1. The influence of social media marketing on customer knowledge management: The role of confidentiality in UAE public banks

Abstract: Social media facilitates banks’ interaction with consumers and provides critical information about their behaviors and preferences. Nevertheless, given the sensitive nature of financial information, maintaining stringent confidentiality is of paramount importance. This study aims to examine the impact of social media marketing (SMM) dimensions on customer knowledge management (CKM) in UAE public banks, focusing on the moderating role of banking confidentiality. The study utilized a quantitative methodology with a correlational framework; data were collected from 283 respondents, who are active customers of First Abu Dhabi Bank, Emirates NBD, and Abu Dhabi Commercial Bank, through a structured questionnaire. These customers were surveyed to understand their interaction with social media campaigns, the information banks request from them, their willingness to share personal data, and their perceptions of safety and legal protections. The results revealed a strong positive relationship between SMM dimensions and CKM, with SMM explaining 65.9% of the variance in CKM outcomes (R² = 0.659, p < 0.01). Among the SMM dimensions, customization had the highest impact (R² = 0.766), followed by word-of-mouth (R² = 0.697) and aesthetics (R² = 0.651). Additionally, confidentiality was found to significantly enhance the effectiveness of SMM, with a moderating effect increasing explained variance by 6.5% (ΔR² = 0.065, β = 0.25, p < 0.01). These findings suggest that public banks in the UAE should integrate personalized SMM strategies with stringent confidentiality measures to optimize CKM. This approach not only enhances customer engagement but also builds trust, fostering sustainable growth in the digital era. 2. Optimizing dormant account management in UAE banking: Legal gaps and proposed reforms

Abstract: The management of dormant accounts and unclaimed balances is a pressing challenge in the banking sector of the United Arab Emirates (UAE), particularly given the complex regulatory landscape. This study analyzes 150 dormant accounts across five major UAE banks (Emirates NBD, First Abu Dhabi Bank, Dubai Islamic Bank, Sharjah Islamic Bank, and Abu Dhabi Commercial Bank) and identifies gaps in the legal framework, including the absence of clear definitions and handling of non-monetary assets.The study investigates the legal framework governing dormant accounts, specifically focusing on the Dormant Accounts System No. 1 of 2020. The results highlight critical issues, including the lack of clear timelines for transferring unclaimed balances and inconsistencies in communication protocols for notifying account holders. Furthermore, the study emphasizes the need for standardized practices across financial institutions in the UAE.To address these challenges, the study proposes legislative amendments to improve asset management and consumer protection. Key recommendations include establishing standardized definitions, implementing automated tracking systems for dormant accounts, and integrating dormant balances into social welfare programs to enhance public trust. These reforms could significantly improve operational efficiency and legal clarity in the UAE banking sector, contributing to a more transparent and effective management of dormant accounts and unclaimed balances. 3. Analyzing the impact of viral marketing on brand equity dimensions in Egypt’s home appliances sector: A customer and legal perspective

Abstract: In the digital age, viral marketing has emerged as a key driver in shaping brand equity, particularly in the highly competitive Egyptian home appliances sector. Traditional marketing strategies have proven insufficient in reaching consumers with the same efficacy as viral campaigns, which leverage digital platforms and consumer networks. This study examines the influence of viral marketing on brand equity dimensions – brand awareness, perceived quality, brand loyalty, and brand association – through the lens of both customer perception and legal frameworks Using a quantitative research design, data were collected from 270 respondents across 389 companies, with a response rate of approximately 70% of the total population in Egypt’s home appliances sector, with simple linear regression analysis employed to assess the relationships. The results indicate a strong positive effect of viral marketing on brand equity. Specifically, viral marketing accounts for 27.2% of the variance in brand awareness (R² = 0.272, p < 0.001), 28.1% in perceived quality (R² = 0.281, p < 0.01), 13.6% in brand loyalty (R² = 0.136, p < 0.05), and 40.9% in brand association (R² = 0.409, p < 0.001). Furthermore, the study finds that regulatory compliance plays a moderating role, ensuring that viral marketing campaigns remain within ethical and legal bounds, thereby enhancing consumer trust and the long-term impact of marketing efforts.These findings highlight the strategic value of integrating viral marketing with a firm understanding of legal frameworks to optimize brand equity in Egypt’s home appliances industry. This research provides actionable insights for brand managers and marketers looking to maximize the efficacy of their viral campaigns while maintaining brand integrity. 4. Generic sofosbuvir and daclatasvir for treatment of hepatitis C virus infection in patients with sickle cell disease

Abstract: Abstract Background and purpose of the study Sickle cell disease (SCD) patients are at a high risk of chronic liver disease (CLD) due to chronic viral hepatitis infection such as hepatitis C virus (HCV) infection, iron overload, and sickle cell hepatopathy. Nowadays, several oral direct-acting antiviral drugs (DAAs) have been developed and approved by the FDA for hepatitis C treatment. However, the safety and efficacy of DAAs in SCD patients remain insufficiently explored. Purpose of the study To evaluate the efficacy and safety of administration of generic sofosbuvir (SOF) and daclatasvir (DCV) for 12 weeks in SCD patients infected with HCV. Methods A retrospective study included 38 SCD patients infected with HCV treated with generic SOF (400 mg) and DCV (60 mg) for 12 weeks without ribavirin. The effectiveness of the HCV treatment was assessed by the sustained virologic response (SVR) at 24 weeks after the end of the treatment (SVR24). Results The SVR24 rate was 100% (38/38).There were insignificant alterations in hemoglobin and total bilirubin levels during HCV treatment or at end of treatment (EOT). The number of anemic patients who needed blood transfusion two weeks before HCV treatment, at week 4 of treatment, and at EOT was 11 (28.9%), 3 (8%), and 1 (3%) respectively. Moreover, the reductions in serum transaminase levels from baseline were statistically significant compared to the EOT. Conclusion Generic SOF and DCV regimens appear to be safe and effective in the treatment of chronic HCV in patients with SCD.

Abstract: Abstract Background Acute coronary syndrome (ACS) is responsible for one-third of total deaths in people older than 35 years old. Number of prognostic models aims to estimate the future risk of mortality, or the combined risk of all-cause mortality or MI has been developed, such as GRACE score. Objective The aim of our study is to assess the utility of GA and CHA2DS2-VASc score in predicting outcomes in patients admitted with acute coronary syndromes. Patients and methods This study was conducted on 90 patients diagnosed with acute coronary syndrome who were admitted receiving PCI, CHA2DS2-VASc score was calculated on admission, and a serum sample for GA was withdrawn within 24 h from admission. Results Combined GA and CHA2DS2-VASc score could significantly predict MACCE with sensitivity 100%, specificity 79%, and diagnostic accuracy 80.9% and significantly predict hospital mortality with sensitivity 100%, specificity 75.3%, and diagnostic accuracy 76.7%; on the other hand, GRACE score can predict MACCE using cutoff point 112.5 with sensitivity 75% and specificity 66%. Conclusion It was concluded that GA and CHADSVASC score can significantly predict major adverse cardiovascular events, in-hospital mortality, and poor clinical outcomes.

