27. Peculiar nuclear clearing composed of microfilaments in papillary carcinoma of the thyroid

Abstract

Background. Papillary carcinoma of the thyroid frequently occurs in young women. Four cases of papillary carcinoma were found in women in their 20s, and the disease was characterized by unusual morphologic and clinical features. Methods. Four cases were analyzed clinically, grossly, microscopically, ultrastructurally, and immunohistochemically and compared with control cases of papillary carcinoma. Results. The tumors histologically showed various appearances with trabecular, solid cribriform, follicular, and papillary patterns. A striking finding was that stratified cancer cell nests with prominent nuclear clearings were scattered in the tumors. Ultrastructurally, the nuclear clearings were composed of fine thread-like fibrils, and they were bound unselectively to various antibodies against hormones and intermediate filaments. The four patients also had common clinicopathologic features as follows: they were all women in their 20s and their preoperative serum thyroglobulin levels were all normal despite the large size of their thyroid tumors. The tumors were grossly well encapsulated, and there was no lymph node metastasis, except in one patient who had only one positive node. Conclusions. The peculiar nuclear features were distinct from intranuclear cytoplasmic inclusions or ground-glass nuclei, and these have not been reported previously to the authors' knowledge in conventional thyroid neoplasms. 28. Diffuse sclerosing variant of papillary carcinoma of the thyroid: clinical importance, surgical treatment, and follow-up study

Abstract

Diffuse sclerosing (excessive fibrous tissue growth) variant of papillary carcinoma of the thyroid is uncommon among papillary carcinomas (characterized by small raised protrusions), although the variant is not rare when only young women are considered. Since the prognosis for this variant is excellent, it is imperative that the condition be promptly and accurately diagnosed. The authors discuss the characteristics of the diffuse sclerosing variant as seen in 14 women, all less than 29 years of age. Autoantibodies against thyroglobulin are common among such patients, and 7 of 10 tested in the present series were positive. It is possible to misdiagnose the condition as Hashimoto's thyroiditis, which was the case in four patients. Another patient had been misdiagnosed with Grave's disease. One of the features of the diffuse sclerosing variant is the presence of psammoma bodies, which may be seen both radiologically and on biopsy. Psammoma bodies are layered calcium concretions which build up around cells. These grainy pathological features get their name from the Greek word for sand. Treatment of diffuse sclerosing papillary carcinoma requires the surgical removal of the entire thyroid; it may be possible to prevent chronic hypoparathyroidism by removing the parathyroid glands from the affected thyroid and reimplanting at least two. Unfortunately, the tumor may invade the laryngeal nerve in the later stages and damage to this nerve is unavoidable if surgery is not carried out prior to the invasion of the nerve. In the present series, all 14 patients are alive and well at an average follow-up of 16 years. These results suggest that conservative surgery should be attempted to preserve function and cosmetic appearance as far as possible; such conservative surgery requires that the diagnosis be made early and that the condition is not mistaken for other thyroid disorders. Abstract

The patient suffered loss of consciousness, dysarthria and right sided hemiparesis. The CT scan and MRI scans were negative. These findings are more in keeping with a diagnosis of Transient Ischemic Attack (TIA) or mild CVA. Hypoglycemia per se does not usually cause hemiparesis. The blood glucose level was low but I am not sure if one can conclude that hypoglycemia caused the above noted neurological signs and symptoms. The authors do not present any data to prove that this patient had "hypoglycemic coma".

Abstract

我々は過去500例以上の乳癌に術前intravenous digital subtraction angiography (IV-DSA)を施行し,乳管内非浸潤癌巣(intraductal cancer spread)をふくめた癌の広がり診断に有用であることを報告してきた.今回IV-DSAにより癌巣の広がりを描出できた非触知乳癌を経験したので報告する.症例は55歳女性で,1982年右乳癌で乳房切除術を施行し,1999年1月マンモグラフィーで左乳房C領域に限局した微小石灰化を認めステレオ下生検(stereotactic needle biopsy)を施行し,非浸潤性乳管癌(ductal carcinoma in situ, DCIS)の診断を得た.触診,超音波検査では異常はなかったがIV-DSAでC領域を中心として乳房ほぼ全域に広がる濃染像を認めた.乳房切除術の適応となる広範囲のDCISと診断したが,患者とも相談の上,quadrantectomyを施行した.組織学的には小浸潤巣(minimal invasive foci)を伴うpapillo-tubular carcinomaで,surgial marginが広範囲にわたって陽性であり,後日腋窩郭清を伴う乳房切除術を追加施行した.なお切除標本には癌遺残が著明であった.非触知乳癌における癌の広がり診断には従来の画像検査に加えてMRI,CTなどが行われるがIV-DSAも重要な検査手段になり得ると考えられた., In a recent case of non-palpable breast cancer we used intravenous digital subtraction angiography to examine the cancerous spread. We describe the usefulness of this technique and report the clinical course of this case. The patient was a 55-year-old female who underwent a mastectomy in 1982 due to cancer in the right breast. Mammography conducted in January 1999 revealed microcalcification in area C of the left breast. According to results obtained from a stereotactic needle biopsy, we made a diagnosis of non-infiltrating ductal carcinoma in situ. Although no abnormal findings were observed in palpation and ultrasound examination, intravenous digital subtraction angiography disclosed high-density lesions extending almost entirely over the left breast, especially in area C. A quadrantectomy was selected as the initial surgery. Histologically the lesion was regarded as being papillo-tubular carcinoma accompanied by minimal invasive foci. The surgical margins proved positive and we performed a mastectomy with dissection of axillary nodes. A substantial volume of cancerous remnant was detected in the resected specimen. This study demonstrates that intravenous digital subtraction angiography is an useful technique for examination of the spread of non-palpable breast cancer.
