6. Complementary alternative medicine in rheumatic diseases: Causes, choices, and outcomes according to patients.

Objective: The knowledge of physicians about complementary and alternative medicine (CAM) applications is limited. However, especially in chronic diseases, patients and their relatives can often refer to CAM applications. Rheumatic diseases are chronic in nature presenting with a wide clinical spectrum. Despite developing treatment options, achieving treatment goals may not always be possible. For this reason, patients seek different treatment and use traditional and complementary medicine. The aim of this study was to investigate causes, consequences, and the frequency of applying to CAM in rheumatic diseases. Methods: Ninety-five patients admitted to the rheumatology outpatient clinic were enrolled in the study. Health assessment questionnaire and short-form-36 were used to determine the quality of life of patients. Anxiety and depression symptoms were screened by the Hospital Anxiety and Depression scale, a questionnaire that was filled-in by the patients themselves. Also, patients were questioned about their place of residence, level of education, diagnosis, CAM modality types, application reasons, and outcomes. Chi-square test was used to analyze categorical data. Parametric data were analyzed using Student t-test, and nonparametric data were analyzed using Mann-Whitney U test. Results: Thirty-two of our patients had applied to CAM modalities (phytotherapy [34.45%], cupping therapy [21.8%], acupuncture [12.5%], hirudotherapy [12.5%], food supplement [12.5%], and ozone treatment [6.25%]). Only 31.3% of the patients informed their doctors about CAM applications. 47.8% of fibromyalgia patients and 29.2% of patients with inflammatory rheumatic diseases had applied to CAM. Gender, working status, income level, smoking, and alcohol habits were not associated with the application to CAM. However, none of the residents of the village, 14.3% of the residents of the district center, and 41.1% of the residents of the city center had applied to CAM modality. The rate of applying to CAM was 18.2% for those who cannot read and write. The application ratio of CAM is over 40% among secondary school, high school, and university graduates. Conclusion: Among patients with rheumatic diseases, application to CAM is quite common. Very few patients inform their physicians about applying to CAM. Contrary to what is presumed, the rate of applying CAM applications is lower among those living in rural areas and with low education levels. [ABSTRACT FROM AUTHOR]

Omurilik Yaralanması (OY) sonrası en büyük kayıp, kişinin hareket fonksiyonunun kaybı olmakla birlikte, yaralanma sonrası meydana gelen ve pek çok fonksiyonu etkileyen komplikasyonlardan birisi de ağrıdır. Ağrı, OY sonrası hastaların büyük bir çoğunluğunda değişik derecelerde görülebilen ve her hastada değişik etkiler yapabilen tedavisi en zor problemlerden biridir. Bizim çalışmamızın amacı; OY'lı hastalarda nöropatik ağrı ve nöropatik olmayan ağrı sıklığını tespit etmek, bu hastalarda, klinik ve demografik parametreler, fonksiyonel durum, ağrı, depresyon ve anksiyete ile yaşam kalitesi arasındaki ilişkiyi incelemektir. Dicle Üniversitesi Tıp Fakültesi Hastanesi Fiziksel Tıp ve Rehabilitasyon servisinde yatarak tedavi gören ve rehabilitasyon polikliniğinde takip edilen OY'lı 84 hasta değerlendirilmeye alındı. Hastaların detaylı genel fizik muayenesi, nörolojik muayenesi, ağrı sorgulaması yapıldı. Ayrıca çalışmaya alınan hastaların depresyon, anksiyete düzeyleri ve yaşam kalitesi değerlendirildi. Hastaların demografik özellikleri, yaralanma tarihi, etyolojisi, hastalık süreleri, komplikasyonları kaydedildi. Hastaların nörolojik seviyesi için 2013 ASIA skalası kullanıldı. Hastaların duyu ve motor seviyeleri kaydedildi. Hastaların spastisitesi Modifiye Ashworth skalası, fonksiyonel ambulasyon seviyesi Walking Index Spinal Cord Injury (WISCI), Fonksiyonel