Abstract

BackgroundDuring the COVID-19 pandemic, decreased volumes of stroke admissions and mechanical thrombectomy were reported. The study's objective was to examine whether subarachnoid haemorrhage (SAH) hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines.MethodsWe conducted a cross-sectional, retrospective, observational study across 6 continents, 37 countries and 140 comprehensive stroke centres. Patients with the diagnosis of SAH, aneurysmal SAH, ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases, 10th Revision, codes. The 3-month cumulative volume, monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before (1 year and immediately before) and during the pandemic, defined as 1 March-31 May 2020. The prior 1-year control period (1 March-31 May 2019) was obtained to account for seasonal variation.FindingsThere was a significant decline in SAH hospitalisations, with 2044 admissions in the 3 months immediately before and 1585 admissions during the pandemic, representing a relative decline of 22.5% (95% CI -24.3% to -20.7%, p

23. Potentiality of Disaster Management Education through Open and Distance Learning System in Bangladesh Open University

Abstract

Bangladesh Open University (BOU) is the only public educational institution in Bangladesh, where, a dual-mode method of learning system has been introduced. Established in 21st October, 1992, the University now accommodates 174,459 learners in 2012. The wide range networking of this university provides it a great prospect to execute a broad spectrum of activities to accomplish its social responsibilities. Despite the aims of BOU at continuous quantitative improvement and greater equity in the society, like most of the ODL method universities in the world, BOU framed a limited objective to create awareness and preparedness about the contemporary global disaster proneness among its learners. Bangladesh for its geographical location, experiences several natural disasters like annual flood, heavy rainfall, cyclone and tidal surge, earth quake, river bank erosion, drought, etc. Lack of awareness of the sustainable use of the natural resources, and the consequences of the heedless consumption of them by the inhabitants enhanced the degree and frequency of these natural disasters during the last decades. The present study emphasizes on the role of Bangladesh Open University in creating awareness among its learners about the causes and pattern of disasters, pre and post disaster management strategies, etc. The study proves that BOU is a unique educational institution which, through the ODL method of teaching, using various educational medium like, tutorial support, printed study materials, electronic media, internet, and cellular phone, etc. can provided a wide range of knowledge about the disaster vulnerability, risk reduction and management strategies to its learners.

26. Psychological distress, General health and Life satisfaction among children with Thalassemia and children undergoing Haemodialysis.

Abstract

Objective: To compare and find out the relationship between psychological distress, general health and life satisfaction among children with thalassemia and children undergoing dialysis. Method: The current study utilized cross sectional research design. A representative group of children with Thalassemia (n=60) and children undergoing dialysis (n=60) with ages ranging from 6-14 years were included in the sample. The sample was drawn from Children's Hospital and Sundas Foundation from February 2023 to August 2023 through a purposive sampling technique. Results: Psychological distress significantly correlates negatively with general health (-.34**) and life satisfaction (-.44**) in Children with Thalassemia. A significant negative relationship is also observed between psychological distress and general health (-.43**) and life satisfaction (-.54**) in Children undergoing Hemodialysis. Children undergoing hemodialysis experience more psychological distress whereas children with thalassemia have better general health. However, no significant difference was found in life satisfaction. The findings of the present study will be helpful for healthcare professionals and clinical psychologists to develop strategies and training programs which can increase resilience among patients. Training the patients and caregivers regarding how to fight chronic illnesses will lead to an improvement in their quality of life. Conclusion: Psychological distress is higher in children undergoing hemodialysis and general health is also not good but the level of satisfaction is almost equal between the two groups. Abstract

Micro and nano structures scatter light and amplify local electric fields very effectively. Energy incident as intense ultrashort laser pulses can be converted to x-rays and hot electrons more efficiently with a substrate that suitably modifies the local fields. Here we demonstrate that coating a plain glass surface with a few micron thick layer of an ubiquitous microbe, {\it Escherichia coli}, catapults the brightness of hard x-ray bremsstrahlung emission (up to 300 keV) by more than two orders of magnitude at an incident laser intensity of 10$^{16}$ W cm$^{-2}$. This increased yield is attributed to the local enhancement of electric fields around individual {\it E. coli} cells and is reproduced by detailed particle-in-cell (PIC) simulations. This combination of laser plasmas and biological targets can lead to turnkey, multi-kilohertz and environmentally safe sources of hard x-rays.

39. How Ethical Leadership Stimulates Academics' Retention in Universities: The Mediating Role of Job-Related Affective Well-Being

Abstract

Purpose: The purpose of this paper is to investigate the relationship between ethical leadership and academics' retention in universities. It draws on the conservation of resources theory to deepen the understanding of a process underlying this relationship whereby academics are more likely to stay in universities through the practice of ethical leadership. Specifically, it advances academics' job-related affective well-being as a potential mediating mechanism, fostered by ethical leadership, which lowers their intention to leave. Design/methodology/approach: This study is conducted through a cross-sectional survey of 303 academics in Australian universities. Univariate, bivariate and multivariate analysis procedures are deployed to analyse academics' data. The research hypotheses are tested through a bootstrapped regression analysis of academics' perceived ethical leadership, affective well-being and intention to leave. Findings: The findings lend support to the hypothesised relations, indicating a significant role of ethical leadership on enhanced intentions of academics to stay in universities by directly conserving their job-related affective well-being. Research limitations/implications: This paper contributes to knowledge of the relationship between ethical leadership and academics' retention by identifying job-related affective well-being as an underlying mechanism in the university sector. Practical implications: This paper has practical implications for higher educational institutes seeking to retain their academic staff. Its findings show that the practice of ethical leadership in universities matters, because it lowers academics' intentions to leave by nurturing their well-being at work. Originality/value: This is the first study to examine the impact of ethical leadership on academics' well-being and intentions to leave in the context of universities in Australia. It is one of the first studies to explore the mediating role of affective well-being in the ethical leadership and leadership and intention to leave relationship.

