1. Design of Polarization-Sensitive Meta Micro-Reflectors in Bragg Grating Waveguide

Abstract: The Society of Photo-Optical Instrumentation Engineers (SPIE), Nanophotonics X 2024 -- 7 April 2024 through 12 April 2024 -- 200395, Bragg-Grating-Waveguide (BGW) delay lines are commonly used in transmission mode, owing to the significant elevation in group index observed at the edge of the stopband. However, achieving delays often necessitates structures with a larger footprint, such as cascaded or spiral designs, which consume a substantial amount of chip estate. Recent approaches to optimize chip space involve round-trip configurations with reflectors, but these can restrict bandwidth and cause mode conversion. Our study introduces an efficient solution: a meta-reflector for TE0 mode, enabling double delays via a single path, while also employing a step taper in TM0 mode to couple the TM signal into a strip waveguide, within a 1.9 × 2.6 µm2 area. Simulations show that in TE mode, there is a peak attenuation of -0.18 dB within the 1530–1590 nm wavelength spectrum, and in TM mode, the loss reaches -4.73 dB across the 1500–1600 nm band. In the computation of losses, the customary impact of the Bragg grating waveguide was disregarded, with attention concentrated exclusively on the design of the reflector. © 2024 SPIE., 2210A, 122E566

2. Optimized amplitude distributions for grating and side lobe suppression in optical phased array antennas

Abstract: The Society of Photo-Optical Instrumentation Engineers (SPIE), Nanophotonics X 2024 -- 7 April 2024 through 12 April 2024 -- 200395, Integrated optical phased arrays (OPAs) are important for various applications such as LIDAR, microscopy, wireless communication, and holography. Traditional one-dimensional N-element OPAs with individual phase shifters for varied steering angles often have emitter spacings that exceed half a wavelength, leading to grating lobes. Moreover, using true-time-delay lines with varied numbers of periods for different delays may cause amplitude inconsistencies and further side lobe elevation. The proposed partially apodised (PA) one-dimensional grating waveguide design controls amplitude while conserving the time delay for targeted steering angles, using distributions as Uniform to minimize grating and side lobes. We fabricated a four-element array with insertion losses of -3 dB for the PA version across the 1530-1580 nm wavelength band. © 2024 SPIE., 2210A, 122E566

3. Ultra-miniaturized crossings based on digitized metamaterials for one-dimensional grating waveguides

Abstract: The Society of Photo-Optical Instrumentation Engineers (SPIE), The challenge of designing crossings in one-dimensional grating waveguides (1DGWs) arises from the noticeable asymmetry in Bloch mode profiles, which causes the guided modes to be compressed toward the outer sidewall. The proposed solution involves using digitized metamaterials in an extensively corrugated 1DGW on a silicon-on-insulator platform. The resulting crossing structure has an ultra-miniaturized and ultra-low loss design, with a broad bandwidth spanning from 1500 to 1580 nm, while maintaining a minimal footprint of merely 2.1×2.1 µm2. The fabricated device is found to have an insertion loss of -2.52 dB within the wavelength range of 1500 to 1580 nm. This design represents a significant advancement in the pursuit of compact and low-energy silicon photonic waveguide crossings. © 2024 SPIE., Scientific and Technical Research Council; TUBITAK, (2210A, 122E566)

4. Periodic Topology Optimization-Based Inverse Design of Slow-Light in Silicon Nitride One-Dimensional Grating Waveguides

Abstract: The Society of Photo-Optical Instrumentation Engineers (SPIE), Integrated Photonics Platforms III 2024 -- 7 April 2024 through 10 April 2024 -- Strasbourg - -201263, Devices engineered for slowing light, utilizing one-dimensional grating waveguides and fabricated from silicon nitride, often necessitate large footprints to secure the required delay, a consequence of the material’s inherently low refractive index. Our approach employs a genetic algorithm to optimize 100×100nm̂2 etchings on a predetermined grating waveguide topology, allowing for either the selective guidance of peak pulse intensity of the output or the augmentation of true time delay within the identical unit length. Within the chosen predetermined topology, the optimal configuration was identified based on the properties of the signal excitation in the time domain. This approach significantly facilitates the application-specific selection of peak intensity decay rate and time delay behavior within a 1D grating waveguide system. © 2024 SPIE., Scientific and Technical Research Council; TUBITAK, (2210A, 122E566)

