Abstract Introduction Neurobrucellosis (NB) can be associated with meningitis and present as a headache with or without meningeal signs. Pseudotumor presentation of NB has been reported to be accompanied by lymphocytic predominant cerebrospinal fluid(CSF) pleocytosis. NB is diagnosed by means of isolation of Brucella from blood or CSF and/or the presence of anti-Brucella antibodies in the CSF. Molecular techniques have been used in chronic or challenging cases of NB. Clinical findings We report on seven cases of NB presenting with different types of headache and signs of meningeal involvement. In five cases, signs of intracranial hypertension were evident in the form of papilledema, sixth nerve palsy and blurred vision. Diagnosis MRIs of the brain revealed signs of intracranial hypertension in three patients, basal meningeal enhancement in one patient and white matter lesions in one patient. Brucella serology in the blood and CSF was negative in all patients. It was interesting that four patients had normocellular CSF analysis with normal glucose and protein results. The diagnosis was made by Brucella PCR in all patients. Conclusion NB should be considered in the differential diagnoses of pseudotumor cerebri syndrome in endemic areas. It is important to employ molecular techniques using sterile CSF samples in the investigation of Brucella.

Background and Objectives: Brucellosis, a zoonotic bacterial disease caused by Brucella, affects humans and domestic animals, leading to significant economic loss. This study examined suspected cases in North Khorasan, Iran, to understand the prevalence of infection and its characteristics in this region. Materials and Methods: Blood specimens were collected from 200 patients suspected of brucellosis after obtaining informed consent. Serum samples were tested using RBPT, Wright, and 2-ME agglutination tests. Blood samples were cultured on Brucella agar, and positive cultures underwent biotyping and PCR assays. A questionnaire identified correlated risk factors. Results: RBPT, Wright, and 2-ME tests showed 25% brucellosis seroprevalence in symptomatic patients. In contrast, the prevalence was 2.5% among those with positive blood cultures. Notably, all culture-positive patients were also serologically positive, with titers exceeding 1:320 in Wright and 2-ME tests. Most positive cases were in people in their 30s, with B. melitensis biovar 1 identified as the causative agent, and the results were confirmed by multiplex PCR. Significant risk factors include contact with livestock and consumption of raw milk (P < 0.0001). Conclusion: The findings highlighted the importance of comprehensive diagnostic approaches for accurate identification of brucellosis. Furthermore, education regarding close contact with animals and pasteurization of dairy products is essential for controlling human brucellosis.

BackgroundOlder adults are at high risk for toxicity due to cancer treatment and increased risk for adverse events related to chemotherapy-induced nausea and vomiting (CINV). Unfortunately, older adults report multiple treatment-related symptoms but use few strategies to self-manage these symptoms due to erroneous beliefs related to the effectiveness of commonly taught self-management strategies. We developed a novel serious game, Managing at Home (MAH), to help older adults learn how to effectively self-manage CINV at home. ObjectiveThis study has 2 aims. Aim 1 is to examine changes in CINV severity, self-management behaviors, functioning, quality of life, cognitive representation, and health care use within the intervention group from baseline (T1) to completion of the study (T6). Aim 2 is to determine the efficacy of the MAH intervention by comparing differences in primary outcomes (CINV severity and health care use) and secondary outcomes (self-management behaviors, functioning, and quality of life) between the intervention and control groups at each follow-up visit (T2-T6) and completion of the study (T6). MethodsThis is a longitudinal randomized clinical trial. We will collect data from 500 older adults receiving cancer-related chemotherapy at baseline (T1) and at each treatment cycle until cycle 6 (T6). Participants will be enrolled if they are 60 years or older of age, are newly diagnosed with cancer, being treated with any chemotherapy agent with moderate or high emetic potential, are on a 2-, 3-, or 4-week treatment cycle, are proficient in English, and have a telephone. Previous diagnosis or treatment for cancer, end-stage disease with less than 6 months to live, and uncorrected visual or hearing impairment are exclusion criteria. ResultsThis study was funded in September 2022 and received institutional review board approval in October 2022. As of July 2023, the enrollment of participants is ongoing and currently has 130 enrolled participants. Data collection and analysis will be complete in 2027. ConclusionsThis study addresses self-management of CINV in older adults using an innovative serious game. The MAH intervention uses simulation and gaming technology to engage older adults in active learning in order to reframe erroneous perceptions about symptom self-management. If shown to be effective, it can easily be adapted to include other cancer-related symptoms or other chronic illnesses. A zoonotic disease called brucellosis can cause flu-like symptoms and heart inflammation. The bacteria responsible for this disease can also enter the brain, causing a condition called neurobrucellosis that can result in long-term neurological problems. In this study, researchers aimed to determine the changes in the hippocampal cells of rats infected with Brucella. For the study, 24 adult male albino rats were inoculated with 1 × 106 CFU Brucella abortus 544. The rats were then deeply anesthetized, and their hippocampus samples were taken for stereological, histological, and molecular studies. The results showed that the infected rats had increased microgliosis and astrogliosis. Furthermore, a high level of caspase-3 in their hippocampal tissue indicated their susceptibility to apoptosis. Additionally, there was a decrease in expression of Ki67, which further supported this. Sholl's analysis confirmed a significant failure in glial morphology. The study demonstrated that the pathogen has the ability to destroy the hippocampus and potentially affect its normal physiology. However, more research is needed to clarify various aspects of neurobrucellosis.

