Foreign language syndrome (FLS) is a rare phenomenon described as transient fixation on a second language following anesthesia administration. There have only been 12 previously reported cases of FLS and none involving female patients or oral and maxillofacial surgery (OMS). While volatile anesthetics were administered during at least 2 cases, at minimum 5 cases occurred without volatile anesthetics - instead manifesting with IV sedation medications commonly used with OMS outpatient procedures (ie, midazolam, fentanyl, and propofol). In our case, a healthy 21-year-old female underwent extraction of wisdom teeth under general anesthesia and developed FLS. Our purpose is not to elucidate the poorly understood pathophysiology of FLS but raise awareness of its potential following OMS procedures. FLS may be difficult to diagnose but appears to be self-limiting and typically resolves within approximately 24 hours. Background/Objectives: This study aimed to evaluate bone mineral density (BMD) discordance and its implications in veterans with unilateral lower-limb amputation, emphasizing the need for comprehensive hip assessments. Methods: Data were collected from 84 male veterans, and BMD was measured using dual-energy X-ray absorptiometry (DXA) at the lumbar spine, intact hip, and amputated hip. Results: The T-scores for the lumbar spine, intact hip, and amputated hip were -0.27 ± 1.69, -0.25 ± 1.20, and -1.07 ± 1.33, respectively. Osteoporosis and osteopenia were present in 19% and 34.6% of patients, respectively. Osteopenia and osteoporosis were most prevalent in the hips on the amputated side (32.1% and 13.1%, respectively), followed by the lumbar spines (22.6% and 8.3%) and the hips on the intact side (17.9% and 2.4%). BMD discordance between the lumbar spine and hip was found in 47.6% of participants, while discordance between both hips was observed in 39.3%. Transfemoral amputees had significantly lower BMD at the amputated hip compared to transtibial amputees (-2.38 ± 1.72 vs. -0.87 ± 1.16, p < 0.001). Conclusions: Veterans with unilateral lower-limb amputation exhibit a high prevalence of osteoporosis and significant BMD discordance, particularly between both hips. These findings underscore the necessity for bilateral hip assessments to ensure the accurate diagnosis and effective management of osteoporosis in this population.

Background: There are not many studies on the differences of ultrasound (US) findings between epidermal cysts (ECs) located in the trunk and those in the extremities., Purpose: To compare the sonographic findings of ECs according to location in the body (trunk vs. extremity) and evaluated the feasibility of strain elastography (SE)., Material and Methods: This is a retrospective study of 76 patients with surgically confirmed non-ruptured EC who underwent US including SE. The US analyses included size, shape, ratio of depth to length (DLR), involvement of more than half the dermis, "submarine sign," and SE characters of each lesion. SE findings were assigned into four grades based on elasticity., Results: The submarine sign was more significantly observed in ECs located in the trunk versus extremities ( P value = 0.004 and 0.035, respectively). Truncal lesions were significantly more likely to possess an ovoid shape ( P  < 0.05) and exhibited higher DLR ( P  < 0.05). There were more cases with low elasticity according to SE (grade 3 or 4) compared to high elasticity (grade 1 or 2). However, we did not observe significant differences between the two locations ( P  > 0.05). More-than-half signs also did not exhibit a significant difference ( P  > 0.05)., Conclusion: The submarine sign, ovoid shape, and tall lesions (higher DLR) are common in the trunk. However, the degree of elasticity and number of more-than-half signs did not differ between the two groups.