Abstract: IntroductionThe role of miRNAs in regulating variable molecular functions has been sought by scientists for its promising utility in regulating the immune response and, hence, in treating various diseases. In hepatocellular carcinoma (HCC) specifically, a reduction in the number and efficiency of circulating and intrahepatic natural killer (NK) cells has been reported. Our project aims to investigate the role of miR-216a-3p in the regulation of NK cell cytotoxicity, especially since it plays a tumor suppressor role in the context of HCC.MethodsTo achieve our aim, we isolated NK cells from the whole blood of 86 patients with HCC and 23 healthy controls. We assessed the expression profile of miR-216a-3p in NK cells of patients and controls. Furthermore, we induced the expression of miR-216a-3p in NK cells isolated from healthy controls, followed by measuring the release of interferon-gamma (IFN-γ), tumor necrosis factor-alpha (TNF-α), perforins (PRF) and granzyme B (GrB) using ELISA as well as NK cells cytolytic activity against Huh7 cells using lactate dehydrogenase (LDH) cytotoxicity assay. After that, we performed an in silico analysis to understand the mechanistic regulation imposed by miR-216a-3p on NK cells to study its impact on one of its potential downstream targets.ResultsOur results have indicated that miR-216a-3p has higher expression in NK cells of patients with HCC, and simulating this elevated expression pattern via forcing miR-216a-3p expression in normal NK cells has negatively impacted the release of TNF- α, IFN- γ, GrB, and PRF. Consequently, a decrease in cell cytolysis was observed. Our in silico analysis revealed that the predicted downstream targets of miR-216a-3p are enriched in the FOXO-signaling pathway. Among those targets is FOXO-1, which has been reported to play a role in NK cell maturation. Thus, we evaluated FOXO-1 expression upon mimicking miR-216a-3p in control NK cells that showed significant downregulation of FOXO-1 on both RNA and protein levels.ConclusionIn conclusion, we report miR-216-3p as a negative regulator of NK cell cytotoxicity.

7. Compressive and bending properties of 3D-printed wood/PLA composites with Re-entrant honeycomb core

Abstract: This study investigates the mechanical behavior of sandwich structures comprising a re-entrant honeycomb core structure created using wood/polylactic acid (PLA) filaments via fused deposition modeling (FDM) technology. The sandwich structures were manufactured with different face layer thicknesses (0 mm, 0.8 mm, and 1.6 mm) and various core topologies. Material characterizations included bending tests, compressive tests, and finite element analysis (FEA) to identify stress concentration areas. In-plane and out-of-plane re-entrant honeycomb core structure specimens were tested to examine the bending properties. In addition, flatwise and edgewise compressive tests were performed to investigate the structures' compressive properties. Furthermore, the modulus of elasticity for each specimen was determined using the finite element technique (FEM). The results reveal that increasing the thickness of the face layer significantly enhances the structure's resistance to bending forces. Furthermore, specimens with an in-plane orientation demonstrated better bending strength compared to those with an out-of-plane orientation due to increased material underloading. In flatwise compressive tests, specimens without a face layer exhibited the highest strength, attributed to their greater displacement. In contrast, edgewise compressive tests showed significant buckling behavior of the face sheet, the maximum stress increased proportionally with the thickness of the face layer, reaching its peak at a skin thickness of 1.6 mm. The findings are validated by ANSYS analysis, which closely mirrors the experimental results, providing insight into flexural modulus, modulus of elasticity, and stress concentration. These findings indicate that architected core structures could be efficiently utilized to improve bending/compressive characteristics and failure mechanisms, providing valuable insights into the mechanical response of sandwich structures for different industrial applications.

8. Stool Lactoferrin as a biochemical marker in Crohn’s disease; correlation with clinical, laboratory and endoscopic indices and its use as a surrogate marker for endoscopic healing.

Abstract: Background A significant limitation of mucosal healing as a treatment target in Crohn’s disease is that Ileocolonoscopy remains the gold-standard for assessing disease activity. Identifying optimal biomarkers and their cut-off values is an unmet need in the context of tight monitoring strategy. Aim To study the performance of Fecal lactoferrin (FL) in patients with Crohn’s disease in comparison to endoscopic assessment, clinical indices and fecal calprotectin (FC) to address whether it correlates with endoscopic severity of inflammation and whether it could be used as a surrogate marker of mucosal healing after therapy. Methods In this prospective study, 35 patients with active Crohn’s disease were recruited. All patients provided stool samples for FL and FC at baseline and underwent colonsocopy. 26 of these patients were followed up after 6-9 months of therapy initiation or upgrade. FL was compared with the simple endoscopic score of Crohn’s disease (SES-CD) ,Harvey-Bradshaw index (HBI) and FC. Data was analyzed to identify cut-off levels for endoscopic response/remission. Results Lactoferrin showed excellent performance in detecting remission (AUC 0.93) with a sensitivity of 85.7% and a specificity of 88.9% at a cut-off of 21.5µg/g. A drop of >26.2% from baseline values is 100% sensitive and specific in detecting endoscopic response (≥50% reduction from baseline SES-CD). Lactoferrin showed strong correlation with SES-CD (r=0.74) and calprotectin (r=0.91) Conclusion FL is a reliable marker of response to therapy and mucosal healing, making endoscopic monitoring of treatment success less necessary. Whether Lactoferrin is superior to Calprotectin or not would require further investigation.

9. Value of autotaxin as a serum marker for liver fibrosis in chronic HCV infected patients receiving direct-acting antiviral therapy

Abstract: Background: Non-invasive biomarkers have largely replaced liver biopsy in assessment of liver fibrosis in chronic HCV. Autotaxin (ATX) is a novel serum marker that may be related to liver fibrosis. Aim: to clarify the role of ATX as a biomarker for the estimation of hepatic fibrosis and to compare its sensitivity and specificity to the well-known fibrosis-4 score (FIB-4) and the AST-to-Platelet Ratio Index (APRI) before treatment with direct-acting antiviral drugs (DAAs) for chronic HCV as well as six months after the end of treatment. Methods: Plasma samples were obtained from 86 chronic HCV patients with different degrees of liver fibrosis. Routine laboratory, transient elastography (TE) and ultrasonographic assessments were done. Enzyme-linked immunosorbent assay (ELISA) technique was used to detect ATX. Results: ATX, FIB-4, and APRI had AUC of 0.57, 0.95, and 0.92, respectively for the detection of cirrhosis (F4). Baseline ATX was higher in cirrhotic group vs. non-cirrhotic group (250 vs. 210) pg/ml, although the difference was not significant (p= 0.3). Significant improvement of all the laboratory parameters, APRI, FIB-4 and liver stiffness occurred at sustained virological response after 24 weeks (SVR24). Non- significant increase of ATX level was noted six months after the end of treatment. Conclusion: ATX should be considered cautiously as a diagnostic marker for liver fibrosis in patients with chronic HCV.

10. Post COVID-19 persistent symptoms and functional status in COVID-19 survivors: a multi-center study

Abstract: Abstract Background Post COVID symptoms are a series of chronic symptoms that patients may experience after resolution of acute COVID-19. Different post-COVID-19 condition phenotypes might exist, although exact causes, management, and outcomes are not known. Aim To assess the functional status among post-COVID-19 survivors and identify the associated socio-demographic, clinical, and laboratory risk factors of the poor functional status among those cases and to identify the most common persistent symptoms among post-COVID-19 survivors after discharge. This was a cross-sectional study conducted on 150 recovered cases who had been infected with COVID-19 as confirmed by swab during hospitalization and being interviewed regarding functional status 6 months post-hospital discharge. Cases were divided into two groups: the decreased functional status group (n = 74) and the non-decreased functional status group (n = 76). Results The present study indicated statistically significant differences between the studied groups; receiving the influenza vaccine was significantly associated with keeping the pre-COVID functional status (p = 0.02). The reduced functional status group had a significantly more severe disease course, prolonged hospital as well as ICU stay (p < 0.001), and worse CT findings than the normal functional status group (p = 0.004). Long-term symptoms such as headache, mood changes, insomnia, hearing problems, dry eyes, breathlessness, and chest tightness were significantly more prevalent among those who reported limitations in their functional status (p < 0.001). There were significantly higher CRP, serum ferritin, and D-dimer in the reduced functional status group. Conclusion The present study highlights that most COVID-19 recovered cases have different degrees of functional limitations ranging from null to severe based on the PCFS scale. These limitations were affected by periodic influenza vaccination, ICU admission, and length of hospital stay. Some laboratory parameters were associated with reduced functionality: CRP, D-dimer, and serum ferritin. Psychological/neurological symptoms and breathlessness were significantly associated with reduced PCFS. This calls for public health action and necessitates widespread health education of post-COVID-19 health consequences.