However, the use of bulky power splitters can occupy a significant amount of space on the chip. A silicon lens is utilized, which also allows a time-domain steering range between ±30° and is custom-developed as an extended-hemispherical lens, and the elements of the antenna array consist of broadband bowtie antennas. Abstract: In this study, simple, fast and reliable cyclic voltammetry, linear sweep voltammetry, square wave voltammetry and differential pulse voltammetry methods were developed and validated for determination of etodolac in pharmaceutical preparations. The proposed methods were based on electrochemical oxidation of etodolac at platinum electrode in acetonitrile solution containing 0.1 M lithium perchlorate. The well-defined oxidation peak was observed at 1.03 V. The calibration curves were linear for etodolac at the concentration range of 2.5-50 µg/ml for linear sweep, square wave and differential pulse voltammetry methods, respectively. Intra- and inter-day precision values for etodolac were less than 4.69, and accuracy (relative error) was better than 2.00%. The mean recovery of etodolac was 100.6% for pharmaceutical preparations. No interference was found from three tablet excipients at the selected assay conditions. Developed methods in this study are accurate, precise and can be easily applied to Etol, Tadolak and Etodin tablets as pharmaceutical preparation. [ABSTRACT FROM AUTHOR]

26. EFFECTS OF LIGHTWEIGHT AGGREGATES ON AUTOGENOUS DEFORMATION IN CONCRETE.

Abstract: To reduce autogenous deformations in high performance concrete at early ages, dispersed saturated lightweight aggregates (LWAs) are used as water reservoirs. For this purpose, in a concrete with low water cement ratio, normal aggregates have been replaced by natural LWAs at size fractions of 2-4mm or 4-8mm at three different volume fractions such as 10, 20 and 30 percent of total aggregate volume of concrete prepared at a constant low water/cement ratio. Effects of volume fraction and average particle size of LWAs on the load-displacement at mid-span curve are investigated by measuring the fracture energy, the characteristic length and final displacement. The results indicate that the inclusion of fine fraction of LWAs in the concrete reduces the autogenous deformation significantly compared to that of the coarse LWA fraction. It is also shown that the fracture energy, final displacement at the mid-span curve of the beam, splitting tensile strength and compressive strength of concrete with fine fraction are higher than those of the concrete with coarse fraction. Increasing the replacement ratio of LWAs mitigates autogenous deformation, while having an unfavorable effect on fracture and mechanical properties of concrete for both fine and coarse fraction replacements. Abstract: Objectives: Hormone replacement therapy (HRT) frozen embryo transfer (FET) cycles are common in assisted reproductive techniques. As the corpus luteum is absent in these cycles, luteal phase support is provided by administering progesterone (P4) through transvaginal, parenteral, or oral routes. Low serum levels of P4 (below 9-10 ng/mL) on the day before embryo transfer (ET) have been associated with unfavorable cycle outcomes. The aim of this study is to investigate whether individualizing luteal support through rescue protocols in patients with low serum P4 levels improves pregnancy outcomes in HRT-FET cycles., Material and Method: This retrospective, single-center cohort analysis includes 1257 cycles involving 942 patients undergoing HRT-FET. Starting in 2019, we have assessed P4 levels before ET day and adjusted MVP doses when P4 levels were <10 ng/mL. In 2021, subcutaneous (SC) P4 was routinely added alongside MVP, with SC doses increased if P4 levels were <10 ng/mL. In this study, Groups 1 and 2 received MVP for luteal support, while Groups 3 and 4 received additional SC progesterone. For patients with P levels below the cut-off level (10 ng/mL) in Groups 2 and 4, the P dose was doubled through a rescue protocol., Results: In the MVP and MVP plus SC groups, 15.8% and 8.9% of the cycles had P4 levels <10 ng/mL, respectively. Ongoing pregnancy rates (OPR) and clinical pregnancy rates (CPR) did not differ between study groups. Regression analysis with a mixed model revealed that age, endometrial thickness, and estradiol levels were confounding factors as well as independent predictors of ongoing pregnancy rates (p<0.05). Pairwise regression analysis revealed no significant differences in pregnancy rates between the groups (p>0.05)., Conclusion: Individualizing luteal phase support based on serum P4 levels on the day of ET in FET cycles with HRT may enhance pregnancy outcomes by either doubling the vaginal dose or increasing the SC dose during MVP plus SC administration. Abstract: Background Lowering fresh gas flow (FGF) can help decrease the carbon footprint of the operating room as FGF levels act as an indirect measure of anesthetic gas waste. Objective The aim of this quality improvement project was to reduce clinician FGF during general anesthesia with clinical decision support (CDS) tools within the electronic health record (EHR) at a single institution. Methods A non-interruptive alert to reduce FGF was coded into the anesthesia intraoperative EHR workspace to alert whenever the 10-minute average FGF exceeded 1 L/min. It was targeted at anesthesia residents, attendings, and certified registered nurse anesthetists at a single US large academic level 1 trauma center. The number of general anesthesia cases with a target FGF of ≤2 L/min and the amount of sevoflurane (L/hr) was tracked on an individual and institutional basis. Results Following CDS implementation from July 2023 through July 2024, 2677 of 4573 (58.5%) had a mean FGF ≤2 L/min, demonstrating a 116.7% increase from our institution's baseline of 27.0% (1200 of 4446 cases) from July 2022 to June 2023, corresponding to a sevoflurane usage reduction of 36.7%. Conclusions Implementing a non-interruptive alert in the EHR altered institution-level behaviors to reduce environmentally harmful anesthetic gas emissions.

