This study aimed to evaluate intercellular bridges in the depth of invasion (DOI) measurement area as prognostic factors in oral squamous cell carcinoma (OSCC).The mode of invasion was determined based on the Yamamoto-Kohama classification system by observing the hematoxylin-eosin-stained whole-slide images of specimens obtained from 78 patients with OSCC, and the clinicopathologic features were characterized. The presence of intercellular bridges was analyzed in 46 patients with Yamamoto-Kohama classification grade ≥3 whose DOI was measured by dividing the measurement area into 3 parts: the surface, center, and front of the tumor.Univariate analyses identified lymph node metastasis, loss of intercellular bridges in the DOI measurement area, DOI of ≥4500 µm, and pattern of invasion 4C-4D as negative prognostic factors. Multivariate analyses revealed that lymph node metastasis and the loss of intercellular bridges in the entire area were independent factors, with hazard ratios of 9.34 (95% confidence interval, 2.09-42.03; P = .003) and 3.64 (95% confidence interval, 1.10-11.99; P = .045), respectively.Loss of intercellular bridges in the DOI measurement area is a negative prognostic factor for OSCC and may be useful in selecting treatment.

This study aimed to evaluate and predict the factors that affect the postoperative maxillary sinus and inferior nasal cavity conditions after Le Fort I osteotomy.The rate of aerated (healthy) sinus area and the inferior nasal airway area were calculated by preoperative computed tomography measurements and at 1 week and 1 year postoperatively. Factors predicting the rate of aerated sinus area and the ratio of the inferior nasal cavity area after 1-week were examined using these variables.The subjects consisted of 112 patients (224 sides) who underwent Le Fort I osteotomy with sagittal split ramus osteotomy. The rate of aerated sinus area after 1 week was significantly correlated with sex, age, and amount of blood loss ( P0.0001). The rate of the inferior nasal cavity area after 1 week was significantly correlated with the anterior impaction amount of the maxilla and the preoperative rate of the inferior nasal airway area ( P= 0.0017).This study suggests that attention should be paid to females, older patients, and the amount of blood loss to prevent sinusitis, and larger maxillary impaction at the anterior site should be carefully planned to prevent severe nasal obstruction immediately after Le Fort I osteotomy.

The purpose of this study was to investigate whether differences in the pattern of the lingual plate split in sagittal split ramus osteotomy (SSRO) affect the remodelling of the split site. Sixty-one patients with mandibular prognathism (122 sides) underwent SSRO. Computed tomography (CT) was performed at 1 week and 1 year after SSRO. Bone splits were classified according to the lingual split scale (LSS) and the lateral bone cut end (LBCE) by evaluating CT images at 1 week. The remodelling at the split sites was evaluated by superimposing the CT images obtained at 1 week and 1 year. Regarding the LSS pattern, significant differences were observed in the distance between anteroposterior ramus points (P = 0.033) and the ramus area in the axial image (P = 0.011). The LBCE pattern also showed a significant difference in the distance between anteroposterior ramus points (P = 0.043). In conclusion, the differences in the lingual plate split and ramus cut end of the SSRO influence the postoperative remodelling in the anteroposterior region of the split site.

Objective The Yamamoto–Kohama criteria are clinically useful for determining the mode of tumor invasion, especially in Japan. However, this evaluation method is based on subjective visual findings and has led to significant differences in determinations between evaluators and facilities. In this retrospective study, we aimed to develop an automatic method of determining the mode of invasion based on the processing of digital medical images. Study Design Using 101 digitized photographic images of anonymized stained specimen slides, we created a classifier that allowed clinicians to introduce feature values and subjected the cases to machine learning using a random forest approach. We then compared the Yamamoto–Kohama grades (1, 2, 3, 4C, 4D) determined by a human oral and maxillofacial surgeon with those determined using the machine learning approach. Results The input of multiple test images into the newly created classifier yielded an overall F-measure value of 87%, (Grade 1: 93%, Grade 2: 67%, Grade 3: 89%, Grade 4C: 83%, Grade 4D: 94%). These results suggest that the output of the classifier was very similar to the judgments of the clinician. Conclusions This system may be valuable for diagnostic support to provide an accurate determination of the mode of invasion.