11. Prevalence and predictors of hepatic steatosis among HIV patients with and without chronic hepatitis C

Abstract: Abstract Background and purpose of the study Hepatic steatosis (HS) is a common and important histologic finding in patients with chronic hepatitis C virus (HCV) infection. However, little is known about this finding in HCV patients co-infected with human immunodeficiency virus (HIV). Purpose of the study To evaluate the prevalence and predictors of HS among HIV patients with and without HCV. Methods A cross-sectional including 47 HIV mono-infected and 50 HIV/HCV patients. Detailed demographic, laboratory and clinical data were collected. A transient elastography (TE) examination with the controlled attenuation parameter (CAP) was performed for all patients. Steatosis was scored on a scale from 0 to 3 according to the percentage of steatosis involving hepatocytes. Results HS was detected in 18 /50 (36%) HIV/HCV co-infected patients and 22 /47 (46.8%) HIV mono-infected patients. In addition, HCV mono-infected patients were more likely to have grade 2 steatosis as compared with HIV/HCV co-infected individuals (25.5% vs. 12%). LSM was significantly higher among HIV/HCV co-infected patients (p = 0.003). When dividing HIV/HCV co-infected and HIV mono-infected group according to CAP values. BMI, abdominal wall thickness, and bright liver were significantly associated with SHS (CAP ≥ 238 dB/m) in both groups. Diabetes was only significantly associated with SHS in HIV/HCV co-infected group BMI was the only independent predictor of SHS among the whole group and subgroup analysis. Conclusion The prevalence of HS was lower in HIV/HCV co-infected patients compared to HIV mono-infected patients; we cannot say that HCV has a protective role against HS in co-infected patients. More fibrosis progression was common in the HIV/HCV co-infected patients.

12. AMP-RNNpro: a two-stage approach for identification of antimicrobials using probabilistic features

Abstract: Abstract Antimicrobials are molecules that prevent the formation of microorganisms such as bacteria, viruses, fungi, and parasites. The necessity to detect antimicrobial peptides (AMPs) using machine learning and deep learning arises from the need for efficiency to accelerate the discovery of AMPs, and contribute to developing effective antimicrobial therapies, especially in the face of increasing antibiotic resistance. This study introduced AMP-RNNpro based on Recurrent Neural Network (RNN), an innovative model for detecting AMPs, which was designed with eight feature encoding methods that are selected according to four criteria: amino acid compositional, grouped amino acid compositional, autocorrelation, and pseudo-amino acid compositional to represent the protein sequences for efficient identification of AMPs. In our framework, two-stage predictions have been conducted. Initially, this study analyzed 33 models on these feature extractions. Then, we selected the best six models from these models using rigorous performance metrics. In the second stage, probabilistic features have been generated from the selected six models in each feature encoding and they are aggregated to be fed into our final meta-model called AMP-RNNpro. This study also introduced 20 features with SHAP, which are crucial in the drug development fields, where we discover AAC, ASDC, and CKSAAGP features are highly impactful for detection and drug discovery. Our proposed framework, AMP-RNNpro excels in the identification of novel Amps with 97.15% accuracy, 96.48% sensitivity, and 97.87% specificity. We built a user-friendly website for demonstrating the accurate prediction of AMPs based on the proposed approach which can be accessed at http://13.126.159.30/ .

Abstract: Background: Research on anxiety in patients with schizophrenia of Arab origin is surprisingly scarce, particularly given that expressions of both psychotic disorders and anxiety disorders can be largely shaped by cultural factors. This study proposes to complement previous research by exploring the psychometric characteristics of an Arabic translation of the Staden Schizophrenia Anxiety Rating Scale (S-SARS) in chronic, remitted patients diagnosed with schizophrenia. As the Arabic version of the Generalized Anxiety Disorder 7-Item Scale (GAD-7) has not been previously validated in Arabic in patients with schizophrenia, this study had as a secondary aim to investigate the psychometric properties of this scale before its use. Method: This cross-sectional study was performed over a period of three months (August–October 2023). A total of 177 chronic inpatients diagnosed with schizophrenia (63.3 % males) who were remitted and clinically stable participated in the study. Results: Confirmatory factor analyses showed that all 10 items loaded onto a single factor and had high factor loading values between .53 and .81. The reliability of the S-SARS in its Arabic version was excellent as attested by a McDonald's omega and a Cronbach's alpha coefficients of .90 and .89, respectively. 14. Thirty versus 60-Watt thulium laser enucleation of prostate: Toward the development of low-power anatomical enucleation of the prostate

Abstract: Introduction: We aimed to study whether using 30 W versus 60 W thulium enucleation of the prostate (ThuLEP) would affect postoperative outcomes in patients with benign prostatic hyperplasia (BPH). Materials and Methods: We prospectively identified male patients with moderate or severe lower urinary tract symptoms due to BPH. We randomized patients into 30 W (Group 1) or 60 W (Group 2) thulium yag laser with a 550 μm laser fiber and a 26 Fr continuous flow resectoscope. We collected data related to prostate size, enucleation time, morcellation time, laser time, perioperative complications, and 1-year functional outcomes. Results: A total of 120 patients were included, with a mean age of 67 years and a mean prostate size of 105 g. The preoperative characteristics were similar across both groups. The mean operative time was shorter in the 60 W group, 74 ± 27 vs. 91 ± 33 min in the 30 W group (P = 0.001), and the mean laser time was 55 ± 20 in 60 W versus 71 ± 25 in 30 W (P = 0.0001). The mean hospital stay was 1 day in both groups and at 1-year follow-up; there was a similar improvement in mean Qmax and International Prostate Symptom Score symptom scores. Discussion/Conclusion: Both 30 and 60 W ThuLEP provided a safe and comparable outcome with a relatively shorter operative time for the 60 W groups. Perhaps using a 30-W setting would be beneficial in the early learning curve or cases with more bleeding capsular perforators; besides, the financial benefit of manufacturing low-cost low-power devices that may help in the widespread of AEEP.

Abstract: The increasing demand for high-grade manganese ores in various industries has led to a growing necessity to enhance the quality of lower-grade manganese ores with time. A representative manganese ore was collected from Um Bogma, Sinai, Egypt, and subjected to various characterization methods including X-ray diffraction (XRD), X-ray fluorescence (XRF) and ore microscopy. XRF analysis revealed a low Mn/Fe ratio of 2.81 (as elements), indicating a low ore grade. For suitability in Mn alloy production at the Sinai Manganese Company (SMC) plant in Abu Zenimah, southeast of Sinai, Egypt, this ratio should ideally exceed 5. Additionally, XRD and ore microscopy identified the mineralogical composition of the ore as pyrolusite, hematite, and manganite, with manganese patches embedded in a ferruginous groundmass at the textural scale. Three different reduction roasting techniques were evaluated, involving conventional heating in a furnace utilizing either CO – CO2 or charcoal as reducing agents, alongside reduction roasting with charcoal utilizing microwave technology. Following each technique, magnetic separation was employed, resulting in the production of high-grade manganese concentrates exhibiting high Mn/Fe ratios ranging from 19.8 to 27.45 and substantial Mn recovery rates ranging from 93.86% to 96.76%.