31. Comparison of the Mitophagy and Apoptosis Related Gene Expressions in Waste Embryo Culture Medium of Female Infertility Types.

Abstract: Mitochondria is an important organelle for the oocyte-to-embryo transition in the early embryonic development period. The oocyte uses mitochondria functionally and its mitochondrial DNA (mtDNA) content as the main energy source in the embryo development at the preimplantation stage. The aim of this study is to compare mitophagic, apoptotic and humanin gene expressions from the culture medium fluid in which embryos are developed and monitored among normoresponder (NOR), polycystic ovary syndrome (PCOS), young and older patients with poor ovarian reserve (POR). The study groups consisted of infertile patients who applied to the Bahçeci Umut IVF Center as NOR (Control), PCOS, POR-Advanced (POR-A) and POR-Young (POR-Y). After the isolation of total RNA from the collected samples, MFN1, MFN2, PINK1, PARKIN, SMN1, SMN2, p53 and Humanin gene expressions were determined by Real Time-PCR. The average age of only the POR-A was determined to be higher than the NOR ( p < 0.001). The MFN1, SMN2 ( p < 0.05), Humanin and p53 gene expressions ( p < 0.001) increased, while PINK1 gene expression decreased ( p < 0.05), in the POR-Y compared to the NOR. A decrease in MFN2, PARKIN ( p < 0.05) and PINK1 gene expressions was determined in the PCOS compared to the NOR ( p < 0.001). Furthermore, a decrease was observed in MFN2, PINK1 ( p < 0.001) and Humanin gene expressions compared to the NOR ( p < 0.05). The current data are the first in the literature determining the apoptotic and mitophagic status of the oocyte. The current results prove that waste embryo culture fluid may provide a non-invasive profile for important cellular parameters such as mitochondrial dysfunction in female infertility. The evaluation of significant cellular parameters can be performed much earlier without any intervention into the embryo.