Background Bilateral sagittal split osteotomy (BSSO) is one of the most frequently used treatments for jaw deformity worldwide. However, lower lip hypoesthesia is a postoperative complication of BSSO. Trigeminal somatosensory evoked potential can be used to evaluate neural pathway abnormalities by measuring latency. The purposes of this study were to measure latency before and after BSSO and to examine the relationship between latency and the duration of hypoesthesia recovery. Study Design and Methods This observational retrospective case-control study analyzed data recorded from 2013 to 2018. We divided the patient data into 5 groups according to recovery time. Using the latency obtained in trigeminal somatosensory evoked potential as the main outcome, we examined the relationship between hypoesthesia recovery time and latency. Results The group with unresolved postoperative hypoesthesia 6 months after surgery had significantly greater latency values extension than the group without postoperative hypoesthesia. Conclusion By measuring the degree of prolongation of latency before and after surgery, it is possible not only to indicate the presence of hypoesthesia but also to predict the hypoesthesia recovery period.

Purpose. The purpose of this study was to examine the location and morphology of sphenoid sinus, pterygoid plate and great palatine canal at the pterygomaxillary region in Le Fort I osteotomy. Method. The subjects consisted of 144 Japanese patients with jaw deformity patients. Lateral cephalograms and computed tomography (CT) were analyzed for all patients. The measurements of the sphenoid sinus, pterygoid plate and great palatine canal were assessed using 2 bilateral sagittal planes (right and left) involving the great palatine canals and mid-sagittal plane involving ANS and PNS. Results. The subjects were divided into three groups (47 class II female, 62 class III female and 35 class III male). Sphenoid sinus existed on the sagittal plane involving the great palatine canal in 81 of 94 sides (86.2%) in class II female, 85 of 124 sides (68.5%) in class III female and 57 of 70 sides (81.4%) male. The distribution was significant difference (P = 0.0386). There was significant difference in the right maxillary sinus length (ML) between class II female and class III female (P = 0.0049). Ramus inclination in cephalometric measurements was selected as a variable related to the vertical distance between the sphenoid sinus wall and PNS (VDS) by stepwise regression analysis (P = 0.003 right, P = 0.0210 left). Conclusions . These results suggest that the sphenoid sinus exist more frequently recognized close to superior site of the pterygoid plate in class II female. However, the location of the sphenoid sinus at the pterygomaxillary junction region could not be predicted accurately using cephalometric measurements analysis.

Background/purpose Dynamic stimulation can induce bone and cartilage growth. The purpose of this study was to examine the effect of low-intensity pulsed ultrasound (LIPUS) on injured temporomandibular joints (TMJs) in a rabbit model. Materials and methods Twenty-four female Japanese white rabbits (age: 12–16 weeks, weight: 2.0–2.5 kg) were equally divided into 4 groups. In two groups, discectomy was performed with (the LD group) and without (the D group) subsequent LIPUS treatment. In the other groups, a sham operation was performed with (the LC group) and without (the C group) subsequent LIPUS treatment. Two animals in each group were sacrificed at each time point (2, 4, and 8 weeks postoperatively). Mandibular measurements were made using three-dimensional computed tomography. We performed histological and immunohistochemical examination of the articular disc, and the cartilage layer and bone at the 30- and 60-degree sites in each condyle. Results There were no statistically significant differences among the groups in terms of thickness of the disc or the fibrous articular zone, or the number of BMP-2 positive cells. In terms of mandibular length, there were differences among the groups after 4 (P = 0.0498) and 8 weeks (P = 0.0260). Specifically, there was a difference between the LC group and the C group after 4 weeks (P = 0.014) and 8 weeks (P = 0.029). Conclusions This study suggests that LIPUS has little effect on cartilage after TMJ injury. It may promote bone growth in a normal TMJ, although discectomy seems to reduce this effect.

