Contains fulltext : 294430.pdf (Publisher’s version ) (Open Access), BACKGROUND: Ta grade 3 (G3) non-muscle-invasive bladder cancer (NMIBC) is a relatively rare diagnosis with an ambiguous character owing to the presence of an aggressive G3 component together with the lower malignant potential of the Ta component. The European Association of Urology (EAU) NMIBC guidelines recently changed the risk stratification for Ta G3 from high risk to intermediate, high, or very high risk. However, prognostic studies on Ta G3 carcinomas are limited and inconclusive. OBJECTIVE: To evaluate the prognostic value of categorizing Ta G3 compared to Ta G2 and T1 G3 carcinomas. DESIGN, SETTING, AND PARTICIPANTS: Individual patient data for 5170 primary Ta-T1 bladder tumors from 17 hospitals were analyzed. Transurethral resection of the tumor was performed between 1990 and 2018. OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: Time to recurrence and time to progression were analyzed using cumulative incidence functions, log-rank tests, and multivariable Cox-regression models with interaction terms stratified by institution. RESULTS AND LIMITATIONS: Ta G3 represented 7.5% (387/5170) of Ta-T1 carcinomas of which 42% were classified as intermediate risk. Time to recurrence did not differ between Ta G3 and Ta G2 (p = 0.9) or T1 G3 (p = 0.4). Progression at 5 yr occurred for 3.6% (95% confidence interval [CI] 2.7-4.8%) of Ta G2, 13% (95% CI 9.3-17%) of Ta G3, and 20% (95% CI 17-23%) of T1 G3 carcinomas. Time to progression for Ta G3 was shorter than for Ta G2 (p < 0.001) and longer than for T1 G3 (p = 0.002). Patients with Ta G3 NMIBC with concomitant carcinoma in situ (CIS) had worse prognosis and a similar time to progression as for patients with T1 G3 NMIBC with CIS (p = 0.5). Multivariable analyses for recurrence and progression showed similar results. CONCLUSIONS: The prognosis of Ta G3 tumors in terms of progression appears to be in between that of Ta G2 and T1 G3. 12. Translating Biochemistry Concepts into Cartoons and Graphic Narratives: Potential and Pitfalls

Abstract

Simple biochemical concepts can be hard to grasp by non-specialists, even when they are related to practical contexts in industry, day-to-day activities, or well-acknowledged pathological conditions. This is especially important in instances where accurate communication of biochemical aspects for different types of stakeholders may be crucial. Examples include interacting with policymakers to establish guidelines, with patients (and/or caregivers) to identify key concepts in promoting awareness and adherence to therapeutic regimens, or with teachers and students for novel approaches in critical thinking. Focusing on our own work in developing communication tools for different purposes, in this review we will focus on some examples of how biochemical concepts can be effectively translated into illustrations and graphical narratives. For this purpose, engagement with target audiences in developing the materials themselves is key. We also discuss how specific projects can be tailored for different purposes, as well as evidence that comic-book strategies are effective in conveying biochemical and biomedical knowledge.

Abstract

When analysing Martorell' s novel we find a recurrent feature suclt as the re-utilisation of differentliterary sourees. This re-utilisations may prevent the reader from grasping the key points of Tirant lo Blanch. From this point of view Rafael Alemany analyses the Valencian author's modus operandi when reusing texts such as Llibre d'ordre de cavalleria, Lancelot en prosa or Voyage d'outre mer and the new spirit present in Tirant. On the other hand, the study of the sources of this Valencian work can enable us to confirm the hypothesis that Tirant lo Blanch was written only by one author.

