3. Minimally Invasive Surgery for Spontaneous Intracerebral Hemorrhage: Meta‐Analysis of High‐Quality Randomized Clinical Trials.

Abstract

Objectives: Spontaneous intracerebral hemorrhage (ICH) poses high mortality and morbidity rates with limited evidence‐based therapeutic approaches. We aimed to evaluate the current evidence for the role of minimally invasive surgery (MIS) in the management of ICH. Methods: This systematic review and meta‐analysis followed recommended guidelines and protocols. Medline, Embase, Scopus, and the Cochrane Library were searched from inception up to April 12, 2024. The inclusion was restricted to randomized clinical trials (RCTs) of high quality, ensuring they were not deemed to have a high risk of bias in any of the Cochrane risk of bias tool (RoB2) domains. Primary outcomes were good functional outcome (modified Rankin scale, 0–3) and mortality beyond 90 days. Secondary outcomes were early mortality within 30 days and rebleeding rates. We pooled odds ratios (ORs) with corresponding 95% confidence intervals (CIs) using random‐effects models. Results: Fourteen high‐quality RCTs were included. There were 3,027 patients with ICH (1,475 randomized to MIS, and 1,452 randomized to medical management or craniotomy). Of included patients, 1,899 (62.7%) were males. MIS resulted in higher odds of achieving long‐term good functional outcome (OR, 1.51 [95% CI, 1.25–1.82]), lower odds of long‐term mortality (OR, 0.72 [95% CI, 0.57–0.90]) and lower odds of early mortality (OR, 0.73 [95% CI, 0.56–0.95]). Rebleeding rates were similar (OR, 1.10 [95% CI, 0.55–2.19]). The treatment effect of MIS was consistent across multiple sensitivity and subgroup analyses, including individuals with deep ICH. Interpretation: This meta‐analysis provides high‐quality clinical trial evidence supporting the use of MIS as a primary treatment strategy in the management of ICH. Abstract

The non-invasive brain sensing modulation technology field is experiencing rapid development, with new techniques constantly emerging. This study delves into the field of non-invasive brain neuromodulation, a safer and potentially effective approach for treating a spectrum of neurological and psychiatric disorders. Unlike traditional deep brain stimulation (DBS) surgery, non-invasive techniques employ ultrasound, electrical currents, and electromagnetic field stimulation to stimulate the brain from outside the skull, thereby eliminating surgery risks and enhancing patient comfort. This study explores the mechanisms of various modalities, including transcranial direct current stimulation (tDCS) and transcranial magnetic stimulation (TMS), highlighting their potential to address chronic pain, anxiety, Parkinson's disease, and depression. We also probe into the concept of closed-loop neuromodulation, which personalizes stimulation based on real-time brain activity. While we acknowledge the limitations of current technologies, our study concludes by proposing future research avenues to advance this rapidly evolving field with its immense potential to revolutionize neurological and psychiatric care and lay the foundation for the continuing advancement of innovative non-invasive brain sensing technologies. Abstract

There are several limited approaches to treat epilepsy in hospitals, for example, using medicines, surgery, electrical stimulation and dietary interventions. Despite the availability of all these new and old approaches, seizure is particularly difficult to manage. The quest for new antiepileptic molecules with more specificity and less CNS toxicity continues for medicinal chemists until a new and ideal drug arrives. This review covers new antiseizure molecules of different chemical classes, the exact mode of action of which is still unidentified. Newer agents include sulfonamides, thiadiazoles, semi- and thiosemicarbazones, pyrrolidine-2,5-diones, imidazoles, benzothiazoles and amino acid deriva tives. These new chemical entities can be useful for the design and development of forthcoming antiseizure agents.