The purpose of this study was to evaluate the association between the immunohistochemistry (IHC) of protein tyrosine kinase 7 (PTK7) expression and clinicopathological factors of oral squamous cell carcinoma (OSCC).Tissue specimens were obtained from 80 patients with primary OSCC. IHC scoring was conducted according to the rate of positive cell and staining intensity. We used the IHC score to classify the degree of PTK7 expression and evaluate clinicopathological factors and prognosis.The number of the high expression group (IHC Score 2 or 3) was 45 cases and that of the low expression group (IHC Score 0 or 1) was 35 cases. A significant difference between high expression and low expression groups was found in the N category (p = .008), degree of differentiation (p .001), and pattern of invasion (p .001). In accordance with the exacerbation of OSCC with respect to three parameters (N category, degree of differentiation, and pattern of invasion), the ratio of high expression of PTK7 increased. The overall 5-year survival rate was 59.3% in the high expression group and 87.3% in the low expression group (p .05). The pathological prognostic signs affecting overall survival were evaluated by univariate analysis and multivariate analysis with Cox proportional hazards model and showed an association with lymph node metastasis and invasion patterns.This study suggests that a high IHC score of PTK7 is a potential biomarker for predicting potential metastasis.

Surface modification of various materials using ultraviolet (UV) irradiation improves their wettability. The purpose of this study was to investigate the wettability of a β-tricalcium phosphate (TCP) surface and the composition changes and bioactivity of β-TCP after UV irradiation. We applied 172 nm UV treatment to a β-TCP surface and measured the contact angle before and after UV irradiation. Energy-dispersive X-ray and Fourier transform infrared spectroscopy examinations were performed on the β-TCP disk with or without UV treatment. In an adhesion test of bone marrow cells using β-TCP disks with and without UV irradiation, cell attachment was measured 10, 30, 50, and 70 h after β-TCP insertion. UV-irradiated β-TCP osteogenesis and absorption of bone substitutes were evaluated using hematoxylin and eosin and tartrate-resistant acid phosphatase (TRAP) staining in a rabbit sinus model. The contact angle on the TCP surface decreased from 70° to 10° owing to UV irradiation. Conversely, UV irradiation did not change the composition of carbon, oxygen, and phosphorus. In the cell adhesion test, UV-irradiated β-TCP significantly increased cell adhesion compared with UV-unirradiated β-TCP after 10 to 30 h of culture. In the rabbit sinus model, TRAP staining showed that UV-irradiated β-TCP significantly increased the number of TRAP-positive cells compared with unirradiated β-TCP granules in the central part of β-TCP. Our results indicate that the UV irradiation of β-TCP improves its clinical utility for surgical bone augmentation in the oral and maxillofacial region.

Prophylactic antibiotic administration aims to maintain the postoperative wound environment. It is difficult to select appropriate prophylactic antibiotics to minimize the development of antibiotic resistant bacteria and the occurrence of complications. The purpose of this study was to compare the prophylactic effect of narrow spectrum antibiotics (cefazolin) with that of wide spectrum antibiotics (piperacillin) in preventing infection following third molar extraction. In addition, the effect of postoperative cefazolin administration was examined. This retrospective study included 350 patients who underwent third molar removal under general anesthesia. The patients were divided into three subgroups: preoperative cefazolin (N = 122), pre- and postoperative cefazolin (N = 101), and pre- and postoperative piperacillin (N = 127). The patients in the piperacillin group were administered the antibiotic preoperatively and postoperatively for 3 days. The patients in the preoperative cefazolin group were administered cefazolin preoperatively only. The patients in the pre- and postoperative cefazolin group were administered cefazolin preoperatively and postoperatively for 1 day. Surgical site infections (SSIs) were identified based on the Clavien-Dindo 30-day postoperative classification. There was a significant difference among the three groups of patients who had third molars classified as position C using the Pell and Gregory classification, according to the degree of impaction of the impacted third molar (P = 0.015). Our analysis showed that the number of SSI did not significantly differ between the three antibiotic treatment groups (P = 0.671). These results suggest that preoperative administration of cefazolin is as effective as postoperative administration of broad-spectrum antibiotics such as piperacillin.

