5. The Wicked role of the medical education department

Abstract

Purpose: This paper aims to describe the role of the Medical Education unit/department (MED) from a world-wide perspective, how it qualifies as a wicked issue, and provide tips on how to tame it. Methods: The authors reviewed the regional and international literature to obtain a framework of functions and then used brainstorming and focus group discussions to define the scope of functions of the MED. Telephone interviews with present or previous chairs of medical education departments in the Kingdom and the region helped define the challenges that are faced. Results: The functions of the MED were found to fall under 10 domains (Table 1) and 40 functions (Table 2). Chal?lenges facing MED role include governance and organizational structure, financial support, manpower, intersection with the roles of other structural entities, and dual reporting of MED staff. Ten tips are described to deal with these challenges. Conclusion: The MED role is complex and faced with many challenges. It has changing and dynamic boundaries, and is entangled with other entities in the college or university. Ten tips are proposed to tackle these challenges and tame the wickedness of the MED role.

Abstract

We compared serum leptin levels during various phases of menstrual cycle and its correlation with serum estradiol between normal weight and overweight/obese young females. Fifty-six young females with normal menstrual cycle were grouped into 26 normal weight and 30 overweight/obese subjects. Serum leptin and estradiol levels were measured during early follicular, pre-ovulatory and luteal phases of menstrual cycle in both groups using ELISA technique. Serum leptin levels were significantly different across different phases of menstrual cycle with a steady increment from follicular phase (9.97 ± 5.48 ng/dl) through pre-ovulatory phase (11.58 ± 6.49 ng/dl) with their peaks in luteal phase (12.52 ± 6.39 ng/dl, p < .001). Same pattern of change during menstrual phases was observed when the normal weight and overweight/obese group were analyzed separately. Serum leptin levels were significantly higher in overweight/obese group compared to normal weight subjects. In any of the study groups, leptin levels were not found to be correlated with estradiol level during different phases of menstrual cycle. [ABSTRACT FROM AUTHOR]

Abstract

We analyzed the association between kisspeptin and plasma adipokines (leptin and adiponectin) in normal-weight and over-weight young females. Thirty young Saudi females, based on their body mass index (BMI), were divided into two groups (15 students/group) as (1) Normal weight (NW): BMI = 18.5–24.99 and (2) Over-weight/obese (OW): BMI ≥ 25. Serum adipokines (leptin, adiponectin) and kisspeptin levels were measured in early follicular, pre-ovulatory, and luteal phase in both groups with ELISA. Menstrual cycle phases were confirmed by serum estradiol levels. There was no association of kisspeptin with leptin in early follicular (r−0.34,P0.31), pre-ovulatory (r−0.32,P0.34) and luteal phase (r0.21,P0.54). Likewise, kisspeptin was not found to be correlated with adiponectin in early follicular (r0.41,P0.21), pre ovulatory (r0.24,P0.48), and luteal phase (r0.40,P0.23) when values recorded during different time points during the cycle were plotted with each other.