9. Pharmacokinetic studies of drug absorption into human skin

Abstract

Optimum therapeutic outcomes require not only proper drug selection but also effective drug delivery and monitoring. The aim of this thesis was to A) study drug delivery through the skin with a liquid formulated to promote absorption, B) develop and validate methods to analyze the drug in the samples obtained, C) assess appropriate methods to measure the transdermal delivery of drug, and D) apply to pharmacodynamics. The stability of a rectal formulation of diazepam, Diastat®, and a quality control of a topical form, TDS® diazepam, were studied using high performance liquid chromatography (HPLC) with ultraviolet absorption detection (UV). It was found that diazepam at 10 mg/mL was stable in solution at various temperatures for at least 4 weeks. A pharmacokinetic study of diazepam delivery from the TDS® delivery system was compared with delivery of the drug following rectal administration of Diastat® in 12 healthy volunteers. The TDS® diazepam was evaluated for safety and no adverse effects or events were observed. The preparation was found to be able to deliver diazepam systemically in humans, the confidence interval (CI) of the ratios for Cmax and AUC of diazepam from the two formulations A (TDS®): B (rectal) were not contained within the bioequivalence limit 80–125%, Cmax (0–72h): 7.3–14% and AUC0-72h: 20–38%. In addition, the 90% CI of desmethyldiazepam (A:B) ratio were not contained within the bioequivalence limit, Cmax (0–72h): 38–54% and AUC0-72h: 33–58%. Although not bioequivalent to Diastat® these finding suggest that skin may be an alternative method of diazepam delivery but further developments and studies would be required. The development and validation of fast, high throughput methods to evaluate tetracaine from skin tape samples was another challenge. Sensitive and reliable capillary electrophoresis with UV and HPLC-UV methods were developed and validated to measure tetracaine in skin using tape samples from volunteers given 1 mL Ametop gel (4% w/w of tetracaine) to support a pharmacokinetic drug delivery study of Ametop. The results from these validation studies demonstrated an equal ability of the two methods to measure tetracaine concentrations reproducibly and accurately. The Bland Altman test was in a range of ± 1.96 SD from the mean (SD = ± 8.02, Mean = 2.23), and percentage error (± 20%.), which show an acceptable difference. The assays were found to possess both the sensitivity and specificity necessary to measure the analyte in the skin tape stripping at the concentrations range in these tapes. Finally, observation of appropriate methods to measure the transdermal drug in vivo techniques, such as microdialysis (MD) and tape stripping (TS) have been employed by plotting a concentration time profile to investigate the capability of measuring tetracaine (pharmacokinetics) in local tissue, instead of measuring tetracaine by conventional systemic measurements. The results showed that the tetracaine Cmax concentration was higher in the stratum corneum compared with the major metabolites of tetracaine, 4- butylaminobenzoic acid (BABA) by 3 and 10 times in MD and plasma, respectively. TS samples reached the maximum concentration quicker than BABA in dialysate and plasma samples (p = 0.002). The median tmax was higher in plasma (IQR -53minutes, 95% CI: - 30– -105) compared with tape samples. The AUC and Cmax for tetracaine were higher in TS compared with BABA in MD and plasma (Mean AUC0-4h: 88582, 55594 and 13208 nM.min: Mean Cmax (0–4h), 850, 459, 110 nM, respectively). In addition, the AUC and Cmax values demonstrated that data from the TS study showed less variability compared with the data from plasma. The most variable data were for MD (CV%; AUC0-4h, 24, 63, and 85%: Cmax (0–4h), 42, 60, 80%, respectively). AUC and Cmax (Bartlett’s test, p = 0.004 for AUC; and Levene’s test, p = 0.042, and 0.028, respectively) This thesis has demonstrated that 1) diazepam was successfully delivered through the skin into the systemic circulation by the TDS® system, 2) novel methods have been developed for the measurement of tetracaine and its metabolite, and 3) the methods have been successfully applied to three different sample types employed in pharmacokinetic studies.