The purpose of this study was to compare bone healing after Le Fort I osteotomy with sagittal split ramus osteotomy (SSRO) in class II and class III patients.The subjects consisted of 46 Japanese class II and III patients. Le Fort I osteotomy was performed in combination with SSRO. They were divided into two groups (23 class II cases and 23 class III cases). Four absorbable plates (uncalcined and unsintered hydroxyapatite and poly-l-lactic acid - uHA/PLLA) and screws were used to fix the maxillary segment in all patients, in the same manner. Postoperative computed tomography (CT) was analyzed for all patients at 1 week and 1 year postoperatively. The anterior and lateral areas between the maxillary segments were measured using two-dimensional frontal and lateral views from the three-dimensional images reconstructed over a constant CT value.There were no significant differences in the area of bone defect healing between classes II and III at 1 year postoperatively, although there were significant differences between the two groups at 1 week postoperatively (p 0.05). Furthermore, there were no significant differences in the areas of bone defect between 1 week and 1 year postoperatively, in both groups.Using measurements based on CT value threshold within 1 year after Le Fort I osteotomy, this study suggests that the areas of bony defect in the region of the anterior and lateral walls of the maxilla do not always decrease in both classes II and III.

Purpose This study was performed to evaluate changes in nasal airflow, nasal airway resistance, nasal cross-sectional area, pharyngeal horizontal area, nasopharyngeal and oropharyngeal volume following Le Fort I osteotomy (L1) impaction with sagittal split ramus osteotomy (SSRO) in classes II and III. Materials and methods The subjects consisted of 35 patients (6 males and 29 females, 70 sides) 17 of which were diagnosed as class II and 18 as class III who underwent L1 and SSRO. Nasal airflow and resistance were measured using the rhinomanometry system (GM NR-6 EXECUTIVE) before and at 1 and 6 months after surgery. Nasal, cross-sectional area, and volume were measured using a 3-dimensional computed tomography respectively, before and 1-year after surgery. Results Although a significant decrease was found in nasal volume after surgery (P = 0.0042), there was no difference between before and after surgery in the nasal airway resistance in class II.A significant decrease in nasal volume was found after surgery (P = 0.0005) and there were no postoperative changes in both nasal airflow and resistance in class III. Conclusion The study suggested that L1 impaction with SSRO did not worsen nasal function such as nasal airflow and nasal airway resistance, although nasal volume significantly decreased in both groups.

The present report describes a case series in which spacers with boluses were used at various sites in the oral cavity to enhance the therapeutic effect of radiation therapy in oral squamous cell carcinoma. In radiotherapy, the surface dose is reduced due to the build-up region of X-rays. In the present study, a bolus was used to complement the build-up region and increase the surface dose effect. A total of 7 patients with oral cancer from a primary care hospital underwent radiation therapy using spacers and added boluses to improve the surface dose effect. The spacer was made from a plastic splint and the bolus was connected to the splint with a quick self-curing resin. There were no complaints of pain or adverse events from the patients while wearing the intraoral splint. A total of 2 of the 7 patients were subsequently confirmed as having progressed disease, and the remaining 5 are currently being managed following a complete response to treatment. The spacers used at various sites of oral squamous cell carcinoma were safe and effective and did not cause any severe adverse effects.

The aim of the study was to examine lateral pterygoid muscle (LPM) and temporomandibular joint (TMJ) disc before and after Le Fort I osteotomy with and without intentional pterygoid plate fracture and sagittal split ramus osteotomy (SSRO) in class II and class III patients. Le Fort I osteotomy and SSRO were performed in class II and class III patients. LPM measurements using oblique sagittal computed tomography (CT) images and TMJ disc position using magnetic resonance imaging (MRI) were examined. Statistical comparisons were performed for the LPM and TMJ between class II and class III patients and between those with and without intentional pterygoid plate fracture in Le Fort I osteotomy. The subjects comprised 60 female patients (120 sides), with 30 diagnosed as class II and 30 as class III. Preoperatively, the width of the condylar attachment, width at eminence, length of the LPM, angle of the LPM, and square of the LPM were significantly smaller in the class II group than in the class III group (p 0.05). After 1 year, the width of the condylar attachment, width at eminence, and angle of the LPM remained significantly smaller in the class II group than in the class III group (p 0.0001). TMJ disc position was significantly related to the width of the condylar attachment of the LPM, both pre- and postoperatively (p 0.0001). However, postoperative disc position did not change in all patients. Next, the class II patients (60 sides) were divided into two groups who underwent Le Fort I osteotomy with or without intentional pterygoid plate fracture. Changes in all measurements of the LPM showed no significant differences between these two groups. Our study suggested that TMJ disc position classification could be associated with the width of condylar attachment of the LPM before and after surgery, while the surgical procedure, including Le Fort I osteotomy with intentional pterygoid plate fracture, might not affect postoperative LMP or disc position in class II patients.

