Abstract

Abstract Background Left ventricular hypertrophy (LVH) is associated with an increased risk of ventricular arrhythmias and cardiovascular mortality. The study objective was to investigate the effect of LVH severity on the complexity and severity of ventricular arrhythmias among a subset of Egyptian hypertensive patients. Results The study cohort consisted of 60 hypertensive patients with LVH as diagnosed by echocardiography. Their mean age was (53.7 ± 12.3) years. 36 males (40%) and 24 females (60%). Diabetes mellitus was found in 26 patients (43%), 41% of these cases were smokers. 48-h Holter monitoring was performed in all cases to assess the frequency of ventricular premature contractions (VPC) and their complexity using the Lown grading. Abstract

The successful execution of spacecraft missions relies heavily on the performance and reliability of their power supply subsystems. Achieving optimal component size and configuration is crucial for efficient power generation, distribution, and storage. Designing an efficient power supply subsystem for spacecraft missions is a complex and multi-objective optimization problem. Traditional optimization techniques, such as genetic algorithm and particle swarm optimization, have been employed, but their limitations in handling discrete variables and achieving global optima necessitate the exploration of alternative methods. In this context, this research paper explores the application of discrete water cycle algorithm (DWCA) optimization as a novel approach for optimizing the component size and configuration of spacecraft power supply subsystems. By emulating the dynamic flow and distribution of water, DWCA seeks to efficiently explore the design space and identify optimal solutions.Through comprehensive experimentation and comparative analysis, this research demonstrates that the proposed DWCA outperforms/surpasses other traditional optimization techniques in optimizing component size and configuration for spacecraft power supply subsystems. It consistently determines designs that are more efficient in power generation and storage. Moreover, DWCA enhances the efficiency and reliability of spacecraft power supply systems, especially in mission-critical scenarios. The successful optimization of spacecraft power supply subsystems is paramount to the overall success of space missions. Therefore, this advancement contributes significantly to the ongoing efforts to ensure the success of future space missions.

Abstract Background Early recognition and appropriate treatment has shown to decrease morbidity and mortality in patients with undifferentiated shock. There are many ultrasound protocols in shock; each protocol combines core ultrasound elements such as IVC and cardiac assessment which includes detection of cardiac tamponade, left ventricular function and right ventricular strain.Valvular assessment is absent in majority of ultasound protocols, while lung ultrasound is included in some of them. Objective In this study we investigated which parameters used in Echo – US protocol help differentiate shock types. Methods This cross sectional study was conducted on 150 patients with shock (140 patients were included while, 10 patients were excluded).Sensitivity and specificity of different parameters used in the Echo-US protocol were analyzed to detect which parameters can diffentiate shock types. Abstract

Abstract This paper aims to present the structural design and the rehabilitation process as well as the site assessment of the reconstruction and rehabilitation of the twelfth-century Grand Al-Nouri Mosque in Al-Nouri Complex, which is a part of the project Revive the Spirit of Mosul in Iraq. UNESCO launched the project Revive the Spirit of Mosul in February 2018 in order to reconstruct the old city after the destruction and looting from the wars with ISIS (Daesh) in 2017. Al-Nouri Complex includes the Great Al-Nouri Mosque and the 45-m-high Al-Hadba Minaret, which are the iconic historic landmarks of the old city of Mosul on the western side of the Tigris River, and it was built by Nur Al-Din Zengi during the twelfth century. Al-Nouri Mosque was reconstructed in 1942 using the original 40 octagonal Moslawi marble columns, and alabaster arches, the mihrab brought from the Ummayad Mosque in Mosul dates back to 956 AD, masonry stones, and a reinforced concrete roof. In this paper, the structural performance of Al-Nouri Mosque has been investigated through site observations, experimental tests, and structural analyses. Due to the several destructive earthquakes recorded around Mosul, a 3D finite element model of the whole mosque using a commercial finite element program has been developed considering the actual material properties and the effect of wind and seismic actions. Generally, the output results of the structural analysis models showed that the obtained compressive stress values did not exceed the compressive strength values defined for the original footing stones and Moslawi marble. Also, the recommended rehabilitation methodology was proposed to enhance the structural characteristics of the dome, the original Moslawi marble columns, and the old stone footings under the marble columns.

