Objective: Since skin diseases may show atypical presentation in children, histopathological evaluation plays a more critical role. The aim of this study is to determine the diagnoses that are finalized after the histopathological evaluation, which departments are frequently consulted, and the methods of follow-up in pediatric dermatology. Materials and Methods: This cross-sectional study was conducted on pediatric and adolescent patients whose biopsy was taken after evaluation in the pediatric dermatology outpatient clinic for 42 months. Results: Of the pediatric patients whose histopathological evaluation was made, 16.7% were consulted to other departments and 70.8% were followed up in the pediatric dermatology clinic with different methods. According to the histopathological evaluation results, 91.9% of the lesions were benign, 5.6% were premalignant, and only 1.5% were malignant. Three patients (1.5%) with malignant lesions in our pediatric dermatology clinic were followed up with pediatric dermatology and pediatric hematology. In comparison, premalignant lesions of 3 patients were excised in pediatric dermatology surgery; 6 patients were excised in plastic surgery, 1 patient in pediatric surgery, and 1 patient in ophthalmology. Conclusion: Pediatric dermatology is a department that should have pediatric dermatopathology and pediatric dermatosurgery units in itself. Background and Design: Darier's disease is a genetic disorder of keratinization with autosomal dominant inheritance. In the article presented here, demographical, clinical and histopathological findings and treatment outcome of Darier's disease are discussed with 9 cases. Materials and Methods: We performed a retrospective study of all the patients diagnosed with Darier's disease at the Department of Dermatology of Istanbul Training and Research Hospital, between 2008 and 2016. Results: During the observation period, we identified 9 patients with Darier's disease; 6 males and 3 females with a mean age of 32.5 years. Two cases were from the same family (mother and child) and no family history was found in the other seven patients. Skin lesions in the form of keratotic papules were noted in seborrhoeic areas, essentially the head-neck and trunk. Four patients had hand involvement, five patients had nail lesions. Conclusion: Darier's disease is a rare genodermatosis and should be considered in the differential diagnosis of dermatoses with keratotic papular lesions. [ABSTRACT FROM AUTHOR]

Objective: Psoriasis is associated with an increased frequency of cardiovascular risk factors. Metabolic syndrome (MS) and the Framingham risk score (FRS) are two different algorithms for evaluating cardiovascular risk. They include different features: waist circumference measurement is included in the MS criteria, whereas smoking, age, and gender are questioned in FRS. This study aimed to evaluate the frequency of MS and FRS in psoriasis patients compared with a control group and investigate the differences between MS and FRS. Methods: This was a cross-sectional study involving 300 psoriasis patients and 177 controls. MS, FRS, and disease severity were evaluated. Results: The frequency of MS was higher in females with psoriasis than in those in the control group (p=0.019). Females in the psoriasis group were more obese than those in the control group (p=0.036). FRS significantly differed between the patients and controls of age >60 years (p=0.006). The risk of hypertension in current and past smokers was higher in the psoriasis patients (OR=2.07 and 2.32-2.48, respectively) than in the control group. There was no statistically significant relationship among MS, FRS, and psoriasis severity (p>0.05). Conclusion: The results of this study support the evaluation of cardiovascular risk assessment in female psoriasis patients with MS and in male and elderly psoriasis patients with FRS. Background and Design: Quality of life scales provide clinicians a systematic and scientifically valid data to evaluate the effectiveness of the treatment options. The Skindex-16 is a dermatology related quality of life measure developed in the United States of America as a skin disease-specific quality-of-life measure. We aimed to generate a semantically equivalent translation of the Skindex-16 to use in Turkish language. Materials and Methods: The Skindex-16 was translated by five Turkish bilingual people. Two native English speakers who were also fluent in Turkish separately translated the initial Turkish translation of the Skindeks-16 back into English. The author of the original Skindex-16 (Chren MM) reviewed both of the backward-translations and evaluated the semantic equivalence of the questionnaire items. Measurement properties were evaluated for the final version of the Turkish version of the Skindex-16. Internal consistency reliability, test-retest reliability, construct validity, content validity, and convergent validity of the Turkish version were examined. Results: The symptom, emotional, and functional scales were internally consistent with Cronbach's α-coefficients of 0.88, 0.91, and 0.91, respectively. Test-retest reliability was determined with readministration of the Skindex-16 72 hours later (symptoms of 0.90, 0.93 and function of emotion 0.96). Construct validity was determined with verification of the hypothesis of the average scale of inflammatory disease being higher than isolated skin lesion (p<0.05) and also with factor analysis. Three factors were extracted in a principal axes factor analysis with varimax rotation. Factors were compatible with the original Skindex-16 scales. Content validity was demonstrated with Skindex-16 items covering 87.2% of the open-ended question. Convergent validity was defined with the correlation between comparative scales of Skindex-16 and Short Form-36. Conclusion: The Turkish version of the Skindex-16 is internally consistent and demonstrates test-retest reliability, construct and content validity. Background and Design: Dermatophytes, yeasts and some moulds settle on the skin and mucosal surfaces in immunocompetent individuals as commensals. Patients with diabetes mellitus, HIV-positive patients, organ transplant recipients and the patients with malignancies are predisposed to develop superficial fungal infections. We aimed to determine the prevalence, clinical and mycological features of superficial fungal infections in patients with hematologic malignancies in this case-control study. Material and Method: Eighty patients with hematologic malignancies (49 men, 31 women) and 50 healthy individuals (22 men, 28 women) randomly selected at our clinical department as controls were included to this study between 2003 and 2004. The mean age was 52±1.85 years in patients and 41.56±2.04 years in controls. All patients were inspected for superficial fungal infections. Skin scrapings and mucosal swabs were obtained from the toe web, inguinal region, any suspicious lesion and oral mucosa. Nail samples were also collected. All samples were examined by direct microscopy and cultured in Sabouraud dextrose agar (SDA). The yeasts were established in germ-tube production. Results: Fifty-six (70%) of 80 patients with hematologic malignancies had fungal colonization, whereas 21 (42%) of 50 controls had. For both groups, oral mucosa was the predominant area that fungus was mostly isolated from. A rising number of non-dermatophyte moulds (26%) was observed. Candida albicans was the predominant agent isolated from the culture. Conclusion: The prevalence of superficial fungal infection was higher in patients with hematologic malignancies (being immunosuppressed) than in the normal population. Candida albicans was the predominant isolated agent that was found in our study. We observed oral mucosa candidal infection mostly. The rising number of non-dermatophyte moulds is attributed to long-term use of antibiotics, cytotoxic chemotherapies and antifungals. [ABSTRACT FROM AUTHOR]