Background: Gram-negative bacteremia is a major cause of death among hematology inpatients who require heavy-dose chemotherapy and hematopoietic stem cell transplantation. Gram-negative bacillus (GNB) is more likely to be detected when the oral health is poor. However, there is a dearth of studies on the relationship between oral assessment and prevalence of GNB in hematology inpatients.Methods: This prospective study aimed to evaluate the relationship between the original point-rating system for oral health examinations (point-oral exam) and the prevalence of GNB in hematology inpatients at the hematology ward of the Yamanashi University Hospital. GNB was detected by cultivating samples from the sputum and blood of each patient.Results: A total of 129 subjects underwent a medical checkup and point-oral exam. The sputum and blood culture results of 55 patients were included in this study. The total points of patients positive for GNB (n = 25, 45.5%) were significantly higher than those who were negative for GNB (total score: median, 25th, 75th, percentile; 6 (4, 7) vs 2 (1, 4); p = 0.00016). Based on the receiver operating characteristic analysis, a cutoff score of 5 proved to be most useful to detect GNB.Conclusion: An oral evaluation with a cutoff value of 5 or higher in the point-oral exam might indicate the need for a more thorough oral management to prevent the development of systemic infections from GNB.

This study aimed to assess mandibular morphologic changes to the condyle, ramus, mandibular body, and inferior mandibular border defect after sagittal split ramus osteotomy in class II and III patients.The relationships among the condyle, ramus, and mandibular body measured by computed tomography preoperatively and postoperatively were assessed and factors related to the reduction of the condylar square and mandibular inferior border defect were examined.Patients included 72 female patients with jaw deformity (36 skeletal class II cases, 36 skeletal class III cases). Postoperative reduction of the condylar square was significantly correlated with preoperative condylar height in patients with class II (P = .0297) vs preoperative condylar height and preoperative mandibular height in patients with class III (P.0001). A mandibular inferior border defect was found in 18 of 72 class II sides (25.0%) and was significantly related to the position of the osteotomy line and attachment side of the inferior border cortex (P.0001).This study's findings suggest that the postoperative reduction of the condyle could be associated with preoperative condylar height. However, the mandibular inferior border defect in class II advancement surgery could be independently associated with technical factors in sagittal split ramus osteotomy.

Resorbable materials are used to fix bony fragments after sagittal split ramus osteotomy (SSRO), but to our knowledge, there is no clear regimen for antibiotic prophylaxis when such materials are used. The purpose of this study therefore was to compare inpatient single-agent and extended dual-agent antibiotic prophylaxis for the prevention of surgical site infections (SSI) in patients after SSRO. This prospective study included 100 patients who underwent SSRO for deformities of the jaw. Cephalometric analysis was performed preoperatively, and at one month and one year postoperatively. Patients were divided into two groups of 50 each. Group A was given cefazolin sodium 1g preoperatively and every eight hours for 48 hours postoperatively. Group B was given cefazolin sodium 1g preoperatively and every eight hours for 48 hours postoperatively, after which they were provided with oral amoxicillin for three days. Both groups were assessed for SSIs for one year postoperatively using the Clavien-Dindo classification. SSIs were documented in seven patients in Group A and six in Group B, with no significant difference between the groups. The two regimens had no significant association with SSI. In conclusion, our results suggest that the inpatient, single-agent, postoperative antibiotic regimen is sufficient to prevent SSI in patients who have SSRO with resorbable plate and screw fixation.

Background Various features are provided in dental implants to improve initial fixation. Purpose To compare the implant stability of the nonself-tapping tapered implant and self-tapping hybrid implant over a 3-month healing period. Materials and methods A randomized controlled trial was conducted. Patients were randomly divided into tapered and self-tapping groups. Patients in the tapered group received NobelReplace Tapered (Nobel Biocare, Sweden) implants, while those in the self-tapping group received NobelSpeedy (Nobel Biocare, Sweden) implants. Implant stability was measured by resonance frequency analysis at surgery and 3 months following implant insertion. Data were analyzed using an independent t-test. Results Forty-three patients (tapered group: 21, self-tapping group: 22) received a total of 88 implants. Initial stability in the tapered group was significantly higher (mean: 60.14, SD: 12.40) than that in the self-tapping group (mean: 54.72, SD: 7.92). Implant stability significantly increased 3 months after implantation in the tapered group (mean: 66.61, SD: 9.00) and self-tapping group (mean: 64.01, SD: 5.78). No significant intergroup difference in implant stability was noted 3 months after surgery. Conclusions The tapered shape affected initial fixation more than the self-tapping function. However, during the second fixation, both implants showed good stability, and the difference disappeared.

