Abstract

International audience; PURPOSE: Our objective was to compare oncologic results of nephron sparing surgery (NSS) versus radical nephrectomy (RN) in T1aN0-x M0 papillary renal cell carcinoma (PRCC). PATIENTS AND METHODS: We retrospectively reviewed 277 patients treated for a pT1aN0M0 PRCC selected from an academic database from 12 centres. We compared the clinico-pathological features by using Chi-square and Student statistical analyses. Survivals analyses using Kaplan-Meier and Log-rank models were performed. RESULTS: The two groups were composed by 186 patients treated by NSS and 91 by RN. The TNM stage was fixed and the two groups were, in terms of age and Fuhrman grade, comparable. Median age at diagnosis was 59 years (27-85). Median tumor size was 2.7 cm (0.4-4). The average follow-up was 49 months (1-246). Very few events arose in both groups: two local recurrences were observed in the NSS group (1.07%), three patients died of cancer in the NSS treated group (1.6%) and five in the RN treated group (5.5%). The five and 10 cancer-specific survival rate were comparable in the two groups (98% vs. 100% and 98% vs. 97%). The specific survival curves were perfectly similar for both groups (log rank test, p=0.25). CONCLUSION: NSS is equivalent to RN as far as oncologic control of pT1aN0M0 PRCC is concerned.

Abstract

To compare the level of surgical training of French interns with that of interns in other European countries.Between 2005 and 2006, an exhaustive questionnaire (www.esnz.net) was sent to 288 young urologists in 30 different European countries. Interns were classified into 3 groups: France (group 1, n = 28), Western Europe (group 2, n = 75) and Eastern Europe (group 3, n = 85). Items concerning surgical practice were analysed.The mean age of the interns was 30.8 years (25.7-35.8), 34.6 years (27.4-48.0) and 31.3 years (25.3-51.3) in groups 1 to 3, respectively. The mean number of months of urology training was 28 months (6-60), 44 months (6-72) and 37 months (2-120), respectively. The mean of weekly working time was 70 hours (40-90), 60 hours (35-90) and 65 hours (40-100). French interns were technically more at ease than interns in groups 2 or 3 for all surgical procedures (open, laparoscopic, endourological). Certain criteria were significantly (p0.05) associated with a better level of technical skills: the number of weekly working hours, the small number of interns per department, the diversity of internships, the use of the intern record and the presence of an active tutorFrench interns appear to learn urological surgical procedures more easily than other European interns. However the scientific training of interns must also be evaluated in order to propose a global assessment system of urology training in Europe.

Abstract

International audience; BACKGROUND: The occurrence of positive surgical margins (PSMs) after partial nephrectomy (PN) is rare, and little is known about their natural history. OBJECTIVE: To identify predictive factors of cancer recurrence and related death in patients having a PSM following PN. DESIGN, SETTING, AND PARTICIPANTS: Some 111 patients with a PSM were identified from a multicentre retrospective survey and were compared with 664 negative surgical margin (NSM) patients. A second cohort of NSM patients was created by matching NSM to PSM for indication, tumour size, and tumour grade. MEASUREMENTS: PSM and NSM patients were compared using student t tests and chi-square tests on independent samples. A Cox proportional hazards regression model was used to test the independent effects of clinical and pathologic variables on survival. RESULTS AND LIMITATIONS: Mean age at diagnosis was 61+/-12.5 yr. Mean tumour size was 3.5+/-2 cm. Imperative indications accounted for 39% (43 of 111) of the cases. Some 18 patients (16%) underwent a second surgery (partial or total nephrectomy). With a mean follow-up of 37 mo, 11 patients (10%) had recurrences and 12 patients (11%) died, including 6 patients (5.4%) who died of cancer progression. Some 91% (10 of 11) of the patients who had recurrences and 83% of the patients (10 of 12) who died belonged to the group with imperative surgical indications. Rates of recurrence-free survival, of cancer-specific survival, and of overall survival were the same among NSM patients and PSM patients. The multivariable Cox model showed that the two variables that could predict recurrence were the indication (p=0.017) and tumour location (p=0.02). No other variable, including PSM status, had any effect on recurrence. None of the studied parameters had any effect on the rate of cancer-specific survival. CONCLUSIONS: PSM status occurs more frequently in cases in which surgery is imperative and is associated with an increased risk of recurrence, but PSM status does not appear to influence cancer-specific survival. Additional follow-up is needed.

