Echinococcosis generally remains asymptomatic for many years, but it can also be fatal if complications like cyst rupture or superinfection occur. It is highly uncommon for a macroscopically non-ruptured cyst to cause death, and its exact mechanism is unknown. In the literature, there are several cases with identical characteristics diagnosed by autopsy. This autopsy case has illustrated the death due to a micro ruptured hydatid cyst. The deceased's lung underwent microscopic inspection, and it revealed broad intraalveolar hemorrhage, edema, severe hyperemia, neutrophil stasis in the interstitial capillary lumens, and a large number of parasites in the capillary lumen. Anaphylaxis against cyst contents that leaked into the bloodstream from liver cysts and non-thrombotic pulmonary embolization were shown to be the causes of mortality. In countries where echinococcosis is endemic, echinococcosis should be considered in all cases of anaphylaxis and sudden death.

The aim of this study was to determine the predictive value of serum interleukin-6 (IL-6), interleukin-8 (IL-8), mannose binding lectin (MBL), procalcitonin (PCT) and C-reactive protein (CRP) levels for bacteremia/sepsis (B/S) at the beginning of a febrile episode in children with chemotherapy-induced febril neutropenia (FN). In a prospective study, 54 febrile neutropenic episodes from 30 pediatric cancer patients were analysed. Samples were obtained in two different clinical periods: afebrile neutropenic (AFN) period after chemotherapy and FN period (on the first day of fever). The number of cases with spontaneous coronary artery dissection (SCAD) is considered to be being underestimated because of a large amount of SCAD leading to sudden death without previous diagnosis. Besides, not only in clinics but also in autopsy practice, correct diagnosis of SCAD is important to prevent forensic malpractice. The article is intended to discuss the pathological findings through the forensic point of view for improving the malpractice expertise in scope of clinicians- timely antemortem diagnosis according to risk factors and in scope of forensic pathologists- the cause of death determination ability according to macroscopical and microscopical findings of the autopsy. In 3 cases reported, the main characteristics were the female sex, pregnancy history and a sudden death without any trauma. However, although there are many women giving birth or using oral contraceptives, only some of them are facing with SCAD. This suggests the possibility of some hereditary factors, whereas hereditary characteristics may be understood in many different ways like hormone-releasing regulating mechanisms as well as immunity, morphology, or any other mechanism. For instance, autoimmunity has been also a hereditary underlying factor for vessel injury considered in presented cases.

Bir kadının, gebelik sürecinde veya gebeliğinin sonlanmasından sonraki 42 gün içerisinde, gebeliğin süresi ve lokalizasyonu ne olursa olsun, gebelik sürecine bağlı ya da gebeliğin ağırlaştırdığı nedenlerle, ancak kaza veya zorlamalı bir olay ile ilişkili olmayan şekilde ölümü Dünya Sağlık Örgütü tarafından anne ölümü olarak tanımlanmaktadır. Çalışmamızda otopsi pratiğinde emboli tanısı için yapılması gerekenlerin literatür verileri eşliğinde tartışılması amaçlanmıştır. Olgunun 38 yaşında, G5 P4 olduğu ve sezaryen sonrası 24 saat içerisinde takipleri sırasında solunum sıkıntısı oluşarak, öldüğü bildirilmiştir. Yapılan otopsisinde vücut kütle indeksi 32,8 kg/m2 (obez) olarak hesaplanmış, her iki akciğerde pulmoner arterlerde trombüs tespit edilmiştir. Trombüs histopatolojik incelenmeyle de doğrulanmıştır. Discussion of the requirements for an emboli diagnosis in an autopsy practice through literature data is aimed in our study. It has been reported that the case was 38 years old, G5 P4 and died with a respiratory distress occurred during the follow ups in 24 hours after the caesarean section. In autopsy, the body mass index was calculated as 32.8 kg/m2 (obese) and thrombus was detected in pulmonary arteries of both lungs. Thrombus was confirmed also through histopathological analysis. Evaluation of the medical history and data obtained from a detailed autopsy are of great importance for maternal deathswhich may occur after a caesarean section and involve a pulmonary thromboembolism diagnosis in order to determine the cause of death and prevent a loss of right in medical malpractice claims.

