Se realizó un estudio comparativo entre el método QuEChERS convencional para la determinación de residuos de plaguicidas en polen y un método modificado, con el propósito de seleccionar el método que funciona adecuadamente. Para ello, se analizaron 58 plaguicidas mediante el uso de cromatografía líquida acoplada a espectrometría de masas; la comparación de los métodos se realizó utilizando diferentes parámetros de validación como: recuperación, precisión (como repetibilidad) y límites de detección, los cuales fueron estimados mediante diferentes aproximaciones. Sumado a esto, se realizó un diseño experimental factorial que permitió evaluar la eficiencia de ambos métodos en diferentes tipos de polen. Los resultados indicaron, que el método desarrollado es apto para el análisis de residuos de plaguicidas en polen. Asimismo, se encontró que el método desarrollado presenta mejores límites de detección, mejor exactitud y mejor selectividad que el método QuEChERS convencional para el análisis de los plaguicidas en estudio en la matriz de polen apícola. Finalmente, se encontró que el método modificado no es apto para el análisis de tres de los 58 plaguicidas estudiados.

Con el fin de estudiar la variación en las señales cromatográficas inducidas por las matrices de análisis (efecto matriz), se estudian tres matrices bióticas de diferente naturaleza (Lactuca sativa L., Physalis peruviana L., y Solanum tuberosum) y un grupo de plaguicidas de diferentes características fisicoquímicas, organoclorados, organofosforados y piretroides, entre otros. Los análisis se realizan por medio de un cromatógrafo de gases con detección simultánea por microcaptura electrónica (μECD) y nitrógeno-fósforo (NPD), acoplados en paralelo. En este estudio se evalúa el efecto matriz por medio de análisis de covarianza y regresión lineal, y se encuentra que cerca del 75% de los plaguicidas estudiados presentan variaciones en la respuesta cromatográfica con respecto a las señales en solvente. Por otro lado se encuentra que la naturaleza de la matriz afecta la respuesta cromatográfica de 18 de los 33 plaguicidas, mientras que la cantidad de matriz inyectada influye en la respuesta de sólo 13 de ellos.In order to study the matrix-induced chromatographic response enhancement (matrix effect), we select three different types of biotic matrices (Lactuca sativa L., Physalis peruviana L. and Solanum tuberosum) and a group of pesticides of different physicochemical characteristics including organochlorine, organophosphorus and pyrethroids pesticides. Este trabajo se propone seguir un hilo que va del Atlas de Claudio Gay (1854) a la pintura La Fundación de Santiago (1889), analizando cómo se instala la cordillera nevada como telón de fondo de la nación. Un paisaje nacional que se construye en el siglo XIX, desde la élite republicana, desde Santiago, y que se reproduce hasta hoy. En un segundo plano, usando como referencia los estudios sobre paisaje y poder y paisaje como lugar de vida, el artículo busca indagar en los márgenes de estas representaciones: ¿Existen otras cordilleras (ocultas) del siglo XIX? ¿Cómo se explica el arraigo popular de este imaginario? Relatos sobre de mineros, arrieros o bandidos pueden ser el otro hilo con que se teje el paisaje de la nación. Ce travail propose de suivre un fil conducteur, de l’Atlas de Gay (1854) à la peinture La Fondation de Santiago (1889), en analysant comment la cordillère enneigée s’installe comme décor de la nation: un paysage national qui se construit au XIXe siècle, au sein de l’élite républicaine, depuis Santiago, et qui se reproduit encore aujourd’hui. Dans un second temps, en se référant aux études sur le paysage et le pouvoir et au paysage en tant que lieu de vie, l’article cherche à questionner au-delà de ces représentations: Existe-t-il d’autres cordillères (occultes) au XIXe siècle ? Comment explique-t-on l’enracinement populaire de cet imaginaire ? Des récits à propos de mineurs, arrieros (muletiers) ou bandits peuvent-ils être l’autre fil avec lequel se tisse le paysage de la nation.

Se desarrolló un método multirresiduo para el análisis de plaguicidas en leche mediante el empleo de cromatografía líquida acoplada a espectrometría de masas. Se estudiaron diferentes variaciones al método de extracción QuEChERS utilizando siete diferentes fases extractantes, dos sales amortiguadoras, tres concentraciones de ácido fórmico en la fase extractante y cuatro tiempos de ultrasonido. De los 58 plaguicidas analizados solo tiociclam y tiodicarb presentaron porcentajes de recuperación inferiores al 70%. Los mejores porcentajes de recuperación y menores coeficientes de variación se obtuvieron empleando el buffer de citrato y acetonitrilo con ácido fórmico al 0,5% como extractante. Por otro lado, no se observaron diferencias estadísticas (P > 0,05) a los tiempos de ultrasonido evaluados. Finalmente, se encontró que el método aplicado es preciso y exacto para el análisis de residuos de plaguicidas de diferentes grupos químicos como: azoles, organofosforados, bencimidazoles, carbamatos, entre otros.