Under the leadership of the C40 Cities Climate Leadership Group (C40), approximately 1100 global cities have signed to reach net-zero emissions by 2050. Accurate greenhouse gas emission calculations at the city-scale have become critical. This study forms a bridge between the two emission calculation methods: (a) the city-scale accounting used by C40 cities-the Global Protocol for Community-Scale Greenhouse Gas Emission Inventories (GPC) and (b) the global-scale gridded datasets used by the research community-the Emission Database for Global Atmospheric Research (EDGAR) and Open-Source Data Inventory for Anthropogenic CO 2 (ODIAC). For the emission magnitudes of 78 C40 cities, we find good correlations between the GPC and EDGAR ( R 2 = 0.80) and the GPC and ODIAC ( R 2 = 0.72). Regionally, African cities show the largest variability in the three emission estimates. For the emission trends, the standard deviation of the differences is ±4.7% yr -1 for EDGAR vs. GPC and is ±3.9% yr -1 for ODIAC vs. GPC: a factor of ∼2 larger than the trends that many C40 cities pledged (net-zero by 2050 from 2010, or -2.5% yr -1 ). To examine the source of discrepancies in the emission datasets, we assess the impact of spatial resolutions of EDGAR (0.1°) and ODIAC (1 km) on estimating varying-sized cities' emissions. Our analysis shows that the coarser resolution of EDGAR can artificially decrease emissions by 13% for cities smaller than 1000 km 2 . We find that data quality of emission factors (EFs) used in GPC inventories vary regionally: the highest quality for European and North American and the lowest for African and Latin American cities. Our study indicates that the following items should be prioritized to reduce the discrepancies between the two emission calculation methods: (a) implementing local-specific/up-to-date EFs in GPC inventories, (b) keeping the global power plant database current, and (c) incorporating satellite-derived CO 2 datasets (i.e. NASA OCO-3)., (© 2023 The Author(s). Objective: To evaluate whether measurement of protruding anterior meniscus in the medial compartment (PAMM) and posterior tibial translation (PTT) can be used as a secondary sign of posterior cruciate ligament (PCL) tear for differential diagnosis of partial or complete tear., Methods: 21 cases with complete PCL tear, 35 cases with partial PCL tear, and 55 normal cases were reviewed in this retrospective study. PTT and PAMM were measured from the MR images associated with each case. Non-parametric data were evaluated using the Kruskal-Wallis test and the Mann-Whitney U -test with Bonferroni correction to compare differences among the three groups: complete tear, partial tear, and controls., Results: There were significant differences in the median values of PAMM among the three groups, and with PAMM increasing as the grade of PCL injury increased. However, there were no significant differences in median PTT among the three groups. Median PAMM in the partial and complete tear groups was significantly higher than in subjects without PCL rupture (adjusted p -value <0.05). Although median PAMM in subjects in the complete tear group tended to be higher than in those with a partial tear, this difference was not statistically significant (adjusted p -values ≥0.418)., Conclusion: PAMM is more useful finding than PTT and can predict PCL tear with or without posterior instability. We analyze airborne measurements of atmospheric CO concentration from 70 flights conducted over six years (2015-2020) using an inverse model to quantify the CO emissions from the Washington, DC, and Baltimore metropolitan areas. We found that CO emissions have been declining in the area at a rate of ≈-4.5 % a -1 since 2015 or ≈-3.1 % a -1 since 2016. In addition, we found that CO emissions show a "Sunday" effect, with emissions being lower, on average, than for the rest of the week and that the seasonal cycle is no larger than 16 %. Our results also show that the trend derived from the NEI agrees well with the observed trend, but that NEI daytime-adjusted emissions are ≈50 % larger than our estimated emissions. In 2020, measurements collected during the shutdown in activity related to the COVID-19 pandemic indicate a significant drop in CO emissions of 16 % relative to the expected emissions trend from the previous years, or 23 % relative to the mean of 2016 to February 2020. Our results also indicate a larger reduction in April than in May. Last, we show that this reduction in CO emissions was driven mainly by a reduction in traffic.