Ambulasyon Skalası (FAS) ve fonksiyonel bağımsızlık düzeyleri Spinal Cord Independence Measure (SCIM) 3 ile değerlendirildi. Hastaların yaşam kalitesini değerlendirmek amacıyla SF-36, depresyon-anksiyete düzeyini ve şiddettini değerlendirmek için Beck Depresyon ve Anksiyete Ölçeği kullanıldı. Nöropatik ağrı, yorgunluk, parestezi düzeyi ve şiddeti için VAS ve LANSS sklası kullanıldı. Çalışmaya alınan hastaların %41,7'sinde nöropatik ağrı mevcut iken. %58.3 hastada nöropatik ağrı yoktu. Nöropatik ağrılı grup ile nöropatik ağrısız grup arasında yaş, cinsiyet ve medeni durum açısından istatistiksel olarak anlamlı bir fark saptanmadı (p>0.05). İki grup karşılaştırıldığında, eğitim seviyeleri arasında anlamlı fark yoktu (p>0.05). Nöropatik ağrılı ve ağrısız gruptaki hastalar arasında; median hastalık süresi ve etyolojik faktörler (travmatik, nontravmatik) açısından istatistiksel olarak anlamlı bir fark saptanmadı (p>0.05). Gruplar karşılaştırıldığında nörolojik seviye, kompletlik durumu ve komplikasyon varlığı açısından gruplar arasında istatiksel olarak anlamlı bir fark yoktu. Her iki grup BECK depresyon, anksiyete skoru, VAS ağrı, yorgunluk, parastezi skorları ve SF -36 skorunun tüm parametreleri açısından karşılaştırıldığında nöropatik ağrılı grupta; VAS ağrı, yorgunluk ve parastezi skorları daha yüksek saptandı ve bu sonuç istatiksel olarak anlamlıydı. Her iki grup asında Beck depresyon ve anksiyete skorları açısından istatiksel olarak anlamlı bir fark saptanmadı. Nöropatik Ağrılı grupta SF-36'nın canlılık, vücut ağrısı, emosyonel durum ve total skor alt parametreleri daha düşük bulundu, bu sonuç istatistiksel olarak anlamlı idi (p≤0.05). Hastaların fonksiyonel ambulasyon seviyeleri (WISCI) ve SCIM 3 skorları karşılaştırıldığında her iki grup arasında istatistiksel olarak anlamlı fark yoktu (p>0.05). Tüm hastaların LANSS skoru ile VAS ağrı, VAS yorgunluk, VAS parastezi ve BECK anksiyete skorları arasında pozitif yönde anlamlı düzeyde korelasyon mevcut iken; SF 36 canlılık, emosyonel durum, vücut ağrısı, sosyal fonksiyonlar, mental sağlık ve total arasında negatif yönde anlamlı düzeyde korelasyon mevcut idi (p≤0.05). Tüm hastaların LANSS skoru ile yaş, cinsiyet, hastalık süresi, ABS skalası, komplikasyon varlığı ve BECK depresyon skoru açısından korelasyon saptanmadı (p>0.05). Sonuç olarak; nöropatik ağrı OY sonrası sık görülen, hastaların yaşam kalitesini ve psikolojik durumunu etkileyen önemli bir komplikasyondur. Anahtar kelimeler: Omurilik Yaralanması, Nöropatk Ağrı, Depresyon, Anksiyete, Yaşam Kalitesi Although the greatest loss is motional functional loss after Spinal Cord Injury (SCI), pain is one of the complications which occurs after the injury and effect several functions. Pain is one of the most difficult problems which has seen among the majority of the patients in different degrees after SCI. The purpose of our study is to detect the frequency of neuropatic and non-neuropathic pain among the SCI patients, to examine the relationship of the pain, functional status, the clinical and demographical properties in these patients and the effects of these parameters on quality of life, depression and anxiety. 84 patients have been evaluated who have treated at Dicle University Hospital Physical Therapy and Rehabilitatiton inpatient clinics and followed at rehabilitation Policlinics. Detailed general physical examination, neurologic examination and pain questionaire of the patients have been performed. In addition; depression, anxiety and quality of life questioned. Demographic properties, date of injuries, etiology, the period of the disease and the complications have been recorded. 