Abstract Background Behavioral risk factors are known to co-occur among youth, and to increase risks of chronic diseases morbidity and mortality later in life. However, little is known about determinants of multiple chronic disease behavioral risk factors, particularly among youth. Previous studies have been cross-sectional and carried out without a sound theoretical framework. Methods Using longitudinal data (n = 1135) from Cycle 4 (2000-2001), Cycle 5 (2002-2003) and Cycle 6 (2004-2005) of the National Longitudinal Survey of Children and Youth, a nationally representative sample of Canadian children who are followed biennially, the present study examines the influence of a set of conceptually-related individual/social distal variables (variables situated at an intermediate distance from behaviors), and individual/social ultimate variables (variables situated at an utmost distance from behaviors) on the rate of occurrence of multiple behavioral risk factors (physical inactivity, sedentary behavior, tobacco smoking, alcohol drinking, and high body mass index) in a sample of children aged 10-11 years at baseline. Multiple behavioral risk factors were assessed using a multiple risk factor score. All statistical analyses were performed using SAS, version 9.1, and SUDAAN, version 9.01. Results Multivariate longitudinal Poisson models showed that social distal variables including parental/peer smoking and peer drinking (Log-likelihood ratio (LLR) = 187.86, degrees of freedom (DF) = 8, p < .001), as well as individual distal variables including low self-esteem (LLR = 76.94, DF = 4, p < .001) increased the rate of occurrence of multiple behavioral risk factors. Individual ultimate variables including age, sex, and anxiety (LLR = 9.34, DF = 3, p < .05), as well as social ultimate variables including family socioeconomic status, and family structure (LLR = 10.93, DF = 5, p = .05) contributed minimally to the rate of co-occurrence of behavioral risk factors. Conclusions The results suggest targeting individual/social distal variables in prevention programs of multiple chronic disease behavioral risk factors among youth.

Background: Brucellosis is one of the most prevalent and common diseases between humans and animals.Prompt diagnosis and timely treatment of this disease can prevent many complications. In this regard, this study aims to comparatively evaluate ELISA, PCR and serological methods to identify Brucella abortus. Methods: In this study, the serum of 100 patients referred to Tonekabon private laboratory from July 2020 to January 2021 was examined by PCR, ELISA and Wright, Coombs Wright, and 2ME methods for the detection of Brucella abortus. Results: In this study, the mean age of the sample was 43.3 ± 18.2 of which 21% were infected with Brucella abortus according to the above serological methods. The purpose of this paper is to investigate decision-making in export financing services by banks in Hungary. The case studies are of one export credit agency and eight commercial banks. The data was collected through interviews and to interpret the data content analysis with an inductive approach was used. The results show that the decision-making system in export financing services in Hungary tends to be centralized and bureaucratic with a mostly participative leadership style. Effectiveness in the decision-making process is influenced by decision-related factors, human-qualification factors, and organizational contextual factors which can be amplified by certain other factors.