Purpose: To evaluate the correlation between Lee's grades and surgical intervention for central lumbar spinal stenosis (CLSS) and to assess whether this grading system can be used as a decision-making tool for the surgical treatment of this condition., Materials and Methods: This retrospective study included 290 patients (M:F = 156:134; mean age, 46 ± 16 years). Radiologists assessed the presence and grade of CLSS at the stenosis point according to Lee's grading system, in which CLSS is classified into four grades according to the shape of the cauda equina. Correlation coefficients ( r s ) between Lee's grades and the operation were calculated with Spearman rank correlation., Results: Among the operated patients, grade 2 was the most commonly assigned grade (50%-58%), grade 3 was less common (35%), and grade 0 was the least common (2%-3%). Among the non-operated patients, grade 1 was the most common (63%-65%), grade 0 was less common (15%-16%), and grade 3 was the least common (8%). The distribution of grades differed between the operated and non-operated groups ( p < 0.001). Less than 25% of patients who underwent surgery were assigned grades 0 and 1, and more than 88% were assigned grades 2 and 3. A moderate correlation was found between the grade and surgical intervention ( r s = 0.632 and r s = 0.583)., Conclusion: Lee's grade was moderately correlated with surgical intervention. Background: To control the surging prices of KF94 masks amid the Covid-19 pandemic in South Korea, the government mandated 80% of medical masks to be sold as "public masks" at a fixed price. The mask prices came down quickly, ensuring public health and suppressing the spread of the pandemic, and the sales of KF94 masks reverted to the free market system on July 12, 2020. This paper aims to evaluate the unintended consequence of public masks to mitigate any negative effects for future deployment., Methods: The relationship between the offline and online prices of KF94 masks and the production quantity of medical masks was estimated using the data after July 12, 2020 (the "free market" period), and, given the regression results, counterfactual analyses were conducted to predict what the offline and online prices of KF94 masks would have been before July 12, 2020 (the "public masks" period) had the sales of medical masks reverted to the free market system a month earlier than July 12, 2020., Results: The prolonged deployment of public masks distorted the ratio of offline and online prices of KF94 masks and kept the prices artificially high at its later stage. How much higher? The online price of privately sold KF94 masks would have been roughly thirty to forty percent lower and the offline price roughly three to four percent lower if public masks ended four weeks earlier., Conclusion: The government intervention achieved its intended consequence of controlling the surging prices of KF94 masks at the early days of Covid-19, but the prolonged use of public masks had the unintended consequence of distorting market outcomes. The lesson? Objective: To compare the convenience and effectiveness of the existing lumbosacral orthoses (LSO) (classic LSO and Cybertech) and a newly developed LSO (V-LSO) by analyzing postoperative data., Methods: This prospective cohort study was performed from May 2019 to November 2019 and enrolled and analyzed 88 patients with degenerative lumbar spine disease scheduled for elective lumbar surgery. Three types of LSO that were provided according to the time of patient registration were applied for 6 weeks. Patients were randomized into the classic LSO group (n=31), Cybertech group (n=26), and V-LSO group (n=31). All patients were assessed using the Oswestry Disability Index (ODI) preoperatively and underwent plain lumbar radiography (anteroposterior and lateral views) 10 days postoperatively. Lumbar lordosis (LS angle) and frontal imbalance were measured with and without LSO. At the sixth postoperative week, a follow-up assessment with the ODI and orthosis questionnaire was conducted., Results: No significant differences were found among the three groups in terms of the LS angle, frontal imbalance, ODI, and orthosis questionnaire results. When the change in the LS angle and frontal imbalance toward the reference value was defined as a positive change with and without LSO, the rate of positive change was significantly different in the V-LSO group (LS angle: 41.94% vs. 61.54% vs. 83.87%; p=0.003)., Conclusion: The newly developed LSO showed no difference regarding its effectiveness and compliance when compared with the existing LSO, but it was more effective in correcting lumbar lordosis.