18. Improving the Operation of Split-Flow Sulfur Recovery Plants with Membrane Technology

Abstract

Sulfur recovery units are used in gas plants and refineries to convert hydrogen sulfide (H2S) to elemental sulfur. H2S occurs naturally in natural gas or is formed as a by-product in refinery and/or petrochemical processing systems (e.g. hydro-treatment). H2S is highly toxic, therefore requiring removal from the gas stream. Efficient processing of H2S and other sulfur-containing compounds is vital in reducing emissions to meet increasingly stringent environmental regulations. In a sulfur recovery unit, an acid gas feed stream containing H2S and a source of oxygen, such as air, are fed to a free-flame furnace, followed by multiple catalytic stages to convert most of the H2S into elemental sulfur. This paper introduces a new approach for the operation of split-flow sulfur recovery plants. For lean acid gas (i.e. low hydrogen sulfide content gas), only a fraction of the acid gas is sent to the reaction furnace of the modified Claus sulfur recovery units. The remaining acid gas, or bypassed feed, is merged with the outlet of the reaction furnace stream, downstream of the waste heat recovery unit and upstream of the first sulfur condenser. One benefit of bypassing the reaction furnace is the reduction of the formation of COS and CS2 within the furnace due to the high concentration of CO2 and kinetic limitation. A shortcoming, however, is that a fraction of the heavy hydrocarbons, i.e. Benzene, Toluene and Xylene (BTX), are not destroyed in the furnace reactor as a result of the bypass. BTX can deposit and rapidly deactivate the catalytic beds. To mitigate this risk, an activated carbon bed is installed to trap these hydrocarbons upstream of the catalyst bed. The cyclical nature of the operation of the activated carbon bed (adsorption, regeneration) as well as the risk inherent to the replacement of the contaminated beds call for an alternative approach. In this paper, the improvements discussed relate to the use of membranes that separate BTX along with H2S. The H2S- and BTEX-rich stream is routed to the reaction furnace, unloading the activated carbon bed and thus improving the sulfur recovery plant operability. An associated benefit is the possibility to route a larger fraction of the feed H2S within the reaction furnace such that the risk of O2 breakthrough to the catalyst bed is reduced while operating at a large by-pass fraction. A series of process simulations with different acid gas compositions and different membrane properties were evaluated to determine the impact of membrane selectivity on the acid gas composition sent to the reaction furnace. The results show a significant reduction of BTX and other hydrocarbons sent to catalytic converters and an acid gas enrichment with significant increase in H2S content in the acid gas feeding the reaction furnace. The ability of the membrane to concentrate BTX along with H2S can lead to a new operating philosophy for lean acid gas sulfur recovery plants.

Abstract

This study was undertaken to determine whether the vertical parasagittal approach or the lateral peri-insular/peri-Sylvian approach to hemispheric surgery is the superior technique in achieving long-term seizure freedom. We conducted a post hoc subgroup analysis of the HOPS (Hemispheric Surgery Outcome Prediction Scale) study, an international, multicenter, retrospective cohort study that identified predictors of seizure freedom through logistic regression modeling. Only patients undergoing vertical parasagittal, lateral peri-insular/peri-Sylvian, or lateral trans-Sylvian hemispherotomy were included in this post hoc analysis. Differences in seizure freedom rates were assessed using a time-to-event method and calculated using the Kaplan-Meier survival method. Data for 672 participants across 23 centers were collected on the specific hemispherotomy approach. Of these, 72 (10.7%) underwent vertical parasagittal hemispherotomy and 600 (89.3%) underwent lateral peri-insular/peri-Sylvian or trans-Sylvian hemispherotomy. Seizure freedom was obtained in 62.4% (95% confidence interval [CI] = 53.5%-70.2%) of the entire cohort at 10-year follow-up. Seizure freedom was 88.8% (95% CI = 78.9%-94.3%) at 1-year follow-up and persisted at 85.5% (95% CI = 74.7%-92.0%) across 5- and 10-year follow-up in the vertical subgroup. In contrast, seizure freedom decreased from 89.2% (95% CI = 86.3%-91.5%) at 1-year to 72.1% (95% CI = 66.9%-76.7%) at 5-year to 57.2% (95% CI = 46.6%-66.4%) at 10-year follow-up for the lateral subgroup. Log-rank test found that vertical hemispherotomy was associated with durable seizure-free progression compared to the lateral approach (p = .01). Abstract

