Abstract

Hemophilia A (HA) is associated with FVIII coagulation insufficiency or inactivity leading to excessive bleeding. Elevated FVIII, on the contrary, is associated with thrombophilia, thrombosis, myocardial infarctions, and stroke. Active FVIII (aFVIII) uses its C2 domain to bind to blood cells' membranes, consequently carrying out its coagulative function. We developed a reliable flow cytometry (FC) method for FVIII detection that can be utilized for assessing surface-bound FVIII on leukocytes in different coagulation/clinical states; we analyzed 49 pediatric subjects, encompassing patients with HA, other coagulopathies, venous thrombosis, and normal coagulation. Interestingly, the total leukocyte surface FVIII showed a declining trend across thrombosis, normal, and hypo-coagulation states. As expected, the leukocytes of HA patients displayed significantly lower levels of cellular-surface FVIII in comparison to patients with thrombosis. However, no significant correlation was observed between circulating levels of FVIII in plasma and the levels of FVIII bound to leukocytes, indicating that the differences in FVIII surface binding are not directly proportional to the availability of FVIII in the circulation and suggesting a specific binding mechanism governing the interaction between FVIII and leukocytes. Intriguingly, when analyzing the distinct blood subpopulations, we observed that surface FVIII levels were significantly elevated in classical monocytes of thrombosis patients compared to HA patients, healthy controls, and patients with other coagulopathies. Our study highlights the reliability of our FC platform in assessing FVIII abundance on leukocytes' membranes across coagulation states. Monocytes, particularly in cases of thrombosis, exhibit active binding of FVIII on their surface, suggesting a potential role in the pathophysiology of thrombosis that requires further investigation. Abstract

This paper explores the neuropsychiatric consequences of SARS-CoV-2 (COVID-19) infection, specifically the use of intravenous immunoglobulin (IVIG) therapy in treating central nervous system (CNS) symptoms associated with COVID-19. The authors searched PubMed and Google Scholar using the keywords "IVIG" and "covid-19 neuropsychiatric symptoms" to find five articles, including three case reports, a retrospective study, and a prospective study, that detail the experiences of individuals with persistent neuropsychiatric symptoms after contracting COVID-19. The neuropsychiatric symptoms reported in the analyzed studies include sleep disturbance, exhaustion, cognitive decline, anxiety, and others. Common treatments for post-COVID neuropsychiatric symptoms include medications, cognitive behavioral therapy, and lifestyle modifications. IVIG therapy to manage CNS symptoms of COVID-19 has shown mixed results in studies, with some showing positive effects while others remain inconclusive. Further research is needed to understand this therapy's potential benefits and limitations fully.

17. A comparative study of ketofol and fentafol for evacuation of retained products of conception (ERPC)

Abstract

Background: Intravenous balanced anesthesia (IVA) is desirable during the evacuation of retained products of conception (ERPC) to avoid the use of inhalational anesthetics agents that may cause uterus relaxation, the possibility of bleeding, and the risk of uterus perforation.Objectives: The aim of this study was to compare the efficacy and safety of ketofol (a mixture of propofol and ketamine) versus fentafol (a mixture of propofol and fentanyl) during the ERPC.Methods: A double-blind, randomized comparative study was conducted among a total of 60 women of childbearing age categorized as grades I and II according to the American Society of Anesthesiologist (ASA), presented for ERPC. The patients were selected and randomized blindly into two groups (K group and F group), with 30 patients in each group. The K group was given ketofol (1ml containing 5mg of propofol and 5mg of ketamine) and F group was given fentafol (1ml containing 5mg propofol and 5mcg fentanyl). An intravenous loading dose of ketofol or fentafol was given slowly, with doses ranging from 1ml to 2ml/10kg, to reach level 5 or 6 of the Ramsay Scale of Sedation (RSS), followed by small incremental doses which were given when RSS dropped to 4. Hemodynamic parameters, success, and side effects were assessed throughout the procedures.Results: K group demonstrated a significant increase in heart rate (HR) and blood pressure (BP), compared to significant decreases in the F group. Decreases in oxygen saturation (SpO2) and respiratory rate (RR) were observed more in the F group. However, no patients developed hypertension, hypotension, apnea, hypoxemia or serious adverse effects. Ketofol showed less propofol consumption and a short recovery time.Conclusions: Both ketofol and fentafol offer optimum conditions for ERPC. Ketofol is characterized by more stable hemodynamic parameters, a smaller dosage and faster recovery. [Ethiop.J. Abstract

Voltage Collapse is instability of a heavily loaded electric power system, which leads to declining voltages and blackout, and it is typically associated with reactive power limitations of the power system. Therefore, knowledge of the critical loading point is vital to ensure a secure mode of operation for the power system. Over the years, different methods were proposed in the literature to identify proximity to voltage collapse. However, these techniques were derived utilizing only the voltage limitation of the load buses. Voltage collapse is often interlinked with static transfer stability limit. In this work, different voltage collapse modes are addressed and a new algorithm that takes into account both load bus voltage and generator bus angle behavior is proposed to estimate the collapse point. The proposed method was examined on several IEEE test systems. Also, superiority of the proposed method in comparison with the other techniques was proven.