Purpose: The purpose of the present study was to correlate specific arthroscopic findings in temporomandibular joint (TMJ) arthroscopy and histologic results from arthroscopic-guided synovial biopsy., Patients and Methods: A retrospective cohort study was completed of patients who had undergone arthroscopy and arthroscopic synovial biopsy from January 1, 2011 to March 31, 2016 at the senior author's practice in Miami, Florida. The demographic, arthroscopic, histologic, serologic, and historical variables were recorded by electronic medical record review. Two-tailed Fisher's exact tests were used to test the correlation of the arthroscopic and histologic variables., Results: A total of 210 patients were included in the present study. The mean age was 43.1 ± 18.8 years, and the study population included 91.7% women. The following histologic findings had significant associations: lymphoplasmacytic infiltrate with petechiae synovitis (P = .0018), chronic inflammation with joint stenosis (P = .019), and crystal deposition with visualization of crystals (P = .0472). Hyperplastic synovium found on histologic examination was associated with both petechiae synovitis and adhesions. Histologic vascular proliferation was associated with synovial plica and joint stenosis., Conclusions: The findings from the present study indicated an association between specific findings during arthroscopy and histologic findings. Temporomandibular joint (TMJ) reconstruction is traditionally invasive. Several investigators have developed animal models, including mouse models, to study the TMJ. However, there are no detailed descriptions of a mouse model to be followed for additional research. The goal of this project was to study minimally invasive TMJ regeneration using tissue engineering in mice. As part of the project, a detailed mouse model was developed, which is described in this article. Eight carcasses were used to study the anatomy of the TMJ of the mouse and 36 mice were used to describe the surgical approach and perioperative management. The study showed similarities and differences when compared to humans. One mouse died suddenly 10 days postoperatively, while 35 mice survived the operation. Keratitis and wound dehiscence were the most common complications. To study emissions of CO 2 in the Baltimore, MD-Washington, D.C. (Balt-Wash) area, an aircraft campaign was conducted in February 2015, as part of the FLAGG-MD (Fluxes of Atmospheric Greenhouse-Gases in Maryland) project. During the campaign, elevated mole fractions of CO 2 were observed downwind of the urban center and local power plants. Upwind flight data and HYSPLIT (Hybrid Single Particle Lagrangian Integrated Trajectory) model analyses help account for the impact of emissions outside the Balt-Wash area. The accuracy, precision, and sensitivity of CO 2 emissions estimates based on the mass balance approach were assessed for both power plants and cities. Our estimates of CO 2 emissions from two local power plants agree well with their CEMS (Continuous Emissions Monitoring Systems) records. For the 16 power plant plumes captured by the aircraft, the mean percentage difference of CO 2 emissions was -0.3 %. For the Balt-Wash area as a whole, the 1 σ CO 2 emission rate uncertainty for any individual aircraft-based mass balance approach experiment was ±38 %. Treating the mass balance experiments, which were repeated seven times within nine days, as individual quantifications of the Balt-Wash CO 2 emissions, the estimation uncertainty was ±16 % (standard error of the mean at 95% CL). Our aircraft-based estimate was compared to various bottom-up fossil fuel CO 2 (FFCO 2 ) emission inventories. Based on the FLAGG-MD aircraft observations, we estimate 1.9±0.3 MtC of FFCO 2 from the Balt-Wash area during the month of February 2015. The mean estimate of FFCO 2 from the four bottom-up models was 2.2±0.3 MtC.

Advances in technology and specialized instrumentation allow surgeons to study, refine, and modify minimally invasive surgery (MIS) to replace standard operations. This has occurred across many surgical specialties. The benefits of MIS include less swelling, less pain, shorter hospital stay, and faster return to daily activities. Oral and maxillofacial endoscopic techniques are used for access to the ramus condyle unit, maxillary sinus, zygoma, orbit, temporomandibular joint, and salivary ductal system. Objective: To evaluate if there is a difference in gait pattern when applying two different shapes of energy storing prosthetic feet for trainstibial amputation we conducted a comparative study. Energy storing prosthetic feet for transtibial amputation are increasing in use, but there are few studies that evaluate the effects of the shape of energy storing feet on gait patterns., Methods: Ten unilateral transtibial amputees were recruited. Two different shapes of dynamic response feet were applied to each subject either 1C30 Trias or 1C60 Triton. The main differences between the two are a split forefoot and the presence of a heel wedge. Spatiotemporal, kinematic, and kinetic data was obtained through gait analysis. Differences between intact and prosthetic side and differences between the two prosthetics were assessed., Results: On a side to side comparison, cadence asymmetry with 1C30 Trias was observed. Ankle plantarflexion at the end of stance and ankle supination at the onset of preswing was smaller with both prosthetic feet compared to the intact side. Other spatiotemporal, kinematic, and kinetic data showed no significant differences in a side to side comparison. In a comparison between the two prosthetics, stance and swing ratio and ankle dorsiflexion through mid-stance was closer to normal with 1C60 Triton than 1C30 Trias. Other spatiotemporal, kinematic, and kinetic data showed no statistically significant differences between prosthetics., Conclusion: Both energy storing feet implants showed symmetric gait in unilateral transtibial amputees who are functionally independent in daily living. And 1C60 Triton showed closer to normal gait patterns than 1C30 Trias in our study.