To develop and validate a model to predict seizure freedom in children undergoing cerebral hemispheric surgery for the treatment of drug-resistant epilepsy. We analyzed 1267 hemispheric surgeries performed in pediatric participants across 32 centers and 12 countries to identify predictors of seizure freedom at 3 months after surgery. A multivariate logistic regression model was developed based on 70% of the dataset (training set) and validated on 30% of the dataset (validation set). Missing data were handled using multiple imputation techniques. Overall, 817 of 1237 (66%) hemispheric surgeries led to seizure freedom (median follow-up = 24 months), and 1050 of 1237 (85%) were seizure-free at 12 months after surgery. A simple regression model containing age at seizure onset, presence of generalized seizure semiology, presence of contralateral 18-fluoro-2-deoxyglucose-positron emission tomography hypometabolism, etiologic substrate, and previous nonhemispheric resective surgery is predictive of seizure freedom (area under the curve = .72). A Hemispheric Surgery Outcome Prediction Scale (HOPS) score was devised that can be used to predict seizure freedom. Children most likely to benefit from hemispheric surgery can be selected and counseled through the implementation of a scale derived from a multiple regression model. Importantly, children who are unlikely to experience seizure control can be spared from the complications and deficits associated with this surgery. The HOPS score is likely to help physicians in clinical decision-making.

Abstract

ObjectiveThis study was undertaken to determine whether the vertical parasagittal approach or the lateral peri-insular/peri-Sylvian approach to hemispheric surgery is the superior technique in achieving long-term seizure freedom.MethodsWe conducted a post hoc subgroup analysis of the HOPS (Hemispheric Surgery Outcome Prediction Scale) study, an international, multicenter, retrospective cohort study that identified predictors of seizure freedom through logistic regression modeling. Only patients undergoing vertical parasagittal, lateral peri-insular/peri-Sylvian, or lateral trans-Sylvian hemispherotomy were included in this post hoc analysis. Differences in seizure freedom rates were assessed using a time-to-event method and calculated using the Kaplan-Meier survival method.ResultsData for 672 participants across 23 centers were collected on the specific hemispherotomy approach. Of these, 72 (10.7%) underwent vertical parasagittal hemispherotomy and 600 (89.3%) underwent lateral peri-insular/peri-Sylvian or trans-Sylvian hemispherotomy. Seizure freedom was obtained in 62.4% (95% confidence interval [CI] = 53.5%-70.2%) of the entire cohort at 10-year follow-up. Seizure freedom was 88.8% (95% CI = 78.9%-94.3%) at 1-year follow-up and persisted at 85.5% (95% CI = 74.7%-92.0%) across 5- and 10-year follow-up in the vertical subgroup. In contrast, seizure freedom decreased from 89.2% (95% CI = 86.3%-91.5%) at 1-year to 72.1% (95% CI = 66.9%-76.7%) at 5-year to 57.2% (95% CI = 46.6%-66.4%) at 10-year follow-up for the lateral subgroup. Log-rank test found that vertical hemispherotomy was associated with durable seizure-free progression compared to the lateral approach (p = .01). Abstract

Introduction: Pharmacogenomics and personalized medicine are the two rapidly emerging areas in medical field which will determine the drug therapy of the future. It is very important for current medical students to be aware of these medical fields and apply these principles in their medical practice. There is huge scope for research in this area, hence an awareness study was conducted among medical students of college of medicine, Shaqra university, Dawadmi. Methods: A cross sectional questionnaire-based study was conducted among the medical students of Shaqra university, Dawadmi by self-administered questionnaire. 150 students participated in the study. Results: Out of the 150 students participated in the study Pharmacogenomics is relevant to my education 25% strongly agree. 45% agreed to recommend pharmacogenomic testing. Familiarity with genetics is only 16% and only 14% are familiar with interpreting results of pharmacogenomic tests. Knowledge testing showed overall less percentage. Only 16% can identify drugs that require pharmacogenomic testing. But 36% are confident to use the results of pharmacogenomics tests to make an appropriate adjustment to a patient's drug therapy. Knowledge about personalized medicine is 31% with scale of 3 out of 7.40% of students agreed to undergo genetic testing. 52% of the participants agreed to know about their risk for developing genetic based chronic diseases. Conclusion: There is genuine interest and awareness about Pharmacogenomics and Personalized medicine among the medical students but the knowledge is lacking. Abstract Introduction Medulloblastoma is one of the most common pediatric brain malignancies. The usual presenting clinical features are related to posterior fossa syndrome or/and hydrocephalus. Cauda equina syndrome is a very rare presentation for this disease. Case presentation We describe the case of a three-year-old boy with cauda equina syndrome as the initial presenting clinical feature for medulloblastoma. He was initially diagnosed as having a spinal tumor by magnetic resonance imaging scan. Subsequently, a cranial magnetic resonance imaging scan revealed a posterior fossa tumor with features of dissemination. He had substantial improvement after treatment. This case report is complemented by a literature review related to this unusual presentation. Conclusions Medulloblastoma primarily presenting with cauda equina syndrome is very rare. However, spinal drop metastasis should be considered in the pediatric age group to avoid suboptimal management.

