Background. The ultrasound method of examination is often used to detect liquid formations of a postoperative wound. For this reason, the search for new criteria for assessing the course of the wound process is currently relevant. Aim. To study the possibility of using the ultrasound method of research to determine the severity of postoperative wound complications according to the ClavienDindo classification and further optimal tactics, diagnosis and treatment. Material and methods. 255 patients after surgery for various urgent diseases of the abdominal organs, as well as patients with elective surgeries were examined. All patients underwent complex clinical, laboratory and instrumental studies. Continuous values that do not have a normal distribution were processed using non-parametric methods the values were presented in the format Me [Q1; Q3], the difference in unrelated groups was assessed by the MannWhitney U test, and in related groups, by the Wilcoxon test. Results. The postoperative period in 58 (22.7%) of 255 patients was complicated by postoperative wound complications, which had I, II, III severity according to the ClavienDindo classification. On the 5th day of ultrasound examination of the surgical site with the III degree of severity of postoperative wound complications, there was a statistically significant increase in the echogenicity width of tissues in the area of the wound defect compared with the III severity of complications. Conclusion. The ultrasound method of research allows to evaluate the effectiveness of reparative processes in damaged tissues and the development of complications, as well as to determine the severity of their course in the postoperative period for planning further surgical tactics.

The present paper aims to study the biological properties of a series of bicyclic monoterpene alcohols. Firstly, we tested the obtained compounds for fungicidal activity against clinical and reference strains of microscopic fungi. Next, we determined the minimum inhibitory concentration of these compounds comparing to other drugs widely used in practical medicine (fluconazole, terbinafine). At this stage, we found that ( −)-myrtenol (47 MIC and 23.5 μg/ml) exhibits the most promising activity against filamentous and yeast fungi, respectively. Then, we have studied the membrane-protective and antioxidant activities of the obtained compounds and found out that ( −)-cis-verbenol and ( −)-myrtenol exhibit the highest activity on the model of erythrocytes oxidative hemolysis. Interestingly, among all the studied bicyclic monoterpene alcohols, the alcohols of the pinane series have been found to be the most promising. The obtained results from the present study suggest that ( −)-myrtenol would be a leading compound for further studies in terms of possible practical application.

Цель исследования — улучшение результатов лечения больных с острой кишечной непроходимо-стью путем разработки и внедрения рациональной программы профилактики и лечения послеоперационных осложнений. Материал и методы. За 4 года в отделение хирургии ГдВВ (Госпиталь для ветеранов войн) г. Казани было госпитализировано 106 больных с различными формами острой непроходимости тонкого кишечника (ОКН). Диагноз устанавливали на основании анамнеза, осмотра, характерных рентгенологических признаков, ультразвукового исследования, рентгеноконтрастной диагностики. Все госпитализированные больные получали стандартное лечение, направленное, прежде всего, на борьбу с шоком и интоксикацией, коррекцию водно-электролитного и белкового баланса, восстановление естественного пассажа по кишечнику. Результаты и их обсуждение. В результате лечения у 83 (78,3%) пациентов явления кишечной непроходи-мости разрешились консервативным путем, в 23 (21,7%) случаях ввиду неэффективности консервативных методов пациенты были прооперированы. Определены подходы инфузионной терапии на различных ста-диях кишечной непроходимости. В послеоперационном периоде частота гнойно-септических осложнений составила 17,3% (4 случая). В послеоперационном периоде умер 1больной. Общая послеоперационная летальность составила 4,3%. Заключение. На программу комплексного метода лечения больных с острой непроходимостью тонкого кишечника, безусловно, оказывает влияние и такие факторы, как возраст больно-го, сроки госпитализации, патологический процесс в брюшной полости, наличие и характер сопутствующей патологии.Ключевые слова: инфузионная терапия, гнойно-септические осложнения, острая тонкокишечная непроходи-мость, периоперационная антибиотикопрофилактика.