Liver plays a major role in metabolism and acts as a source of energy for the body by storing glycogen. With the growing interest and investigation in the biological effects in recent years, it is important and necessary to develop accurate and comparable analytical methods for elements in bio-samples. It has, however, been 10 years since the tissue sample (bovine liver) of CCQM-K49 key comparison. The purpose of CCQM-K145 is to ensure the comparable and traceable measurement results for essential and toxic elements such as P, S, Zn, Mn, Ni, Mo, Sr, Cr, Co, Pb, As and Hg in bovine liver among NMIs and other designated measurement bodies worldwide. The comparison was agreed by IAWG as 6th IAWG Benchmarking Exercise with Zn and Ni as exemplary elements at the meeting in Korea in the early October 2016. The results of CCQM-K145 are expected to cover the measurement capability and support CMCs claiming for inorganic elements in the similar biological tissue materials and food samples. 30 NMIs and DIs registered in CCQM-K145. With respect to the methodology, a variety of techniques such as IDMS, ICP-OES, ICP-MS(non-ID), AAS and NAA were adopted by the participants. For Zn, Ni, Sr, Pb and Hg measurements, most participants chose ID-ICP-MS method, which showed the better performance in terms of consistency and reliability of the measurement results. In aspect of the traceability for the measurement results in CCQM-K145, most participants used their own (in house) CRMs or other NMI's CRMs to guarantee trace to SI unit. Most participants used similar matrix CRMs for quality control or method validation. Base on different statistic way to calculate the reference mass fraction values and associated uncertainties for each measurand, removal of the suspected extreme values, and discussion at the IAWG meetings, the median values are proposed as the KCRV for Zn, Ni, Mn, Mo, Cr, Pb and Hg; the arithmetic mean values are proposed as the KCRV for P, S, Sr, Co and As. In general, the performances of the majority of CCQM-K145 participants are very good, illustrating their measurement capabilities for Zn, Ni, P, S, Mn, Mo, Sr, Cr, As, Co, Pb and Hg in a complex biological tissue matrix. Bovine liver contains many kinds of nutrients and microelements, it can be regarded as a typical representative material of biological tissue and food. In CCQM-K145, the analytes involved alkali metals and transition elements, metalloids / semi-metals and non metals with a range of mass fraction from mg/g to μg/kg. CCQM-K145 also tested the ability of NMIs/DIs to determine elements that were easy to be lost and polluted, and interfered significantly. The chemical pretreatment methods of samples used in the comparison is suitable for general food and biological matrix samples. A variety of measurement methods used in the comparison represent the main instrumental technology for elemental analysis. Therefore, for supporting CMC claim, CCQM-K145 is readily applicable to measurement of more elements in a wide range of biological materials (including liquids and solids) and meat products. Finalmente, se encontró que el método modificado no es apto para el análisis de tres de los 58 plaguicidas estudiados. A comparative study between the conventional and the modified QuEChERS method was performed in order to select the methodology that works better for the determination of pesticide residues in pollen. For this study, 58 pesticides were analyzed by liquid chromatography coupled to mass spectrometry. Comparison was made using different validation parameters for each method as recovery, precision (as repeatability), limits of detection, which were estimate using different approaches. In addition, a factorial design was used to evaluate the efficiency of both methods in different types of pollen. The results showed that the modified method is appropriate for the analysis of pesticide residues in pollen. It was also found that this method has better detection limits, better accuracy and better selectivity than the conventional QuEChERS method for pesticide analysis in bee pollen. Finally, it was found that the modified method is not suitable for the analysis of three of the 58 pesticides studied. Com o objetivo de validar o bom funcionamento de uma análise modificada de resíduos de pesticidas no pólen foi realizado um estudo comparativo com o método QuEChERS. Por isso, 58 pesticidas foram analisados usando espectrometria de massa por cromatograf ia líquida; comparação dos métodos foi realizada utilizando diferentes parâmetros de validação, como a recuperação de precisão (como repetibilidade) e limites de detecção que foram estimadas por diferentes abordagens. Somado a isso, um planejamento fatorial permitiu avaliar a eficiência de ambos métodos em diferentes tipos de pólen. Os resultados indicaram que o método desenvolvido é adequado para a análise de resíduos de pesticidas no pólen. Constatou-se também que o método desenvolvido tem melhores limites de detecção, melhor precisão e maior seletividade do que o método QuEChERS para análise de pesticidas no estudo da matriz de pólen de abelha. Finalmente, verificou-se que o método modificado não é adequado para a análise de 3 dos 58 pesticidas, os quais apresentaram recuperações inferiores ao 70%.