Focal hyperhidrosis is usually localized to the axillae, palms and soles. Less frequently, hyperhidrosis may be confined to the forehead and may have negative impact on patient's quality of life. A 34-year-old man presented to our clinic with the complaint of frontal hyperhidrosis. He was treated with botulinum toxin A. Thirty points were marked over the forehead and at each injection point, 0.15 ml (3U) botulinum toxin A were injected intracutaneously. Hyperhidrosis was significantly reduced and the effect lasted for 12 months. Skindex-29, a quality-of-life measure for skin disease, was administered to the patient at the beginning and at the end of second week of botulinum toxin A injection. There was a significant improvement on the Skindex-29 scale at the end of the treatment. There was no any side effect detected during and after the treatment. Introduction: Pemphigus vulgaris (PV) is an autoimmune disease primarily affecting the oral mucosa., Objectives: This study aimed to determine the demographic, clinical and treatment characteristics of PV patients with oral mucosal involvement and to assess the impact on their quality of life., Methods: We conducted a prospective observational study among 106 patients diagnosed with PV and presenting oral mucosal involvement. Demographic data, clinical and treatment characteristics, and quality of life questionnaires were recorded., Results: The study included 106 patients, 55 (51.89%) were male and there was a predominance of the mucocutaneous subtype in 83 individuals (78.38%). Oral mucosa was the initial site of manifestation in 44 patients (41.51%). Bilateral buccal mucosa was the most frequently affected site. The predominant symptom reported was a burning sensation, noted in 91 patients (85.85%). Oral mucosal examination revealed erosions in 85.85% of the patients. Systemic steroids were the most commonly administered treatment, and rituximab was used in 18 patients (16.98%). A positive and significant correlation was found between pemphigus severity and Oral Health Impact Profile-14, Dermatology Life Quality Index and Dermatological Quality of Life Scale scores (P < 0.05). The presence of superficial ulcers, flaccid bullae, lesion diameter ≥1 cm, and >10 lesions were factors that markedly diminished quality of life. Complete response to treatment was noted in all patients administered rituximab., Conclusions: The most common area of involvement was bilateral buccal mucosa, and the severity of PV closely correlated with a decline in quality of life measures. These results highlight the need for careful clinical oversight of PV, taking into account its effects on patients quality of life.

Introduction: Dermatoscopy is a frequently used non-invasive diagnostic procedure., Objectives: Considering that mycosis fungoides (MF) mimics parapsoriasis clinically in early stages, we aim to determine the dermatoscopic criteria and the histopathological correlations in patients with MF and small plaque parapsoriasis (SPP)., Methods: This prospective study involved 28 patients with clinical and histopathological diagnoses of MF and 31 patients with SPP. Videodermoscopy and USB capillaroscope were used to evaluate the patients vessels at ×200 magnification. Vascularity was evaluated through microvascular density (MVD) scoring involving CD34 antibody staining., Results: Fifty-nine patients were included in this study. The scores corresponding to the presence of short linear vessels, linear-curved vessels, branching linear vessels, and non- structured orange-colored areas were significantly higher in the MF patients than in the SPP patients (P < 0.05). The highest MVD (P = 0.01) scores were also higher in the MF patients than in the SPP patients., Conclusions: The SPP and early-stage MF patients differed in their MVD scores, and the findings correlated with the dermatoscopy and lesional capillaroscopy findings. Differentiating features between SPP and MF were thus identified.

Introduction: Granuloma annulare (GA) is a non-infectious granulomatous disease that can affect children and adults. Although many studies have been conducted in adult GA patients, the literature on pediatric GA cases is limited., Objectives: Therefore, this study aimed to examine the demographic, clinical, and pathological features of pediatric GA cases., Methods: This study was performed retrospectively in a single-center tertiary dermatology hospital. Demographic characteristics and clinical and histopathological features were recorded., Results: Forty-one participants were included in this study, of which 66% were females. The mean age was 3.8 ± 2.6 years, and the mean lesion duration was 7.5 ± 10.3 months. The involvement of 78% of the patients was localized, and the remaining 22% was generalized. Asthma (30%) was the most common comorbid disease. Histopathological examination was performed on 21 patients, and the infiltrate pattern was interstitial in 71% of the cases and palisadic in 29%. Generalized distribution, trunk involvement, and concomitant disease tended to be higher in patients with an interstitial pattern than in those with a palisadic pattern., Conclusions: Atopy and asthma should be questioned in pediatric GA cases. There are differences between involvement, distribution, concomitant disease, and histopathological patterns, which may indicate differences in pathogenesis.