Abstract: Graph neural network (GNN) is a formidable deep learning framework that enables the analysis and modeling of intricate relationships present in data structured as graphs. In recent years, a burgeoning interest has arisen in exploiting the latent capabilities of GNN for healthcare-based applications, capitalizing on their aptitude for modeling complex relationships and unearthing profound insights from graph-structured data. However, to the best of our knowledge, no study has systemically reviewed the GNN studies conducted in the healthcare domain. This study has furnished an all-encompassing and erudite overview of the prevailing cutting-edge research on GNN in healthcare. Through analysis and assimilation of studies, current research trends, recurrent challenges, and promising future opportunities in GNN for healthcare applications have been identified. China emerged as the leading country to conduct GNN-based studies in the healthcare domain, followed by the USA, UK, and Turkey. Among various aspects of healthcare, disease prediction and drug discovery emerge as the most prominent areas of focus for GNN application, indicating the potential of GNN for advancing diagnostic and therapeutic approaches. This study proposed research questions regarding diverse aspects of GNN in the healthcare domain and addressed them through an in-depth analysis. This study can provide practitioners and researchers with profound insights into the current landscape of GNN applications in healthcare and can guide healthcare institutes, researchers, and governments by demonstrating the ways in which GNN can contribute to the development of effective and efficient healthcare systems.

Abstract: Knee osteoarthritis (KOA) is a prevalent condition characterized by gradual progression, resulting in observable bone alterations in X-ray images. X-rays are the preferred diagnostic tool for their ease of use and cost-effectiveness. Physicians use the Kellgren and Lawrence (KL) grading system to understand the severity of an individual condition of KOA. This system categorizes the disease from normal to a severe stage. Early detection of the condition with this approach enables knee deterioration to be slowed down with therapy. In this study, we aggregated four datasets to generate an extensive dataset comprising 110,232 raw images by applying an augmentation technique called deep convolutional generative adversarial network (DCGAN). We employed advanced image pre-processing methods (adaptive histogram equalization (AHE), fast non-local means), including image resizing, to generate a substantial dataset and enhance image quality. Our proposed approach involved developing a modified compact convolutional transformer (CCT) model known as KOA-CCTNet as the foundational model. We further investigated optimal configurations by adjusting various parameters and hyperparameters in the final model to handle large datasets and address training time concerns efficiently. We investigated optimizing its configurations by adjusting numerous parameters and hyperparameters to efficiently manage extensive data and address concerns related to training time. Simulation results indicated that our proposed model outperforms other transfer learning models (Swin Transformer, Vision Transformer, Involutional Neural Network) in terms of accuracy. The test accuracy for the ResNet50, MobileNetv2, DenseNet201, InceptionV3, and VGG16 was 80.77%, 79.98%, 80.23%, 76.89%, and 79.58%, respectively. All of them were surpassed by our proposed KOA-CCTNet model, which had a test accuracy of 94.58% while classifying KOA X-ray images. Furthermore, we reduced the number of images to assess the model’s performance and compared it to existing models. However, by employing a large datahub, our proposed approach provides a unique and effective way to diagnose KOA grades with satisfying results.

18. An Alzheimer’s disease category progression sub-grouping analysis using manifold learning on ADNI

Abstract: Abstract Many current statistical and machine learning methods have been used to explore Alzheimer’s disease (AD) and its associated patterns that contribute to the disease. However, there has been limited success in understanding the relationship between cognitive tests, biomarker data, and patient AD category progressions. In this work, we perform exploratory data analysis of AD health record data by analyzing various learned lower dimensional manifolds to separate early-stage AD categories further. Specifically, we used Spectral embedding, Multidimensional scaling, Isomap, t-Distributed Stochastic Neighbour Embedding, Uniform Manifold Approximation and Projection, and sparse denoising autoencoder based manifolds on the Alzheimer’s Disease Neuroimaging Initiative (ADNI) dataset. We then determine the clustering potential of the learned embeddings and then determine if category sub-groupings or sub-categories can be found. We then used a Kruskal–sWallis H test to determine the statistical significance of the discovered AD subcategories. Our results show that the existing AD categories do exhibit sub-groupings, especially in mild cognitive impairment transitions in many of the tested manifolds, showing there may be a need for further subcategories to describe AD progression.

Abstract: SARS-CoV-2, responsible for the global COVID-19 pandemic, has manifested significant cardiovascular implications for the infected population. These cardiovascular repercussions not only linger beyond the initial phase of illness but have also been observed in individuals who remain asymptomatic. This extended and pervasive impact is often called the post-acute COVID-19 syndrome (PACS) or “Long COVID”. With the number of confirmed global cases approaching an alarming 756 million, the multifaceted challenges of Long COVID are undeniable. These challenges span from individual health complications to considerable burdens on worldwide healthcare systems. Our review comprehensively examines the complications of the persistent cardiovascular complications associated with COVID-19. Furthermore, we shed light on emerging therapeutic strategies that promise to manage and possibly mitigate these complications. We also introduce and discuss the profound concerns regarding the potential transgenerational repercussions of SARS-CoV-2, emphasizing the need for a proactive and informed approach to future research and clinical practice.

Abstract: Background: Several industrial products [such as paints, cosmetics, and petrol] contained metals like lead [Pb], which are harmful to people. The reproductive organs are the primary organs affected by heavy metal toxicity through the production of oxidative stress. Selenium and curcumin could help to counteract this toxicity.Aim of the work: We investigated the possible role of Selenium or curcumin in improving the testicular toxicity caused by the intake of Lead orally in albino rats.Materials and Methods: 40 adult male albino rats were selected and unsystematically separated into 4 groups [10 rats/each] included: Group 1: control group; Group 2: Pb-exposed group; Group 3: Pb plus Selenium and Group 4: Pb plus Curcumin. Blood samples were collected for examination of serum biochemical parameters, serum Testosterone, LH, FSH levels, Total Anti-oxidant Capacity [TAC], testicular tissue oxidative stress parameters and testicular histology were evaluated.Results: Oral administration of Pb to rats for 4 weeks disturbs the testicular structure and function with a significant increase in the oxidative stress enzymes, deposition of collagen fibers and anticaspase-3 in the testis. While, administration of either Selenium or Curcumin with Pb-exposure induced significant improvement of all the testicular changes.Conclusion: Chronic lead exposure causes deteriorating changes in the testicular structures and functions which could be improved by oral intake of either Selenium or Curcumin by their antioxidant, antifibrotic, and antiapoptotic properties.

21. Comparing the effect of oral ivabradine versus oral propranolol premedication during controlled hypotensive anesthesia in functional endoscopic sinus surgery

Abstract: Background Reflex tachycardia that occurs during nitroglycerin-induced hypotensive anesthesia in functional endoscopic sinus surgery (FESS) increases both the risk of intraoperative bleeding and the duration of surgery. Our clinical trial aimed to compare the effect of oral ivabradine versus oral propranolol as a premedication before nitroglycerin-induced hypotensive anesthesia on the reduction of reflex tachycardia in FESS.Methods In this clinical, comparative, prospective, randomized, controlled, double-blinded study, 40 patients were divided into two equal groups (20 each); group P received oral propranolol (10 mg tablet) and group I received oral ivabradine (5 mg tablet) in the evening before the surgery and 1 hour before the induction of anesthesia.Results The reduction in heart rate (HR) was statistically significantly higher in group I than in group P. Conversely, the reduction in mean blood pressure (MAP) was statistically significantly higher in group P than in group I at baseline (pre-induction). However, after that, MAP reduction was statistically non-significant between both groups. Meanwhile, there was no statistically significant difference between both groups as regards duration of surgery, duration of deliberate hypotension, intraoperative NTG infusion doses, and the amount of bleeding.Conclusion Premedication with oral propranolol 10 mg or oral ivabradine 5 mg before FESS proved to be safe and effective in the reduction of reflex tachycardia that occurs during controlled nitroglycerin induced hypotensive anesthesia in FESS. However, ivabradine was more effective with higher safety profile than propranolol.