Histoplasmosis is an endemic mycosis of the Americas, Africa, and Asia. In Spain, it is the most common imported endemic mycosis appearing in the literature, and its incidence is on the rise. Proper differential diagnosis of the disease must be taken into consideration by otorhinolaryngologists, as the clinical manifestations of histoplasmosis may simulate more prevalent diseases such as cancer or tuberculosis. We present the case of a Spanish patient with focal involvement of the larynx and offer a review of the relevant literature.

El cálculo de los límites de detección (LD) es una práctica que ha generado diversas discusiones en la comunidad científica, debido a las diferentes definiciones y los procedimientos para realizar su estimación. En este estudio se aplicaron, confirmaron y compararon seis de los métodos más empleados en la estimación de LD en el análisis de residuos de plaguicidas. Los métodos evaluados correspondieron a IUPAC (por sus siglas en ingles, International Union of Pure and Applied Chemistry), la EPA (por sus siglas en ingles, Enviroment Protection Agency), t99, raíz media cuadrática del error (RMSE), Hubaux-Vos y propagación de errores. Este estudio se realizó sobre cinco productos vegetales empleando el método QuEChERS en la determinación de 31 plaguicidas, y el análisis de las muestras se realizó mediante cromatografía líquida acoplada a espectrometría de masas. Los resultados mostraron que los LD estimados con los diferentes métodos no presentaron una variación significativa con la matriz de análisis, exceptuando el método de la EPA. El Servicio de Salud Metropolitano Sur Oriente inauguró el año 2003 la Unidad de Hospitalización de Corta Estadía (UHCE). El equipo profesional diseña una ficha precodificada con el objetivo de hacer un diagnóstico de la población, de un total de 156 pacientes registrados se rescatan 114 fichas (73,07%). El análisis de la información hace posible determinar datos biosociodemográficos, identificar antecedentes psiquiátricos de la población usuaria, conocer datos estadísticos de la UHCE. Resultados: La población es joven, (82% entre 14 y 50 años); mayormente femenina (81,6%); con educación (25,5% básica y el 74,6 tiene media o superior); diversa situación familiar (36,8% solteros; 17,5% separados; 43% casados y convivientes). La mayor demanda de atención la proporcionan las comunas de Puente Alto y la Florida. Un 65,5% viene derivado por intento suicida. Dentro de los métodos registrados destacan: fármacos (80%); ahorcamiento (5,6%); arma blanca (4%); asfixia (4,4%). Considerando que la tasa de suicidio en Santiago especialmente para varones aumentó a 15 x cien mil habitantes y que el método más utilizado es el ahorcamiento, nosotros concluimos que estamos atendiendo a los intentadores de suicidio, que usan los métodos menos efectivos o menos violentos. Se hace necesario redefinir políticas para la detección precoz de estos casos. La ficha precodificada ha permitido un diagnóstico de la población atendida y rediseñar un nuevo instrumento The Southeastern Metropolitan Health Service launched in 2003 the Short Residential Hospitalisation Unit (UHCE). The professional team, developed a pre-codified form that aimed at registering the characteristics of the attending population. Of a total number of 156 registered patients, it was possible to analyse 114 forms (73,07%). The information analysis allowed to determine biosociodemographic data; to identify psychiatric records of the people who benefit from these attentions; and to know statistical data from the UHCE. The results: The population was young (82% between 14 _50); mostly female (81,6%); well educated (25,5% only primary school against a 74,6% of secondary or higher education); with a diverse family situation (36,8% single; 17,5% divorced; 43% married and living in partnership). The major attention requirements are located at the boroughs of Puente Alto and La Florida. A 65,5% arrived as a consequence of suicidal attempt. Within the methods used in these attempts it is possible to highlight: drugs (80%); hanging (5,6); white weapons (4%); asphyxia (4,4%). Therefore, and considering that suicidal rates in Santiago has risen, specially among males, where it reaches 15 cases for every 100 thousand people, and that the most frequent method used by suicides is hanging, we conclude that we are treating the suicidal attempts that use the lesser effective and violent methods. Then it is necessary to rethink policies in order to make an early diagnosis of these cases. The pre-codified form has allowed to diagnose population treatments and redesign a new instrument