Abstract

Background/objective: Small bowel atresia commonly causes neonatal intestinal obstructions. Technical problems are associated with the surgical management of atresia using primary end–to–end anastomosis. Furthermore, the significantly dilated proximal loop may be associated with the stasis of intestinal fluid, thus increasing pressure on the anastomosis and prolonging fasting time before initiation of oral intake. This study aimed to perform antimesenteric tapering of the proximal loop using a linear stapler to reduce its diameter and facilitate anastomosis with the distal loop. Methods: This retrospective study included 57 neonates diagnosed with jejunoileal atresia. They were categorised into two groups: Group A (n = 29), which included neonates treated using antimesenteric sleeve enteroplasty tapering using a linear stapler and Group B (n = 28), which included neonates treated at the primary end of the proximal loop to the side of the distal loop anastomosis. Results: The mean operative time was 122.5 min in group A vs. 118 min in group B, and the mean duration to reach full oral intake was 17 days in group A vs. 20.2 days in group B (p = 0.03). The mean length of hospital stay was 25 and 35 days in groups A and B, respectively (p = 0.042). Conclusion: Tapering the proximal dilated bowel loop to achieve anastomosis with the distal loop improved the transient time, reduced stasis and its associated translocation and colonisation, and allowed for early initiation and maintenance of oral intake. All these parameters shortened the overall length of hospital stay.

Abstract

The scientific community recognizes that the depletion of natural resources and solid waste management pose inherent challenges in building material production and disposal, particularly in concrete manufacture and deconstruction. Recycling and reusing construction waste is proposed as a solution to these issues in the literature. Repurposing waste materials as additives in concrete represents an alternative approach. For instance, ceramic tile waste (CTW) may replace a portion of the aggregate and cement used in concrete. This study extensively examines the literature on cement-based composites that utilize CTW in lieu of cement and aggregate. A thorough evaluation of mechanical, durability, and fresh properties is conducted. The physical and chemical attributes of CTW are based on extensive scientific research. Prior studies suggest that the use of ceramic tile aggregate (CTA) to concrete in the appropriate proportions could enhance its durability and strength. Finely ground ceramic tile powder (CTP) with high silica content can potentially enhance the strength and durability of concrete, according to prior research studies. The impact of CTP on the chemical resistance, drying shrinkage and fire resistance abilities of concrete are subsequently evaluated. Employing waste materials as a concrete component in a circular economy to promote environmental protection and the development of sustainable cities and communities has received broad support from the academic community.

Abstract

Abstract Background Early diagnosis and appropriate management of shock aimed at prevention of prolonged hypoperfusion has shown to decrease morbidity and mortality in patients with undifferentiated shock. However, there is often a challenge in emergency department (ED) – where diagnosis is mainly based on clinical signs and standard monitoring parameters. Early use of point of care ultrasound could reduce the diagnostic time and improve diagnostic accuracy. Purpose The aim of this study is to investigate the accuracy of echocardiography - ultrasound protocol to identify the cause of shock in ED. Method The study was conducted on 150 shocked patients admitted to emergency department of Alexandria Main University Hospital from December 2018 to December 2020. The study was conducted to reach initial impression about shock etiology which was then compared to final diagnosis to determine accuracy, agreement, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV). Results One hundred forty patients were included in the study (10 patients were excluded). The protocol was 100% accurate for diagnosing cases with obstructive and mixed obstructive distributive shock. It showed excellent rule- out characteristics for cardiogenic shock (sensitivity and NPV = 100%). There was almost perfect agreement between provisional and final shock type for mixed distributive cardiogenic shock (kappa 0.915). Echo- US protocol had lowest agreement and PPV for patients with hypovolemic shock Kappa 0.48 and 35% respectively. Conclusion The Echo- US protocol showed a high accuracy in identifying shock etiology in ED and is likely a promising diagnostic tool in emergency care.