Abstract: Purpose Diabetes mellitus (DM) increases the risk of morbidity and mortality after liver resection. Albuminuria is associated with a higher risk for all-cause and cardiovascular mortality. This study evaluated albuminuria as a predictor of the outcome of living donor liver transplantation (LDLT) in patients with pre-existing DM.Methods This retrospective study involved 103 type II diabetic patients with end-stage liver disease who received LDLT. Preoperative spot urine albumin: creatinine ratio was used to determine the degree of albuminuria. The primary outcome measure was the impact of urinary albumin excretion on the 3-year mortality rate after LDLT in this diabetic cohort.Results Hepatitis C virus infection was the main cause of cirrhosis. Albuminuria was detected in 41 patients (39.8%); 15 had macroalbuminuria, while 26 had microalbuminuria. Patients with microalbuminuria were significantly older than those with macroalbuminuria and normal albumin in urine. After 3 years, twenty-four patients (23.3%) died within 3 years after LT. Myocardial infarction was the leading cause of death (25%). Albuminuria was an independent factor affecting 3-year mortality with an odds ratio of 5.17 (95% CI: 1.86–14.35).Conclusion Preoperative albuminuria is an independent factor affecting mortality within 3 years after LDLT in type II diabetic patients. Myocardial infarction was the leading cause of death in 25% of cases, followed by hepatocellular carcinoma recurrence, sepsis, and graft failure.KEY MESSAGESDiabetes mellitus (DM) increases the risk of morbidity and mortality after liver resection.Albuminuria is associated with a higher risk for all-cause and cardiovascular mortality.Preoperative albuminuria is a significant predictor of mortality within 3 years after LDLT in diabetic patients.

Abstract: Background: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: High FiO2 may be cost-effective (cheaper and effective). In Nigeria, the average cost for high FiO2 was $216 compared with $222 for low FiO2 leading to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the average cost for high FiO2 was $184 compared with $195 for low FiO2 leading to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the average cost for high FiO2 was $1164 compared with $1257 for low FiO2 leading to a −$93 (95% CI: −$132 to −$65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this.

Abstract: Background Fiberoptic endoscopic evaluation of swallowing (FEES) has been recommended to assess aspiration in stroke. This study aimed to determine the diagnostic and prognostic roles of FEES in the early assessment of aspiration, intensive care unit (ICU) stay and mortality in acute stroke patients. Methods Fifty-two patients with acute stroke admitted to the Alexandria Main University Hospital were included. Complete examinations and assessment of aspiration using the 8-point penetration-aspiration scale (PAS) with FEES protocol were performed. Results The patients were classified into three groups: normal with no or low risk of aspiration (n=15, 27.3%; PAS level 1), low to moderate risk (n=8, 14.5%; PAS level 2–4), and high risk (n=32, 58.2%; PAS ≥5). Abstract: Urban flooding is a problem faced by most countries because of climate change. This paper aims to study the most appropriate position or arrangements of grate inlets to upgrade drainage efficiency at less cost. Different arrangements of grates were studied and their efficiency was determined. A comparison between the total grate’s efficiency was conducted and the best arrangement was selected. Additionally, a dimensional analysis equation was developed to determine the total efficiency of the system. Finally, the FLOW-3D program was used to simulate the laboratory results using different discharges and numbers of inlets. The error of calculation ranged between 5% and 8%. Therefore, the results indicated that this program is a powerful tool for predicting the discharge efficiency and velocity direction for large discharges. A comparison was made between this study and previous studies. The results indicated that the same trend existed. A new equation was developed to correlate discharge efficiency (E) with relative total discharge Q and number of inlets. The equation can be used by planning engineers to conduct initial planning of storm drainage layout systems and achieve cost saving.

Abstract: Mesenchymal stromal/stem cells (MSCs) are multipotent cells that reside in multiple tissues are capable of self-renewal and differentiation into various cell types. These properties make them promising candidates for regenerative therapies. MSC identification is critical in yielding pure populations for successful therapeutic applications; however, the criteria for MSC identification proposed by the International Society for Cellular Therapy (ISCT) are inconsistent across different tissue sources. This study aimed to identify potential markers to be used together with the ISCT criteria to provide a more accurate means of MSC identification. Thus, we carried out a computational comparative analysis of the gene expression in human and mouse MSCs derived from multiple tissues to identify the differentially expressed genes that are shared between the two species. We show that six members of the proteasome degradation system are similarly expressed across MSCs derived from bone marrow, adipose tissue, amnion, and umbilical cord. Additionally, with the help of predictive models, we found that the expression profile of these genes correctly validated the identity of the MSCs across all the tissue sources tested. Moreover, using genetic interaction networks, we showed a possible link between these genes and antioxidant enzymes in the MSC antioxidant defense system, thereby pointing to their potential role in prolonging the life span of MSCs. According to our findings, members of the proteasome degradation system may serve as stemness-related markers.

Abstract: Abstract Background Gastric antral vascular ectasia (GAVE) is an uncommon but important cause of gastrointestinal bleeding in cirrhotic patients. Argon plasma coagulation (APC) is the standard therapy for GAVE. Endoscopic band ligation (EBL) is an emerging, safe and effective treatment for GAVE. The best way of applying EBL in the management of GAVE is not clear yet. Aim We aimed to determine the safety and efficacy of APC alternating with EBL versus EBL alone for managing GAVE in cirrhotic patients. Patients and methods Forty cirrhotic patients with bleeding GAVE were randomized to receive either APC alternating with EBL (20 patients) or EBL alone (20 patients) until GAVE lesions are eradicated. Gastroscopy was done 6 months after eradication of GAVE lesions to document recurrence. Cessation of bleeding, rise of hemoglobin level, need for transfusion, hospitalization, complications, number of sessions, and recurrence of GAVE in both groups were statistically analyzed. Results Patients in both groups showed significantly high rate of bleeding cessation, improvement in hemoglobin levels, reduction in transfusions, and hospitalizations. There was no statistically significant difference regarding the recurrence of GAVE between the two groups. There were no complications seen in the combined therapy group. Twenty percent of the patients in the EBL group had complications including hypertrophied polyp formation and post-band ulcerations. Conclusion APC alternating with EBL and EBL alone are effective methods in the treatment of bleeding GAVE. Combined therapy has the potential to decrease the number of banding sessions and the number of rubber bands required to treat GAVE, consequently decreasing the incidence of band-related complications.

However, hydrolysis and absorption are not always absolute, and some lactose will enter the colon where the gut microbiota is able to hydrolyze lactose and produce metabolic byproducts.MethodsHere, the impact of lactose on the gut microbiota of healthy adults was examined, using a short-term, in vitro strategy where fecal samples harvested from 18 donors were cultured anaerobically with and without lactose. The data were compiled to identify donor-independent responses to lactose treatment.Results and discussionMetagenomic sequencing found that the addition of lactose decreased richness and evenness, while enhancing prevalence of the β-galactosidase gene. Taxonomically, lactose treatment decreased relative abundance of Bacteroidaceae and increased lactic acid bacteria, Lactobacillaceae, Enterococcaceae, and Streptococcaceae, and the probiotic Bifidobacterium. This corresponded with an increased abundance of the lactate utilizers, Veillonellaceae. These structural changes coincided with increased total short-chain fatty acids (SCFAs), specifically acetate, and lactate. These results demonstrated that lactose could mediate the gut microbiota of healthy adults in a donor-independent manner, consistent with other described prebiotics, and provided insight into how dietary milk consumption may promote human health through modifications of the gut microbiome.