El presente trabajo se centra en el estudio de los Frataraka, dinastas iranios que gobernaron en la Pérside durante el período helenístico. Se presenta, primeramente, una introducción al sistema de organización territorial del imperio seléucida, la principal potencia hegemónica del Próximo Oriente y Oriente Medio del momento. A continuación, se exponen los principales testimonios de que disponemos para el estudio de los dinastas, se describen brevemente los rasgos fundamentales que los caracterizan y se esbozan los principales debates académicos actuales que los atañen, entre los que destaca la discusión sobre la naturaleza de la relación entre la Pérside y el imperio seléucida., Aquest treball es centra en l'estudi dels Frataraka, dinastes iranians que van governar a la Pèrsida durant el període hel·lenístic. Es presenta, primerament, una introducció al sistema d'organització territorial de l'imperi selèucida, la principal potència hegemònica al Pròxim Orient i Orient Mitjà del moment. A continuació, s'exposen els principals testimonis de que disposem per l'estudi dels dinastes, es descriuen breument els trets fonamentals que els caracteritzen i s'esbossen els principals debats acadèmics actuals que hi tenen relació, entre els quals destaca la discussió sobre la natura de les relacions entre la Pèrsida i l'imperi selèucida.

Item does not contain fulltext, BACKGROUND: The pathological existence and clinical consequence of stage T1 grade 1 (T1G1) bladder cancer are the subject of debate. Even though the diagnosis of T1G1 is controversial, several reports have consistently found a prevalence of 2-6% G1 in their T1 series. However, it remains unclear if T1G1 carcinomas have added value as a separate category to predict prognosis within the non-muscle-invasive bladder cancer (NMIBC) spectrum. OBJECTIVE: To evaluate the prognostic value of T1G1 carcinomas compared to TaG1 and T1G2 carcinomas within the NMIBC spectrum. DESIGN, SETTING, AND PARTICIPANTS: Individual patient data for 5170 primary Ta and T1 bladder tumors from 17 hospitals in Europe and Canada were analyzed. Transurethral resection (TUR) was performed between 1990 and 2018. OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: Time to recurrence and progression were analyzed using cumulative incidence functions, log-rank tests, and multivariable Cox regression models stratified by institution. RESULTS AND LIMITATIONS: T1G1 represented 1.9% (99/5170) of all carcinomas and 5.3% (99/1859) of T1 carcinomas. According to primary TUR dates, the proportion of T1G1 varied between 0.9% and 3.5% per year, with similar percentages in the early and later calendar years. We found no difference in time to recurrence between T1G1 and TaG1 (p = 0.91) or between T1G1 and T1G2 (p = 0.30). Time to progression significantly differed between TaG1 and T1G1 (p < 0.001) but not between T1G1 and T1G2 (p = 0.30). Multivariable analyses for recurrence and progression showed similar results. CONCLUSIONS: The relative prevalence of T1G1 diagnosis was low and remained constant over the past three decades. Time to recurrence of T1G1 NMIBC was comparable to that for other stage/grade NMIBC combinations. Abstract

Two etiopathogenic types of vulvar squamous cell carcinoma (VSCC) have been described: human papillomavirus (HPV)-associated and HPV-independent. Precursor lesions, frequently identified in the adjacent skin, are also distinct in the 2 types of VSCC: high-grade squamous intraepithelial lesions (HSILs) in HPV-associated VSCC and differentiated vulvar intraepithelial neoplasia (dVIN) or vulvar acanthosis with altered differentiation in HPV-independent VSCC. Although HPV-independent precursors mimicking HSIL have been described in the vulva, their frequency and morphologic spectrum have not been completely characterized. We explored, in a large series of HPV-independent VSSC, the frequency and the histologic features of precursors mimicking HSIL. We included 779 DNA HPV-negative/p16-negative VSCC with at least 1\xE2\x80\x89cm of adjacent skin. We evaluated the histologic and immunohistochemical (p16 and p53) characteristics of the intraepithelial lesions, focusing on precursors mimicking HPV-associated vulvar HSIL. A total of 254 tumors (33%) had adjacent premalignant lesions. Of them, 186 (73%) had dVIN, 22 (9%) had vulvar acanthosis with altered differentiation, and 46 (18%) had lesions that mimicked HSIL. The mean age of the patients with these HSIL-like lesions was 72\xC2\xB115 years. Twenty-six of these HSIL-like lesions had basaloid morphology, 13 warty, and 7 mixed basaloid/warty features. All the HSIL-like precursors were DNA HPV-negative/p16-negative; 74% of them showed p53 abnormal staining and 35% of them had areas of conventional dVIN. In conclusion, about one fifth of the HPV-independent precursors mimic HSIL, showing either basaloid or warty features. Older age and the presence of areas of typical HPV-independent intraepithelial lesions, together with p16 negativity, should raise suspicion of an HPV-independent etiology.

