Abstract

ABSTRACTIntroductionPolymer-lipid hybrid nanoparticle (PLN) is an emerging nanoplatform with distinct properties and functionalities from other nanocarrier systems. PLN can be optimized to overcome various levels of drug delivery barriers to achieve desired therapeutic outcomes via rational selection of polymer and lipid combinations based on a thorough understanding of their properties and interactions with therapeutic agents and biological systems.Areas coveredThis review provides an overview of PLN including the motive and history of PLN development, types of PLN, preparation methods, attestations of their versatility, and design strategies to circumvent various barriers for increasing drug delivery accuracy and efficiency. It also highlights recent advances in PLN design including: rationale selection of polymer and lipid components to achieve spatiotemporal drug targeting and multi-targeted cascade drug delivery; utilizing the intracellular lipid transport mechanism for active targeting to desired organelles; and harnessing bioreactive lipids and polymers to magnify therapeutic effects.Expert opinionA thorough understanding of properties of PLN components and their biofate is important for enhancing disease site targeting, deep tumor tissue penetration, cellular uptake, and intracellular trafficking of PLN. For futuristic PLN development, active lipid transport and dual functions of lipids and polymers as both nanocarrier material and pharmacological agents can be further explored.

18. Two-Step Targeted Hybrid Nanoconstructs Increase Brain Penetration and Efficacy of the Therapeutic Antibody Trastuzumab against Brain Metastasis of HER2-Positive Breast Cancer.

Abstract

Therapeutic antibodies (e.g., trastuzumab, TRA) against human epidermal growth factor receptor 2 (HER2)-positive breast cancers have shown benefits in controlling primary tumors, yet are ineffective against brain metastases due to their inability to cross the blood-brain barrier (BBB). A novel hybrid nanoconstruct system is designed to deliver trastuzumab to brain metastasis of HER2-positive breast cancer via a two-step sequential targeting approach. Self-assembly of a polysorbate 80 (PS 80)-containing polymer, lipid, and polymer-conjugated TRA forms hybrid nanoconstructs (TRA-terpolymer nanoparticles (TPN)) with high encapsulation efficiency and bioactivity. The PS 80 moiety enables the first-step targeting and receptor-mediated trancytosis across BBB is demonstrated in vitro with a 3D human BBB model in healthy and brain tumor-bearing mice. The subsequent partial dissociation of the nanoconstructs exposes the encapsulated TRA for the second-step targeting to HER2-positive cancer cells in the brain. Intravenously injected TRA- TPN delivers 50-fold TRA compared to free TRA to the brain metastasis of HER2-positive breast cancer. Treatment with TRA-TPN increases tumor cell apoptosis by 4-fold, inhibits tumor growth by 43-fold, and prolongs median survival by >1.3-fold compared to free TRA, without causing noticeable organ toxicity. These findings suggest the two-step targeted nanoconstruct system is promising for shuttling therapeutic antibodies to treat central nervous system diseases. [ABSTRACT FROM AUTHOR]

Abstract

Objective: The current study aimed to develop a matrix type sustained release Diclofenac tablet, using hydrophilic hydroxypropyl methylcellulose (HPMC) and hydrophobic polymer cetyl alcohol (CA). Materials and Methods: Two different polymers, that is, Methocel K15MCR® and CA were used in various proportions as release controlling factor. Matrix tablets were prepared by wet granulation technique. The physicochemical properties of the granules and tablets were evaluated. In vitro dissolution studies of prepared matrix tablet and patent product Voltaren SR® tablet (VSR) were performed at pH 7.4 phosphate buffer at 100 rpm, and at 37 ± 0.5°C, and subjected to in vitro bioequivalence study in terms of similarity and difference factors. Stability studies were conducted for 6 months using optimized formulation for extended period of time, both at room temperature and accelerated conditions. The dissolution data were fit to Zero-order, First-order, Higuchi, and Korsmeyer-Peppas equations. Results: The formulated tablets showed acceptable weight variation, hardness, drug content uniformity with sustained release matrix characteristics. Hydrophilic Methocel K15 MCR® matrices-based tablets showed zero-order and hydrophobic CA matrices-based tablets followed first-order kinetics except for formulation six (F6 showed zero-formulations containing CA showed better dissolution properties with respect to formulations containing Methocel K15 MCR® in terms of similarity and difference factor. Furthermore, the formulations F4, F5, and F6 exhibited similar drug release profile as compared with VSR tablet, which indicated that these formulations could be bioequivalent with VSR tablet in vitro. Tablets were stable both at room temperature and as well as at accelerated conditions. Conclusion: The present study demonstrated that Diclofenac could be successfully prepared using an appropriate amount of Methocel K15 MCR® and CA in the form of matrix tablets with similar dissolution profile of patent product Voltaren SR®.
