Abstract

CIC‐rearranged sarcomas comprise a group of exceptionally aggressive round‐cell sarcomas. These tumors most commonly demonstrate CIC::DUX4 fusion and show similar histopathology to Ewing sarcomas, though lesions mimicking vascular neoplasms have recently been described. Here, we describe a case of a patient with CIC::DUX4 fusion sarcoma identified using RNA‐based molecular testing who was initially diagnosed with an endothelial neoplasm. The tumor showed extensive vasoformative growth, complete WT1 negativity, and global positive staining for ERG, CD31, and DUX4 by immunohistochemistry. Methylation testing of the tumor clustered more closely with angiosarcomas than with CIC‐rearranged sarcomas. Our findings suggest that CIC::DUX4 fused neoplasms may demonstrate a more diverse phenotypic range than previously appreciated and offer evidence that both molecular and immunohistochemical studies are needed for accurate diagnosis. Abstract

Purpose: We have shown that the aged microenvironment increases melanoma metastasis, and decreases response to targeted therapy, and here we queried response to anti-PD1.Experimental Design: We analyzed the relationship between age, response to anti-PD1, and prior therapy in 538 patients. We used mouse models of melanoma, to analyze the intratumoral immune microenvironment in young versus aged mice and confirmed our findings in human melanoma biopsies.Results: Patients over the age of 60 responded more efficiently to anti-PD-1, and likelihood of response to anti-PD-1 increased with age, even when we controlled for prior MAPKi therapy. Placing genetically identical tumors in aged mice (52 weeks) significantly increased their response to anti-PD1 as compared with the same tumors in young mice (8 weeks). These data suggest that this increased response in aged patients occurs even in the absence of a more complex mutational landscape. Next, we found that young mice had a significantly higher population of regulatory T cells (Tregs), skewing the CD8+:Treg ratio. FOXP3 staining of human melanoma biopsies revealed similar increases in Tregs in young patients. Depletion of Tregs using anti-CD25 increased the response to anti-PD1 in young mice.Conclusions: While there are obvious limitations to our study, including our inability to conduct a meta-analysis due to a lack of available data, and our inability to control for mutational burden, there is a remarkable consistency in these data from over 500 patients across 8 different institutes worldwide. Abstract

The arena of melanocytic histopathology has experienced tremendous growth in the last decade. The advancement is attributed to incorporating various molecular tests in benign, intermediate, and malignant melanocytic tumors. Most molecular testing has been mainly applied in clinically advanced-stage melanoma to determine the molecular alteration to help guide therapy (e.g., BRAF inhibitors in BRAF mutated melanomas). However, with more availability and, to a certain degree, affordability of certain molecular tests, multiple studies have been conducted on benign/intermediate lesions in an attempt to understand further the driving molecular alterations allowing for the proliferation of certain melanocytic lineages. This review article discusses and illustrates examples of recently recognized entities with their corresponding genomic alterations in the Spitz lineage, blue nevi, deep penetrating melanocytomas, and pigmented epithelioid melanocytomas. Abstract

In summary, this report documents an association of CCV with an underlying vasculopathy related to a heterozygous prothrombin G20210A mutation. Cutaneous collagenous vasculopathy in a middle-aged woman with a history of prothrombin G20210A thrombophilia It has been theorized that the aberrant collagen deposition may be induced by repeated vascular injury caused by microvascular thrombi.2,3 CCV has been reported in a patient with cryofibrinogenemia3 as well as in a patient whose biopsies revealed occlusive dermal thrombi but in whom a limited hypercoagulability work-up revealed no underlying disorder.5 In this case, CCV arose in the setting of a recently diagnosed, symptomatic hypercoagulability syndrome.