This study was performed to evaluate the relationship between condylar height reduction and changes in condylar surface computed tomography (CT) values in jaw deformity patients following orthognathic surgery. Mandibular advancement by sagittal split ramus osteotomy (SSRO) with Le Fort I osteotomy was performed in class II patients, and mandibular setback by SSRO with Le Fort I osteotomy was performed in class III patients. The maximum CT values (pixel values) at five points on the condylar surface and the condylar height, ramus height, condylar square, ramus angle, and gonial angle in the sagittal plane were measured preoperatively and 1 year postoperatively. Disc position was classified as anterior disc displacement (ADD) or other types by using magnetic resonance imaging (MRI). Ninety-two condyles of 46 female patients were prepared for this study. Their temporomandibular joints (TMJs) were divided into two groups based on class (46 joints in class II and 46 joints in class III) and two groups based on the findings (25 joints with ADD and 67 joints with other findings). ADD with and without reduction was observed in two joints in the class III group and in 23 joints in the class II group. The distribution of ADD incidence had not changed 1 year after surgery. Condylar height decreased 1 year after surgery in both class II patients (mandibular advancement) (p < 0.0001) and class III patients (mandibular setback) (p = 0.0306). Similarly, condylar height decreased 1 year after surgery both in patients who showed ADD (p = 0.0087) and those with other types (p = 0.0023). Significant postoperative increases at all angle sites on the condylar surface were found in the class II (p < 0.05) and ADD (p < 0.05) groups. This study showed that an enhanced condylar surface CT value might be one sign of condylar height reduction related to sequential condylar resorption, in combination with ADD.

Purpose The purpose of this study was to examine the effect of injecting basic fibroblast growth factor following surgical induced anterior disc displacement in temporomandibular joints (TMJ). Materials and methods Adult male Japanese white rabbits (n = 16; 2.0–2.5 kg; 10 weeks old) were assigned to experimental and control groups. In the experimental group, anterior disc displacement was induced in the bilateral TMJ. Recombinant human basic fibroblast growth factor (rh bFGF) 0.1 μg/1 μL aqueous solution was injected into the left retro-discal connective tissue close to the disc (ADL group), and saline alone was injected into the same site on the right (ADR group). In the control group, a sham operation without disc position change was performed in the bilateral TMJ (CR group and CL group). Four animals from the experimental (ADR and ADL) and control (CR and CL) groups were sacrificed at 1 and 12 weeks postoperatively to evaluate the mandibular morphology and computed tomographic (CT) value of the condylar head, using 3 dimensional computed tomography. Furthermore, cartilage layers and disc tissue were examined histologically. Results Regarding CT value at the 0° site of the condylar surface, ADR showed the lowest value after 1 week (P = 0.0325). However, there were no significant differences among the 4 groups regarding CT values at the other degree sites after 1 and 12 weeks. Regarding mandibular length, ADR showed the lowest value after 12 weeks (P = 0.0079). In condylar width, ADR showed the lowest value after 1 week (P = 0.0097). Conclusion This study suggested that surgically induced anterior disc displacement could affect condylar morphology in the early stage, and could decrease mandibular length in the late stage. However, bFGF injection into the TMJ might prevent the degenerative change derived from anterior disc displacement and inhibition of sequential mandibular growth.

