Background: Preoperative left ventricular dysfunction is an independent risk factor for perioperative mortality and morbidity and is also associated with postoperative low cardiac output syndrome. IABP improved outcomes in high-risk cardiac patients. However, installation of the balloon has many disadvantages. Recently, a new drug named levosimendan is commonly used as an inotropic support with no increase oxygen demand of myocardium. Objective: The purpose of our study was to compare the efficiency of levosimendan versus intra-aortic balloon pump in patients with poor left ventricular function undergoing coronary artery bypass grafting (CABG) with ejection fraction less than 35%. Patients and Methods: Forty patients with elective multi-vessel off-pump CABG at Al-Azhar University, Air force Specialized and Al-Assema Hospitals were randomized into 2 groups, according to the treatment they received – either levosimendan (Levo group) or intra-aortic balloon counter pulsation (IABP group), and compared with respect to The use of inotropic drugs, Hemodynamics, ICU stay, hospital stay and in-hospital mortality. Results: There were no differences in preoperative characteristics including patients’ age, sex, BMI, and comorbidities. The Levosemindan group had shorter ICU and hospital stays as compared to IABP group with comparable hemodynamic functions, time of mechanical ventilation and mortality rate.

Abu Dhabi's onshore oil fields are generally mature and contain carbonate reservoirs with light oil and gas. Various well types have been employed, such as vertical, highly deviated, and horizontal wells with cased hole or openhole completions. Surveillance of wellbore and formation fluid attributes is essential for reservoir management decisions and hydrocarbon production enhancement. This paper discusses surveillance techniques to understand multi-phase flow characteristics and reservoir saturation. Formation saturation monitoring and production profiling have been performed actively using pulsed neutron logging (PNL) and array production logging (APL) techniques. Challenges in employing the APL method in Abu Dhabi onshore fields include (1) many wells are horizontal with barefoot completions, (2) horizontal sections extend typically more than 2000-ft long with irregular and undulating trajectories, and (3) wells contain asphaltene, debris, and other materials preventing optimal spinner flowmeter functionality. After reviewing each well condition, well environment-specific combinations of APL, nuclear production logging applications for holdups and water velocity calculations, and an advanced three-phase formation saturation analysis technique were determined. This approach overcame several challenges to deliver surveillance objectives. We demonstrate four case examples, delineating well-based production and formation saturation profiles in various conditions. Two nuclear measurements exhibiting different sensitivities to oil and gas were combined to compute three-phase formation saturation. When a horizontal openhole wellbore was severely under-gauged, the pulsed neutron-based holdup application was used to avoid an APL tool deployment that might result in tool damage and unsatisfactory data acquisition. Additionally, for wells with good wellbore conditions, pulsed neutron-based and APL-based holdup data sets were acquired, and analysis results were compared. A stationary water velocity calculation method when water cut was high was also adopted to identify downhole water sources, and in-situ water production profiles from APL and nuclear applications were compared. An effort to evaluate production profile and in-situ saturation effectively from highly deviated- and horizontal wellbores to improve hydrocarbon production is described. The delineation of production profiles and formation fluid distribution allowed operators to determine reservoir and production management strategies.

In this work, a printed coplanar waveguide (CPW) fed single band antenna based on expanded graphite material is introduced. The proposed antenna is based on a CPW-monopole antenna with a U-shape conductor strip connected with the ground. Expanded graphite, a grade of graphene, is used as a conductor to design the uniplanar antenna over a flexible paper substrate. The antenna is designed for 2.4 GHz applications. The antenna design procedures are discussed. The material preparation and analysis are illustrated. Finally, the antenna fabrication and measurements of the reflection coefficient are discussed. The measured antenna reflection coefficient agrees with the simulated one, ensuring the antenna validity for serving the required applications. The radiation antenna parameters are discussed and simulated results from two-simulation software are included for comparison. The antenna has a simulated gain of 4 dBi and simulated efficiency of around 90% at 2.4 GHz.

Physical, FTIR, ultrasonic, and dielectric characteristics of calcium lead-borate glasses: 50B2O3–30CaO–20PbO–xEr2O3–yNd2O3:` (x,y) = (0,0; BCP), (0,1;1Nd), (1,0;1Er), and (1,1;1Nd–1Er) mol% have been examined. Nd3+ ions increased density up to 3857 kg/m3, meanwhile, molar volume barely changed. Er2O3 mol% decreased density down to 3641 kg/m3, while molar volume increased up to 2.75 × 10–5 m3. As well, Nd2O3 mol% + Er2O3 mol% decreased density down to 3556 kg/m3, while molar volume increased up to 2.91 × 10–5 m3. Er3+ and Nd3+ ions have different effects on the formation rate of BO−4 and BO2O− structural units. PbO in the glassy systems can be incorporated into the glass as network-forming Pb–O groups (PbO4 and/or PbO3). Introducing of Nd3+ ions induces changes in the local field on the lead ions giving rise to the formation of [PbO4] units. The addition of Er 3+ and/or Nd3+ to glass systems revealed that there was observed withdrawal of the area of PbO4- absorption IR band and increasing both of longitudinal and shear velocity. The elastic moduli of the investigated glasses have a fingerprint of density behavior versus rare-earth content. Dielectric constant and ac conductivity were obviously enhanced by the insertion of Nd3+/Er3+ ions separately or together into borate glass matrix referring to the creation of non-bridging oxygen.

