevention and early detection represent global health challenges. Worldwide public health services must implement strategies to face this global burden and improve the health equity of the population. This scoping review will evaluate national cancer plans and Plans of Non-communicable Diseases for preventing oral and oropharyngeal cancer and confront the different national strategies adopted. These research results may subsidize future decisions for public health policies.

Abstract

Introduction: Syphilis is an infectious disease caused by the anaerobic bacteria Treponema pallidum, and can be transmitted either by sexual contact or vertically (maternal-fetal transmission). The clinical manifestations of syphilis are divided into three phases: the first two being highly infectious. The lack of knowledge of the disease by pregnant women can lead to congenital syphilis, which can cause death of newborns, or dire consequences to their development, as it occurs with the Hutchinson’s triad. Due to a rise in syphilis cases, the disease has caused an increasing rebound of concern in health professionals, both in the public and private spheres. Objective: Thus, the goal of this essay is, by means of narrative literature review, to approach the main aspects of the disease, clarifying the diagnosis, its clinical presentation, oral manifestations, treatment and prevention. Sources of data: A bibliographic survey has been made via Pubmed, Lilacs, and Google Scholar data, through the keywords “Sífilis”, “Syphilis”, “Sífilis congênita”, “Congenital Syphilis” “Atenção primária em saúde”, “Sistema Único de Saúde”. Synthesis of data: 41 articles in English or Portuguese, complete and free, published since 2006 were selected. It has been seen that the increase in the number of cases is related to the lack of self-care, along with failures in the process of diagnosis, prevention, and treatment of the disease. Conclusion: It was concluded that dentists have an important role in early diagnosis and treatment of the disease, and must have the knowledge of its most common clinical manifestations, being able to work in an interdisciplinary fashion. Introdução: A sífilis é uma doença infecciosa causada pela bactéria anaeróbica Treponema pallidum, e pode ser transmitida através do contato sexual ou verticalmente, da mãe contaminada para o seu feto. Suas manifestações clínicas ocorrem em três estágios, sendo os dois primeiros altamente infecciosos. O desconhecimento da sífilis pela gestante pode implicar na sífilis congênita, que por vezes acarreta a morte do recém-nascido ou severas consequências ao seu desenvolvimento, como a tríade de Hutchinson. Devido ao aumento de casos de sífilis na população, esta doença voltou a ser motivo de preocupação dos profissionais de saúde, tanto no âmbito público quanto no privado. Objetivo: Portanto, o objetivo deste trabalho é, através de uma revisão narrativa da literatura, discorrer sobre aspectos da doença, elucidando o diagnóstico, suas características clínicas, manifestações orais, tratamento e prevenção. Fonte dos dados: Foi realizado um levantamento bibliográfico nas bases de dados Pubmed, Lilacs e Google Scholar, com os descritores “Sífilis”, “Syphilis”, “Sífilis congênita”, “Congenital Syphilis”, “Atenção primária em saúde”, “Sistema Únic de Saúde”. Síntese dos dados: Foram selecionados 41 artigos escritos em inglês e português, em sua versão completa e gratuita, e publicados a partir do ano de 2006. Foi verificado que o aumento dos números de casos se relaciona com a falta de autocuidado, e falhas no processo de diagnóstico, prevenção e tratamento da doença. Conclusão: Concluiu-se que o cirurgião-dentista tem papel importante no diagnóstico precoce e tratamento da doença, devendo ter conhecimento de suas manifestações clínicas mais comuns e ser capaz de trabalhar de maneira interdisciplinar.

Abstract

Background Melanoma is an aggressive malignant tumor, rarely observed in the oral cavity. The aim of this study was to describe the clinicopathologic features of a series of oral melanomas. Material and Methods A retrospective descriptive study was performed. A total of 15,482 biopsy records from two oral and maxillofacial pathology services in Brazil were analyzed. All cases of oral melanomas were reviewed, and clinical, demographic, histopathological data, treatment, and follow-up status were collected. In addition, immunohistochemistry stains (pan-cytokeratin AE1/AE3, vimentin, α-SMA, CD45, S-100 protein, HMB-45, Melan A, and Ki-67) were performed. Results The series comprised of 5 males (71.4%) and 2 females (28.6%), with a mean age of 58.0 ± 9.2 years (range: 45-69 years) and a 2.5:1 male-to-female ratio. The gingiva (n = 3, 42.8%) and hard palate (n = 2, 28.6%) were the most common affected sites, presenting clinically as ulcerated swellings with a brown to black color. Cervical lymph node metastasis was detected in three patients during the first examination. Microscopically, 6 cases (85.7%) were melanotic, and one (14.3%) was amelanotic. Most cases (n = 4, 57.1%) presented a predominance of epithelioid cells. S-100 and HMB-45 were positive in all cases (n = 7, 100.0%). In contrast, only 4 cases (57.1%) were positive for Melan-A. The proliferative index with Ki-67 was high, with labeling index ranging from 70.0% to more than 90% of positive cells. Five patients died from complications of the tumors after a mean follow-up period of 7.8 months. Conclusions Melanoma is an aggressive malignant tumor that rarely occurs in the oral cavity. It occurs mainly in adult and elderly patients and often is diagnosed in advanced stages. The current findings were similar to previous studies and reflected the characteristics of the services from where lesions were retrieved. Abstract