43. Academics' Perceptions of Bullying at Work: Insights from Pakistan

Abstract

Purpose: Despite an extensive history of research into workplace bullying and the psychosomatic harm associated with it in western contexts, research into the occurrence and manifestation of bullying behavior in the academic workplaces of non-western countries is sparse. In response to this gap, the purpose of this paper is to start a research conversation by reporting an empirical enquiry into the occurrence, forms and perceptions of workplace bullying among academics in Pakistan. Design/Methodology/Approach: This study was conducted with a representative sample of academics in a large Pakistani province through a cross-sectional survey. Findings: This study reveals that workplace bullying is prevalent among academics in the Pakistani context, with up to half of them regularly exposed to practices such as excessive work monitoring, undermining of professional competence, lack of recognition of work contributions and obstruction of important work-related matters. Research Limitations/Implications: The findings underscore the need for developing broader institutional actions, clear policies and grievance procedures to discourage bullying at work in Pakistan. Higher educational managers will find the results useful for development of anti-bullying policies and codes of conduct. Originality/Value: This is the first study to examine the perceptions, occurrence and demographic risk factors associated with workplace bullying among academics in the Pakistani context.

Abstract

This article is a literature review and case report of two patients with CLOVES syndrome who were treated with sclerotherapy. The first case involved an 18-month-old boy who received sclerotherapy injections for a large venous and lymphatic malformation. After two sessions of sclerotherapy, the lesion completely resolved with no recurrence. The second case was a 1-month-old baby girl who developed respiratory distress and died before receiving sclerotherapy. CLOVES syndrome is a rare overgrowth syndrome characterized by lipomatous overgrowth, vascular malformations, epidermal nevi, and spinal/skeletal deformities. Sclerotherapy with bleomycin has shown promising results in treating the vascular component of CLOVES syndrome. [Extracted from the article]

Abstract

Background and Objectives: Declines in stroke admission, IV thrombolysis (IVT), and mechanical thrombectomy volumes were reported during the first wave of the COVID-19 pandemic. There is a paucity of data on the longer-term effect of the pandemic on stroke volumes over the course of a year and through the second wave of the pandemic. We sought to measure the effect of the COVID-19 pandemic on the volumes of stroke admissions, intracranial hemorrhage (ICH), IVT, and mechanical thrombectomy over a 1-year period at the onset of the pandemic (March 1, 2020, to February 28, 2021) compared with the immediately preceding year (March 1, 2019, to February 29, 2020)., Methods: We conducted a longitudinal retrospective study across 6 continents, 56 countries, and 275 stroke centers. We collected volume data for COVID-19 admissions and 4 stroke metrics: ischemic stroke admissions, ICH admissions, IVT treatments, and mechanical thrombectomy procedures. Diagnoses were identified by their ICD-10 codes or classifications in stroke databases., Results: There were 148,895 stroke admissions in the 1 year immediately before compared with 138,453 admissions during the 1-year pandemic, representing a 7% decline (95% CI [95% CI 7.1–6.9]; p < 0.0001). ICH volumes declined from 29,585 to 28,156 (4.8% [5.1–4.6]; p < 0.0001) and IVT volume from 24,584 to 23,077 (6.1% [6.4–5.8]; p < 0.0001). Larger declines were observed at high-volume compared with low-volume centers (all p < 0.0001). There was no significant change in mechanical thrombectomy volumes (0.7% [0.6–0.9]; p = 0.49). Stroke was diagnosed in 1.3% [1.31–1.38] of 406,792 COVID-19 hospitalizations. SARS-CoV-2 infection was present in 2.9% ([2.82–2.97], 5,656/195,539) of all stroke hospitalizations., Discussion: There was a global decline and shift to lower-volume centers of stroke admission volumes, ICH volumes, and IVT volumes during the 1st year of the COVID-19 pandemic compared with the prior year. Mechanical thrombectomy volumes were preserved. These results suggest preservation in the stroke care of higher severity of disease through the first pandemic year., Trial Registration Information: This study is registered under NCT04934020.

Abstract

This article discusses the establishment of pediatric interventional neuroradiology services in a public children's hospital in Pakistan. The study analyzes data from neuro-interventional procedures performed between March 2022 and April 2023, including patient information, success rates, and procedure-related problems. The results show that despite challenges, such as lack of evidence and equipment, the facility was able to perform a total of 289 procedures, with varying levels of success. The article emphasizes the importance of training more physicians in neurointervention and providing angiography equipment to government hospitals in order to improve access to advanced neurointervention services in low-middle-income countries.
