2. Epidemiology, clinical and complications profile of diabetes in Saudi Arabia : A review

Abstract

Diabetes mellitus is emerging as a major public health probelm in Saudi Arabia in parallel with the worldwide diabetes pandemic, which is having a particular impact upon the Middle East and the third world. This pandemic has accompanied the adoption of a modern lifestyle and the abandonment of a traditional lifestyle, with a resultant increase in rates of obesity and other chronic non-communicable diseases. The indigenous Saudi population seems to have a special genetic predisposition to develop type 2 diabetes, which is further amplified by a rise in obesity rates, a high rate of consanguinity and the presence of other variables of the insulin resistance syndrome. We highlight the epidemiology, clinical and complications profiles of diabetes in Saudi people. Diabetes is well studied in Saudi Arabia; however, there seems to be little research in the area of education and health care delivery. This is of paramount importance to offset the perceived impact on health care delivery services, to lessen chronic diabetes complications, and to reduce the expected morbidity and mortality from diabetes.

Abstract

During the last decades, the coastal areas of the Kingdom of Saudi Arabia, on the Red Sea and the Arabian Gulf, have been subjected to intense economic and industrial growth. As a result, it may be expected that the overall environmental status of Saudi Arabian coastal marine waters has been affected by human activities. As a consequence, adequate management of the Saudi Arabian coastal zone requires an assessment of how the various pressures within this zone impact the quality of seawater and sediments. To this end, environmental surveys were conducted over 15 hotspot areas (areas subject to environmental pressures) in the Saudi Arabian coastal zone of the Red Sea and over three hotspot areas in the Saudi Arabian waters of the Arabian Gulf. The survey in the Red Sea, conducted in June/July 2021, acquired measurements from hotspot areas spanning most of the Saudi coastline, extending from near the Saudi–Jordanian border in the north to Al Shuqaiq and Jizan Economic City (close to the Saudi–Yemen border) in the south. The survey in the Arabian Gulf, carried out in September 2021, included the areas of Al Khobar, Dammam and Ras Al Khair. The main objective of both cruises was to record the physical and biogeochemical parameters along the coastal waters of the kingdom, tracing the dispersion of contaminants related to specific pressures. Taken together, these cruises constitute the first multidisciplinary and geographically comprehensive study of contaminants within the Saudi Arabian coastal waters and sediments. The measurements acquired revealed the influence of various anthropogenic pressures on the coastal marine environment of Saudi Arabia and also highlighted a strong influence of hydrographic conditions on the distribution of biochemical properties in the Red Sea and the Arabian Gulf. 17. Occupational stress and associated sources and risk factors among nurses in Gaza strip, Palestine: A cross‐sectional survey

Abstract

Abstract Aim To determine the occurrence of occupational stress among Palestinian nurses, and their associated sources and risk factors. Design A cross‐sectional descriptive design. Methods A total of 250 registered nurses from eight governmental hospitals, using a convenience sampling method. Data collection were conducted using the 30‐items self‐reported Occupational Stress Scale from December 2022 to March 2023. Descriptive statistics, independent sample t‐test, one‐way ANOVA, and multiple linear regression analysis were applied to analysis data. Data analysis included descriptive statistics, independent sample t‐test, one‐way ANOVA, and multiple linear regression analysis. Results The prevalence of high occupational stress levels was 64.8% (Mean = 3.9 out of 5). The main sources of stress are too much responsibility and work, understaffing, lack of promotion and recognition, inadequate pay, time pressure, and management style. The results regression analysis demonstrated that male nurses with a Masters or PhD degree and those working in fixed shifts experienced higher occupational stress. Moreover, participants who worked overtime hours were more susceptible to stress. Conclusions The research indicates that occupational stress presents a notable challenge for nurses in the Gaza Strip, Palestine. It suggests that in order to alleviate this stress, decision‐makers in healthcare policy and hospital management should prioritize the execution of strategies aimed at addressing the primary stressors and risk factors identified. Reporting Method This study adhered to the STROBE guidelines. Public Contribution A total of 250 registered nurses were taken part in this study by answering a self‐administered study survey.