Wildfire smoke presents a growing threat in the Western U.S.; and human health, transportation, and economic systems in growing western communities suffer due to increasingly severe and widespread fires. While modelling wildfire activity and associated wildfire smoke distributions have substantially improved, understanding how people perceive and respond to emerging smoke hazards has received little attention. Understanding and incorporating human perceptions of threats from wildfire smoke is critical, as decision-makers need such information to mitigate smoke-related hazards. We surveyed 614 randomly selected people (in-person) across the Boise Metropolitan Area in Idaho and 1,623 Boise State University affiliates (online), collecting information about their level of outside activity during smoke event(s), knowledge about the source of air quality information and effective messaging preference, perception of wildfire smoke as a hazard, and smoke-related health experiences. This relatively large dataset provides a novel perspective of people's perception of smoke hazards, and provides crucial policy-relevant information to decision-makers. Dataset is available to the public and can be used to address a wide range of research questions.

Abstract There remains a significant gap in our knowledge of the synergistic nature of family dynamics, child characteristics, and child-rearing features in the etiology of obesity from childhood through adolescence. We assessed the associations of family dynamics (poverty, family structure), child characteristics (child temperament), and child-rearing features (maternal depression, maternal sensitivity, and type of child care) with the development of childhood obesity. Children (n = 1240) whose weights and heights were measured at least once for ten time points (from 2 years through 15 years) from the NICHD Study of Early Child Care and Youth Development were included. Generalized estimating equation (GEE) was used to examine the associations of family and individual factors with the childhood obesity after adjusting for covariates. Adjusted GEE models showed that living below poverty level was associated with an increased odds of obesity (odds ratio = 1.62, 95% confidence interval 1.05, 2.53). Among these key family and individual factors, poverty status was observed to be the strongest predictor of obesity of offspring across time. Findings highlight the importance of systemic-level public health changes in obesity reduction efforts and suggest that poverty-reduction based prevention and intervention are likely more effective targets than more individual/family specific targets.

IntroductionDirect seeding of rice is one of the most common farming methods in the world. Economic efficiency, faster and easier cultivation, reduction of growth period and earlier maturity (7-10 days), needing to less labour, higher water efficiency, more mechanization and reduction of rice vulnerability due to end-of-season drought are some benefit of direct rice cultivation; while, the most important problem in this planting system is weed control, and if weeds are controlled properly; the yield of rice in direct seeding will be almost equal to that of transplanting system. So, this study was conducted to evaluate the effect of weed management methods on different cultivars of rice in direct seeding system.Materials and MethodsThis research was performed as a factorial in randomized complete block design in three replications at Rice Research Institute, Amol in 2018. Treatments include three different rice cultivars of Tarom Hashemi, Shiroodi and Neda under six weed management methods (no weeding, twice weeding (40 days after emergence and in the earing stage), using rice husk mulch (0.5 kg m-2 after sowing), application of triafamone+ethoxysulfuron herbicide (30 g ai ha-1 in 5 days after emergence in two-leaf stage), application of triafamone+ethoxysulfuron herbicide (30 g ai ha-1 in 5 days after emergence in two-leaf stage) + weeding (40 days after emergence) and complete weeding (all weeds are removed during the growing season) were considered. The studied traits included: plant height, tiller number, panicle length, 1000-grain weight, filled grain number, empty grain number, panicle number per square meter, grain yield, biological yield and total weed density and dry weight.Results and DiscussionThe results showed the highest plant height (137.7 cm) and number of tillers (24.7) was obtained in Tarom Hashemi and Shiroodi cultivars under complete weeding, respectively. The results of mean comparison of the main effects of the experimental treatments indicate that the filled grain number in Tarom Hashemi, Neda and Shiroodi cultivars were 67.6, 69.3 and 90.1 per ear and 1000-seed weight in these cultivars, was 19.9, 18.6 and 21.1 g, respectively. When Shiroodi cultivar had more seeds per ear and 1000 seeds weight. Also, the maximum filled grain number (94.9) and the highest 1000-seed weight with 22.7 g was observed in the complete weed control treatment. The highest panicle number per square meter (316.7) was related to Shiroodi cultivar and complete weed control, which had no significant different with herbicide + weeding and twice weeding in the same cultivar, as well as complete weeding in Neda cultivar. The highest grain yield (8192.5 kg ha-1) was observed in Shiroodi cultivar + complete weeding, which had no significant different with Neda + complete weeding and Shiroodi + herbicide + weeding. Lack of weed control in Tarom Hashemi, Neda and Shiroodi cultivars reduced 38.2, 63.3 and 61.7% of grain yield compared to complete weed control, respectively. The results showed that at all three stages of sampling, the lowest weed density and dry weight in all three sampling stages was related to herbicide + weeding treatment in Shiroodi cultivar which had good control over weed and was able to reduce weed density and its dry weight.ConclusionIn general, it can be concluded that although the yield of rice in the direct planting system was the highest in the absence of weed contamination; however, since the application of triafamone+ethoxysulfuron herbicide along with weeding caused reduction the density and dry weight of weeds, and also, improved the yield and yield components of all three rice cultivars, so this management method can be used to control weeds more effectively and achieve proper yield in this system.