Abstract

Objectives: To evaluate the effects of a practical introduction to urology session on the recruitment of Parisian interns since introduction of the National Classifying Examination., Materials and Methods: Since 2004, all surgery interns appointed in Paris have participated in a practical introduction to urology session before starting work in hospital. They were asked to complete a questionnaire. The prospective survey studied: age, gender, urology training as a medical student and the intern's preferences in relation to 11 surgical specialties. Interns were recontacted annually to determine their choice of specialization., Results: Population. 255 interns participated, including 145 females (56.9%) and 110 males (43.1%). The mean age was 24.6 +/- 5 years (range: 22-31). 173 interns were attached to a Parisian teaching hospital (67.8%) and 82 (32.2%) were attached to a provincial teaching hospital. 52 interns (21.2%) had completed at least one attachment in a urology department during their medical training. Surgical specialties. Orthopaedics was most frequently listed (n = 48 ; 28.9%). Urology was chosen by 32 interns (12.5%), 28 of whom had completed an urology attachment during their medical training. At the end of the introductory training sessions, 18 interns were tempted by urology. At one year, 31/34 interns (91.2%) confirmed their choice in favour of urology., Conclusion: A practical training sessions very early in the training of young surgeons is a good solution to introduce them to urology. Based on a more informed choice, the most motivated interns are encouraged to preferentially choose this discipline.

Abstract

Objectives: To compare open (OPN) and laparoscopic (LPN) partial nephrectomy (PN) techniques in the light of a French multicentre series., Material and Methods: Data corresponding to 741 PN (91 laparoscopic and 650 open procedures) were compared in terms of the indications, tumour diameter, operative data, complication rates and length of hospital stay., Results: Tumours were smaller in the LPN group (2.7 vs 3.4 cm, p = 0.001). There were fewer malignant tumours (71.1% vs 80% p = 0.05) and fewer NP by necessity (20.9% vs 31.4%. p = 0.04) in the LPN group than in the OPN group. There were fewer hilar tumours in the LPN group than in the OPN group (LPN: 4% vs OPN: 14.8%, p = 0.03). Pedicle clamping was performed less frequently in the LPN group (33% vs 50.2%, p = 0.002) but for a significantly longer mean duration (35 minutes vs 19 minutes, p = 0.0001). The mean operating time was longer in the LPN group (163 vs 150 minutes, p = 0.02). The surgical complication rate (17.6% vs 14.3%), transfusion rate (6.6% vs 10.5%) and mean blood loss (363 vs 434 ml) were not significantly different between the 2 groups. There were significantly more urinary fistulas (12.1% vs 2.5%, p < 0.001) and medical complications (24.2% vs 14%, p = 0.01) in the laparoscopy group, but, in the longer-term, urinarvfistula rates were comparable in the 2 groups. The length of hospital stay was shorter for LPN (9.1 vs 11.2 days, p = 0.009)., Conclusion: This comparative series, reflecting initial experience, shows that laparoscopic partial nephrectomy achieves similar operative and perioperative results to those of open partial nephrectomy. However, the indications for laparoscopic partial nephrectomy remain selective, as the pedicle clamping time and medical complication rates are higher with laparoscopic surgery. Experience and technical progress in laparoscopic partial nephrectomy should make the operative technique comparable to that of open surgery.

Abstract

The authors report two clinical cases of cystic nephroma in adults observed in the department over the last 10 years. The nonspecific clinical findings and the poor contribution of imaging examinations make this rare disease difficult to diagnose. Only histological examination can confirm the diagnosis and eliminate multilocular cystic renal carcinoma or necrotic pseudocystic carcinoma. Based on a review of the literature, the authors emphasize recent pathophysiological and immunohistochemical data that could improve the management of this benign disease, in which malignant transformation is also possible.

Abstract

The surgical management of stress urinary incontinence has been revolutionized with the development of minimally invasive suburethral tape techniques, which ensure a high continence rate by means of a rapid procedure with a short hospital stay. Although morbidity and adverse effects are uncommon, they must be kept in mind in order to adopt an appropriate strategy designed to prevent their development. Urinary retention is the most frequent postoperative complication of retropubic tape. The recent transobturator tape technique modifies the location of the tape with an angulation more close resembling that of fascial support tissues, thereby decreasing the theoretical risk of retention. The authors report the first case of complete urinary retention and discuss the mechanism and therapeutic strategy.