Abstract

Background: The characteristics, outcomes, and risk factors for in-hospital death of critically ill intensive care unit (ICU) patients with coronavirus disease-2019 (COVID-19) have been described in patients from Europe, North America and China, but there are few data from COVID-19 patients in Middle Eastern countries. The aim of this study was to investigate the characteristics, outcomes, and risk factors for in-hospital death of critically ill patients with COVID-19 pneumonia admitted to the ICUs of a University Hospital in Egypt. Methods: Retrospective analysis of patients with COVID-19 pneumonia admitted between April 28 and July 29, 2020 to two ICUs dedicated to the isolation and treatment of patients with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections in Cairo University Hospitals. Diagnosis was confirmed in all patients using real-time reverse transcription polymerase chain reaction on respiratory samples and radiologic evidence of pneumonia. Results: Of the 177 patients admitted to the ICUs during the study period, 160 patients had COVID-19 pneumonia and were included in the analysis (mean age: 60 ± 14 years, 67.5% males); 23% of patients had no known comorbidities. The overall ICU and hospital mortality rates were both 24.4%. The ICU and hospital lengths of stay were 7 (25–75% interquartile range: 4–10) and 10 (25–75% interquartile range: 7–14) days, respectively. In a multivariable analysis with in-hospital death as the dependent variable, ischemic heart disease, history of smoking, and secondary bacterial pneumonia were independently associated with a higher risk of in-hospital death, whereas greater PaO2/FiO2 ratio on admission to the ICU was associated with a lower risk. Conclusion: In this cohort of critically ill patients with COVID-19 pneumonia, ischemic heart disease, history of smoking, and secondary bacterial pneumonia were independently associated with a higher risk of in-hospital death.

Abstract

Abstract Background The accurate non-invasive diagnosis of spontaneous bacterial peritonitis (SBP) in patients with decompensated liver cirrhosis has not been achieved yet. The aim of the study was to obtain an unmistakable diagnosis of SBP using a new simple serum bioscore, made by combined measurement of procalcitonin (PCT), erythrocyte sedimentation rate (ESR), and C-reactive protein (CRP), which we called the PEC index. This cross-sectional analytic study comprised 178 cirrhotic patients with ascites (60 patients with SBP and 118 patients with sterile ascites), after excluding non-SBP infection, during the period from March 2019 until September 2019. In all participants, serum levels of PCT, ESR, and CRP were measured, and PEC index was calculated [PEC index = PCT × (ESR + CRP)]. Results Patients with SBP (n = 60) had significantly higher serum PEC index than those with sterile ascites (n = 118) (41.0/31.2–93.0 vs. 9.9/5.9–15.0, P < 0.001). PEC index distinguished culture positive cases significantly (P < 0.001). Using receiver operating characteristic (ROC) statistics, the sensitivity and specificity of PCT, at a cutoff value of 0.590 ng/mL, for SBP diagnosis, were 81.67% and 93.33%, respectively (area under the curve [AUC] = 0.879; 95% confidence interval [CI] 0.809–0.948). The sensitivity and specificity of ESR, at a cutoff value of 27.0 mm/hour, were 73.33% and 61.67%, respectively (AUC = 0.679; 95% CI 0.581–0.776). The sensitivity and specificity of CRP, at a cutoff value of 21.0 mg/L, were 93.33% and 51.67%, respectively (AUC = 0.736; 95% CI 0.639–0.833). While, the sensitivity and specificity of PEC index, at a cutoff value of 20, were highest (98.33% and 96.67%, respectively, AUC = 0.977; 95% CI 0.940–0.996). Conclusion Serum PEC index makes an accurate noninvasive diagnosis of SBP, after excluding other infections.

