10. How to treat hemorrhagic shock and head trauma in the emergency department: isotonic or hypertonic saline?

Abstract

Aims: This study was performed to compare the effects of physiological saline and hypertonic saline administered at an early stage in a model of traumatic brain injury associated with hemorrhagic shock. Material and methods: Twenty-eight male Sprague Dawley rats were divided into four groups. The rats in the control group (S) underwent a sham experimental hemorrhagic shock followed by a sham operation. The rats in the trauma group (T) underwent a hemorrhagic shock followed by head trauma and no treatment. The rats in the NS group underwent a hemorrhagic shock followed by head trauma and received 0.9% NaCl. The rats in the HS group underwent a hemorrhagic shock followed by head trauma and received a 7.5% NaCl solution. The weight-drop method was used for achieving head trauma. After the head trauma, hypovolemia was induced by the controlled hemorrhage of 30% of the blood volume. The animals were exposed to hypovolemic shock for a further 30 min prior to fluid resuscitation. Each animal received a single volume infusion of their assigned fluid within few minutes. The effects of different fluids were evaluated after 24-hours by their brain water contents, and histological, and biochemical tests. Results: Group T had a significantly higher mean value for brain water content than did the NS group (P < 0.0001). Also, the HS group had significantly higher mean values for brain water content than the NS group (P = 0.003). Edema and bleeding were more marked in the HS group compared to the NS group (both, P < 0.001) in the histopathological evaluation. Leukocyte accumulation was significantly increased in the untreated rats compared to the HS and NS groups (both, P < 0.001). More red neurons were observed in the rats in the T group than in the NS and HS groups (both, P < 0.001). The mean serum osmolarity was higher in the T group compared to NS, HS, and S groups (all, P < 0.001). The mean plasma ADH levels and the mean plasma aldosterone levels were significantly higher in the T group than in the S, NS, and HS groups (all, P < 0.0001). Conclusion: Although not significantly different in biochemistry, animals treated with HS early in hemorrhagic shock secondary to head injury had more brain water than those the received NS as defined histopathologically. Therefore, in emergency settings, NS should be used safely in the early stage of hemorrhagic shock secondary to head trauma.

Abstract

OBJECTIVE: To investigate the reaction of versican and heparin-binding EGF-like growth factor (HB-EGF) molecule concentrations to acute radiation exposure in normal bladder and rectal tissue samples in order to gain more insight into the effects of cancer radiotherapy. STUDY DESIGN: Four groups with 6 male adult Swiss Albino mice per group were investigated. The mice bladder and rectum tissue samples were subjected to a 10-Gy single-dose radiation exposure in the pelvic region with a Co-60 teletherapy device and investigated 1, 2, and 7 days after radiation exposure, with 1 reference group which was not exposed to radiation. RESULTS: In the immunohistochemical examination of the tissue samples with anti-versican and anti-HB-EGF primary antibodies was observed a statistically significant increase 7 days after radiation exposure. CONCLUSION: The observed increase of versican and HB-EGF concentrations in the normal tissue matrix after radiation exposure may play a role in the side effects of radiotherapy. © Science Printers and Publishers, Inc.

16. Ondansetron: an agent to be included in the emergency treatment of migraine headache?

Abstract

Objective: To compare the efficacy of metoclopramide to ondansetron on migraine headache in the emergency setting. Methods: The study was conducted in two University-based EDs. Patients who had a migraine consistent with the description and classification of the International Headache Society, were included in the study. Numeric rating scales elicited before and after the administration of intravenous 1 g paracetamol plus 10 mg metoclopramide combination and 1 g paracetamol plus 4 mg ondansetron combination were compared. Both treatment protocols were completed in 15 minutes. Results: Thirty patients were delivered paracetamol and metoclopramide, whereas another thirty were administered paracetamol and ondansetron combination. The reduction in pain after the commencement of the therapy among patients in the ondansetron and metoclopramide groups revealed significantly lower pain scores at 30, 45, 60 minutes and discharge time compared with the baseline pain score (p=0.000). The magnitude of difference between the mean baseline pain score and the pain score at 30 minutes (Delta NRS30), baseline and 45 minutes (Delta NRS45) baseline and 60 minutes (Delta NRS60), baseline and discharge(Delta NRS-T) were not found to be significant in the two study groups (p=0.618, p=0.239, p=0.232, p=0.796 respectively). Conclusions: Ondansetron and metoclopramide appear to be equally effective in relieving migraine headache in the emergency setting.

