3. Ecchordosis physaliphora: a cautionary tale.

Abstract

Background: Ecchordosis physaliphora is a congenital, benign lesion originating from notochordal remnants along the craniospinal axis, most frequently located at the level of the clivus and sacrum. Sometimes ecchordosis physaliphora is difficult to recognise and treat, with a total of twenty-six cases described in the literature. Methods: This study reports on three cases of previously undiagnosed ecchordosis physaliphora presenting with cerebrospinal fluid rhinorrhoea and meningitis. Conclusion: Endoscopic transclival or transsphenoid surgery including three-layer (fat, fascia and nasoseptal flap) reconstruction was used in all cases with complete resolution of the symptoms. 4. Late detection of hearing loss in a case with middle ear congenital abnormality

Abstract

The present paper describes a rare case of a unilateral congenital ossicular abnormality that presented in adulthood. A unilateral conductive hearing loss was revealed during a routine examination for a professional driving license. Differential diagnosis included otosclerosis and ossicular anomalies. Our patient, a 40-year-old man, had no history of otitis media or trauma to the head and he was not aware of his hearing loss. Exploratory tympanotomy revealed absence of the long process of the incus and of the stapes. A remnant of the underdeveloped stapes footplate was removed and reconstruction of the defect using a 5.0-mm malleus-oval window prosthesis had an excellent result with complete restoration of the air-bone gap. Abstract

"Silent Sinus Syndrome" is a quite rare condition of otherwise asymptomatic maxillary sinusitis that presents with enophthalmos. Despite the fact that the "Silent Sinus Syndrome" presents with enophthalmos, these patients are finally treated by the otorhinolaryngologist, who should be familiar with this condition, in order to facilitate prompt diagnosis and treatment. We present the case of a 33 year old man with enophthalmos and no other associated symptom that was caused by chronic rhinosinusitis. Functional endoscopic sinus surgery was the treatment of choice. No reconstruction of the orbital floor was performed. One year follow up, following surgical treatment revealed an excellent result.