Abstract

Introduction: Morel–Lavallee lesion (MLL) is an infrequent or underreported serious consequence of closed degloving injuries. We aimed to describe the clinical presentation and management of pelvic MLL in obese patients. Materials and Methods: A retrospective analysis was conducted for pelvic trauma patients with a diagnosis of MLL between 2010 and 2012. Patients' demographics, presentations, management, and outcomes were analyzed and compared based on the body mass index (BMI) and injury severity. Results: Of 580 patients with pelvic region injuries, 183 (31.5%) had MLL with a mean age of 30.1 ± 12.2 years. The majority (75.4%) of MLL patients had a BMI ≥30 and 44% patients had pelvic fracture. Based on the initial clinical examination, MLL was diagnosed in 84% of patients and clinically missed in 16% of patients. Nonoperative management (NOM) was performed in 93.4% of patients, while primary surgical intervention was indicated in 6.6% of patients. Failed NOM was observed in seven cases, of them five were obese. The overall mortality in MLL patients was 12.6% and the frequency of deaths was nonsignificantly higher in Grade I obese patients. Multivariate analysis showed that injury severity score (odds ratio [OR]: 1.25, 95% confidence interval [CI]: 1.05–1.50) and Glasgow coma scale (OR: 0.72, 95% CI: 0.56–0.92) were the predictors of mortality in patients with MLL irrespective of BMI. Conclusions: One-third of pelvic region injuries have MLL and three-quarter of them are obese. This significant association of obesity and MLL needs further prospective evaluation.

Abstract

Introduction: Despite global efforts to contain the illness, COVID-19 continues to have severe health, life, and economic repercussions; thus, maintaining vaccine development is mandatory. Different directions concerning COVID-19 vaccines have emerged as a result of the vaccine’s unpredictability. Aims: To study the determinants of the attitudes of healthcare workers (HCWs) to receiving or refusing to receive the vaccine. Methods: The current study adopted an interviewed questionnaire between June and August 2021. A total of 341 HCWs currently working at Assiut University hospitals offered to receive the vaccine were included. Results: Only half of the HCWs (42%) accepted the COVID-19 vaccine. The most common reason that motivated the HCWs was being more susceptible than others to infection (71.8%). On other hand, the common reasons for refusing included: previously contracted the virus (64.8%); did not have time (58.8%); warned by a doctor not to take it (53.8%). Nearly one-third of nonaccepting HCWs depended on television, the Internet, and friends who refused the vaccine for information (p < 0.05). In the final multivariate regression model, there were six significant predictors: sex, job category, chronic disease, being vaccinated for influenza, and using Assiut University hospital staff and the Ministry of Health as sources of information (p < 0.05). Conclusion: Misinformation and negative conceptions are still barriers against achieving the desired rate of vaccination, especially for vulnerable groups such as HCWs.

Background: Malignant pleural mesothelioma (MPM) is a highly aggressive tumor that arises from the surface cells of the pleura with a poor survival rate. Fibulin-3 is a protein biomarker found in blood and pleural fluid of patients with mesothelioma and can reliably predict the presence, or absence, of mesothelioma cancer cells. The possible role of fibulin-3 in diagnosis of MPM was studied. Patients and methods: Sixty patients were included in the study, 30 with pleural effusions due to MPM and another 30 with non mesothelioma malignant pleural effusion (MPE). Plasma and pleural effusion fibulin-3 levels were estimated for all patients using an enzyme-linked immunosorbent assay (ELIZA). Results: Plasma and pleural effusion fibulin-3 levels were significantly higher in patients with MPM (113 ± 3.7 ng/ml and 594.2 ± 65.7 ng/ml, respectively) compared to those with non mesothelioma MPE (44.4 ± 7.1 ng/ml and 187.3 ± 14.5 ng/ml, respectively) (P

Abstract

Customer satisfaction is a universal concern that impacts enterprises of all sizes, whether they are for-profit or non-profit, and whether they operate on a global or local scale. Therefore, it is crucial for every firm, including hospitals, to have a comprehension of the aspects that impact customer satisfaction. A study proposed that the correlation between the use of resources and patient satisfaction may be due to the perception that patients have. They believe that doctors who allow them to stay in the hospital for a longer period or conduct additional tests demonstrate a greater concern for their well-being. Consequently, these patients feel more confident that their concerns are being acknowledged and dealt with by their physicians. A comprehensive analysis of primary care patients revealed numerous studies that established a direct correlation between meeting patient expectations and their overall satisfaction with the care received. However, it was also observed that physicians often misinterpret patients' desire for information as an expectation for a specific course of action. [ABSTRACT FROM AUTHOR]