Abstract

WOS: 000278590700012, In recent years, Crimean Congo haemorrhagic fever disease (CCHF) has created a serious health problem in our country. The disease takes place among haemorrhagic viral diseases. This study was made with CCHF diagnosed patients accepted in emergency services to determine the diagnostic criteria and cost effectivness for CCHF. CCHF patients who were accepted in our emergency service between 2004 and 2008 were analyzed retrospectively. Sensitivity, specificity, positive predictive and negative predictive value was calculated only for the patients who had 5 pathological laboratory parameters together (thromobcytopenia, high LDH, AST, ALT and CPK), and cost effectiveness analysis was made. During the five-year period, 687 patients had been evaluated. Significant pathologic results were established for alanine amino transferase (ALT), aspartate amino transferase (AST), creatine phosphokinase (CPK) enzymes, platelet counts and lactate dehydrogenase (LDH). Fatigue, fever, bleeding, gastro intestinal symptoms and diffuse pain were the symptoms seen according to their frequencies. The patients were frequently admitted in July. Mortality, costs per patient and emergency service's crowdedness can be reduced by rapid diagnosis which can easily be made with a brief history of the patient's occupation, physical examination and mentioned laboratory results sensitivity, specificity, positive predictive and negative predictive value.

18. Coagulation markers in stroke patients: worth ordering in the emergency department?

Abstract

Introduction: Cerebrovascular accidents (CVA) are sudden focal or global cerebral dysfunction, vascular in origin, lasting 24 hours or more in duration. Many advanced laboratory and radiological methods are employed in the diagnosis. This study was designed to evaluate if markers of coagulation cascade can be of value in conjunction with adjunctive,methods for expedient diagnosis of CVA. Materials and Methods: All consecutive patients admitted to the state hospital-based emergency department (ED) due to CVA between June 2004 and September 2004 were enrolled in the study. The control group comprised 70 healthy volunteers over 50 years of age. Blood samples for coagulation factors and ECG were obtained upon admission. Patients were categorized according to the detected levels of coagulation factors (high, normal and low). These categories were compared to each other respecting the outcome (CVA verified or ruled out). Results: A total of 74 patients with CVA were enrolled. Levels of D-dimer, prothrombin time (PT), activated partial thromboplastin time (aPTT), and fibrinogen were found to be higher in CVA group compared to the control group. There was not any significant difference between ischemic and hemorrhagic CVA with regard to D-Dimer, fibrinogen, PT, aPTT levels. Conclusion: D-Dimer and fibrinogen may be useful laboratory tests in the diagnosis and management of stroke.

Abstract

Anaphylaxis and acute allergic reactions may sometimes be fatal. They occur within minutes in a sensitized individual. So quick diagnosis and management are necessary issues. In the literature, cases are widely reported against allergens found in drugs, foods and their additives, radiocontrast material, bee stings, and many other materials. Here, we present a 37-year-old woman who developed an anaphylactic reaction to normal saline infusion during evaluation for her acute abdominal pain. We found only one report about normal saline allergy in the literature (Litvin ME, Shemchuck AS, Lisetskii VA. Anaphylactic shock caused by intravenous injection of isotonic solution of sodium chloride. Klin Khir 1976;(7):59-61). © 2009 Elsevier Inc. All rights reserved.