Abstract: Purpose: The main purpose of this study is to compare the validity of transcervical embryoscopy method with standard uterine evacuation method in detecting more accurate karyotypes in miscarriages below tenth week of pregnancy. Additionally, the frequency and distribution of fetal morphological abnormality were evaluated., Methods: A prospective study was carried out at the Gazi University Faculty of Medicine, Department of Obstetrics and Gynecology. Patients with missed abortions between sixth and tenth gestational weeks were included in the study group, and fetal morphological examination and direct embryonic biopsy were performed by transcervical embryoscopy. The control group consisted of patients who experienced miscarriage and genetic material obtained from routine uterine evacuation between February and October 2023., Result: A total of 60 patients in the study group and 189 patients in the control group were evaluated. The median ages, previous miscarriage numbers, median gravida numbers, and median gestational weeks were comparable between groups. Chromosomal abnormality was detected in 24 (42.8%) and 52 embryos (29.9%) in the study and control groups, respectively (p = 0.004). Culture failure rates were 6.6% (n = 4) and 7.9% (n = 15) in the study and control groups, respectively. In the study group, 12 embryos had a morphological abnormality in which 6 of them had normal karyotype., Conclusion: Direct embryonic biopsy with transcervical embryoscopy is an effective method to exclude maternal decidual cell contamination and placental mosaicism in miscarriages for karyotype analysis. 33. Physical and well-being effect of scapular kinesio taping combined with conventional physiotherapy in shoulder impingement syndrome: A randomized controlled study.

Abstract: Background: Kinesio taping (KT) is one of the treatment methods used on patients with shoulder impingement syndrome (SIS). There are different results regarding its effectiveness in the literature., Objective: To investigate the effects of scapular KT combined with a conventional physiotherapy program on scapular dyskinesia, shoulder pain, upper extremity function, and well-being in patients with SIS., Methods: The study was conducted with 60 outpatients diagnosed with SIS, aged 40-65 years. The patients were divided into two groups: KT [conventional physiotherapy program + scapular KT (targets scapular retraction and is applied along the inferior-medial edge of the scapula, starting from the processus coracoids), n= 30] and control [conventional physiotherapy program, n= 30]. In before- and after-treatment evaluations, the Lateral Scapular Slide Test (LSST) for scapular dyskinesia, a Visual Analogue Scale (VAS) for shoulder pain, and the Disabilities of the Arm, Shoulder, and Hand (DASH) for upper extremity function were used. In addition, at the end of treatment, a Kinesio taping Satisfaction Survey, created by the researchers, was filled out by the KT group for the assessment of well-being., Results: The interaction effect of Group*Time was not statistically significant in all outcome measures (p> 0.05). However, the main effect of both group and time was statistically significant in the DASH-Function/Symptom, VAS-Rest, VAS-Activity, and VAS-Night (p< 0.05). Moreover, only the main effect of time was statistically significant in LSST-1 and LSST-3 (p< 0.05). In the KT group, the satisfaction level was 8.50 ± 1.69 and the recommendation level was 8.72 ± 1.81., Conclusion: Both conventional physiotherapy programs and additional scapular KT improved scapular dyskinesia, reduced pain, and increased the upper extremity function. Adding scapular KT to treatment did not change the results, but it had positive psychological effects and yielded a high satisfaction rate.