Background and Aim: Brucellosis is an infectious disease in humans and livestock. The disease is endemic in many regions of Iran, for example, Hamedan Province. Knowledge of infection rate and associated risk factors is essential to control and prevent the disease. The study aimed to estimate the prevalence of brucellosis and associated risk factors in cattle, sheep, and goats in Famenin, Hamedan Province, West of Iran. Materials and Methods: Blood samples of 1758 animals (1470 sheep, 190 goats, and 98 cattle) were obtained in different rural regions of Famenin. The samples were evaluated to detect of Brucella-antibodies using rose Bengal plate test (RBPT), Wright standard tube agglutination test (SAT), and 2-Mercapto-Ethanol (2-ME) techniques. The risk factors associated with brucellosis such as age, gender, history of vaccination against brucellosis, and abortion history in animals were evaluated. In the sampling process, the critical gaps related to the distribution of brucellosis in the herds and regions are identified for designing the strategies to prevent and control the disease. Results: About 6.88% and 89.31% of animals had a history of abortion and vaccination against brucellosis, respectively. Most of the animals were female (92.49%) and in the range of 2–3 age old (39.8%). The antibodies to the Brucella-infection in animals were 2.73% with RBPT and 1.30% with SAT and 2-ME. The prevalence of brucellosis was detected 1.3% among individual animals and 11% among herds. This rate was 1.43% for sheep and 1.05% for goats, with no significant statistical difference. No seropositive case was detected in cattle samples using RBPT, STAT, and 2-ME. The highest rate of brucellosis (6.25%) was detected in Emamzadeh-Pirnahan region (22.2% goats and 5.6% sheep). In sheep, most cases of the disease were in 3–4 age-old group (1.92%), animals without a history of abortion (1.58%), and without a history of vaccination against brucellosis (2.80%). Furthermore, 5.94% of males and 1.11% of females were detected positive for brucellosis (p < 0.001). The chance of brucellosis in rams was 5.6 folds higher than in others (odds ratio = 5.64). Brucellosis in goats was detected 2.94% and 1.89% in the age groups 0.05) was observed between prevalence of brucellosis and risk factors. In farmers, low level of information about the transmission and also control and preventive methods of the disease was dominant. Consumption of traditional and unpasteurized dairy products is also very common in the studied regions. Conclusion: This is a comprehensive evaluation of animal brucellosis parallel to humans' cohort study in the Famenin region for the first time. Although the rate of brucellosis in animals is low in the region, explaining the risk factors to farmers, mass vaccination, regular screening of animals, and culling the positive animals are very important for controlling and reducing the disease in the region.