Abstract

In this study, it was aimed to examine the effects of U rtica dioica L. ( UD) that has antioxidant feature in the experimental testicular I/R model in rats in terms of anti-apoptotic and antioxidative effects. In our study, 24 male rats were divided into three groups: control group, I/R group and I/R + UD (2 mg kg−1) group. Seminiferous tubule calibre measurement, Johnson score, haematoxylin-eosin staining, proliferative cell nucleus antigen ( PCNA) immunohistochemical staining and TUNEL as histopathological have been conducted. The structural deterioration in the testicular on I/R group has reduced after the treatment of UD. Our data indicate a significant reduction in the activity of in situ identification of apoptosis using terminal dUTP nick end labelling ( TUNEL), and there was a rise in the expression of proliferating cell nuclear antigen ( PCNA) in testis tissues of UD-treated rats in the I/R group. The I/R + UD group showed a decrease in malondialdehyde levels and an increase in the activities of superoxide dismutase, catalase and glutathione peroxidase in comparison with the I/R group. It could be concluded that protective effects of UD on the I/R testicles are via reduction of histological damage, apoptosis, oxidative stress and lipid peroxidation. [ABSTRACT FROM AUTHOR]

Abstract

The aim of this study was to investigate the protective effects of fish omega-3 (n-3) fatty acids on doxorubicin (DOX)-induced acute cardiotoxicity. A total of 24 rats were divided into three groups: control, DOX-treated, and DOX treated with fish n-3 fatty acids. Control group received 0.4 ml/kg/day of saline intragastrically. The rats in the fish n-3 fatty acid-pretreated group were given 400 mg/kg/day fish n-3 fatty acids for 30 days by intragastric intubation. To induce acute cardiotoxicity, DOX (30 mg/kg) was injected intraperitoneally by a single dose and the rats were killed after 48 h. DOX treatment caused severe damage in heart tissues. Disorganization of myocardial muscle fibers, myofibrillar loss, and cardiotoxic myocardial fibers with cytoplasmic vacuoles were seen. Fish n-3 fatty acid-treated rats showed an improved histological appearance in the DOX-treated group. Our data indicate a significant reduction in the activity of terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick end labeling in cardiomyocytes of the DOX-treated group with fish n-3 fatty acids therapy. The DOX-treated with fish n-3 fatty acids group showed a significant decrease in malondialdehyde levels, and an increase in superoxide dismutase and glutathione peroxidase activities in comparison with the DOX-treated group. This study showed that fish n-3 fatty acids may be a suitable cardioprotector against acute toxic effects of DOX. [ABSTRACT FROM AUTHOR]

Abstract

Among the family of stainless steels, cast austenitic stainless steels (CASSs) are preferably used due to their high mechanical properties and corrosion resistance. These steels owe their properties to their microstructural features consisting of an austenitic matrix and skeletal or lathy type δ-ferrite depending on the cooling rate. In this study, the solidification behavior of CASSs (304L and 316L grades) was studied using ThermoCalc software in order to determine the solidification sequence and final microstructure during cooling. Theoretical findings were supported by the microstructural examinations. For the mechanical characterization, not only hardness measurements but also tribological studies were carried out under dry sliding conditions and worn surfaces were examined by microscopy and 3D profilometric analysis. Results were discussed according to the type and amount of microstructural features.

Abstract

Acute pharyngitis in adults is primarily a viral infection; only about 10% of cases are of bacterial etiology. Most cases of bacterial pharyngitis are caused by group A beta-hemolytic streptococci (GABHS). One laboratory method for the diagnosis of GABHS is rapid antigen diagnostic testing (RADT), which can be processed during an emergency department visit and which has become a popular alternative to throat swab cultures. We conducted a study to define the sensitivity and specificity of RADT, using throat culture results as the gold standard, in 100 emergency department patients who presented with symptoms consistent with streptococcal pharyngitis. We found that RADT had a sensitivity of 68.2% (15 of 22), a specificity of 89.7% (70 of 78), a positive predictive value of 65.2% (15 of 23), and a negative predictive value of 90.9% (70 of 77). We conclude that RADT is useful in the emergency department when the clinical suspicion is GABHS, but results should be confirmed with a throat culture in patients whose RADT results are negative.