Abstract

The building sector is a significant contributor to global carbon emissions, primarily due to the production of cement, a crucial construction material. The need for sustainable building practices has spurred research into alternative materials to traditional cement and sand. Two such materials, ultrafine coal bottom ash (uCBA) and spent garnet (SG), have shown potential as substitutes for cement and fine aggregate, respectively. This research examines the effectiveness of uCBA as a partial replacement for cement and SG as a partial replacement for fine aggregate in concrete, at varying levels of 3%, 6%, and 9% for uCBA and 10%, 20%, and 30% for SG. Initially, the chemical composition analysis was conducted using an X-ray fluorescence spectrometer (XRF), and the particle morphology of uCBA, OPC, and SG was examined through field emission scanning electron microscopy (FE-SEM). Then, the study evaluates the performance of these materials based on slump value, compressive strength, and splitting tensile strength, with specimens undergoing 28, 56, and 90-day curing periods. Results indicate that integrating uCBA and SG in concrete at levels of 3% and 10%, respectively, enhances compressive and splitting tensile strength while decreasing environmental impact. Moreover, workability improves with higher substitution levels. Overall, this research indicates that uCBA and SG hold great potential as replacements for cement and fine aggregate in concrete, providing a means to mitigate the environmental impact of construction.

Abstract

Background: Study of expression of lymphocyte subsets could help to understand the pathophysiology of COVID-19 infection, development of novel treatment protocols and reducing rate of mortality among COVID-19 infected patients. The aim of this work was to correlate expression of lymphocyte subsets with the inflammatory markers and clinical status of patients with COVID-19 infection. Methods: This prospective study was performed at Zagazig University Isolation Hospitals and clinical pathology department, Zagazig, Egypt on 80 persons who were enrolled in this study; 40 COVID-19 patients with pneumonia made up group I, and the following individuals made up group II: 40 COVID-19 patients without pneumonia. Immunophenotyping of CD4 CD8 were measured by Flow Cytometry in both groups. Results: Patients had a considerable drop in CD4 CD8 levels with pneumonia than those without pneumonia Conclusions: Low levels of CD4 CD8 in COVID-19 patients are associated with a higher risk of pneumonia. 45. A Novel Battery Management and Recovery Control Algorithm for Mitigating the Memory Effect in Nickel Hydrogen Batteries for Space Missions.

Abstract

The spacecraft’s (SC) electric power system (EPS) is one of the vital systems. It could limit the SC capabilities or reduce the SC lifetime. The EPS should provide the required energy for the SC loads to perform their tasks successfully until End-of-Life (EOL). Furthermore, the EPS should guarantee the power needed to execute the customer requirements under all degradations (battery degradation, radiation, etc.) and other external factors (orbital degradation). The SC’s primary power generation sources are solar arrays and storage batteries. This paper investigates a SC with a Nickel Hydrogen Storage Battery (NHSB) consisting of 17 cells: 4 measured cells and 13 unmeasured cells. In this paper, two distinct objectives have been achieved: The first objective is formulating a control algorithm for battery testing and reconditioning (CABTR). This algorithm is applied to an NHSB in an orbital setting mode, rectifying the memory effect in the NHSB and restoring the battery’s total capacity. The second objective constructs an environment that effectively simulates the dynamic behavior of the battery during in-orbit operations. This simulated environment calculates crucial parameters such as the state of charging (SOC), battery temperature, voltage, and pressure of the NHSB during reconditioning. This computational analysis assures the robustness of the CABTR, and confirms the optimal battery performance. Subsequent validation encompasses ground tests and real-time telemetry data, which affirms the battery’s dynamic behavior and the developed algorithms’ efficacy. [ABSTRACT FROM AUTHOR]