El analisis de la informacion hace posible determinar datos biosociodemograficos, identificar antecedentes psiquiatricos de la poblacion usuaria, conocer datos estadisticos de la UHCE. Resultados: La poblacion es joven, (82% entre 14 y 50 anos); mayormente femenina (81,6%); con educacion (25,5% basica y el 74,6 tiene media o superior); diversa situacion familiar (36,8% solteros; 17,5% separados; 43% casados y convivientes). La mayor demanda de atencion la proporcionan las comunas de Puente Alto y la Florida. Un 65,5% viene derivado por intento suicida. Dentro de los metodos registrados destacan: farmacos (80%); ahorcamiento (5,6%); arma blanca (4%); asfixia (4,4%). Considerando que la tasa de suicidio en Santiago especialmente para varones aumento a 15 x cien mil habitantes y que el metodo mas utilizado es el ahorcamiento, nosotros concluimos que estamos atendiendo a los intentadores de suicidio, que usan los metodos menos efectivos o menos violentos. Se hace necesario redefinir politicas para la deteccion precoz de estos casos. La ficha precodificada ha permitido un diagnostico de la poblacion atendida y redisenar un nuevo instrumento

18. Anestesia endovenosa en perros mediante el uso de propofol en dosis única, premedicado con acepromazina-atropina y xilazina-atropina

- Abstract
Twenty mongrel dogs, both sexes, of 1 to 6 years were divided into two groups of ten animals each. A dose of 1.5 mg/kg of acepromazine was administered to the first group (A.A.P) of 16.2±1,63 kg body weight. The second group (X.A.P) of 11.9±1.7.1 kg. body weight, received a 3 mg/kg i.m. dose of xylazine. Both groups received atropine 0.1 mg/kg s.c. ten minutes before the administration of propofol 5 mg/kg i.v. The effects of Propofol on latency period, surgical anesthesia duration, recovery period, respiratory rate, heart rate, arterial blood pressure, and body temperature were evaluated. Adverse reactions to propofol were registered. The results of the anesthesiological variable significantly differed between the two groups: induction of anesthesia was 0.45+0.03 min in the first and 0.26+0.03 in the second group. Surgical anesthesia period was 12.3+1.89 min in the first and 25.2+1.78 min in the second group, and recovery period was 4.5+0.63 min and 10.1+0.98 min in group 1 and 2, respectively. The physiological variables in both groups were maintained without significant modification during the surgical anesthesia period; respiratory rate had an initial average of 14.3+2.45 and 13.0+1.54 breaths/min in group 1 and 2 respectively. The heart rate was 175+11.81 beats/min in the first and 148.,4+9.04 beats/min in the second group; the average arterial blood pressure was 102.6+5.69 and 111.8+10.43 mm Hg for the first and second group, respectively. Body temperature in the first group was 38.5+0.17 and 38.7+0.2 ºC for the second group. Adverse reactions were muscle twitching (3 cases) and opisthotonus (1 case) in group 1; transitory apnea (2 cases) in group 2, during the initial anesthesia period. It may be concluded that a single dose of propofol premedicated either with atropine-acepromazine or atropine-xylazine combination induces an adequate surgical anesthesia in dogs, without significant changes on physiological parameters., Se efectuó la valoración de la acción anestésica del propofol en perros, mediante administración endovenosa en dosis única, con dos protocolos de premedicación: atropina - acepromacina, y atropina - xilacina.Se trabajó con 20 perros mestizos de uno a seis años de edad, clínicamente sanos, divididos al azar en dos grupos de 10 cada uno. Al primer grupo, de 16.2±1.63 kg, se administró acepromacina en dosis de 1.5 mg/kg i.m. , al grupo 2, de 11.9±1.71 kg, se le aplicó xilacina en dosis de 3 mg/kg i.m. A ambos grupos se inyectó atropina en dosis de 0.1 mg/kg s.c. Se administraron 10 min. más tarde 5 mg/kg de propofol en la vena femoral, luego se evaluó el efecto del propofol sobre las variables: período de latencia, duración de la anestesia quirúrgica , período de recuperación anestésica, frecuencia respiratoria, frecuencia cardíaca, presión arterial y temperatura corporal. Además se determinaron las reacciones adversas.Los resultados difirieron significativamente entre los dos grupos. La inducción anestésica fue de 0.45+0.03 min. en el primero (A.A.P) y 0.26+0.03 min. en el segundo (X.A.P). La duración de la anestesia quirúrgica fue de 12.3+1.89 min y 25.2+1.78 min y el período de recuperación de 4.5+0.63 min y 10,1+0,98 min en el grupo 1 y 2, respectivamente. Las variables fisiológicas en ambos grupos se mantuvieron sin modificaciones estadísticamente significativas durante el transcurso de la anestesia quirúrgica; la frecuencia respiratoria tuvo un valor inicial promedio de 14.3+2.45 y 13.0+1.54 ciclos/min. en el grupo 1 y 2, respectivamente. La frecuencia cardíaca fue de 175+11.81 lat/min en el grupo 1 y 148.4+9.04 lat/min en el grupo 2. La presión arterial promedio fue de 102.6+5.69 y 111.8+10.43 mm Hg para el grupo 1 y 2 respectivamente y la temperatura corporal en el grupo 1 fue de 38.5+0.17 y en el grupo 2 de 38.7+0.20 ºC. Las reacciones adversas se presentaron al inicio de la anestesia; en el grupo 1 tremores musculares (3 casos) y opistótono (1 caso), en el grupo 2 apnea transitoria (2 casos). Se concluye que propofol premedicado con atropina - xilacina reduce el período de latencia, aumenta el tiempo de anestesia y recuperación sin alterar significativamente las variables fisiológicas.