Abstract: Assisted reproduction technology has two significant problems: low success rates and multiple pregnancies. Because of these problems, the priority in IVF clinics is to develop a potential diagnostic test that can be used to select the embryos with the ultimate developmental competence. Aneuploidy screening as embryo selection criteria will ensure that the transferred embryos are euploid and high implantation rate. We hypothesize that aneuploidy in human preimplantation embryos could be discriminated by their amino acid metabolism profile in the spent culture media. Preimplantation genetic testing for aneuploidy results and spent embryo culture medium amino acid content were analyzed for 58 couples. The next-generation sequencing technique was used and coupled with TE biopsy. Forty euploid and 71 aneuploid blastocysts were evaluated. Embryos were cultured individually until day 5 or 6 of embryo development. Spent culture medium was collected after finishing the culture. There was no statistical difference between D3 and D5 embryo morphology between euploid and aneuploid embryos ( p > .05). Eight amino acids, including SER, GLY, HIS, ARG, THR, ALA, PRO, and TYR, were detected in the culture medium from the blank control group, euploid group, and aneuploid group. Only TYR amino acid concentration was found significantly higher in the aneuploid group compared to the euploid group ( p < .003). Tyrosine amino acid levels equal to and above 76.38 µmol/L could be considered aneuploid. Aneuploid embryos demonstrate altered amino acid turnover in vitro relative to euploid counterparts. A noninvasive method of amino acid profiling will be of value as a tool for routine preimplantation embryo selection among all patient groups.

Abstract: Background: In cardiac tamponade, coronary sinus (CS) as an intrapericardial structure can be easily compressed, whereas inferior vena cava (IVC) dilates. This inverse relationship may augment their roles in the evaluation of tamponade imaging., Aim: We assessed the usefulness of computerized tomographic measures of CS diameter and also CS/IVC ratio to predict tamponade in clinically stable patients with large pericardial effusion., Methods: Sixty-six clinically stable patients who had large pericardial effusions were included. Coronary sinus diameter was measured from the point at 1 cm proximal to the CS ostium. Inferior vena cava diameter was measured from the segment between its right atrial orifice and hepatic vein., Results: Patients with tamponade had smaller CS diameter and CS/IVC ratio. After adjusting with other parameters, only either CS diameter or CS/IVC ratio predicted tamponade, respectively. (Nagelkerke r value for CS was 53.7% and 72.1% for CS/IVC ratio). In Receiver Operating Characteristic Curve analysis, a cutoff value of 6.85 mm for CS diameter had 82.6% sensitivity and 83.7% specificity and a cutoff value of 27% for CS/IVC ratio had 87.0% sensitivity and 86.0% specificity for predicting cardiac tamponade., Conclusions: The tomographic measures of both the CS diameter and the CS/IVC ratio predicted tamponade in clinically stable patients with large pericardial effusion. Compared with CS diameter, CS/IVC ratio seemed to be a more powerful predictor of tamponade.