Abstract

Purpose: Up to 50% of patients do not achieve significant left ventricular ejection fraction (LVEF) recovery after primary percutaneous intervention (PPCI) for STEMI. We aimed to identify the echocardiographic predictors for LVEF recovery and assess the value of early follow‐up echocardiography (Echo) in risk assessment of post‐myocardial infarction (MI) patients. Methods: One hundred one STEMI patients undergoing PPCI were enrolled provided EF below 50%. Baseline echocardiography assessed LVEF, volumes, wall motion score index (WMSI), global longitudinal strain (GLS), global circumferential strain (GCS), and E/e'. Follow‐up echocardiography after 6 weeks reassessed left ventricular volumes, LVEF and GLS.GCS was not assessed at follow up. Patients were classified into recovery and non‐recovery groups. Predictors of LVEF recovery and major adverse cardiovascular events (MACE) at 6 months were analysed. Results: The mean change of EF was 8.04 ± 3.32% in group I versus ‐.39 ± 5.09 % in group II (p <.001). Recovered patients had better baseline GLS, baseline GCS, E/e', and follow‐up GLS. Multivariate regression analysis revealed E/e', GCS, and follow‐up GLS after 6 weeks to be strong independent predictors for LVEF recovery. Composite MACE was considerably higher in group II (32.7% vs. 4.1%, p <.001) mainly driven by higher heart failure hospitalisation Multivariate regression analysis revealed baseline GLS, E/e', and ejection fraction (EF) percentage recovery as strong independent predictors for MACE. Conclusions: Multiparametric echocardiographic approach incorporating LVEF, strain parameters, and diastolic function could allow early optimal risk stratification after STEMI treated with PPCI. Follow‐up GLS and LVEF percentage change are the strongest predictors for early LV recovery and long term clinical outcome, respectively. [ABSTRACT FROM AUTHOR]

Abstract

Today, the subject of a building's resistance to lateral loads is one of the most important concerns of structural engineers. The partitions and infilled walls are non-structural elements that are important due to their effects on the lateral resistance of the building frame. Recently, it has been observed that great damage is occurring to infilled walls, partitions, and buildings in an earthquake-prone area. Infilled walls are effective at increasing the hardness and resistance of building frames, which changes the seismic properties of structures. Therefore, the study of interactions between the structural frame and the infilled walls is essential for a better understanding of structural behaviors. In this paper, the effect of infilled walls is investigated on the behaviour of steel frames using ABAQUS software. Modeling is carried out for different types of infilled materials, including brick and panel, as well as different thicknesses of the infills. It was observed that with an increase in the thickness of infills from 7 to 20 cm, the final capacity and energy absorption increased by 78%. Also, the panel-infilled frames have 18% more capacity and 3.8% more energy absorption than the brick-infilled frame in the same full state. As a result, panel-infilled frames outperform brick-infilled frames in terms of performance.&nbsp

Abstract

Smart phones are an indispensable tool in modern day-to-day life. Their widespread use has spawned numerous applications targeting diverse domains such as bio-medical, environment sensing and infrastructure monitoring. In such applications, the accuracy of the sensors at the core of the system is still questionable, since these devices are not originally designed for high accuracy sensing purposes. In this thesis, we investigate the accuracy limits of one of the commonly used sensors, namely, a smart phone accelerometer. As a use case, we focus on utilizing smart phone accelerometers in structural health monitoring (SHM). Using the already deployed network of distributed citizen-owned sensors is considered a cheap alternative to standalone sensors. These devices can capture floors vibration during disasters, and consequently compute the instantaneous displacement of each floor. Hence, damage indicators defined by government standards such as peak relative displacement can be estimated. In this work, we study the displacement estimation accuracy and propose a zero-velocity update (ZUPT) method for noise cancellation. Theoretical derivation and experimental validation are presented, and we discuss the impact of sensor error on the achieved building classification accuracy. Moreover, in spite of the presence of sensor error, SHM systems can be resilient by adopting machine learning. Several algorithms such as support vector machine (SVM), K-nearest neighbor (KNN) and convolutional neural network (CNN) are adopted and compared. Techniques for addressing noise levels are proposed and the results are compared to regular noise cancellation techniques such as filtering.Finally, since most previous work focused on modelling the sensor chip error itself, we study other sources of error such as sampling time uncertainty which is introduced by the device operating system (OS). That type of error can be considered a major contributor to the overall error, specially for sufficiently large signals. Hence, we propose a novel smart device accelerometer error model that includes the traditional additive noise as well as sampling time uncertainty errors. The model is validated experimentally using shake table experiments, and maximum likely-hood estimation (MLE) is used to estimate the model parameters. Moreover, we derive the Cramer-Rao lower bound (CRLB) of acceleration estimation based on the proposed model.