Pharmacokinetic variables of propofol were studied in 8 foals. Plasma levels of propofol at different time after a single intravenous dose of 2.4 mg/kg bw, were determined by HPLC. An open two compartments model was used to evaluate plasma levels of propofol and values of t1/2a, t1/2b,Vdc, Vdss, Vdb, Cltotaly MRT were obtained. Propofol pharmacokinetic disposition showed a rapid distribution and removal from organic tissues, and the reported pharmacokinetic variables contribute to determine the appropriate dose to be given ., Se estudian las características farmacocinéticas de propofol en 6 potrillos de 10 a 15 días de edad. Se determinaron parámetros farmacocinéticos de propofol, cuantificando sus concentraciones sanguíneas en función del tiempo por HPLC, tras su administración de 2,4 mg/kg por vía endovenosa al grupo de animales. Los datos de concentración se interpretaron por un modelo abierto de 2 compartimentos, obteniéndose, entre otros, los valores de t1/2a, t1/2b,Vdc, Vdss, Vdb, Cltotaly MRT. El análisis de los parámetros farmacocinéticos del propofol, indican que éste posee una rápida y pronta distribución a los tejidos y una rápida eliminación del organismo. Los parámetros farmacocinéticos obtenidos contribuyen a efectuar cálculos para una correcta dosificación.

20. Determinación de la dosis letal media (DL50) de alcaloides del lupino en pollas de reposición blancas y marón

- Abstract
DL50of total alkaloids, as well as of two individual alkaloids (spartein and lupanin) found in bitter lupine seeds were determined for ISAbrown and Shavercross pullets. Three hundred chickens were used for each genetic line: 100 to calculate the dosification of the alkaloids administered, 100 for the spartein dossification and 100 for the lupanin dosification. In order to determine the DL50, the maximal and minimal lethal dose had to be determined first. The three types of products were administered with a catheter directly to the crop. After alkaloids were administrated, birds were kept under observation for several hours. The results showed that DL50for spartein was 655 (509-856) mg/kg for the ISAbrown line and 425 (303-544) mg/kg for the SHAVERcross line. DL50for lupanin was 1.131 (929-1378) mg/kg (ISAbrown line) and 1.271 (1027-1573) mg/kg (SHAVERcross line). DL50for total alkaloids in lupine seeds was 958 (854-1070) mg/kg (ISAbrown line) and 961 (890-1037) mg/kg (SHAVERcross line). As a conclusion lupanin was the alkaloid that presented the highest DL50values followed by the total alkaloids and finally spartein., Se determinó en pollas de reposición, líneas ISA brown y SHAVER cross, la DL50tanto de alcaloides totales como de esparteína y lupanina en forma individual, presentes en las semillas de lupinos amargos. Se emplearon 300 pollos por cada línea genética: 100 para los cálculos de las dosis administrando alcaloides totales, 100 para la dosis de esparteína y 100 para lupanina. En la determinación de la DL50hubo que calcular la DLmáxy la DLmin, realizándose administraciones de los 3 tipos de productos, con sonda directamente a la ingluvia y observándose las aves por un período de horas post administración. Los resultados de estos estudios establecieron una DL50de esparteína de 655 (509-856) mg/kg (línea ISAbrown) y 425 (303-544) mg/kg (línea SHAVERcross). En el caso de la lupanina la DL50fue de 1,131 (929-1378) mg/kg (línea ISAbrown) y 1,271 (1027-1573)mg/kg (línea SHAVERcross). Para los alcaloides totales de la semilla de lupino, se obtuvo una DL50de 958 (854-1070) mg/kg (línea ISAbrown) y 961 (890-1037) mg/kg (línea SHAVERcross). Se concluye que la lupanina fue el alcaloide que presentó los valores de DL50más altos, seguido por los alcaloides totales y finalmente por la esparteína.