2013 ASIA scale has been used for the neurologic level of the patients. Spasticity of the patients have been assessed by Modified Ashworth Scale, the functional ambulation levels of the patients have been assesed by Walking Index Spinal Cord Injury (WISCI), Functional Amulation Scale (FAS) and the functional independency levels of the patients have been assessed by Spinal Cord Independence Measure (SCIM) 3 scale. SF-36 has been used to evaluate the quality life of the patients while Beck Depression and Anxiety Scale have been used to evaluate the level and the intensity of the depression and anxiety. VAS and LANSS have been used for the level and the intensity of the neuropatic pain. While the 41,7 % (n=35) of the patients those were included in the study have neuropatic pain, in the 58,3 % (n=49) of the patients neuropatic pain haven't been detected. When the two groups who have neuropathic pain and who have no pain compared significant differences couldn't be found according to their age, sex and marital status between these two groups. Also there haven't been significant differences in educatioanl level when these two groups have compared. There have not detected any significant differences in terms of median disease period and etiologic factors (as traumatic, non-traumatic) between two groups (p>0.05). There haven't been any significant difference statistically in terms of neurologic level, status of completeness and the existence of complications when these two groups have been compared (p>0.05). When the VAS pain, VAS fatigue, VAS parasteshia, BECK depression, BECK anxiety score and the all parametres of the SF-36 scores have been compared between these two groups while VAS pain, VAS fatigue, VAS parasteshia scores are higher in the group which has neuropathic pain and these differences were statistically meaningful at level of significance (p≤0.005), there were no statistically significiant differences according to BECK depression and BECK anxiety score between two groups (p>0.05). SF-36 paramaters vitality, bodily pains, emotional status, total scores have been found lower in neuropathic pain group and this result was statistically meaningful (p≤0.005). When the Functional Ambulation Levels ( WISCI ) and SCIM 3 scores of the patients have been compared there haven't found any significant differences statistically between the two groups. In all patients there have been a positive correlation between the LANSS and VAS pain, fatigue, parasthesia socres and negative correlation between vitality, bodily pain, emotional status, social functions, mental health, total score of SF 36 paramaters (p≤0.005). There was no correlation between LANSS and age, gender, marital status, AIS scale, existence of complications and BEC depression score (p>0.005). Consequently, Neuropatic Pain is an important complication which has occured frequently after SCI and effects the quality of life and psychological status of the patients. Resveratrol, a phytochemical, acts several cellular signaling pathways and has anti‐inflammatory potentials. The purpose of this study is to research the therapeutic effect of resveratrol in collagen‐induced arthritis (CIA) model in rats and whether resveratrol affects the activities of signaling pathways those are potent pathogenic actors of rheumatoid arthritis. Arthritis was induced by intradermal injection of chicken type II collagen combined with incomplete Freund's adjuvant in Wistar albino rats. One day after the onset of arthritis (day 14), resveratrol (20 mg/kg/day) was given via oral gavage, until day 29. The paws of the rats were obtained for further analysis. Tissue Wnt5a, mitogen‐activated protein kinase (MAPK), Src tyrosine kinase and signal transducer, and activator of transcription‐3 (STAT3) mRNA expressions were determined by real‐time polymerase chain reaction. Resveratrol ameliorated the clinical and histopathological (perisynovial inflammation and cartilage‐bone destruction) findings of inflammatory arthritis. The tissue mRNA expressions of Wnt5a, MAPK3, Src kinase, and STAT3 were increased in the sham group compared to the control group. Resveratrol supplement decreased their expressions. The present study shows that Src kinase, STAT3, and Wnt signaling pathway are active in the CIA model.