Abstract

En este trabajo estudiamos un grupo de etnónimos turcos mencionados en el Cantar de la hueste de Ígor -una obra maestra de la literatura rusa antigua- cuya etimología es controvertida. Comparamos las opiniones de otros investigadores sobre este tema y presentamos los resultados de un análisis del texto en su contexto histórico-literario. En aquest treball estudiem un grup d'etnònims turcs esmentats en el Cantar de l'host d'Ígor -una obra mestra de la literatura russa antiga-, l'etimologia de la qual és controvertida. Comparem les opinions d'altres investigadors sobre aquest tema i presentem els resultats d'una anàlisi del text en el seu context historicoliterari. In this paper we analyze a group of Turkic ethnic names mentioned in the Tale of Igor's Campaign -a masterpiece of ancient Russian literature- with a controversial etymology, by comparing the opinions of various researchers on this topic and by presenting the results of an analysis of the text against its historical and literary context.

Item does not contain fulltext, BACKGROUND: The European Association of Urology (EAU) prognostic factor risk groups for non-muscle-invasive bladder cancer (NMIBC) are used to provide recommendations for patient treatment after transurethral resection of bladder tumor (TURBT). They do not, however, take into account the widely used World Health Organization (WHO) 2004/2016 grading classification and are based on patients treated in the 1980s. OBJECTIVE: To update EAU prognostic factor risk groups using the WHO 1973 and 2004/2016 grading classifications and identify patients with the lowest and highest probabilities of progression. DESIGN, SETTING, AND PARTICIPANTS: Individual patient data for primary NMIBC patients were collected from the institutions of the members of the EAU NMIBC guidelines panel. INTERVENTION: Patients underwent TURBT followed by intravesical instillations at the physician's discretion. OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: Multivariable Cox proportional-hazards regression models were fitted to the primary endpoint, the time to progression to muscle-invasive disease or distant metastases. Patients were divided into four risk groups: low-, intermediate-, high-, and a new, very high-risk group. The probabilities of progression were estimated using Kaplan-Meier curves. RESULTS AND LIMITATIONS: A total of 3401 patients treated with TURBT ± intravesical chemotherapy were included. From the multivariable analyses, tumor stage, WHO 1973/2004-2016 grade, concomitant carcinoma in situ, number of tumors, tumor size, and age were used to form four risk groups for which the probability of progression at 5 yr varied from <1% to >40%. Limitations include the retrospective collection of data and the lack of central pathology review. CONCLUSIONS: This study provides updated EAU prognostic factor risk groups that can be used to inform patient treatment and follow-up. Abstract

Introduction: New coronavirus SARS-CoV2 has spread very quickly throughout our country. A lower incidence has been described in the population of pediatric age, with cases presenting a milder course than in adulthood. Case series: We present 5 clinical cases of children admitted to our hospital for respiratory infection by SARS-CoV2 during the first weeks of the outbreak in our country. Discussion. The spectrum of severity of the disease is very wide. In this case series, we found from upper respi-ratory symptoms to one severe patient with acute respiratory distress syndrome, requiring admission to the intensive care unit: The cases were diagnosed made by polymerase chain reaction. Serological tests were performed in all cases to detect antibodies against the virus. The treatment administered consisted of supportive therapy in all cases. Other drugs such as antivirals or immunomodulators were added in severe cases, according to the experience in adults, because of the absence of clinical trials carried out in the pediatric population., Objetivos: El nuevo coronavirus SARS-CoV-2 se ha extendido de forma muy rápida por nuestro país. Se ha descrito una menor incidencia en la edad pediátrica, observándose en estos casos un curso más leve que en la edad adulta. Casos clínicos: Se presenta una serie de cinco casos de niños ingresados en nuestro hospital por infección respiratoria por SARS-CoV-2 durante las primeras semanas del brote. Discusión y conclusiones: El espectro de gravedad de la enfermedad es muy amplio. En la muestra estudiada encontramos desde cuadros respiratorios de vías altas hasta un caso grave, que desarrolló un síndrome de distrés respiratorio agudo y precisó ingreso en la Unidad de Cuidados Intensivos. El diagnóstico de los casos se efectuó mediante reacción en cadena de polimerasa y posteriormente se realizaron en todos los casos pruebas serológicas para la detección de anticuerpos frente al virus. En cuanto al tratamiento administrado, consistió en una terapia de soporte en todos los casos, a la que se han añadido otros fármacos, como antivirales o inmunomoduladores en los pacientes graves, según la experiencia en adultos, dada la ausencia de ensayos clínicos realizados en la población pediátrica.