Purpose The purpose of this study was to examine the relationship between occlusal force and condylar morphology in class II and III after sagittal split ramus osteotomy (SSRO) with Le Fort I osteotomy. Materials and methods The subjects were 42 female patients who underwent bi-maxillary surgery, and were divided into 2 groups (21 class II and 21 class III cases). They were selected randomly from among patients that underwent surgery from 2012 to 2017. Condylar height, ramus height, ramus inclination and condylar square were assessed by computed tomography (CT), preoperatively and 1 year postoperatively. Occlusal contact area and occlusal force were measured pre- and 1, 3, 6 and 12 months post-operative. Results Condylar height and ramus height of the class II advancement cases decrease more significantly than those of class III. There were no significant differences in time-course changes of occlusal force and area between the class II and III. In the class II cases, occlusal force at 1 and 3 months was significantly lower than the preoperative value (P = 0.0009, P = 0.0002). On the other hand, in class III, occlusal force at 1, 3 and 6 months (P = 0.0038, P = 0.0031, P = 0.0283) was significantly lower than the preoperative value. Conclusion This study suggested that occlusal force of the class II advancement cases reached the pre-operative level earlier than that of the class III setback cases, even though condylar height decreased after surgery in the class II cases. However, a correlation between occlusal force change and condylar height reduction after surgery could not be found.

The purpose of this study was to compare mandibular bone healing after advancement or setback surgery using sagittal split ramus osteotomy (SSRO).and Methods. The subjects were 50 patients (100 sides) who underwent bi-maxillary surgery, and were divided into 2 groups (25 class II advancement cases and 25 class III setback cases). They were selected randomly from the patients who underwent surgery between 2012 and 2017. Ramus square, ramus length and ramus width were measured in the horizontal plane image of computed tomography (CT), before and immediately after the operation, and at 1 year after the operation.Ramus square in the class III cases significantly increased in 1 year after the operation (P 0.0001), meanwhile there was no change after 1 year in the class II cases. Before the operation, there were no significant differences in the all measurements between classes II and III. However, for ramus width, class III was significantly larger than class II immediately after (P = 0.0014) and at 1 year after the operation (P = 0.0003).This study suggested that post-operative change in ramus morphology was different between class II advancement surgery and class III setback surgery.

The purpose of this study was to examine changes in lateral pterygoid muscle and temporomandibular joint (TMJ) disc position in classes II and III patients, before and after bi-maxillary surgery. The subjects were comprised of 48 patients (96 sides), 23 of whom were diagnosed as class II and 25 as class III patients who underwent Le Fort I osteotomy and sagittal split ramus osteotomy (SSRO). The cross-sectional measurements of the lateral pterygoid muscles were measured at two levels of horizontal plane images (condyle and mandibular notch levels) by computed tomography (CT), before and 1 year after the operation. The purpose of this study was to examine the changes in the mandibular border movement between class II and class III jaw deformity patients before and after orthognathic surgery, by using the same device.and Methods: Eighty one patients (28 in class II and 53 in class III) who underwent sagittal split ramus osteotomy (SSRO) with Le Fort I osteotomy using absorbable plate fixation and 27 controls with normal occlusion were enrolled. Mandibular border movement (observed using a kinesiograph) was recorded with a mandibular movement measure system (K7) before surgery, and at 6 months and 1 year after surgery. Time-course changes of 5 components of the mandibular border movement (MVO: Maximum vertical opening, CO to MAP: Maximum antero-posterior movement from centric occlusion, MLDL: maximum lateral deviation left, MLDR: maximum lateral deviation right, CO to MO: centric occlusion to maximum opening) were compared between classes II, III and controls statistically. The relationship between lateral cephalometric measurements and the components of mandibular border movement was also examined.There was a significant difference in CO to MAP (P = 0.0025) and CO to MO (P 0.0001) between class II and class III in the time-course change. In class III, mean and standard deviation of MVO were 44.5 ± 6.7 mm before surgery and 39.8 ± 6.8 mm after 1 year. Mean and standard deviation of CO to MAP were 25.2 ± 6.8 mm before surgery and 21.5 ± 7.9 mm after 1 year. Mean and standard deviation of CO to MO were 53.4 ± 9.0 mm before surgery and 47.3 ± 8.4 mm after 1 year. In class II, mean and standard deviation of MVO were 38.8 ± 5.8 mm before surgery and 36.2 ± 7.4 mm after 1 year. Mean and standard deviation of CO to MAP were 18.0 ± 6.3 mm before surgery and 17.8 ± 7.4 mm after 1 year. Mean and standard deviation of CO to MO were 43.1 ± 7.5 mm before surgery and 39.6 ± 10.5 mm after 1 year. In MVO, CO to MAP and CO to MO, the values after 1 year did not significantly reach the pre-operative values in class III (P = 0.0001, P = 0.0007 and P 0.0001), although there was no significant difference between pre-operation and after 1 year in class II. In CO to MO, class II (mean and standard deviation 39.6 ± 10.5 mm) and class III (mean and standard deviation 47.3 ± 8.4 mm) still remained smaller values than control (mean and standard deviation 52.7 ± 9.2 mm) after 1 year (P 0.0001 and P = 0.0095).This study suggests that bi-maxillary surgery can have more influence on the reduction in the range of mandibular border movement including vertical or antero-posterior motion than lateral deviation motion, in both groups. The difference in the time-course change in the mandibular border movement between the groups might depend more on the mandibular length than on the movement direction of the mandible by surgery such as advancement or setback.