Abstract: Background: To examine the differences in intraocular pressure (IOP) measurements obtained with Goldmann applanation tonometry (GAT) and Pascal dynamic contour tonometry (DCT) in subjects with thin corneas and to correlate these with structural and functional parameters of glaucoma damage., Patients and Methods: One hundred and thirty participants (130 eyes) with central corneal thickness lower than 500 µm were included in five groups in this cross-sectional observational study: 52 eyes with primary open angle glaucoma (POAG), 19 eyes with normal tension glaucoma (NTG), 27 eyes with ocular hypertension (OHT), 21 participants suspected of having glaucoma (GS), and 11 healthy subjects. The measurements were obtained with GAT and DCT in a masked fashion. The corrected GAT-IOP for central corneal thickness (CCT), the difference between DCT and GAT (ΔIOP = DCT-GAT) and corrected ΔIOP (corrected ΔIOP = DCT-corrected GAT) were calculated. Age, mean deviation (MD) from the most recent reliable visual field examination, average retinal nerve fibre layer thickness (RNFL), cup-to-disc ratio (CDR), ocular pulse amplitude (OPA), and treatment status were recorded for statistical analysis., Results: In all of the subjects (n = 130), the mean DCT, GAT and corrected GAT values were 17.6 ± 3.4 mmHg, 13.2 ± 3.8 mmHg, and 15.5 ± 2.1 mmHg, respectively. ΔIOP and corrected ΔIOP were 4.4 ± 2 mmHg and 1.7 ± 2 mmHg, respectively. ΔIOP was correlated negatively with MD (rs = -0.32, p < 0.0001) and average RNFL thickness (r = -0.46, p < 0.0001) and positively with CDR (rs = 0.50, p < 0.0001). The mean IOP measured by GAT differed statistically significantly between eyes with glaucoma (n = 71, POAG and NTG) and eyes without damage (n = 59; OHT, GS, and healthy) (p < 0.0001), whereas the mean IOP by DCT did not (p = 0.935). The mean ΔIOP values were also statistically significantly higher in the glaucomatous group, with and without correction for CCT (p < 0.001). OPA and glaucoma diagnoses statistically significantly predicted ΔIOP (R(2) = 0.41, F2.127 = 47.46, p < 0.0001)., Conclusion: Greater underestimation of IOP by GAT was observed in the glaucomatous eyes with thin corneas. Abstract: Background: An adequate pool of free intracellular cholesterol is essential for steroidogenesis in gonads and LDL is the major source of cholesterol used in this pathway. Effect of peripheral LDL on the synthesis of steroids is dose dependent and although LDL levels around 100 mg/dl is demonstrated to be safe in terms of steroidogenesis, effect of LDL levels <70 mg/dl with higher doses of statins on steroidogenesis remains controversial., Material and Methods: Androgen and gonadotropin levels are prospectively evaluated at baseline and after 12 weeks of treatment in 77 male coronary heart disease patients receiving high doses of atorvastatin (40-80 mg daily) targeting serum LDL levels <70 mg/dl and in 83 male coronary heart disease patients receiving regular doses of atorvastatin (10-20 mg daily) targeting serum LDL levels <100 mg/dl., Results: At the end of the study, mean LDL levels of the high and regular dose atorvastatin groups were 77+/-9 mg/dl and 98+/-10 mg/dl respectively. After twelve weeks of treatment, there were no significant alterations in serum total testosterone, free testosterone, sex hormone binding globulin, luteinizing hormone and follicle stimulating hormone levels between two groups., Conclusion: High dose atorvastatin in order to reach serum LDL levels around 70 mg/dl seems to be as safe as regular doses in order to reach serum LDL levels around 100 mg/dl, in terms of gonadal steroidogenesis in men with coronary heart disease.

Abstract: Scarce data exist on the relationship of C-reactive protein (CRP) or plasminogen activator inhibitor-1 (PAI-1) to the occurrence of heart failure (HF) or cardiogenic shock (CS) after acute myocardial infarction (AMI) and on the relationship between these biomarkers and mortality in CS patients. Thus, we compared high-sensitivity CRP and PAI-1 antigen plasma levels on admission among 3 age- and gender-matched AMI patients groups (consisting of 60 patients with CS, 60 with HF, and 60 without HF on admission), after determining that PAI-1 levels did not vary significantly diurnally in these groups by comparing the data among subgroups which were divided according to admission time within the groups. For CS patients, we also conducted regression analyses to examine the relations of these biomarkers to mortality. CRP levels both in CS (P < 0.001) and HF (P < 0.05) patients were significantly higher compared to those without HF, PAI-1 levels in CS patients were significantly higher compared to both those with (P < 0.05) and without HF (P > 0.01), and CRP and PAI-1 were independent predictors of in-hospital (Odds ratio [OR] = 6.12, 95% confidence intervals [95%CI] = 1.47-25.54 and OR = 5.92, 95%CI = 1.31-26.77, respectively) and 1-year mortality (OR = 5.53, 95%CI = 1.21-25.17 and OR = 5.48, 95%CI = 1.09-27.52, respectively) in CS patients. In conclusion, at admission, CRP is associated with the occurrence of CS and HF and PAI-1 is associated with the occurrence of CS after AMI, and they are of prognostic value in CS complicating AMI.