Abstract

Soil improvement using chemical admixtures has been extensively investigated in the recent past and these admixtures are abundantly being used on various construction projects. However, there is a need to elucidate the long-term performance of stabilized geomaterials under aggressive pore fluid environments such as high salinity of groundwater in coastal areas. This study presents the effects of the concentration of pore fluid salinity on the post-stabilization behavior of cement-treated Marl specimens to brine solution from Sabkha deposits and rejected water from a water treatment plant. Unconfined compressive strength (UCS), mineralogy, chemical, and microstructural evaluations have been carried out in this regard. minerals like albite and muscovite completely leached out of the samples after 28 days of soaking in a saline environment. A new mineral Attapulgite was formed at various concentration levels of the pore fluid. Significant changes in the chemical composition were also observed in various phases of the experiments. Moreover, at all salinity levels, the UCS initially increased to a maximum of 75 % which later reduced to a value less than the original strength whereas the elastic modulus (E50) increased as the soaking period progressed. Lastly, it is emphasized that the engineering properties of treated soils exposed to aggressive pore fluids should be carefully assessed for a reliable prediction of the long-term performance of geotechnical structures.

21. A universal hydro-mechanical coupled behavior model for clay-bearing strata—Molecular-level simulation approach

Abstract

Abstract Clay minerals in soils and rocks exhibit large volume change upon interaction with water and this behavior becomes even more complex when the strata are being stressed by the engineering and environmental loads. Therefore, a realistic prediction of the hydro-mechanical behavior of the clay-bearing strata is always a challenge due to their coupled swelling-mechanical response in the cases of geotechnical and geoenvironmental engineering problems, nuclear waste storage in clay-bearing rock repositories, shale gas extraction, and other uses of clay in the manufacturing industry. All the existing behavior models have restricted applications in the engineering and other fields of practice mainly due to the partial consideration of the structure and fabric of clay-bearing strata in the model formulation. In this study, a hydro-mechanical behavior model has been formulated using the parameters acquired from the molecular-level simulations and modeling of the volume change and stress–strain behavior of the clay-bearing structure. The Molecular Mechanics and Molecular Dynamic simulations were performed on the natural structure of the clay-bearing strata formulated using Monte Carlo technique. The mathematical model, developed from the simulation results, can predict the overall hydro-mechanical behavior of clay-bearing strata for all possible combinations of clay minerals, non-clay minerals, salts causing cementation of the soil/rock structure, confining pressures, and the induced strain levels. The developed model has successfully been validated through laboratory and field testing on the clay-bearing strata in both the elastic and elasto-plastic regions of the stress–strain behavior and also from the data of two (02) swelling clays (MX-80 and FEBEX Bentonite) from the existing literature, supporting the universal nature of the developed behavior model.

في السنوات الأخيرة عملت المملكة العربية السعودية على إيجاد بيئة جاذبة للاستثمار وزيادة الإنتاجية وتعزيز التنافسية، والتركيز على قطاعي الصـناعة والتكنولوجيا, كما أولت المملكة اهتماماً واضحاً بالرياضة لتحسين جودة الحياة لدى كافة شرائح المجتمع و تحقيق مستهدفات رؤيتها 2030 وحيث أن صناعة المنسوجات تشكل إحدى المقومات الأساسية والحضارية بالنسبة لأي مجتمع, وعليه يهدف هذا البحث الى تحسين خواص الأداء الوظيفي للنسيج المستخدم في ملابس رياضة الفروسية بالمملكة، فتمثلت فرضية البحث في التحقق من ان معالجة النسيج الشائع الاستخدام في ملابس رياضة الفروسية بالمملكة العربية السعودية بجزيئات الفضة النانوية قد يحسن خواص الأداء الوظيفي لها، واتبع البحث المنهج التجريبي بمعالجة عينة البحث لنسيج القميص المخلوط من القطن 95% والاسباندكس 5% بجزيئات الفضة النانوية بثلاث تركيزات مختلفة (30-60-90 ميكروجرام\مل) وتقييم أداء النسيج قبل و بعد المعالجة من حيث مقاومة البكتريا و بعض الخواص الفيزيقية و الميكانيكية، وتوصلت الدراسة الى ان معالجة النسيج بجزيئات الفضة النانوية قد حسن بعض خواص الأداء ومن أبرزها مقاومة البكتيريا والتي تزداد بشكل طردي بزيادة تركيز جزيئات الفضة النانوية، وكذلك يزداد وزن المتر المربع للعينة والصلابة في اتجاه السداء واللحمة وقوة الشد في اتجاه السداء بزيادة تركيز محلول المعالجة، وخلص البحث الى توصيات من أبرزها تشجيع الاهتمام بمعالجة الألياف المخلوطة بتقنية النانو لإيجاد أقمشة مستدامة ومناسبة للملابس الرياضية.