Abstract: Propolis is a naturally occurring substance with beneficial properties; bees produce it from various plant sources, and it is an anti-inflammatory and therapeutic resinous substance. This study aimed to enhance the biological features of propolis extract by loading it onto active film. Firstly, extraction was performed using three solvent systems, and their total phenolic, flavonoid, and antioxidant activity was measured. Propolis ethanol extract (EEP) was evaluated for phenolic fraction content and then chosen to prepare a chitosan-loaded emulsion with several concentrations. The antibacterial, anti-mycotic, and anti-mycotoxigenic properties of the extract and nanoemulsion were assessed. PPE’s cytotoxicity and nanoemulsion were evaluated using brine shrimp and cell line assays. Results indicate higher phenolic (322.57 ± 4.28 mg GAE/g DW), flavonoid (257.64 ± 5.27 mg QE/g DW), and antioxidant activity of the EEP. The phenolic fraction is distinguished by 18 phenolic acids with high p-hydroxybenzoic content (171.75 ± 1.64 µg/g) and 12 flavonoid compounds with high pinocembrin and quercetin content (695.91 ± 1.76 and 532.35 ± 1.88 µg/g, respectively). Phenolic acid derivatives (3,4-Dihydroxybenzaldehyde, 3,4-Dihydroxyphenol acetate, and di-methoxy cinnamic) are also found. Concentrations of 50, 100, 150, and 200 ng EEP loaded on chitosan nanoemulsion reflect significant antibacterial activity against pathogenic bacteria, particularly methicillin-resistant Staphylococcus aureus (MRSA) and toxigenic fungi, particularly Fusarium. Among the four EEP-loaded concentrations, the nanoemulsion with 150 ng showed outstanding features. Using a simulated medium, 150 and 200 ng of EEP-loaded chitosan nanoemulsion concentrations can stop zearalenone production in Fusarium media with complete fungi inhibition. Also, it reduced aflatoxins production in Aspergillus media, with fungal inhibition (up to 47.18%). These results recommended the EEP-chitosan application for pharmaceutics and medical use as a comprehensive wound healing agent.

Incompatibility with integrated circuits or complex fabrication requirement in most of the conventionally used filters necessitates the development of a simple, on-chip platform for easy-to-use wavelength detection system. Also, intensity fluctuations hinder precise, noise free detection of spectral information. Here we propose a novel approach of utilizing wavelength sensitive photocurrent across semiconductor heterojunctions to experimentally validate broadband wavelength detection on an on-chip platform with simple fabrication process. The proposed device utilizes linear frequency response of internal photoemission via 2-D electron gas in a ZnO based heterojunction along with a reference junction for coherent common mode rejection. We report sensitivity of 0.96 μA/nm for a broad wavelength-range of 280 nm from 660 to 940 nm. Simple fabrication process, efficient intensity noise cancelation along with heat resistance and radiation hardness of ZnO makes the proposed platform simple, low-cost and efficient alternative for several applications such as optical spectrometers, sensing, and Internet of Things (IOTs).

Abstract: To fulfill the demands of high-speed photonic applications, researchers, and engineers have been working to improve the modulation bandwidth (MBW) of semiconductor lasers. We extend our prior work on modeling a vertical-cavity surface-emitting laser (VCSEL) with multiple transverse-coupled-cavities (MTCCs) to evaluate the feasibility of boosting MBW beyond 100 GHz in this study. Because of the strong coupling of slow-light feedback from nearby lateral transverse coupled cavities (TCCs) into the VCSEL cavity, the laser has a high modulation performance. The intensity modulation response of the VCSEL design using one, two, four, and six TCCs is compared. Due to the optical-feedback (OFB) from short TCCs, which achieves 3 dB MBW reaching 170 GHz, photon–photon-resonance (PPR) is projected to occur at ultra-high frequencies beyond 145 GHz. In terms of the Fourier spectrum of the relative intensity noise (RIN), we characterize the noise features of the MTCC-VCSEL in the ultra-high bandwidth domain.

Like other high-throughput biological experiments, metabolomics produces vast volumes of complex data. The application of machine learning (ML) to analyze data, recognize patterns, and build models is expanding across multiple fields. In the same way, ML methods are utilized for the classification, regression, or clustering of highly complex metabolomic data. This review discusses how disease modeling and diagnosis can be enhanced via deep and comprehensive metabolomic profiling using ML. We discuss the general layout of a metabolic workflow and the fundamental ML techniques used to analyze metabolomic data, including support vector machines (SVM), decision trees, random forests (RF), neural networks (NN), and deep learning (DL). Finally, we present the advantages and disadvantages of various ML methods and provide suggestions for different metabolic data analysis scenarios.

We aimed to describe the aIMT in infants with congenital heart disease undergoing cardiac surgery over the first year of life and explore its association with cardiopulmonary bypass, growth velocity, and a diagnosis of left heart obstruction. Methods and Results A prospective cohort study measuring mean and maximum aIMT preoperatively, at 3 months, and 1 year of age in neonates with congenital heart disease undergoing cardiac surgery. Twenty‐four infants with a median gestation of 39 weeks and a median birth weight of 3184 g were included. Sixteen (67%) infants had left outflow tract obstruction. Gestation correlated inversely with baseline mean aIMT (β=−0.027, P=0.018) and positively with the percentage of increase in mean and maximum aIMT between baseline and 3 months (β=17%, P=0.027 and β=15%, P=0.023). The presence of left outflow obstruction was significantly associated with increasing mean and maximum aIMT between baseline and 1 year (mean aIMT change: β=34%, P=0.017 and maximum aIMT change β=43%, P=0.001). Abstract: Mosquito-borne infections are a global health threat. Different species of mosquitoes transmit viruses and cause several human diseases. In this study, in silico molecular docking of 23 phytochemicals of Phoenix dactylifera was performed to look for potential hits that bind effectively at the active site of different protein targets of the dengue virus (2FOM, 3U1I, and 2BMF) and Aedes aegypti mosquito (1YIY, 1PZ4 and 3UQI). The docking results of coumestrol to 2FOM resulted in four hydrogen bonds and ten hydrophobic interactions with binding energy of −9.5 kcal/mol. Similarly, the docking simulation of 2FOM to pinoresinol formed seven covalent bonds resulting in − 9.5 kcal/ mol energy. There were also two hydrophobic (THR289 and THR450) and one π-cation (LYS515) interactions with amino acid residues. Similarly, isofucosterol exhibited the best binding conformations with the lowest binding energy values with the two target proteins 1YIY and 1PZ4 of Ae.egyptia. The docking simulation of 1YIY to isofucosterol resulted in one hydrogen bond with binding energies of – 10.3 kcal/mol and 16 hydrophobic interactions with different amino acid residues. A similar observation of target protein 1PZ4 was noted in isofucosterol resulting in −9.7 kcal/mol energy. From the docking studies reported in this paper, promising candidates can be further optimized and studied in vitro.