Abstract: Background: Coronary slow flow (CSF) phenomenon is characterized by delayed opacification of coronary vessels in a normal coronary angiogram. Although clinical and pathological features have been previously described, the underlying pathophysiology has not been fully elucidated. We aimed to determine and compare the carotid intima-media thickness (CIMT) values of SCF patients and healthy subjects, and to correlate patients' values with intimal thickness and TIMI frame counts of their coronary arteries (assessed by intravascular ultrasound)., Methods: The study population consisted of 50 patients with SCF [38(76%) male, aged 53+/-7 years] and 40 normal subjects [22(55%) male, aged 51+/-8 years]. CIMT values, intravascular ultrasonographies and TIMI frame counts of the patients, and CIMT of the controls were investigated., Results: Upon intravascular ultrasonography investigation, the common finding was longitudinally extended massive calcification throughout the epicardial coronary arteries in 44 (88%) of patients. Mean coronary intimal thickness was 0.52+/-0.1mm. CIMT was significantly increased in SCF patients than controls (0.84+/-0.14 vs. 0.66+/-0.13, p<0.0001). Besides, CIMT was significantly correlated with coronary intima-media thickness. TIMI frame counts were positively correlated with CIMT and coronary intimal thickness (p<0.05 and p<0.05 respectively)., Conclusion: These results support the theory that SCF is the result of coronary atherosclerosis which is the part of systemic involvement.

41. Direct stenting versus predilatation and stenting technique when using paclitaxel-eluting stents.

Abstract: Direct stenting without predilatation is a well-defined, feasible method with bare metal stents. Direct stenting has also been shown to be safe and feasible with drug-eluting stents, however, there is much less evidence with this type of device when compared with bare metal stents. Three hundred and sixty-four coronary lesions in 257 consecutive patients (mean age, 57.4 +/- 9.8 years; 63 women) who had undergone elective stenting either with or without predilatation via a paclitaxel-eluting stent between March 2003 and March 2006 were retrospectively analyzed. Quantitative coronary angiography analysis was compared between the two groups of procedures; stenting with predilatation and direct stenting. All procedures were uneventful. No deaths occurred during the follow-up period. Direct stenting when compared with the predilatation technique, significantly decreased both procedure time (32.1 +/- 17.9 minute versus 41.2 +/- 18.6 minute, P < 0.0001) and fluoroscopy time (10.6 +/- 7.8 minute versus 15.5 +/- 7.6 minute, P < 0.0001). There was no difference in quantitative analysis parameters of coronary angiography during follow-up or the rates of stent thrombosis and restenosis. Direct stenting seems to be as feasible and safe as conventional predilatation and stenting in selected cases. Direct stenting seems to decrease procedural time and radiation exposure without any negative effect on quantitative analysis parameters of coronary angiography with approximately a one-year follow-up period.

Abstract: Objectives: To investigate the late outcomes of sirolimus-eluting stent implantation in patients with coronary artery disease., Background: Drug-eluting stents reduce intimal hyperplasia, which is the main cause of in-stent restenosis. Sirolimus-eluting stents significantly reduce clinical and angiographic restenosis and improve event-free survival., Methods: The study population consisted of 207 patients (273 stents) who had undergone coronary Cypher stent implantation. Patients were eligible for enrollment if there was symptomatic coronary artery disease or positive exercise testing, and angiographic evidence of single or multivessel disease with a target lesion stenosis of > or = 70% in a > or = 2.25 mm vessel. Follow-up coronary angiography was performed 18 months after stent deployment. Patients were followed-up for a mean of 24.7 +/- 7.4 months., Results: All patients survived after stent implantation, but 5 (2.4%) patients experienced acute ST elevation myocardial infarction and 4 (1.9%) patients developed non-Q wave myocardial infarction following angioplasty. Recurrent angina pectoris was observed in 16 (7.7%) patients (11 stable angina pectoris and 5 unstable angina pectoris). Angiographic evidence of restenosis was observed in these 20 (9.66%) patients. The 5 other patients had noncritical angiographic restenosis. Eleven (5.3%) patients underwent angioplasty because of restenosis, and coronary artery bypass grafting was conducted in the other 9 (4.3%) patients., Conclusion: The results of the present study indicate that Cypher stents could be implanted with a very high success rate and have encouraging long-term angiographic and clinical results.