Bu çalışmada Türkiye'den çeşitli lokalitelerden bulunmuş olan Sırtlangiller (Hyaenidae) ailesine ait fosiller incelenmiştir. Anadolu, Avrupa'dan köken almış olan Hyaenidae ailesinin doğuya geçişi sırasında önemli bir konumdadır. Bu nedenle bu çalışma ile ailenin evrimsel geçmişini anlamaya katkıda bulunmak ve Türkiye'deki durumunun ortaya konulması amaçlanmıştır. Bu çalışmada Anadolu Geç Miyosen lokalitelerinden Çobanpınar (MN 13), Orta Sinap (MN 9-10), Pınaryaka (MN 9-12) (Ankara), Bayraktepe (MN 9-10), Gülpınar (MN 11-12) (Çanakkale), Gökeşme (MN 12) (Kırşehir), Karaağaç (MN 12) (Muğla) ve Karain (MN 11-12) (Nevşehir)'den dental bulgular incelenmiştir. Çalışma sonucunda beş adet cinse ait sekiz adet tür belirlenmiştir: Ictitherium viverrinum (Orta Sinap, Pınaryaka, Çobanpınar, Bayraktepe, Karaağaç), Ictitherium cf. viverrinum (Karain), Hyaenictitherium wongii (Karaağaç, Bayraktepe, Gülpınar), Adcrocuta eximia (Çeltek, Gülpınar, Çobanpınar), Protictitherium crassum (Orta Sinap), Protictitherium intermedium (Gülpınar), Protictitherium cf. cingulatum (Bayraktepe), Lycyaena sp. (Çeltek). Bulgular, genelde karnasiyal dişler üzerinde yapılan multivaryasyon analizleri ile biyokronolojik, paleobiyocoğrafik ve paleoekolojik açıdan değerlendirilmiş ve Anadolu Geç Miyosen sırtlan bulguları Yunanistan-İran biyoprovensindeki ve Çin'deki Geç Miyosen sırtlan bulguları ile morfolojik ve kronolojik açıdan uyumlu olduğu görülmüştür. Bulgular, Türkiye'de bu aile ile ilgili bilinen biyokronolojik ve paleocoğrafik dağılımı ile Anadolu Geç Miyosen'inde nadir bilinen Lycyaena gibi avlanan sırtlan bulgularına katkıda bulunmuştur.Bulguların biyometrik değerlerinden edinilen veriler, söz konusu memeli lokalitelerindeki Valesiyen-Turoliyen artiodactil (Bovidae, Suidae, Giraffidae) ve perissodactil (Equidae, Rhinocerotidae) faunaları ile birlikte değerlendirildiğinde, Batı ve Orta Anadolu'nun Geç Miyosen'de ağaçlık, otlak ve açık alan (savan) olabileceği sonucuna varılmış olup konu ile ilgili önceki çalışmaları desteklemektedir. In this study it is investigated that the fossils belong the family Hyaenidae from some Turkish localities. Anatolia is an important position during the migration from Europe originated hyaenids to the East. In this study it is intended that contributes to understand evolutionary history of the family and reveal their situation in Turkey.Some dental discoveries from Çobanpınar (MN 13), Orta Sinap (MN 9-10), Pınaryaka (MN 9-12) (Ankara), Bayraktepe (MN 9-10), Gülpınar (MN 11-12) (Çanakkale), Gökeşme (MN 12) (Kırşehir), Karaağaç (MN 12) (Muğla) and Karain (MN 11-12) (Nevşehir) were examined in this work. Eight species belong to five genera is identified in the result: Ictitherium viverrinum (Orta Sinap, Pınaryaka, Çobanpınar, Bayraktepe, Karaağaç), Ictitherium cf. viverrinum (Karain), Hyaenictitherium wongii (Karaağaç, Bayraktepe, Gülpınar), Adcrocuta eximia (Çeltek, Gülpınar, Çobanpınar), Protictitherium crassum (Orta Sinap), Protictitherium intermedium (Gülpınar), Protictitherium cf. cingulatum (Bayraktepe), Lycyaena sp. (Çeltek). The findings