Abstract: Climate change might have direct impacts on water quantity in Egypt and lead to indirect effects on Mediterranean saltwater intrusion to groundwater, which exposes agriculture to vulnerability. This study investigated impacts of climate change on agriculture, with particular regard to food security and socioeconomy, and quantified the effectiveness of cropping pattern adaptation measures by integrating three mathematical models. The BlueM model was used for hydrological simulations of Nasser Lake under flooding scenarios to predict the water supply from the High Aswan Dam. The water and salinity balance (WB-SAL) model was adopted to estimate the water salinity in the Nile Delta. The simulated results from the BlueM and WB-SAL models were integrated with the agricultural simulation model for Egypt (ASME) to project cropping patterns, food security, and socioeconomy throughout the country. The results showed that future climate change will directly affect the total crop area; crop areas for 13 crop types; the self-sufficiency of wheat, rice, cereal, and maize supplies; and socioeconomic indicators. The proposed cropping pattern adaptation measures focus on fixing the crop areas of rice and orchards and providing half of the population with lentils, maize, onion, vegetables, milk, and meat. The adaptation measures have the potential to promote food security without causing deterioration of the socioeconomic situation. However, water availability has much more significant effects on food security and socioeconomy than cropping pattern adaptation measures do. Accordingly, the country should rationalize water use efficiency and increase water supply.

Abstract: Abstract Background Ophthalmic regional anesthesia is now the most common anesthetic technique used for eye surgeries. There are various modes of needle-based ophthalmic anesthesia which are retrobulbar, peribulbar, and episcleral. The sub-Tenon episcleral anesthesia technique became a potential alternative to the retrobulbar and peribulbar anesthesia in most of the anterior and posterior segment eye surgeries; this is due to its better safety profile and tolerability than the other blocks. The aim of this study was to compare between medial episcleral block and peribulbar block in intracapsular cataract surgery as regards anesthesia and akinesia of the eye, the need of supplementation of local anesthetic, and finally the safety profile of each block. This was a prospective, comparative, randomized, double-blinded clinical study. It was carried out on 60 patients that were scheduled for intracapsular cataract surgery in ophthalmic surgery unit. The patients were randomly allocated into two equal groups; group A received medial canthus episcleral block technique and group B received peribulbar block technique. Results Results of this prospective, comparative, randomized, double-blinded study showed no statistical difference between the two groups as regards demographic and vital data. As regards Akinesia score, the ESA group had better akinesia score at 1, 5, and 10 min and at the end of surgery than PBA group (P value, 0.001). No patient in the ESA group received supplemental injection via inferotemporal peribulbar block technique, while 66.7% of PBA group was in need of supplementation. Regarding time to onset of acceptable akinesia score; ESA group had a faster onset with high statistical significance (P value, 0.001). Numeric pain scale was better in ESA group than PBA group with high statistical significance. There were chemosis after injection in two of the ESA group (6.6%). On the other hand, slight pricking pain at the end pf surgery developed in two cases in the PBA group. Conclusion Medial canthal episcleral technique proved to be superior in motor akinesia score, time to onset of acceptable akinesia score, and numeric pain scale in comparison to peribulbar anesthesia with high statistical significance between the two groups. Both techniques proved to be safe with no incidence of major complications.

Abstract: Summary: Background: Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide. Methods: A multimethods analysis was performed as part of the GlobalSurg 3 study—a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital. Findings: Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. Abstract: PurposeObesity is associated with fat accumulation in ectopic sites such as the pancreas, the so-called pancreatic steatosis (PS). Bariatric surgery has been shown to be associated with reducing pancreatic fat. This study investigated the effect of laparoscopic sleeve gastrectomy (LSG) on pancreatic volume and its fat content and glucose homeostasis.MethodsThe study enrolled 54 patients subjected to LSG. Metabolic variables and pancreatic exocrine function were assessed immediately before surgery and 12 months after. MRI of the abdomen was performed to measure pancreatic fat content and its total volume and visceral adipose tissue (VAT).ResultsSurgery resulted in a significant reduction in body weight and BMI. HbA1c, fasting insulin, C-peptide levels, HOMA-IR, and Hs-CRP levels decreased significantly. Surgery resulted in significant improvement in lipid profile except for HDL-cholesterol and liver function tests. Total VAT volume decreased significantly. Total pancreas volume decreased by a mean of 9.0 cm3 (95% CI: 6.6–11.3). The median change of pancreatic fat was −26.1% (range: −55.6 to 58.3%). Pancreatic lipase decreased significantly (P < 0.001). There was a positive correlation between the percentage of total weight loss and decrease in pancreatic fat volume (r = 0.295, P = 0.030).ConclusionWeight loss after LSG is associated with a reduction of total VAT volume, total pancreatic volume, and pancreatic fat content. These changes are associated with improved glucose homeostasis, reduced systemic inflammation, and decreased pancreatic lipase secretion.

Abstract: Heat dissipation is one of the major problems in the construction of electronic devices. Reversible computing is one of the emerging computing paradigms to overcome heat dissipation problem. One of the nanoscale devices that has low power consumption and can be used to construct digital logic components is quantum-dot cellular automata (QCA). In this paper, the benefits of reversible computing of QCA will be used to propose QCA designs for two main reversible gates, the CNOT and the TOFFOLI gates. The proposed CNOT gate has been used to design QCA for double CNOT gate to show its reversibility, and QCA design for the SWAP circuit that contains 3 CNOT gates. The suggested TOFFOLI gate is utilized to construct QCA for double TOFFOLI gate and a reversible circuit which consists of several reversible gates. The suggested QCA designs for double CNOT, double TOFFOLI and reversible circuits such as SWAP circuit have been proposed by adding cells with certain clocks to the wires that connects the proposed gates. The proposed QCA designs show better cell count, area used in the construction, number of majority gate, wire crossover, and/or clock cycle delay when compared with relevant designs in literature.

40. Hexagonal transverse-coupled-cavity VCSEL redefining the high-speed lasers

Abstract: Vertical-cavity surface-emitting lasers (VCSELs) have emerged as a vital approach for realizing energy-efficient and high-speed optical interconnects in the data centers and supercomputers. Indeed, VCSELs are the most suitable mass production lasers in terms of cost-effectiveness and reliability. However, there are still key challenges that prevent achieving modulation speeds beyond 30s GHz. Here, we propose a novel VCSEL design of a hexagonal transverse-coupled-cavity adiabatically coupled through a central cavity. Following this scheme, we show a prototype demonstrating a 3-dB roll-off modulation bandwidth of 45 GHz, which is five times greater than a conventional VCSEL fabricated on the same epiwafer structure. This design harnesses the Vernier effect to increase the laser’s aperture and therefore is capable of maintaining single-mode operation of the laser for high injection currents, hence extending the dynamic roll-off point and offering increases power output. Simultaneously, extending both the laser modulation speed and output power for this heavily deployed class of lasers opens up new opportunities and fields of use ranging from data-comm to sensing, automotive, and photonic artificial intelligence systems.

Abstract: The technologically-relevant task of feature extraction from data performed in deep-learning systems is routinely accomplished as repeated fast Fourier transforms (FFT) electronically in prevalent domain-specific architectures such as in graphics processing units (GPU). However, electronics systems are limited with respect to power dissipation and delay, due to wire-charging challenges related to interconnect capacitance. Here we present a silicon photonics-based architecture for convolutional neural networks that harnesses the phase property of light to perform FFTs efficiently by executing the convolution as a multiplication in the Fourier-domain. The algorithmic executing time is determined by the time-of-flight of the signal through this photonic reconfigurable passive FFT ‘filter’ circuit and is on the order of 10’s of picosecond short. A sensitivity analysis shows that this optical processor must be thermally phase stabilized corresponding to a few degrees. Furthermore, we find that for a small sample number, the obtainable number of convolutions per {time, power, and chip area) outperforms GPUs by about two orders of magnitude. Lastly, we show that, conceptually, the optical FFT and convolution-processing performance is indeed directly linked to optoelectronic device-level, and improvements in plasmonics, metamaterials or nanophotonics are fueling next generation densely interconnected intelligent photonic circuits with relevance for edge-computing 5G networks by processing tensor operations optically.