46. Exploring the Relationship Between Knowledge, Attitudes, Self-Efficacy, and Infection Control Practices Among Saudi Arabian Nurses: A Multi-Center Study.

Abstract

Introduction: Healthcare-associated infections are a significant risk to patients and the healthcare system. Differences in knowledge, attitudes, and practices among nurses have limited standardized precautions. Improving self-efficacy can enhance compliance with standard precautions, protect patients, and preserve the credibility of healthcare institutions. Aim: This study aimed to determine the mediating effects of self-efficacy on nurses' knowledge, attitude, and compliance with standard precautions in Saudi Arabia. Methods: This cross-sectional study was conducted in four hospitals in Dharan, Saudi Arabia, between July and October 2023. The respondents were selected through a multistage sampling of 305 nurses from wards and special areas. Results: participants demonstrated a moderate level of knowledge (M = 19.00, SD = 2.17), positive attitudes (M = 55.86, SD = 10.22), intermediate compliance (M = 72.44, SD = 11.47), and moderate self-efficacy (M = 29.99, SD = 11.40). Respondents who possessed more knowledge demonstrated positive attitudes toward standard precautions (r = 0.256, p < 0.001) and were more likely to comply (r = 0.376, p < 0.001). Higher self-efficacy levels were significantly positively correlated with knowledge (r = 0.391, p < 0.001), attitude (r = 0.311, p < 0.001), and compliance (r = 0.385, p < 0.001). The direct effect of knowledge on compliance was estimated to be 0.115 (p < 0.001), while the direct effect of attitude was slightly weaker (0.014, p = 0.049). Self-efficacy indirectly increased compliance through its influence on knowledge and attitude, with indirect effects of 0.039 (p < 0.001) and 0.008 (p = 0.002), respectively. Conclusions: Nurses with higher knowledge of infection control and positive attitudes toward established protocols were more likely to comply consistently. This association is further strengthened by self-efficacy. This confidence enhanced their understanding of the specific procedures, reinforced their positive beliefs about the importance of protocols, and ultimately led to greater adherence. Abstract

Immunotherapy, particularly that based on blocking checkpoint proteins in many tumors, including melanoma, Merkel cell carcinoma, non-small cell lung cancer (NSCLC), triple-negative breast (TNB cancer), renal cancer, and gastrointestinal and endometrial neoplasms, is a therapeutic alternative to chemotherapy. Immune checkpoint inhibitor (ICI)-based therapies have the potential to target different pathways leading to the destruction of cancer cells. Although ICIs are an effective treatment strategy for patients with highly immune-infiltrated cancers, the development of different adverse effects including cutaneous adverse effects during and after the treatment with ICIs is common. ICI-associated cutaneous adverse effects include mostly inflammatory and bullous dermatoses, as well as severe cutaneous side reactions such as rash or inflammatory dermatitis encompassing erythema multiforme; lichenoid, eczematous, psoriasiform, and morbilliform lesions; and palmoplantar erythrodysesthesia. The development of immunotherapy-related adverse effects is a consequence of ICIs' unique molecular action that is mainly mediated by the activation of cytotoxic CD4+/CD8+ T cells. ICI-associated cutaneous disorders are the most prevalent effects induced in response to anti-programmed cell death 1 (PD-1), anti-cytotoxic T-lymphocyte-associated antigen-4 (CTLA-4), and anti-programmed cell death ligand 1 (PD-L1) agents.