Abstract

Here, the first accurate study is presented of the room-temperature and 100 K structures of one of the first organic spin liquids, 􏰗-(BEDT-TTF)2Ag2(CN)3. It is shown that the monoclinic structure determined previously is only the average one. It is shown that the exact structure presents triclinic symmetry with two non-equivalent dimers in the unit cell. But surprisingly this does not lead to a sizeable charge disproportionation between dimers. The difference from the analogue compound 􏰗-(BEDT-TTF)2Cu2(CN)3 which also presents a spin liquid phase is discussed in detail. The data provided here show the importance of the anionic layer and in particular the transition metal position in the process of symmetry breaking. The possible impact of the symmetry breaking, albeit weak, on the spin-liquid mechanism and the influence of various disorders on the physical properties of this system is also discussed.

Definició de procediments i mètodes d'assaig a IDIADA per la homologació de components i/o vehicles segons el Reglament UN 134 (Uniform provisions concerning the approval of motor vehicles and their components with regard to the safety-related performance of hydrogen-fuelled vehicles (HFCV)) i el Reglament (EC) 79/2009 (Type-approval of hydrogen-powered motor vehicles). Per tant, podriem definir com a objectiu del TFG, estudiar i desenvolupar la capacitat d'assaig, ser capaços de millorar els procediments existents realitzats a Idiada i generar els que manquin segons la reglementació vigent, poder ser mes òptims i tenir més precisió en els assajos. A més a més, per tal de contextualitzar i harmonitzar el contingut principal del TFG, m'agradaria realitzar un estudi del funcionament dels vehicles d'hidrogen i el seu mecanisme (estat de l?art).

Abstract

There are at least two different etio-pathogenic pathways for the development of vulvar squamous cell carcinoma (VSCC): one associated with infection by human papillomavirus (HPV) and another independent of HPV. We aimed to describe the histological characteristics of HPV-associated and HPV-independent tumors and to determine the best strategy to identify HPV in VSCC. A single paraffin block was available for review from a series of 1594 VSCCs. In all cases HPV DNA detection was analyzed using the SPF10PCR/DEIA/LiPA25 system and p16 immunohistochemistry (IHC). A tumor was considered as unquestionably HPV-associated if both HPV DNA and p16 IHC were positive. A tumor was considered indisputably HPV-independent if both HPV DNA and p16 IHC were negative. Two groups of tumors were classified as non-conclusive: 1) HPV DNA+/p16-; and 2) HPV DNA-/p16+. WHO typing and a thorough histological evaluation were conducted in all cases. 441 tumors were HPV DNA+ with 367 cases (23.0%) being HPV DNA+/p16+. These HPV DNA+/p16+ tumors were more frequently basaloid or warty (49.8%), but 36.5% were of the keratinizing type. 1153 tumors were HPV DNA-, with 1060 cases (66.5%) being HPV DNA-/p16-. These HPV DNA-/p16- tumors were mostly keratinizing (81.2%) but were occasionally basaloid or warty (5.2%). The features of HPV DNA-/p16+ cases (n=93) were similar to those of the HPV-associated VSCC, and HPV DNA+/p16- (n=74) cases had a more diverse profile, although they were more similar to HPV-independent tumors.