Abstract Background Various functional resonance imaging, magnetoencephalographic and lesion studies suggest the involvement of the insular cortex in the control of swallowing. However, the exact location of insular activation during swallowing and its functional significance remain unclear. Case presentation Invasive electroencephalographic monitoring was performed in a 24-year-old man with medically intractable stereotyped nocturnal hypermotor seizures due to a ganglioglioma. During stimulation of the right inferior posterior insular cortex with depth electrodes the patient spontaneously reported a perception of a "stutter in swallowing". Stimulation of the inferior posterior insular cortex at highest intensity (4 mA) was also associated with irregular and delayed swallows. Swallowing was not impaired during stimulation of the superior posterior insular cortex, regardless of stimulation intensity. Conclusions These results indicate that the right inferior posterior insular cortex is involved in the neural circuitry underlying the control of swallowing.

The ventriculoperitoneal (VP) shunt is a common procedure in pediatric neurosurgery that carries a risk of complications at cranial and abdominal sites. We report on the case of a child with shunt infection and malfunction. The peritoneal catheter was tethered within the abdominal cavity, precluding its removal. Subsequently, laparoscopic exploration identified a knot at the distal end of the peritoneal catheter around the omentum. A new VP shunt was inserted after the infection was healed. This type of complication occurs rarely, so there are a limited number of case reports in the literature. This report is complemented by a literature review.

Transorbital intracranial penetrating injury is an uncommon mechanism of head injury. These injuries can be occult during the initial clinical presentation. Certain patients develop an intracranial cerebral infection. Herein, we report a 5-year-old child with an occult transorbital intracranial penetrating injury caused by a pen. A retained pen tip was found at the superior orbital roof and was not noticed at initial presentation. This was complicated by a right frontal lobe cerebral abscess. This paper emphasizes the importance of orbitocranial imaging in any penetrating orbital injury. A review of the literature on intracranial infection locations in relation to the route and mechanism of injury is included to complement this report.

Transorbital slow-penetrating injury is an uncommon type of head injury that is seen more often in the pediatric age group. This type of injury can be occult, which is often associated with serious complications. We report on a 4-year-old female who presented with orbital swelling after an unwitnessed right orbital injury following a fall on her face at her school. Three days after injury, the presence of a foreign body was discovered on imaging study when she presented with orbital swelling and purulent discharges. She recovered well after surgical and medical management. This paper sheds light on the importance of high suspicion for the presence and early surgical management of a penetrating foreign body.

Mucormycosis is the third most common invasive fungal infection that particularly occurs in immunocompromised patients. Intracranial and extracranial arteriovenous vasculopathy is a complication that makes this disease more complex and difficult to treat. We describe a 23-year-old female, who presented to her local hospital with acute blindness and diabetic ketoacidosis-induced coma requiring intensive care treatment. She was found to have lesions in the nasal sinuses, orbit, and frontal base. The left carotid artery was occluded from its origin in the neck to the supraclinoid segment and left cavernous sinus involvement. No cerebral infarction was noted. Biopsies obtained by endonasal debridement confirmed mucormycosis. In addition to antimicrobial therapy, she underwent several multidisciplinary approaches to treat her disease. Multiple endonasal, and cranial procedures were done including bilateral orbital exenteration. After prolonged treatment on the intensive care unit she made a remarkable recovery to the point where she was communicating verbally and had normal limb movements and later discharged home. She remained alive and well for two months, but later succumbed to a recurrence of her disease. In conclusion, mucormycosis-induced vasculopathy is a complex problem, which merits aggressive treatment of this invasive disease. It is normally regarded as an indicator of grave prognosis.