are determined with multivariation analysis in terms of biochronology, palaeogeoraphy and palaecology, and it is seen that Anatolian late Miocene hyaenids is compatible with late Miocene hyaenids in Greco-Iran bioprovinces and China. This work contributes that the data known about the family of the biochronological and the palaeogeographical distribution in Turkey and the discoveries of the hunting hyaenas like genus Lycyaena.Once the data obtained from analyzes are evaluated with Vallesian-Turolian artiodactyl (Bovidae, Suidae, Giraffidae)and perissodactyl (Equidae, Rhinocerotidae) findings in the mammalian localities mentioned, it was concluded that the western and the central Anatolia could be woodland, grassland and open areas (savanna) in late Miocene, and previous studies about that subject support this conclusion. 182

67 ÖZET Akut lenfoblastik lösemide ilaçlara direnç tedavideki başarıyı sınırlayan önemli bir faktördür. İlaç direnci karmaşık bir dizi mekanizma ile oluşmakta p-gp ile ilgili klasik multidrug rezistans en çok araştırılanıdır. Çalışmamızda çocukluk çağı ALL'lerinde genel ilaç direncini MTT ile tespit ederek prognostik önemini değerlendirdik. Diğer faktörlerle birlikteliklerini araştırdık. SSK Bakırköy Doğumevi Kadın ve Çocuk Hastalıkları Eğitim Hastanesi, Pediatrik Hematoloji-Onkoloji Ünitesi'nde Ocak 1999-Mart 2001 tarihleri arasında ALL nedeniyle izlenen 30 hastadan tedavi öncesinde ilaç direncini saptamak için örnek gönderildi. Hastalarımızda prednizolona duyarlılık %33.3, daunuribisine ve L- Asparaginaze duyarlılık %20, vinkristine duyarlılık ise %13.3 olarak bulundu. Duyarlı olduğu sayısına göre ise hastalarımızın %6.6'sı yüksek derecede hassas, %16.6'sı orta derecede hassas ve %76.6'sı dirençli gruba aitti. Tedavi sonrasında hastalarımızın tam remisyon oranı %96.6 idi. 2 vakamız daha sonra geç relaps yaparken 3 hastamızda uzayan aplazi döneminde kanama ve enfeksiyon nedeniyle kaybedildi. 20 aylık genel sürvi %86.62, EFS ise %72.6 idi. İlaç duyarlılığına göre EFS süreleri arasında anlamlı bir fark yoktu. Çeşitli faktörlerin prognoza etkisi değerlendirildiğinde sadece Hb^10 g/dl, L2 morfolojisi ve 33. gün kemik iliği cevabının olmaması ile kötü prognoz arasında anlamlı ilişki saptadık. İlaç duyarlılığının diğer faktörlerle ilişkisine baktığımızda T ve Mix hücreli lösemi, CD33 ve CD34 pozitifliği ve CD22 negatifliği ile ilaç direncinin arttığını gördük. İstatistiksel açıdan destekleyememize rağmen direnç arttıkça hastalarda indüksiyona cevapsızlık, erken relaps ve geç relaps eğilimi mevcuttu. Tedavi cevabı ile prognostik faktörler değerlendirildiğinde de translokasyon varlığı, clgM pozitifliği, 33. gün kemik iliği68 cevabının olmaması, yüksek risk grubu, HbâlO g/dl, lökosit>20.000/mm3 ve TdT negatifliğinin kötü prognoz ile ilişkisini saptadık. Bu çalışmanın sonucunda MTT'nin kişinin tedaviye cevabı hakkında faydalı bilgiler verebileceği, ancak belirgin kişisel farklılıklar nedeniyle hastanın diğer bütün prognostik faktörlerle değerlendirilerek uygun tedavi stratejilerinin belirlenmesi gerektiğini düşünmekteyiz. 78