Propofol’s pharmacokinetic disposition presented a rapid distribution and removal from organic tissues. It is concluded that propofol is an alternative to be considered in anaesthetic protocols in horses, and the pharmacokinetic variables presented contribute to determine the appropriate dose to be given., Se estudian las características farmacocinéticas de propofol en 6 caballos. Se determinaron parámetros farmacocinéticos de propofol cuantificando sus concentraciones sanguíneas en función del tiempo por HPLC, tras su administración de 2.4 mg/kg por vía endovenosa al grupo de animales. Los datos de concentración se interpretaron por un modelo abierto de 2 compar-timentos, obteniéndose, entre otros, los valores de t1/2α, t1/2β, Vdc, Vdss, Vdβ, Cltotaly MRT. Las variables farmacodinámicas se encuentran acordes a la disposición cinética de este fármaco. El análisis de los parámetros farmacocinéticos del propofol indica que éste posee una rápida y pronta distribución a los tejidos y una rápida eliminación del organismo. El propofol se presenta como una alternativa anestésica factible de ser incorporada dentro de un protocolo anestésico en caballos. Los parámetros farmacocinéticos obtenidos contribuyen para una correcta dosificación ya sea para mantener un estado de anestesia por infusión continua o por inyecciones repetidas.

El Servicio de Salud Metropolitano Sur Oriente inauguró el año 2003 la Unidad de Hospitalización de Corta Estadía (UHCE). El equipo profesional diseña una ficha precodificada con el objetivo de hacer un diagnóstico de la población, de un total de 156 pacientes registrados se rescatan 114 fichas (73,07%). El análisis de la información hace posible determinar datos biosociodemográficos, identificar antecedentes psiquiátricos de la población usuaria, conocer datos estadísticos de la UHCE. This paper presents a detailed geological map at the 1:20,000 scale of the Tocomar basin in the Central Puna (north-western Argentina), which extends over an area of about 80 km2 and displays the spatial distribution of the Quaternary deposits and the structures that cover the Ordovician basement and the Tertiary sedimentary and volcanic units. The new dataset includes litho-facies descriptions, stratigraphic and structural data and new 234U/230Th ages for travertine rocks. The new reconstructed stratigraphic framework, along with the structural analysis, has revealed the complex evolution of a small extensional basin including a period of prolonged volcanic activity with different eruptive centres and styles. The geological map improves the knowledge of the geology of the Tocomar basin and the local interplay between orogen-parallel thrusts and orogen-oblique fault systems. Background: The occurrence of secondary vascular pathology in paragangliomas is extremely rare, particularly in carotid body tumors (CBTs)., Case Description: A 73-year-old female presented with computed tomography angiography revealing a right CBT with a large intratumoral contrast collection. Digital subtraction angiography confirmed a CBT with an internal carotid artery (ICA) intratumoral pseudoaneurysm. Interventional neuroradiology performed coil embolization of the pseudoaneurysm, and a covered stent was placed in the ICA. Surgical intervention was deferred, and the patient was closely monitored with Doppler ultrasound., Conclusion: Diagnostic imaging is crucial in the management of CBT, offering detailed assessment of tumor morphology and their anatomical relationships, particularly in identifying coexisting vascular anomalies. Recognition of vascular pathologies, such as intratumoral pseudoaneurysms, is critical, as failure to do so could lead to life-threatening complications, including severe bleeding during surgical resection. In cases where surgical resection is high risk due to tumor classification or patient comorbidities, palliative endovascular treatment presents a viable alternative. This approach not only reduces tumor vascularity and provides symptomatic relief but also minimizes procedural risks. Dural arteriovenous fistulas (DAVFs) are rare but significant intracranial vascular malformations that are usually idiopathic and can lead to severe complications like venous hypertension and intracranial hemorrhage. We present 2 cases of DAVF occurring after venous sinus stenting (VSS) for presumed idiopathic intracranial hypertension (IIH) in a 51-year-old and a 52-year-old female. In both patients, Cognard type 1 DAVF was detected by Catheter angiography and successfully obliterated with Onyx™ embolization. These cases suggest that VSS may either unmask pre-existing DAVFs by altering intracranial pressure dynamics or induce new fistula formation by angiogenic factors or stent-induced hemodynamic changes. Clinicians should maintain an index of suspicion for DAVF in patients with persistent symptoms post-VSS and catheter angiography might be needed to detect the DAVFs., (© 2024 The Authors. 41. Latin American consensus on the treatment of melasma.