Key Clinical Message Vanishing bile duct syndrome (VBDS) has been postulated that may be related to Hodgkin's lymphoma (HL). In the present study, we present a 75‐year‐old male patient with HL who received chemotherapy but has not received any radiotherapy. The patient's condition worsened in further days, and he died with the diagnosis of cirrhosis and hepatic failure.

Brucellosis is one of the most common zoonoses in the Middle East. It is causing economic losses to the livestock industry and has a great public health concern. Little is known about the genetic diversity and distribution of brucellae in Iran. Therefore, forty Brucella spp. strains (B. abortus and B. melitensis) isolated from animals and humans were analyzed by whole genome sequencing (WGS) technology using single nucleotide polymorphism (SNP) analysis and core genome multilocus sequence typing (cgMLST). Brucella isolates were obtained from lymph nodes (cows and camels), milk (cows, camels and sheep), and aborted foetus samples (sheep and goats), as well as cerebrospinal fluid and blood of humans. The isolates were originating from thirteen provinces of Iran and isolated between 2015 and 2020. According to in-silico MLST, ST8 and ST2 were the most frequent sequence types in B. melitensis and B. abortus, respectively. Based on phylogeographic reconstruction using cgSNP analysis, the investigated Iranian B. melitensis strains belonged to the American and Mediterranean lineages of the B. melitensis phylogeny. Furthermore, cgSNP analysis revealed a similarity between Iranian B. abortus isolates and strains from Iraq and Egypt. Therefore, the origin of the Iranian strains can be suggested to be strains from neighboring and Middle East countries. Moreover, cgMLST analysis showed that the Iranian B. melitensis strains were closely relative to strains recovered from sheep and humans in Iraq, Afghanistan, Syria, Turkmenistan, and Pakistan. In the current panel of strains, cgMLST and cgSNP analysis provided an appropriate and accurate tool for effective traceback analyses for Brucella spp. from Iran. The results of cgSNP and cgMLST helped to understand the geographic distribution and interspecies transmission of Iranian strains and highlight the importance of specific brucellosis control measures in Iran with regard to the One-Health approach.

In the 2020s, Export credit agencies alongside other industries face many critical challenges such as new technologies (like Artificial Intelligence and Blockchain), environmental issues, and new financial crises that force change or modification to the organizations. State-owned ECAs have some limitations in their regulations and are inherent in the way of the change process. This paper aims to investigate the management challenges in ECAs in facing the changes needed in the 2020s and how they face them. ECAs in Iran and Hungary are used as case studies. Content analysis with an inductive approach is done to interpret the conducted interviews.

Brucellosis is one of the most important zoonotic diseases in many regions worldwide. This study aimed to investigate the antimicrobial properties of hydro-ethanolic extracts of propolis (EEP) samples collected from six different regions of Iran against five Brucella melitensis (B. melitensis) clinical isolates causing human brucellosis and an antibiotic-resistant B. abortus vaccinal strain (RB51). Brucella clinical isolates were first carefully identified using conventional molecular typing and Brucella bio-typing methods. Different Brucella strains were then confronted with EEPs using the disk-diffusion agar method to evaluate the antimicrobial activity of each propolis extract. Chemical composition of EEPs was then determined using HPLC-DAD, and the main phenolic compounds were quantified. It was found that all EEPs displayed significant antimicrobial activities against Brucella strains, though to varying extents. All tested clinical strains were susceptible to different EEPs with inhibition zones ranging from 18 to 38 mm diameter. Interestingly, the RB51 vaccine strain was more susceptible to EEP6 (from Markazi province), compared to conventional antibiotics used in the treatment of brucellosis. Substantial differences observed in EEP antimicrobial activity could be due to their distinct botanical origins and chemical compositions as confirmed by our HPLC analysis. The promising inhibitory effect of some propolis preparations against a broad spectrum of Brucella strains points to the need for further studies in the context of systematic clinical investigations and opens up the way for the development of natural complements in support of conventional antibiotic therapy.