This paper presents a compact dual-band filtering antenna without extra employing of filter structures. The antenna is designed using a planar dual-composite right/left-handed (D-CRLH) transmission line unit cell, where the filtering function is achieved through current cancellation between the D-CRLH resonators. The antenna is designed to function at 3.0 and 5.1 GHz, which can serve different WLAN applications. The antenna is a co-planar waveguide fed with a very compact size of only 30 × 16 mm2. Compared to the conventional patch antenna, the antenna size is only 17% at 3.0 GHz and 31% at 5.1 GHz. Despite the small size, the antenna preserves a good omni-directional radiation pattern at the two resonant frequencies with a measured realized gain of 2 and 2.7 dB, respectively. At the stopband in-between the two resonant bands, the reflection coefficient is almost 0 dB at 4.25 GHz and complete non-radiation is proved with a −11 dB measured realized gain. The different antenna filtering functions are verified by full-wave simulation and measurements.

A Fractal metasurface-based absorber is designed to obtain a wideband absorption in THz frequency band. The developed absorber has a broadband absorption over more than 600 GHz bandwidth, from 0.6 THz to 1.2 THz, with a minimum absorption percentage of 70% and total absorption from 1 to 1.1 THz. The proposed fractal structure is investigated using a Modified Split Ring Resonator (MSRR). The absorption coefficient of the proposed fractal metasurface is compared with the modified initially SRR metasurface. The results indicate that by using fractal design, the metasurface operating band is summed up, resulting in a wide bandwidth. The absorption coefficient of the proposed fractal metasurface is studied using two different simulation software programs to ensure the obtained results, with a high agreement between the results of them. The designed metasurface has a similar absorption response for the different polarization of the incident wave. The impedance of the designed metasurface indicates that, for the operating band, the fractal metasurface has a surface impedance of around 377 Ω, indicating a good matching between the designed metasurface and air. Different incidence and polarization angles are studied with results showing its insensitivity to both of them. The designed metasurface is compared with previously published works, showing wider bandwidth, higher absorption, and incidence and polarization insensitivity. The proposed metasurface absorber can be used for biomedical applications.

BACKGROUND: Atherosclerosis is a widespread and devastating disease and one of the leading causes of death worldwide. So much is there to understand about atherosclerosis. And although a lot is already discovered, yet most of the studies are performed in cell cultures and animal models. Recent technologies for genetic engineering and imaging are mainly performed on animal models, with few studies in human tissues. A better understanding of their role is required. AIM: We aim to study the expression of CD 34 hematopoietic progenitor stem cell, CD 68 macrophages, and smooth muscle actin (SMA)-positive smooth muscle cells (SMCs) in the human coronary arteries and correlate their differential expression with the atherosclerosis progression. RESULTS: CD 68 and CD 34 expression increase as the atherosclerotic process proceeds from early atheroma to advanced atheroma and start to decrease as the process proceeds to fibroatheroma with a significant p < 0.001. Conversely, SMA expression decreases as the atherosclerotic process progresses with a significant p < 0.001. CONCLUSION: CD34 progenitor cells in conjunction with CD 68 macrophages have a major role in the development of atherosclerosis, whereas the SMCs are minimal in the early stages and reach their maximal levels during the stage of fibroatheroma.

Background Comparison between the clinical outcomes of mid-urethral sling procedure for stress incontinence with and without repair of asymptomatic stage II cystocele is made. Methods This is a prospective randomized study of 72 female patients with stress urinary incontinence and asymptomatic stage II cystocele. The patients were divided into two groups: group 1 treated with trans-obturator tape only and group 2 with repair of cystocele by anterior colporrhaphy in the same session. We compared between both groups in cure rate and voiding function after 1 year. Results The cure rate of group 1 was 66.7%, while for group 2 it was 90% after 12 months (p Conclusion Concomitant repair of stage II asymptomatic cystocele with mid-urethral sling improves the cure rate of stress urinary insentience and reduces the incidence of irritative urinary symptoms.