Purpose The purpose of this study was to compare the stability of the chin between absorbable plate and screws with a template device and titanium plate after advancement genioplasty in class II patients. Patients and methods The subjects consisted of 22 Japanese class II patients who underwent genioplasty advancement in combination with bi-maxillary surgery. After genioplasty horizontal osteotomy, the template plate and screws were fixed at the central region of the chin temporarily. Then, two absorbable bi-cortical screws (uncalcined and unsintered hydroxyapatite and poly- l -lactic acid: uHA/PLLA) were used and fixed bilaterally. After removal of the template plate and screws, one absorbable plate and screws were added to fix the segment in the advancement genioplasty (n = 14). The remaining 8 patients underwent genioplasty advancement surgery with the conventional titanium plate. For all patients, lateral cephalograms were obtained pre- and immediately after surgery and at 1 year after surgery. Change in the Pogonion (Pog) and Menton (Me) points and the corresponding soft tissue points (PogS and MeS) were evaluated. Results Although there were no significant differences in the change from before to immediately after surgery between the absorbable and titanium groups, there were significant differences in the Pog (Y) (P = 0.0379) and PogS (Y) (P = 0.0379) from immediately after surgery to after 1 year between both groups. Conclusion This study shows that predicted advancement of the chin in the absorbable group could be achieved by using a template and screws, and likewise in the titanium group. However, this study suggested that vertical relapse to the inferior site or resorption at the antero-superior edge of the segment could occur in the absorbable group.

Gram-negative bacteremia is a major cause of death among hematology inpatients who require heavy-dose chemotherapy and hematopoietic stem cell transplantation. Gram-negative bacillus (GNB) is more likely to be detected when the oral health is poor. However, there is a dearth of studies on the relationship between oral assessment and prevalence of GNB in hematology inpatients.This retrospective study aimed to evaluate the relationship between the original point-rating system for oral health examinations (point-oral exam) and the prevalence of GNB in hematology inpatients at the hematology ward of the Yamanashi University Hospital. GNB was detected by cultivating samples from the sputum and blood of each patient.A total of 129 subjects underwent a medical checkup and point-oral exam. The sputum and blood culture results of 55 patients were included in this study. The total points of patients positive for GNB (n = 25, 45.5%) were significantly higher than those who were negative for GNB (total score: median, 25th, 75th, percentile; 6 [4, 7] vs 2 [1, 4]; P = .00016). Based on the receiver operating characteristic analysis, a cutoff score of 5 proved to be most useful to detect GNB.An oral evaluation with a cutoff value of 5 or higher in the point-oral exam might indicate the need for a more thorough oral management to prevent the development of systemic infections from GNB.

The purpose of this study was to evaluate the relationship between computed tomography (CT) values of the condylar surface and temporomandibular joint (TMJ) disc position in the sagittal plane before and after sagittal split ramus osteotomy (SSRO) setback surgery, retrospectively. The subjects were 75 patients (150 condyles) who underwent bilateral SSRO setback surgery. They were divided into two groups (42 symmetric patients and 33 asymmetric patients). Maximum CT values (pixel values) of five points of the condylar surface and condylar height, length, fossa height, fossa length, and ramus angle in the sagittal plane were measured preoperatively and 1 year postoperatively. Disc position was classified as anterior disc displacement, anterior type, fully covered type, and posterior type, both pre- and postoperatively, using magnetic resonance imaging (MRI). Postoperative value was significantly higher than preoperative one in CT value of 135° (P = 0.0199) and 180° (0.0363), in the non-deviation side in the asymmetry group.