Background Angina bullosa haemorrhagica (ABH) is characterized by the recurrent appearance of blood blisters on the oral mucosa, mainly in adults' soft palate. In general, the blisters rupture spontaneously, lacking the necessity for biopsy. We report the clinical features of 23 ABH cases, emphasizing the clinical behavior and the management of these conditions. Material and Methods A retrospective descriptive cross-sectional study was performed. A total of 12,727 clinical records of oral and maxillofacial lesions from four dental services in Brazil were analyzed. Clinical data were collected from the clinical records and evaluated. Results The series comprised 12 males (52.2%) and 11 females (47.8%), with a mean age of 56.8 ± 14.6 years (ranging: 24-82 years) and a 1.1:1 male-to-female ratio. Most of the lesions affected the soft palate (n = 15, 65.2%). Clinically, the lesions presented mainly as an asymptomatic (n = 17, 73.9%) blood-filled blister that ruptured after a few minutes or hours, leaving an erosion. The masticatory trauma was the most frequent triggering event. No patient had coagulation disorders. A biopsy was performed in only four cases (17.4%). Treatment was symptomatic with a favorable outcome. Conclusions ABH is still poorly documented in the literature, and its etiology remains uncertain. ABH mainly affects the soft palate of elderly adults and has a favorable evolution in a few days. The therapeutic approach is often focused only on the relief of symptoms. Abstract

Background: To evaluate the prevalence and clinicopathological features of a large series of gingival neoplasms in Brazil. Material and Methods: All gingival benign and malignant neoplasms were retrieved from the records of six Oral Pathology Services in Brazil, during a 41-year period. Clinical and demographic data, clinical diagnosis, and histopathological data were collected from the patients' clinical charts. For statistical analysis, the chi-square, median test of independent samples and the U Mann-Whitney tests were used, considering a significance of 5%. Results: From 100,026 oral lesions, 888 (0.9%) were gingival neoplasms. There were 496 (55.9%) males, with a mean age of 54.2 years. Most cases (70.3%) were malignant neoplasms. Nodules (46.2%) and ulcers (38.9%) were the most common clinical appearance for benign and malignant neoplasms, respectively. Squamous cell carcinoma (55.6%) was the most common gingival neoplasm, followed by squamous cell papilloma (19.6%). In 69 (11.1%) malignant neoplasms, the lesions were clinically considered to be inflammatory or of infectious origin. Malignant neoplasms were more common in older men, appeared with larger size, and with a time of complaint shorter than benign neoplasms (p<0.001). Conclusions: Benign and malignant tumors may appear as nodules in gingival tissue. In addition, malignant neoplasms, especially squamous cell carcinoma, should be considered in the differential diagnosis of persistent single gingival ulcers. [ABSTRACT FROM AUTHOR]

Abstract

Objectives: To investigate the potential effect of fatty acid synthase (FASN) inhibitor orlistat to enhance the effectiveness of chemotherapy drugs widely used to treat oral squamous cell carcinomas (OSCC), such as 5‐fluorouracil, cisplatin, and paclitaxel. Methods: The OSCC SCC‐9 LN‐1 metastatic cell line, which expresses high levels of FASN, was used for drug combination experiments. Cell viability was analyzed by crystal violet staining and automatic cell counting. Apoptosis and cell cycle were analyzed by flow cytometry with Annexin‐V/7‐AAD and propidium iodide staining, respectively. Cyclin B1, Cdc25C, Cdk1, FASN, and ERBB2 levels were assessed by Western blotting. Finally, cell scratch and transwell assays were performed to assess cell migration and invasion. Results: Inhibition of FASN with orlistat sensitized SCC‐9 LN‐1 cells to the cytotoxic effects of paclitaxel and cisplatin, but not 5‐fluorouracil, which was accompanied by a significant reduction in cyclin B1. The suppression of proliferation, migration, and invasion of SCC‐9 LN‐1 cells induced by orlistat plus cisplatin or paclitaxel was not superior to the effects of chemotherapy drugs alone. Conclusion: Our results suggest that orlistat enhances the chemosensitivity of SCC‐9 LN‐1 cells to cisplatin and paclitaxel by downregulating cyclin B1. [ABSTRACT FROM AUTHOR]

Abstract

This study aimed to evaluate the contribution of oral and maxillofacial pathology laboratories (OMPLs) in Brazilian public universities to the diagnosis of lip, oral cavity, and oropharyngeal squamous cell carcinoma (SCC). A cross-sectional study was performed using biopsy records from a consortium of sixteen public OMPLs from all regions of Brazil (North, Northeast, Central-West, Southeast, and South). Clinical and demographic data of patients diagnosed with lip, oral cavity, and oropharyngeal SCC between 2010 and 2019 were collected from the patients' histopathological records. Of the 120,010 oral and maxillofacial biopsies (2010-2019), 6.9% (8,321 cases) were diagnosed as lip (0.8%, 951 cases), oral cavity (4.9%, 5,971 cases), and oropharyngeal (1.2%, 1,399 cases) SCCs. Most cases were from Brazil's Southeast (64.5%), where six of the OMPLs analyzed are located. The predominant profile of patients with lip and oral cavity SCC was Caucasian men, with a mean age over 60 years, low schooling level, and a previous history of heavy tobacco consumption. In the oropharyngeal group, the majority were non-Caucasian men, with a mean age under 60 years, had a low education level, and were former/current tobacco and alcohol users. According to data from the Brazilian National Cancer Institute, approximately 9.9% of the total lip, oral cavity, and oropharyngeal SCCs reported over the last decade in Brazil may have been diagnosed at the OMPLs included in the current study. Therefore, this data confirms the contribution of public OMPLs with respect to the important diagnostic support they provide to the oral healthcare services extended by the Brazilian Public Health System.
