8. Number needed to treat: A primer for neurointerventionalists.

Abstract

Background: The number needed to treat is a commonly used statistical term in modern neurointerventional practice. It represents the number of patients that need to be treated for one patient to benefit from an intervention. Given its growing popularity in reflecting study results, understanding the basics behind this statistic is of practical value to the neurointerventionalist. Method(s): Here, we review the basic theory and calculation of the number needed to treat, its application to stroke interventions, and its limitations. In addition, we demonstrate several simple methods of calculating the number needed to treat utilizing recent thrombectomy trial results. By presenting the number needed to treat as a universal metric, we provide a comprehensive comparative of the number needed to treat for key stroke therapies, including mechanical thrombectomy, tissue plasminogen activator, carotid endarterectomy, and prevention with antiplatelet and statin drugs. Conclusion(s): In comparison with available stroke therapies, mechanical thrombectomy stands out as the most effective acute intervention in patients with emergent large-vessel occlusions. Understanding how the number needed to treat is derived and its implications helps provide perspective to clinical trial data, identify health-care resource priorities, and improve communication with patients, health-care providers, and additional key stakeholders.Copyright © The Author(s) 2019.

Abstract

Background The Journal of NeuroInterventional Surgery (JNIS) published its first volume in 2009. Over the ensuing years, JNIS flourished and has published a considerable number of high-profile articles. Citation analysis is a method of quantifying various metrics related to scholarly publications. Objective To apply citation analysis to the 100 most cited papers in the history of JNIS. Methods The most cited articles in JNIS were identified by using the Web of Science database. The top 100 articles were ranked according to their number of citations. Further information was obtained for each article, including citations per year, year of publication, authorship, article topics, and article type and level of evidence. Results The total number of citations for the 100 most cited articles in JNIS ranged from 18 to 132 (median 26.0). Most articles (75%) were published between 2012 and 2015 and originated in the USA (79%). Eighteen authors have contributed five or more articles to the top 100 list. The most common topics are related to acute ischemic stroke and cerebral aneurysm. Conclusions This study highlights the influence of JNIS over its first decade by providing a comprehensive list of the 100 most cited articles and their authors as well as topics covered. This study also highlights the important factors driving the growth of JNIS.Copyright © Author(s) (or their employer(s)) 2018.

Abstract

BACKGROUND AND PURPOSE: Stent-assisted revascularization increases prevailing recanalization rates (≅50%–69%) for vessel occlusions recalcitrant to thrombolytics. Although balloon-mounted coronary stents can displace thrombus (via angioplasty) and retain clot along vessel walls, intracranial self-expanding stents are more flexible and exert less radial outward force during deployment, increasing deliverability and safety. To understand the effectiveness of self-expanding stents for recanalization of acute cerebrovascular occlusions, we retrospectively reviewed our preliminary experience with these stents. MATERIALS AND METHODS: Eighteen patients (19 lesions) presenting with a clinical diagnosis of acute stroke underwent catheter-based angiography documenting focal occlusion of an intracranial artery. A self-expanding stent was delivered to the occlusion and deployed. Stent placement was the initial mechanical maneuver in 6 cases; others involved a combination of pharmacologic and/or mechanical maneuvers prestenting. GP IIb/IIIa inhibitors were administered in 10 cases intraprocedurally or immediately postprocedurally to avoid acute in-stent thrombosis. RESULTS: Stent deployment at the target occlusion (technical success) was achieved in all cases. Thrombolysis in Cerebral Ischemia (TICI)/Thrombolysis in Myocardial Ischemia (TIMI) 2/3 recanalization (angiographic success) was achieved in 15 of 19 lesions (79%). All single-vessel lesions (n = 8) were recanalized, but only 7 of 11 combination internal carotid artery and middle cerebral artery lesions were recanalized. No intraprocedural complications occurred. Seven in-hospital deaths occurred: stroke progression, 4; intracranial hemorrhage, 2; respiratory failure, 1. Seven patients had ≥4-point National Institutes of Health Stroke Scale improvement within 24 hours after the procedure, 6 had modified Rankin Score (mRS) ≤3 at discharge, and 4 had mRS ≤3 at 3 months. Overall, revascularization and improvement in clinical outcome were more likely to occur in women. CONCLUSION: Feasibility of self-expanding stents for treatment of acute symptomatic intracranial occlusions is shown. For single-vessel lesions, stent placement with concomitant administration of IIb/IIIa inhibitors contributed to the achievement of recanalization rates exceeding those currently reported for other means of thrombolysis.

Atypical cavernous malformations are lesions with distinct anatomic features and locales. This subgroup includes purely extraxial lesions as well as those involving the ventricular system and cranial nerves. The epidemiology, symptoms, surgical approach, and therapeutic implications are unique to each atypical lesion. These features are reviewed, and the indications as well as caveats for successful surgical treatment are offered.

Abstract

Ischemia of the posterior circulation related to diseases of the vertebral artery is associated with a significant incidence of delayed stroke and morbidity. Although medical therapies, including anticoagulant and antiplatelet agents, typically are used as the first line of treatment, many surgical techniques are available for the management of refractory cases. Atheroclerosis and compression by fascial bands or osteophytes represent the most frequent types of pathology affecting the vertebral artery. Surgical techniques vary according to the location and type of disease process. Vertebral endarterectomy, transpositions, and bypasses are discussed, and surgical approaches are reviewed in the context of the location and type of disease.