- Abstract
Melasma is a chronic, relapsing hyperpigmentation disorder that primarily affects photoexposed areas, occurring most frequently in adult women with darker skin phototypes. The primary factors contributing to its development include sun exposure, sex hormones (e.g., pregnancy), and genetic predisposition. Melasma is highly prevalent in Latin America, where many countries lie in intertropical zones and exhibit significant ethnic diversity because of centuries of intermixing among Native Americans, Europeans, and Sub-Saharan Africans. Nine Latin American experts formulated a DELPHI-based consensus to develop a valuable approach for treating melasma in this diverse population. After establishing an accurate diagnosis, assessing the impact on quality of life, and determining disease severity, the consensus recommends mitigating known triggers and promoting rigorous photoprotection. Active therapy should be tailored based on individual characteristics (e.g., pregnancy status, previous treatments, skin sensitivity). Treatment options include topical depigmenting agents, systemic therapies, and procedural interventions such as laser therapy, microneedling, and chemical peels. Periodic reassessment of the treatment is essential, with strategies adjusted if targeted outcomes are not achieved. Once clinical remission is attained, patients should continue using topical depigmenting agents and maintain strict photoprotection measures to prevent recurrence., (© 2024 the International Society of Dermatology.)

This case report aims to describe the clinical presentation, imaging findings, diagnostic challenges, and management of a patient with a cerebellopontine angle lesion. A 63-year-old woman presented with progressive headaches, tinnitus, right ear pressure, and dizziness. Initial imaging studies (computed tomography and magnetic resonance imaging) suggested either a thrombosed aneurysm or a lipoma. However, advanced imaging with cone beam computed tomography provided a definitive diagnosis of temporal bone exostosis. This case highlights the importance of cone beam computed tomography in diagnosing complex intracranial lesions due to its superior spatial resolution and lower radiation dose., (© 2024 The Authors. Type 2 diabetes and its associated cardiovascular risk is an escalating epidemic that represents a significant public health burden due to increased morbidity and mortality, disproportionately affecting disadvantaged communities. Poor glycaemic control exacerbates this burden by increasing retinal, renal, and cardiac damage and raising healthcare costs. This predicament underscores the urgent need for research into cost-effective approaches to preventing diabetes complications. An important but often overlooked strategy to improve metabolic control in diabetic patients is the treatment of periodontitis. Our aim is to assess whether the inclusion of periodontitis treatment in diabetes management strategies can effectively improve metabolic control, and to advocate for its inclusion from an equity perspective. We conducted a comprehensive review of the literature from 2000 to 2023. We analyzed the pathophysiological links between periodontitis, diabetes, and atherosclerotic cardiovascular disease, all of which have inflammation as a central component. We also examined the inequalities in health care spending in this context. The utilization of eco-innovative products has witnessed a surge in adoption, driven by their inherent capacity to address pressing environmental concerns. To comprehensively fathom the underlying motivations propelling consumers to embrace these products, we conducted an in-depth investigation employing "The Not Company" (Chile) as a compelling case study. We conducted qualitative interviews with a cohort of 20 Chilean consumers, guided by the Theory of Planned Behavior theoretical framework. The research methodology harnessed the principles of thematic analysis, yielding insights that underscore the significance of key determinants in shaping consumers' choices towards eco-innovative products. Specifically, our findings highlighted that consumer choices in this domain are profoundly influenced by their attitudes, subjective norms, and perceived behavioral control. Moreover, within these overarching categories, we unearthed sub-themes illuminating the intricate influences guiding consumer choices. These sub-themes encompassed beliefs about food manufacturing and packaging, the persuasive impact of social media and advertising, and the indelible impressions left by prior encounters with eco-innovative products. This study highlights consumers' fundamental role in the broader eco-innovation landscape, particularly within the food industry context.

- Abstract
Acute Kidney Injury (AKI) is a frequent complication in intensive care unit (ICU) patients that increases mortality and chronic kidney disease (CKD) development. AKI is associated with elevated plasma fibroblast growth factor 23 (FGF23), which can be modulated by erythropoietin (EPO) and Klotho. We aimed to evaluate whether a combined biomarker that includes these molecules predicted short-/long-term outcomes. We performed a prospective cohort of ICU patients with sepsis and previously normal renal function. They were followed during their inpatient stay and for one year after admission. We measured plasma FGF23, EPO, and Klotho levels at admission and calculated a combined biomarker (FEK). A total of 164 patients were recruited. Of these, 50 (30.5%) had AKI at admission, and 55 (33.5%) developed AKI within 48 h. Patients with AKI at admission and those who developed AKI within 48 h had 12- and 5-fold higher FEK values than non-AKI patients, respectively. Additionally, patients with higher FEK values had increased 1-year mortality (41.9% vs. 18.6%, p = 0.003) and CKD progression (26.2% vs. 8.3%, p = 0.023). Our data suggest that the FEK indicator predicts the risk of AKI, short-/long-term mortality, and CKD progression in ICU patients with sepsis. This new indicator can improve clinical outcome prediction and guide early therapeutic strategies.