Abstract: Cardiac valves exhibit highly complex structures and specialized functions that include dynamic interactions between cells, extracellular matrix (ECM) and their hemodynamic environment. Valvular gene expression is tightly regulated by a variety of mechanisms including epigenetic factors such as histone modifications, RNA-based mechanisms and DNA methylation. To date, methylation fingerprints of non-diseased human aortic and mitral valves have not been studied. In this work we analyzed the differential methylation profiles of 12 non-diseased aortic and mitral valve tissue samples (in matched pairs). Analysis of methylation data [reduced representation bisulfite sequencing (RRBS)] of 16,101 promoters genome-wide revealed 584 differentially methylated (DM) promoters, of which 13 were reported in endothelial mesenchymal trans-differentiation (EMT), 37 in aortic and mitral valve disease and 7 in ECM remodeling. Both functional classification as well as network analysis showed that the genes associated with the DM promoters were enriched for WNT-, Cadherin-, Endothelin-, PDGF-, HIF-1 and VEGF- signaling implicated in valvular physiology and pathophysiology. Additional enrichment was detected for TGFB-, NOTCH- and Integrin- signaling involved in EMT as well as ECM remodeling. This data provides the first insight into differential regulation of human aortic and mitral valve tissue and identifies candidate genes linked to DM promoters. Our work will improve the understanding of valve biology, valve tissue engineering approaches and contributes to the identification of relevant drug targets.

Abstract: Stress response helps microorganisms survive extreme environmental conditions and host immunity, making them more virulent or drug resistant. Although both reductionist approaches investigating specific genes and systems approaches analyzing individual stress conditions are being used, less is known about gene networks involved in multiple stress responses. Here, using a systems biology approach, we mined hundreds of transcriptomic data sets for key genes and pathways involved in the tolerance of the model microorganism Escherichia coli to multiple stressors. Specifically, we investigated the E. coli K-12 MG1655 transcriptome under five stresses: heat, cold, oxidative stress, nitrosative stress, and antibiotic treatment. Overlaps of transcriptional changes between studies of each stress factor and between different stressors were determined: energy-requiring metabolic pathways, transport, and motility are typically downregulated to conserve energy, while genes related to survival, bona fide stress response, biofilm formation, and DNA repair are mainly upregulated. The transcription of 15 genes with uncharacterized functions is higher in response to multiple stressors, which suggests they may play pivotal roles in stress response. In conclusion, using rank normalization of transcriptomic data, we identified a set of E. coli stress response genes and pathways, which could be potential targets to overcome antibiotic tolerance or multidrug resistance.

Abstract: In order to map and differentiate ore-bearing hydrothermal alteration zones at the northwestern part of Allaqi-Heiani, we use a combined method of the Advanced Spaceborne Thermal Emission and Reflection Radiometer (ASTER) image processing, field relationship, mineralogical features, and rock chemical compositions. Hydrothermal alteration zones are identified on the basis of Advanced Spaceborne Thermal Emission and Reflection Radiometer data. Propylitic, phyllic, argillic, and silicification zones are recorded in different types of igneous and metamorphic rocks typically with chromite, goethite, ilmenite, galena and gold. Crosta and CEN techniques using ASTER are successfully applied to highlight the hydrothermal alteration zones and the structural elements represented by lithologic contacts and faults/fracture zones. Combining data of lineaments, alteration zones and ore mineralogy studied after obtaining a consequence of each map allowed predicting and mapping areas of probable high mineral resources. According to petrographic and remote sensing studies, types of alteration include sericitization, chloritization, epidotization, kaolinitization, carbonatization and silicification. The results of remote sensing in the study area are confirmed by X-ray diffraction (XRD), petrography, and SEM. In the studied district, there are ophiolitic ultramafic rocks, island-arc assemblage of metasediments, volcaniclastics and metagabbro-diorite, syntectonic granite and dykes. Accordingly, to the present study can identify the most potential areas of mineralization taking us into consideration the overlapping of high lineament density areas with abundant alterations.

46. Evaluation of local scour around bridge piers for various geometrical shapes using mathematical models

Abstract: Bridges are one of the important structures across the Nile River; these bridges may be subjected to collapse if there is no monitoring or safety precaution for local scour during high floods. This research was commenced in order to evaluate the scour around bridge piers. 1-D and 2-D mathematical models were applied to Aswan and El Minia bridges, in the 1st and 4th reaches, respectively. The two bridges are considered two major bridges so their impacts should be studied. Total Scour (local, contraction and long-term degradation) was evaluated and the expected morphological changes were estimated. The local scour and contraction scour at Aswan Bridge is higher than El-Minia Bridge due to the big dimensions of piers. 2D model gives accurate results especially in the non-uniform cross sections. 47. Dynamic operation rules of multi-purpose reservoir for better flood management

Abstract: This research was commenced to enhance flood management via improving the operation rules of multi-purpose reservoirs (i.e. Aswan High Dam Reservoir “AHDR”). Literature in the field of reservoir operation rules was reviewed. Available data, about Aswan High Dam (AHD) together with its reservoir releases, were assembled and analyzed. Accessible numerical models were investigated to select the applicable model in terms of the assembled available data. Tenth scenarios (the current scenario and 9 proposed scenarios) for AHDR operation rules were designed and simulated. These scenarios include climate change, static operation (fixed level at beginning of water year from the 1st of August) and dynamic operation. Results were obtained and compared to the existing operation rules. The comparison considered the storage, water release, hydropower generation and total losses. In addition, the comparison considered the dam safety. The comparison indicated that the dynamic operation rules would achieve satisfying results; this scenario will provided an increase in the yearly AHDR storage from 3% to 4% during 9 years. The total increase in the storage was 22.46 BCM during 9 years without any reduction in the release downstream AHD and the hydropower generation. Abstract: The aim of this study was to assess the expression of Th17-associated gene, AMPs genes and SIRT-1 protein in patients with inflammatory bowel disease (IBD). Material and Methods: We studied a group of 20 IBD patients, together with 20 subjects served as controls. Colonoscopy, terminal ileoscopy, and colonoscopic biopsy were performed for histopathology diagnosis, and quantitative gene expression of Th17-associated gene, CAMP, Elafin and SLPI by real-time PCR. SIRT-1 protein level expression was assessed by western blot. Results: The expression of the four studied genes—elafin, SLPI, CAMP and Th17-associated gene—by relative quantification was higher in the patient group than in the control group. Abstract: The incidence of pediatric cancer is lower than that of adult cancer worldwide. However, the former has detrimental side effects on the health of individuals, even after the cancer is cured, due to the impact of treatment on development. Recently, correlations have been made between the gut microbiome and cancer in several studies but only on adult participants. There is always a complication of dealing with pediatric cancer treatment protocols because they usually include a combination of chemotherapy, radiotherapy, and intensive prophylactic antibiotics. In the current study, a pilot study was conducted to analyze ten fecal samples from three pediatric cancer patients, suffering from rhabdomyosarcoma near their pelvic region, and two healthy individuals. A correlation between microbial composition and response to treatment was reported, in which the responders had generally a lower microbial diversity compared to non-responders. In addition, nucleotide changes and deletions in the tested 16S rRNA sequences post radiotherapy were detected. Despite the small sample size used in the experiments due to the uncommon rhabdomyosarcoma in children, the results can help in understanding the influence of radiotherapy on the gut microbiome in pediatric cancer patients. More work with larger sample size and different cancer types need to be conducted to understand the influence of radiotherapy on gut microbiome to mitigate the deleterious impact of radiation on treated children.