The increasing mandate for fresh-like food products and the possible hazards of chemically preserved foods necessitate the search for alternatives. Bacteriocins represent a promising food biopreservative. In the present study, one hundred enterococci isolates recovered from Egyptian raw cow milk and homemade dairy products were screened for bacteriocin production. The overall detection rate was 10%. Three isolates, namely, Enterococcus faecalis (OE-7 and OE-12) and Enterococcus hirae (OE-9), showed the highest antibacterial activity with narrow spectrum against multidrug-resistant (MDR) Gram-positive foodborne bacteria: Enterococcus faecalis and Staphylococcus aureus. The antimicrobial activity was completely abolished by trypsin and proteinase K but not affected by lipase and/or amylase indicating the protein nature of the antimicrobial activity. Optimum conditions for bacteriocin production were cultivation in MRS broth at 37 °C, pH 6-6.5 for 16-24 h. The tested bacteriocins exhibited bactericidal activity on S. aureus subsp. aureus ATCC 25923; such activity was further investigated by transmission electron microscopy that revealed leakage and lysis of treated cells. Characterization of tested bacteriocins revealed high activity in a wide range of pH and temperature, storage stability, and heat resistance. PCR analysis revealed that the tested isolates produced multiple enterocins showing homology with the enterocins L50A, AS-48, and 31. Finally, this study reported potent antibacterial activity of bacteriocins derived from dairy products Enterococci against MDR foodborne and spoilage pathogens. The potency, specificity, and stability of these bacteriocins presented promising perspectives for application as biopreservatives in the food industry. The biopreservation of foods by bacteriocins produced by lactic acid bacteria recovered directly from foods remains an innovative approach.

Treatment options for patients with metastatic CRC (mCRC) expanded during the last two decades, with introduction of new chemotherapeutic and targeted agents. Egypt is a lower middle-income country; Egyptian health care system is fragmented with wide diversity in drug availability and reimbursement policies across different health care providing facilities. We report the results of consensus recommendations for treatment of patients with metastatic colorectal cancer developed by Egyptian Foundation of Medical Sciences (EFMS), aiming to harmonize clinical practice through structured expert consensus-based recommendations consistent with the national status. EFMS recommendations could be utilized in other countries with similar economic status.Methods: EFMS recommendations were developed using a modified Delphi process, with three rounds of voting till the final recommendations were approved. A non-systematic review of literature was conducted before generating the provisional statements. Content experts were asked to vote on some recommendations in two different resource groups (restricted resources and non-restricted resources). External review board of experts from a low income and lower-middle countries voted on the applicability of EFMS recommendations in their countries.Results: The current recommendations highlighted the discrepancy in health care between restricted and non-restricted resources with expected survival loss and quality of life deterioration. Access to targeted agents in first line is very limited in governmental institutions, and no access to agents approved for third line in patients who failed oxaliplatin and irinotecan containing regimens for patients treated in restricted resource settings.Conclusion: Management of mCRC in developing countries is a challenge. The currently available resource-stratified guidelines developed by international cancer societies represent a valuable decision-making tool, adaptation to national status in each country based on healthcare system status is required.Keywords: colorectal cancer CRC, consensus recommendations, metastatic CRC, management in a lower-middle income country

An experiment was performed during two winter successive seasons of (2016/2017-2017/2018), at a private farm in Wadi El-Natrun area, El-Behera Governorate, Egypt, to study the effect of applied irrigation water (IR) methods: fuzzy Logic controller (FLC) and manual irrigation water addition (Manual) based on ETc100% at different humic acid rates “HA” (10, 15 and 20 kg ha-1) under surface and sub-surface drip irrigation systems on marketable yield, crop quality parameters, actual evapotranspiration (ETa), water use efficacy (WUE) and irrigation water use efficiency (IWUE) for sugar beet roots and spinach leaves crops under surface (SDI) and sub- surface drip (SSDI). The results showed that, the marketable yield and studied quality parameters of sugar beet roots and spinach leaves gave the highest values under FLC and SSDI treatment for both seasons. While, the seasonal ETa of sugar beet and spinach crops gave the lowest values: 436.24and 141.82 mm; 429.16 and 139.93 mm for both seasons respectively, under FLC and SSDI treatment. The results recorded that, the marketable yield and studied quality parameters of sugar beet and spinach crops gave the highest values under FLC, HA =20 kg ha-1 and SSDI treatment for both seasons. While, the IR and ETa of both crops gave the lowest values under FLC, HA =15 kg ha-1 and SSDI treatment. Finally, the WUE and IWUE of both crops gave thehighest values under FLC, HA =15 kg ha-1 and SSDI treatment. This study concluded that the cultivation of sugar beet and spinach crops under FLC, HA =15 kg ha-1 and SSDI treatment can possibly save of the applied irrigation water about 35 and 45% for the 1st season while, were 41 and 47% for the 2nd season respectively, moreover, increased marketable yield 35 and 45% for the 1st season while, were 41 and 47% for the 2nd season respectively, compared with that under the control treatment (Manual, HA =20 kg ha-1 and SDI treatment).