Open fractures often require complex treatments, especially those with joint involvement or critical bone defects. Managing both combined injuries present even greater challenges and is not without complications. We present the case of a young patient with an open fracture of the femoral condyle and loss of bone stock in the articular surface. In this case, a combined osteosynthesis approach was employed, utilizing cannulated screws and a femoral condyle arthroplasty with bone cement as a salvage measure, allowing a favorable clinical and functional outcome at the 3-year follow-up. Amicrobial Pustulosis of the Folds is a relapsing, chronic and rare neutrophilic dermatosis, characterized by papulopustular, eczematous and aseptic lesions on skin folds. This disorder usually occurs predominantly in females (30 years of age average) with a history of an autoimmune disorder, especially systemic lupus erythematosus. There is no standard therapy, but systemic corticosteroids, alone or in combination with other immunosuppressive drugs, are usually the first-line therapy. We report two females aged 37 and 20 years with the disease but without associated autoimmune diseases. They were successfully treated with non-steroidal treatments.

Objectives: The problem is to determine whether, after a disaster, unemployment is a possible cause of post-traumatic stress disorder (PTSD) - the classic model, or whether PTSD is a possible cause of unemployment - the alternative model., Material and Methods: The study was conducted on a probabilistic sample of 26 213 adults who provided responses using the Davidson Trauma Scale and lived in regions near or far from the epicenter (Cobquecura, the coast of Chile) of the earthquake and tsunami of February 27, 2010., Results: Independent of the proximity to the epicenter, there is an association between PTSD, unemployment, female sex and the poverty line. For regions close to the epicenter, the alternative model has better adjustment indicators than the classic model., Conclusions: Given the adjustment of the alternative model, the occurrence of PTSD cases is more likely to explain the unemployment condition. - Abstract
Cross-country mountain biking is an Olympic sport discipline with high popularity among elite and amateur cyclists. However, there is a scarcity of data regarding the key determinants of performance, particularly in young cross-country cyclists. The aim of this study was to examine the physiological profile of youth national-level cross-country cyclists and to determine those variables that were able to best predict the performance in an official race. Ten youth cross-country cyclists of a national team underwent a complete evaluation that included anthropometric assessments, laboratory tests to evaluate the wattage at blood lactate thresholds and at maximal oxygen uptake (PO
VO2max ), and field tests to make an in-depth power profile of the athletes. The data obtained in the above-mentioned tests was analysed along with total and partial race times during a competition belonging to the Union Cycliste Internationale (UCI) calendar. In the present study, large and statistically significant correlations (r = -0.67 to -0.95, p ≤ 0.05) were found between maximal and submaximal indices of aerobic fitness and cycling performance, especially when they were normalised to body mass. A multiple regression analysis demonstrated that the wattage at 2 mmol/L, 4 mmol/L and POVO2max were able to explain 82% of the variance in total race time. In summary, the results of this study support the use of maximal and submaximal indices of aerobic power as predictors of performance in youth cross-country cyclists.- Published

50. The Economics of Honey Bee (Hymenoptera: Apidae) Management and Overwintering Strategies for Colonies Used to Pollinate Almonds.

- Abstract
Commercial honey bee (Apis mellifera L.) colonies significantly contribute to agricultural productivity through crop pollination. Almond production requires the most colonies because there are more than a million acres of orchards that require cross-pollination for nut set. With the rising costs of managing and transporting colonies to almond orchards combined with the high colony losses beekeepers routinely experience, we asked if renting colonies for almond pollination was profitable. We conducted a longitudinal study on 190 colonies from their establishment in April until they were placed in almond orchards 10 mo later. In the fall, equal numbers of colonies were placed either in cold storage (CS) facilities or in outdoor apiaries for the winter. We found that the cost of overwintering colonies in CS was lower than in apiaries, but CS did not reduce overwintering losses. A key finding from our study is that there is little or no profit in renting colonies for almond pollination once summer management and overwintering costs are considered. Our only profitable venture was honey production in the summer. We propose alternative management strategies to lower costs and make almond pollination profitable. We also developed a decision tool for selecting colonies to overwinter in CS and reduce expenditures on those that will not reach sufficient size for almond pollination. Our study exposes the unsustainable financial burden experienced by migratory beekeepers that is not included in estimates of yearly colony losses, and underscores the urgent need for forage plantings to generate revenue from honey and improve overwinter survival., (Published by Oxford University Press on behalf of Entomological Society of America 2019.)