Abstract Background Brucellosis is an endemic zoonotic disease with rising health and economic concerns in many areas worldwide. Musculoskeletal pains are among the main complications of human brucellosis, which are often difficult to diagnose due to the variability of clinical symptoms. Brucellar discitis is a very disabling problem in some chronic forms of the disease which may lead to serious vertebral and neurological consequences. Case presentation In this case report, we reported the isolation of Brucella abortus from lumbar disc bulging in a woman who had rheumatoid arthritis and diabetes mellitus as underlying conditions. The patient had several negative brucellosis serological tests and dorsolumbar pains with urinary incontinence over a 2-month period. The diagnosis was confirmed by magnetic resonance imaging (MRI) examination of lumbar spine as well as disc culture. MRI examination was performed without intravenous contrast and revealed the presence of disc bulging, left foraminal narrowing at L5-S1, left foraminal narrowing, anterolisthesis grade II at L4-L5. The diagnosis was also confirmed by isolation of B. abortus biovar 1 from bulging disc culture. The isolate was characterized by AMOS PCR, Bruce-ladder PCR and biotyping, resulting in the identification of B. abortus from L4-L5 and L5-S1 disc bulging regions. The patient was treated with two drugs i.e. doxycycline and rifampin for 3 months. In the follow-up, in addition to improving the patient’s general condition, low-back pain was also significantly reduced. Conclusions MRI, serology, cultural and molecular test along with patient history are important to make a rapid diagnosis of brucellosis’ discitis, thereby decreasing the delay for the brucellosis treatment. The present report suggests that the infection by Brucella spp. should be fundamentally considered among the causative agents of back pain especially in the endemic areas of Brucella infections.

Abstract Background Obesity has been generally understudied in Asian Americans. It is important to identify subgroups of Asian Americans at high risk of obesity to help develop targeted interventions for those subgroups. This study aimed to examine the disparities in obesity among Asians (i.e., Chinese, Filipino, Japanese, Korean, and Vietnamese) living in California. Methods A sample of Adult Americans in California (n = 47,970) including Asian American adults (n = 3810) aged 18 years or older were obtained from the 2013–2014 California Health Interview Survey (the U.S. nation’s largest state cross-sectional health survey). In this study, we tried to calculate Iran’s green growth index and compare it with four selected OECD countries in the continent of Asia, Turkey, Japan, Korea, and Israel in 2015 by applying the Green Index, and Technique for Order of Preference by Similarity to Ideal Solution. Our research provides a new methodology to rank different countries based on green growth indicators. Applying both methods, Iran ranked fourth among the selected countries. Iran, in comparison to Turkey, Japan, and Korea, is behind in developing sustainability and green economy indices.

Brucellosis is recognized as a major public health concern leading to critical economic losses in livestock animals. The present study assessed Brucella spp. isolated from aborted ovine and caprine fetuses in different parts of Iran between 2016 and 2019. It used classic and molecular methods in order to determine the Brucella species carrying higher risks of abortion complications in these animals. A total of 189 samples from 35 cases/case series from milk (16 sheep, and 8 goats), 19 abomasum content (sheep), and 146 aborted fetuses (116 sheep, and 30 goats) were bacteriologically examined. Subsequently, the resultant Brucella isolates were further characterized by phenotypic and molecular approaches. The multiplex Polymerase chain reaction (PCR) (Bruce-ladder) and IS711-based PCR were performed on all the extracted DNA to evaluate the presence of Brucella spp. As suggested by the obtained results, all recovered isolates from ovine and caprine abortion samples were either B. melitensis or B. abortus. An issue of concern was the implication of B. melitensis vaccine strain Rev1 in a small portion of sheep and goat abortion cases. Despite the recent B. abortus burden in ovine, aborted cases were predominantly associated with B. melitensis infections in both ovine and caprine, and B. melitensis biovar 1 was responsible for the majority of studied cases. These data and the techniques implemented in the present study can shed light on the level of implication of different Brucella species in ovine and caprine abortion in Iran. The present study identified Brucella agents responsible for abortion in small ruminants at the biovar level. Therefore, it provides precious information for future control programs and vaccination strategies in Middle Eastern regions.