Objective: To analyze the effect of lumbar strengthening exercise in lower-limb amputees with chronic low back pain., Methods: We included in this prospective study 19 lower-limb amputees who had experienced low back pain for longer than 6 months. Participants were treated with 30-minute lumbar strengthening exercises, twice weekly, for 8 weeks. We used the visual analog scale (VAS), and Oswestry low back pain disability questionnaire, and measured parameters such as iliopsoas length, abdominal muscle strength, back extensor strength, and back extensor endurance. In addition, we assessed the isometric peak torque and total work of the trunk flexors and extensors using isokinetic dynamometer. The pre- and post-exercise measurements were compared., Results: Compared with the baseline, abdominal muscle strength (from 4.4±0.7 to 4.8±0.6), back extensor strength (from 2.6±0.6 to 3.5±1.2), and back extensor endurance (from 22.3±10.7 to 46.8±35.1) improved significantly after 8 weeks. The VAS decreased significantly from 4.6±2.2 to 2.6±1.6 after treatment. Furthermore, the peak torque and total work of the trunk flexors and extensors increased significantly (p<0.05)., Conclusion: Lumbar strengthening exercise in lower-limb amputees with chronic low back pain resulted in decreased pain and increased lumbar extensor strength. This work reports on designing of first successful MOF-sensitizer based solid-state photovoltaic device, perticularly with a meaningful output power conversion efficiency. In this study, an intrinsically conductive cobalt-based MOFs (Co-DAPV) formed by the coordination between Co (II) ions and a redox active di(3-diaminopropyl)-viologen (i.e., DAPV) ligand is investigated as sensitizer. Hall-effect measurement shows p-type conductivity of the Co-DAPV film with hole mobility of 0.017 cm 2 V -1 s -1 , suggesting its potential application as hole transporting sensitizer. Further, the energy levels of the highest occupied molecular orbital (HOMO) and the lowest unoccupied molecular orbital (LUMO) of Co-DAPV are well-matched to be suitably employed for sensitizing TiO 2 . Thus, by layer-by-layer deposition of hole conducting MOF-sensitizer onto mesoporous TiO 2 film, a power conversion efficiency of as high as 2.1% is achieved, which exceeds the highest efficiency values of MOF-sensitized liquid-junction solar cells reported so far.

We investigated the major trends in health aid financing in the Democratic People's Republic of Korea (DPRK) by identifying the primary donor organizations and examining several data sources to track overall health aid trends. We collected gross disbursements from bilateral donor countries and international organizations toward the DPRK according to specific health sectors by using the Organization for Economic Cooperation and Development creditor reporting system database and the United Nations Office for the Coordination of Humanitarian Affairs financial tracking service database. We analyzed sources of health aid to the DPRK from the Republic of Korea (ROK) using the official records from the ROK's Ministry of Unification. We identified the ROK, United Nations Children's Fund (UNICEF), World Health Organization (WHO), United Nations Population Fund (UNFPA), and The Global Fund to Fight AIDS, Tuberculosis and Malaria (GFATM) as the major donor entities not only according to their level of health aid expenditures but also their growing roles within the health sector of the DPRK. We found that health aid from the ROK is comprised of funding from the Inter-Korean Cooperation Fund, private organizations, local governments, and South Korean branches of international organizations such as WHO and UNICEF. We also distinguished medical equipment aid from developmental aid to show that the majority of health aid from the ROK was developmental aid. This study highlights the valuable role of the ROK in the flow of health aid to the DPRK, especially in light of the DPRK's precarious international status. Although global health aid from many international organizations has decreased, organizations such as GFATM and UNFPA continue to maintain their focus on reproductive health and infectious diseases.

Initial nitridation of the Si(100) surface is investigated using photoemission, ion-scattering and ab initio calculations. After dissociation of NO or NH3, nitrogen atoms are found to spontaneously form a stable, highly coordinated N[triple bond]Si(3) species even at room temperature. The majority of the N species is incorporated into the subsurface Si layers occupying an interstitial site, whose atomic structure and unique bonding mechanism is clarified through ab initio calculations. This unusual adsorption behavior elucidates the atomistic mechanism of initial silicon nitride formation on the surface and has important implication on the N-rich layer formation at the SiO(x)N(y)/Si interface.
